Abstract

背景 
全世界的医师每年执行几百万例外科手术过程。这些过程中的许多作为开放手术被执行,并且越来越多的过程使用最低限度地侵入的手术(例如内窥镜检查过程、关节镜检查过程以及腹腔镜检查过程)被执行。作为一个例子,美国美容整形外科学会报道,2007年美国发生超过450,000例的抽脂手术过程。 
外科手术过程涉及皮肤和相关的组织中的严重的创伤,例如切口。在许多情况下,切口在过程结束时使用机械装置(例如钉或缝线)被闭合,或使用粘合剂被闭合。此后,创伤经常仅仅用干燥的无菌绷带覆盖。当然,通常不只在表皮处有破裂。 
使用许多外科手术过程,特别是以最低限度地侵入的技术执行的那些过程,很多破裂或损伤在表皮以下或在皮下水平。再次作为一个例子,在一种类型的抽脂手术过程中,在引入肿胀性流体(盐水、温和止痛药和肾上腺素)之后,外科医生将使用套管针和具有抽吸作用的套管来除去脂肪区域。在这么做时,在远离放置套管的切口或放置设备的其他切口的组织部位处形成皮下空隙和其他组织缺陷并非罕见。被损伤的组织将需要时间和护理来愈合,并且带来很多潜在的并发症和风险,列举几个,包括水肿、血清肿、血肿、另外的瘀伤以及瘀癍。 
简要概述 
本文通过例证性的实施方案处理用于对切口和对被损伤的皮下组织进行外科手术后创伤护理的设备、系统和方法的缺点。根据一个例证性的实施方案,用于处理皮下的被损伤的组织的减压系统包括具有倾斜的末端和由医疗垫枕材料形成的成形敷料垫枕。成形敷料垫枕用于放置在患者的表皮上并且被实质上依尺寸制造成覆盖被损伤的皮下组织。减压系统还包 括用于在患者的表皮的一部分和成形敷料垫枕上提供流体密封的上覆盖布(over-drape);减压源;以及减压接驳体(reduced-pressure interface)。减压接驳体用于向成形敷料垫枕传送减压。系统还包括用于流体地耦合减压源和减压接驳体的减压传送导管。成形敷料垫枕具有当被放置在减压下时产生并且均匀地分布压缩力的特征。作为成形敷料垫枕的特征的一部分,还可以产生闭合力。

切口12可以使用任何机械闭合工具例如钉或缝线来闭合,或可以使用粘合剂来闭合,但是在该例证性的实施方案中被示为使用缝线13来闭合。减压系统10通常用于处理区域,并且特别地通常用于处理皮下组织部位20和围绕皮下组织20的组织,但是减压系统10也可以用于处理围绕切口12的更有限的区域。

上覆盖布62可以耦合于成形敷料垫枕32。如果耦合是期望的,那么 耦合可以以许多方式出现。可以使用粘合剂例如丙烯酸粘合剂、硅酮粘合剂、水凝胶、水解胶体等来耦合上覆盖布62和成形敷料垫枕32。可以通过使用任何技术--包括但不限于焊接(例如超声焊接或RF焊接)、粘结、粘合剂、粘固剂等--来粘结上覆盖布62和成形敷料垫枕32。上覆盖布62和成形敷料垫枕32可以被部分地耦合、完全耦合或根本不耦合。可以向结合物添加结构,以使上覆盖布62在期望的方向上表现为各向异性,即,产生各向异性的盖布材料。各向异性的盖布材料被配置为在给定的方向或轴线上比另一个方向或轴线有更大程度的移动、收缩或膨胀。这种行为还在下文结合图9被讨论。如本文所使用的,术语“耦合”包括通过单独的物体耦合并且包括直接耦合。术语“耦合”还包括两个或更多个组分,其借助于从同一片材料形成的组分中的每个而彼此连续。此外,术语“耦合”可以包括化学的(例如通过化学键)、机械的、热的或电的耦合。流体耦合意指流体在指定的部件或位置之间的连通中。

减压子系统80包括减压源82或治疗单元。减压源82可以是真空泵、 壁式吸入装置或其他源。减压源82提供作为系统10的一部分的减压。虽然施加于组织部位的减压的量和性质将通常根据应用变化,但是减压将通常在-5mm Hg至-500mm Hg之间并且更通常在-100mm Hg至-300mm Hg之间。

在操作中,减压系统10可以在手术室中在外科手术过程之后被应用在患者身上或应用在其它地方。可以包括舒适层(见例如图16)的成形敷料垫枕32的第二表面36将靠着患者的表皮14放置,其中成形敷料垫枕32被放置在被损伤的皮下组织部位20上并且一部分被放置在切口12上。 敷料组件30可以对在由护理提供者执行的过程中涉及的典型应用被预先依尺寸制造或当时依尺寸制造。敷料组件30可以被依尺寸制造、成形并且配置成在不同的组织应用(例如腹部的、胸、大腿、手足等)中起作用。 
如果上覆盖布62尚未耦合(见其他例证性的实施方案)于成形敷料垫枕32,则上覆盖布62将被放置在成形敷料垫枕32的第一表面34上,额外的部分延伸超出外周边缘38以形成盖布延伸部分64。然后盖布延伸部分64可以被胶带粘在下面(见图2中的172)或用形成上覆盖布62与患者的表皮14之间的流体密封的粘合剂70(图1)粘住。流体密封仅仅需要足以允许减压系统10将减压保持在期望的位置。然后如果尚未安装减压接驳体92,则将应用减压接驳体92,并且减压传送导管90将在一端被耦合。然后,减压传送导管90的另一端将被耦合于减压源82。然后,减压源82可以被激活并且减压可以被传送至成形敷料垫枕32。 
当在成形敷料垫枕32处的压力被减少时,成形敷料垫枕32压缩并且横向地收缩,并且形成半刚性的基材或柔性较低的基材。减压通过成形敷料垫枕32传输,使得减压在切口12的点处被施加于患者的表皮14。至少在愈合过程的早期阶段并且由于某些类型的创伤,减压可以通过切口12传输并且进入皮下组织20,以及压力的减少可以直接帮助闭合缺陷,例如皮下空隙22,并且通常向区域提供稳定性。被传送至成形敷料垫枕32的减压还产生再次可以提供稳定性、治疗并且也可以闭合或帮助闭合皮下空隙22的压缩力24。压缩力24优选地不止在表皮14处。例如,压缩力24可以在皮下组织20的水平或其他皮下身体结构处施加力。 
当上覆盖布62与成形敷料垫枕32在减压的影响下横向地收缩时,并且当压缩力作用于表皮14时,产生净闭合力26,净闭合力26可以帮助保持切口12闭合并且可以通常对区域提供另外的稳定性。切口12的有效抗张强度可以被增加。闭合力26可以部分地依赖于成形敷料垫枕32与表皮14之间的摩擦以将闭合力传递至表皮14,并且可以涉及力经由粘合剂70或(如果使用胶带(图2中172))通过摩擦从盖布延伸部分64到表皮14的力的传输。同时,被传送至成形敷料垫枕32并且通过成形敷料垫枕32传送的减压帮助从切口12除去任何渗出液或其他流体。在一个方面,减 压系统10抑制表皮14中褶皱的形成。系统10可以向表皮14传送平均量的力,将表皮14保持在用于愈合的平滑的或无褶皱的构型中。 
减压系统10可以避免皮肤刺激,例如可能由二级剪切力、二级应变或其他效应导致的患者表皮14的起泡。为了这个目的,成形敷料垫枕32的末端33可以被成形为提供径向压缩力的均匀分布。末端33是成形敷料垫枕32的外部成形部分,并且外周边缘通常是成形敷料垫枕32的最外侧的部分或接合患者的皮肤的最外侧的部分。末端33可以采取很多不同的形状,以帮助均匀地分布压缩力或以其他方式避免应力梯级。末端33的可能的形状包括以下形状:如图1所示的斜切(或有角度的、倾斜的、或锥形的)表面、如图2所示的弓形形状、或分布所述力的其他形状。相反地,当使用具有正方形边缘的垫枕时,当上覆盖布被应用在垫枕上并且进入患者的表皮时,可以形成“帐篷区域”。“帐篷区域”可以导致皮肤刺激,除非采取其他措施。成形敷料垫枕32避免“帐篷区域”。敷料垫枕的成形边缘或末端允许产生压缩力而没有大的“边缘效应”;即,不使剪切力或应力上升到导致皮肤刺激(例如红斑或水泡)的水平。成形敷料垫枕32的成形部分逐渐地分布力以避免刺激。这种小心地将力施加于皮肤以避免刺激的方式通常被称为“均匀分布”压缩力,但并不在文字意义上被严格地使用。作为另一个抵抗皮肤刺激的预防措施,内层可以被添加在成形敷料垫枕32和患者的表皮14之间(见例如图11中的857)或放置在其他位置,如下文结合其他例证性的实施方案进一步解释的。 
在手术室中应用减压系统10并且允许减压系统10保持在患者身上直到已经发生足够的愈合可能是期望的。在这方面,可能期望从透视材料形成上覆盖布62、成形敷料垫枕32和任何其他层,以允许护理提供者获得关于切口12和被损伤的皮下组织20的愈合的视觉线索,而不必除去敷料组件30。

现在将进一步描述所产生的力。环境压力在上覆盖布162的第一表面161上提供竖直力131,并且成形敷料垫枕132的收缩产生压缩力124以提供指向表皮114并且到达皮下水平(即,到达皮下组织118)的力。同时,可以产生横向力或闭合力。闭合力通过成形敷料垫枕132被传送至表皮。力127是通过成形敷料垫枕132收缩和压缩而导致的向内的收缩力。当成形敷料垫枕132收缩和压缩时,闭合力通过成形敷料垫枕132传送至表皮114。同时,对于本例证性的实施方案,当减压被施加时,上覆盖布162被拉入接近末端133的区域中,如箭头128所表示的。因为盖布延伸部分164被固定于表皮114,所以力128的水平分量将向内拉表皮,如向内的闭合力129所示出的。

很多材料可能用于各个层741、732、747。透气干燥层741可以例如从允许流体流入成形敷料垫枕735的亲水无纺材料形成。透气干燥层741可以是帮助避免皮肤刺激或以其他方式增强舒适的舒适层。成形敷料垫枕735可以由可以储存相对大量的流体的相对薄的吸收性结构或材料形成。例如,成形敷料垫枕735可以由经常被称为“水凝胶”、“超吸收剂”或“水解胶体”的类型的超吸收性聚合物(SAP)形成。成形敷料垫枕735也可以由上文提到的歧管材料中的任一个形成。不透气层747可以由很多不同的材料形成,例如将保持流体不泄漏的聚乙烯膜。可以添加另外的基材。各种层741、732、747可以被密封,或与粘合剂(例如热熔性粘合剂)结合,或被热胶粘,或使用任何技术或工具被耦合。

在一个实施方案中,成形敷料垫枕1132可以包括多个柔性凹口1178。柔性凹口1178可以是在如所示的成形敷料垫枕1132中的横向凹口或横向切口,并且此外或可选地,可以是一个或多个纵向凹口或纵向切口或其他切口。切口可以使用锯(或有凹口的刀刃)、热封刀或其他工具产生。柔性凹口1178增强成形敷料垫枕1132的柔性。当敷料组件1130被施用在患者的关节或其他运动的区域上时,增强的柔性可能是特别地有用的。例如,如果成形敷料垫枕1132在膝盖上被使用,那么成形敷料垫枕1132可能需要弯曲或延伸高达100%或更多,并且柔性凹口1178或脊帮助提供期望的柔性。此外,可以添加多个褶皱1173,以便于运动,如下文进一步描述的。

密封子系统1160在敷料组件1130和患者的表皮的至少一部分上提供流体密封。密封子系统1160包括上覆盖布1162,上覆盖布1162可以被形成为具有第一上覆盖布部分1163和第二上覆盖布部分1165。第一上覆盖布部分1163在成形敷料垫枕1132的第一侧1134上延伸,并且进一步延伸以形成具有第一侧1166和面向内的第二侧(未明确地示出)的盖布凸缘或盖布延伸部分1164。孔1181在第一上覆盖布1163的一部分上形成。孔1181用于允许与减压接驳体(例如图1中的减压接驳体92)流体连通。

一个或多个释放构件1182可以可释放地耦合于第二盖布部分1165的第一侧1167。在图15的例证性的实施方案中示出了四个释放构件1182。释放构件1182在敷料组件1130的部置期间提供硬度和帮助。释放构件1182通常是被保持在第二盖布部分1165的第一侧1167上的铸涂纸或膜。

第二盖布部分1265的面向内的第二侧(未明确地示出,但是为与第一侧1267相对的侧)可以被粘合剂覆盖。在部置前的状态中,该粘合剂 被底部释放构件1286和侧部释放构件1287覆盖。 
一旦被组装,系统1210的部分与图15的系统1120的部分相似。用法和设计可以变化,但是在一个例证性的实施方案中,系统1210的部分可以被部置,如将要描述的。底部释放衬里1286被除去,并且第二盖布部分1265的面向内的第二侧上的被暴露的粘合剂被放置为靠着在一端开始的患者的表皮的一部分,并且可以被放置在线形创伤上。在平稳地施用第二盖布部分1265之后,侧部释放构件1287被除去。在上覆盖布1262的第一侧1267上的释放构件被除去。减压接驳体被耦合于第一上覆盖布部分1263中的孔1282。中央释放构件1284已经在制造期间被除去。 
关于制造上文描述的系统和部件,已经示出了部件和它们的组装。在将上覆盖布施用和耦合于成形敷料垫枕的第一表面时,利用按压来除去任何褶皱可能是期望的,否则褶皱可能产生或保留。成形敷料组件的医疗垫枕材料可以使用冲切或使用刳刨机用手切割。 
根据另一个例证性的实施方案,用于处理组织部位的减压系统包括指向力构件,指向力构件具有非正交的边缘,例如弯曲的边缘、倾斜的或成角度的边缘或与盖布的一部分粘合的边缘,指向力构件用于在被放置在减压下时均匀地分布力。指向力构件可以被形成为具有用于传输流体的多个通道的泡沫构件。减压系统还包括用于在指向力构件的至少一部分和患者的表皮上提供流体密封的盖布。系统还可以具有用于流体地耦合减压源和指向力构件的减压导管。在一个例证性的实施方案中,指向力构件是具有锥形边缘的泡沫构件。当减压通过盖布被减压源传送至内部部分时,减压使指向力构件施加力。力可以包括逆着患者的表皮或其他组织的可以穿透至大于1毫米、大于2毫米、大于3毫米、大于4毫米、大于5毫米、大于7毫米以及更深的竖直力。竖直力可以帮助使死区以及空隙靠近。力可以是或包括闭合力。 
根据另一个例证性的实施方案,减压力产生敷料组件包括具有倾斜的边缘的用于在被放置在减压下时均匀地分布力的指向力构件。指向力构件具有顶侧和底侧。指向力构件由具有多个通道的医疗垫枕材料形成。流动通道可以是互相连接的,例如泡沫。敷料组件还可以包括用于在指向力构 件的至少一部分和患者的表皮上提供流体密封的盖布。指向力构件可以具有成角度的末端。可选地,指向力构件可以具有弓形末端。敷料组件也可以具有耦合于指向力构件的底侧的舒适层。舒适层可以是耦合于指向力构件的底侧的透气干燥层,或帮助避免皮肤的浸渍或任何种类的皮肤刺激的任何其他材料。 
根据另一个例证性的实施方案,处理患者身上的被损伤的皮下组织的方法包括将成形敷料垫枕定位在被损伤的皮下组织上。成形敷料垫枕具有倾斜的末端并且由医疗垫枕材料形成。方法还包括在患者的表皮的一部分和成形敷料垫枕上部置上覆盖布以提供流体密封,并且提供减压源。方法还包括将减压接驳体耦合于盖布并且将减压传送导管流体地耦合于减压源和减压接驳体。方法还涉及激活减压源以向成形敷料垫枕提供减压来产生压缩力和闭合力。压缩力可以在皮下组织或其他皮下身体结构处实现。 
虽然已经在某些例证性的非限制性实施方案的上下文中公开了本发明以及其优点,但是应当理解,可以作出各种改变、取代、置换和更改而不偏离由所附的权利要求限定的本发明的范围。应认识到,结合任何一个实施方案描述的特征也可以适用于任何其他实施方案。 

Claims (27)

1.一种用于处理皮下的被损伤的组织的减压系统,所述减压系统包括:
成形敷料垫枕,其具有倾斜的末端和多个横向凹口,并且由医疗垫枕材料形成,所述成形敷料垫枕用于放置在患者的表皮上并且被实质上依尺寸制造成覆盖被损伤的皮下组织;
上覆盖布,其用于在所述患者的表皮的一部分和所述成形敷料垫枕上提供流体密封;
减压源;
减压接驳体,其耦合于所述上覆盖布,所述减压接驳体用于向所述成形敷料垫枕传送减压;
减压传送导管,其用于流体地耦合所述减压源和所述减压接驳体;
其中所述成形敷料垫枕还包括:
超吸收性材料,其具有第一表面和面向内的第二表面,并且可操作以随着液体的添加而增加刚性;
透气干燥层,其具有第一表面和第二表面;
不透气层,其具有第一表面和第二表面;
其中所述超吸收性材料的所述第二表面至少部分地耦合于所述透气干燥层的所述第一表面,并且所述不透气层上的所述第二表面和所述超吸收性材料的所述第一表面至少部分地耦合;并且
其中所述成形敷料垫枕具有当被放置在减压下时产生并且均匀地分布径向压缩力的特征。
2.根据权利要求1所述的系统,其中所述成形敷料垫枕包括具有大于25.6kg/m3的密度的网状泡沫。
3.根据权利要求1所述的系统,其中所述成形敷料垫枕包括具有大于32.0kg/m3的密度的网状泡沫。
4.根据权利要求1所述的系统,其中所述成形敷料垫枕包括具有大于80.0kg/m3的密度的网状泡沫。
5.根据权利要求1所述的系统,其中所述成形敷料垫枕包括具有密度小于表皮密度的网状泡沫。
6.根据权利要求1所述的系统,其中所述径向压缩力在比所述表皮深的组织部位处实现。
7.根据权利要求1所述的系统,其中所述成形敷料垫枕的所述末端在横截面中具有梯形形状。
8.根据权利要求1所述的系统,其中所述成形敷料垫枕在至少两个正交的横截面中具有弓形形状。
9.根据权利要求1所述的系统,其中所述成形敷料垫枕还包括纵向凹口。
10.根据权利要求1所述的系统,
其中所述成形敷料垫枕包括具有大于25.6kg/m3的密度的网状泡沫;
其中所述成形敷料垫枕的所述末端在横截面中具有梯形形状。
11.根据权利要求1所述的系统,其中所述成形敷料垫枕具有第一表面和面向内的第二表面,并且其中所述上覆盖布耦合于所述成形敷料垫枕的所述第一表面的至少一部分。

17.根据权利要求1所述的系统,其中所述成形敷料垫枕包括由亲水泡沫形成并且具有第一表面和面向内的第二表面的第一垫枕层;由疏水泡沫形成并且具有第一表面和面向内的第二表面的第二垫枕层;并且其中所述第一垫枕层的所述第一表面和所述第二垫枕层的所述面向内的第二表面被相邻地布置。
18.根据权利要求1所述的系统,其中所述成形敷料垫枕包括:
由亲水泡沫形成并且具有第一表面和面向内的第二表面的第一垫枕层,所述亲水泡沫的所述面向内的第二表面用于靠着所述患者的表皮放置;
由疏水泡沫形成并且具有第一表面和面向内的第二表面的第二垫枕层;以及
其中所述第一垫枕层的所述第一表面和所述第二垫枕层的所述面向内的第二表面被相邻地布置。
19.根据权利要求1所述的系统,其中所述成形敷料垫枕具有第一表面和面向内的第二表面,并且其中所述成形敷料垫枕的所述第一表面的至少一部分和所述末端的至少一部分耦合于所述上覆盖布。
20.根据权利要求1所述的系统,其中所述成形敷料垫枕包括超吸收性材料。
21.根据权利要求1所述的系统,其中所述成形敷料垫枕包括超吸收性材料,所述超吸收性材料在减压下可操作以产生随着流体到所述超吸收性材料的添加而增加的所述径向压缩力。
22.根据权利要求1所述的系统,其中所述上覆盖布在所述成形敷料垫枕上延伸,并且其中所述上覆盖布包括各向同性材料。

所述系统还包括内层,所述内层与所述盖布延伸部分的至少一部分在面向内的表面上耦合并且与所述成形敷料垫枕的至少一部分在面向内的表面上耦合,且所述内层被形成为具有处理区域孔。
