Continucare Corporation (Continucare) is a provider of primary care physician services. The Company, through its network of 18 medical centers, provide primary care medical services on an outpatient basis. Continucare also provide practice management services to independent physician affiliates (IPAs) at 21 medical offices.The Company’s medical centers and IPAs are located in Miami-Dade, Broward and Hillsborough Counties, Florida. As of June 30, 2009, the Company provided services to or for approximately 25,800 patients on a risk basis and approximately 8,800 patients on a limited or non-risk basis. Additionally, as of June 30, 2009 Continucare also provided services to over 4,000 patients on a non-risk fee-for-service basis. On August 12, 2009, the Company completed the acquisition of Professional Sleep Diagnostics, Inc. (PSD).
