Abstract

背景 
在世界各地的医生每年进行数百万手术过程。许多过程作为打开手术被进行,且越来越多的过程采用微创手术来进行,诸如内窥镜、关节镜、和腹腔镜过程。作为一个例子,美国美学整形外科学会报道美国2007年有超过450,000例的抽脂术过程。 
手术过程涉及表皮和相关组织中的明显伤口,如切口。在许多实例中,在过程结束时,使用诸如U形钉或缝合线的机械装置来闭合切口或使用粘合剂来闭合切口。此后,通常仅用干燥的、消过毒的绷带来覆盖伤口。当然,通常不只是在表皮处由破裂。 
对于许多手术过程,特别是那些用微创技术完成的手术过程,大量破裂或损伤是在表皮之下或在皮下水平处。再次作为一个例子,在一种类型的抽脂术过程中,在引入肿胀液(盐水、轻微镇痛剂和肾上腺素)之后,外科医生将使用套管针和有吸力的套管,以除去脂肪区域。这样做时,经常见到在远离切口(通过其放置套管)或其他切口(通过其放置设备)的组织部位处形成皮下空隙和其他组织缺损。受损的组织将需要时间和护理来治愈并引起许多潜在的并发症和危险,列举一些,包括浮肿、血清肿、血肿、进一步的瘀伤以及瘀斑。

详细描述

在治疗期间,注意避免皮肤刺激,诸如由于二次剪切、二次应变(strain)或其他影响的患者的表皮14的起泡。为了帮助避免皮肤刺激,外围边缘39可以被成形或成角度,或者可以在超吸收性敷料垫枕31与患者的表皮14之间增加内层。

进入层138具有第一表面140和面向内的第二表面142。进入层138是用来允许流体离开患者的表皮114上的线形伤口112并穿过进入层138,而不保留在表皮114上。换句话说,进入层138起到有效地允许仅在一个方向上流动的作用。此单向作用有助于避免表皮114的浸渍。进入层138还帮助将减压管输送或分布至线形伤口112。进入层138可以被描述为透气的干燥层。许多材料可以被用于进入层138,诸如亲水性无纺布材料。 
顶层144具有第一表面146和面向内的第二表面148。顶层144可以是不透气层。顶层144可以具有通过顶层144形成的孔149以接纳流体地连接至减压源的减压。顶层144帮助在超吸收性层132之上提供密封。许多材料可以被用于顶层144,诸如聚乙烯膜,其将避免流体漏出。在可选择的实施方式中,顶层144可以被省略且盖布单独被用于限制流体在超吸 收性敷料组件130内。 
额外的接驳体透气层194可以被增加在顶层144的第一表面146上以起到过滤器的作用。接驳体透气层194覆盖孔149。接驳体透气层194允许减压的传送并防止部分超吸收性层132进入置于超吸收性敷料组件130的顶部上的减压接驳体,如图1的减压接驳体92。 
超吸收性层132具有第一表面134和面向内的第二表面136。超吸收性层132帮助输送减压至进入层138并继续到线形伤口112。超吸收性层132可操作来容纳相对大量的流体且可操作来帮助起用于产生压缩力(如,图1中的力24)的敷料垫枕的作用。 
超吸收性层32(图1)和超吸收性层132可以由通常称为“水凝胶”、“超吸收体”或“水解胶体”的类型的超吸收聚合物(SAP)形成。也可以使用超吸收性球体,其将输送减压直到超吸收性球体变得饱和。为了允许使用减压而没有远程罐或在人类患者中带有相对小的远程罐,带有许多手术应用使超吸收性层132可操作来容纳至少300毫升的流体是合乎需要的。在一些应用中,又可以是合乎需要的是,在超吸收性敷料132与减压源之间的减压传送导管(如,图1中的90)中提供超吸收性材料以进一步容纳流体。 
当流体被加入到超吸收性层132中时,敷料垫枕131变得更坚硬且在减压作用下,这导致增大的压缩力,诸如图1中的力24。流体可以以渗出液或来自线形伤口112的其他流体的形式出现,或者可以是通过注入或其他方式有意加入的补给流体,诸如盐水。 
现在参考图3,超吸收性敷料组件230可以与用于传送减压的减压接驳体292和注入端口233一起形成。注入端口233便于流体注入到超吸收性敷料组件230的超吸收性层中。 
现在参考图4,超吸收性敷料组件330可以具有第一接驳体392或减压接驳体,用于将减压传送至超吸收性层,并且还可以包括第二接驳体335或流体传送接驳体,用于将诸如盐水的流体传送至超吸收性层。 
无论是通过注入端口233(图3)或第二接驳体335(图4)或其他方 式,都可以将流体加入到超吸收性层以增强超吸收性层的硬度,并且这提供液体受控的垫枕。液体的加入控制硬度,硬度又可以控制在减压作用下产生的压缩力。如果流体由渗出(如,流血)伤口来供给,那么由额外的流体-渗出液产生的额外的压缩力帮助使敷料稍微自调节或自调整。这在战场上的伤口治疗是特别有用的。产生的压缩的量也可以受到盖布的弹性的影响;其越是可伸长,将会产生越小的压缩力。可以设置转换器和控制器,这便于压缩力的测量并且能够调节通过第二接驳体335供给(或通过第一接驳体392除去)的流体的量以在期望的水平或期望的参数内调整压缩。 
根据另一个例证性实施方式,制造用于与减压伤口治疗系统一起使用的敷料组件的方法包括步骤:形成用于贴着伤口放置的且具有第一表面和面向内的第二表面的透气的流体受限的干燥层;邻近透气的干燥层布置具有第一表面和面向内的第二表面的超吸收性层;邻近超吸收性层的第一表面布置具有第一表面和面向内的第二表面的不透气层。制造的方法还包括将盖布放置在不透气层的第一表面之上;以及将减压接驳体流体地连接至超吸收性层。 
虽然已经在某些例证性的、非限制性的实施方式的背景下公开本发明及其优点,但是应当理解,可以做出各种变化、替换、置换和改变而并没有偏离如由所附权利要求所限定的本发明的范围。将认识到,结合任一个实施方式描述的任何特征也可以被应用于任何其他实施方式。 

Claims (17)

1.一种用于与减压治疗系统一起使用的敷料组件,所述敷料组件包括:
透气的流体受限的干燥层,其用于贴着患者上的伤口放置且具有第一表面和面向内的第二表面;
超吸收性层,其具有第一表面和面向内的第二表面,所述超吸收性层的面向内的第二表面邻近所述透气的流体受限的干燥层的第一表面布置;
不透气层,其具有第一表面和面向内的第二表面,所述不透气层的面向内的第二表面邻近所述超吸收性层的第一表面布置;以及
注入端口,其流体地连接至所述超吸收性层且适于将流体加入到所述超吸收性层,其中所述超吸收性层适于随着所述流体的加入而变得更坚硬。
2.如权利要求1所述的敷料组件,还包括盖布和减压接驳体,所述盖布在所述不透气层的第一表面之上延伸,所述减压接驳体流体地连接至所述超吸收性层且与密封子系统一起可操作来接收减压。
3.如权利要求2所述的敷料组件,还包括盖布外延,所述盖布外延由延伸超出所述透气的流体受限的干燥层、所述超吸收性层和所述不透气层的外围边缘的所述盖布的一部分形成。
4.如权利要求2所述的敷料组件,还包括盖布外延,所述盖布外延由延伸超出所述透气的流体受限的干燥层、所述超吸收性层和所述不透气层的外围边缘的所述盖布的一部分形成;以及还包括密封装置,所述密封装置用于将所述盖布外延密封至患者的表皮。
5.如权利要求1所述的敷料组件,其中所述超吸收性层可操作来容纳至少300毫升的流体。
6.一种用于治疗伤口的减压治疗系统,所述系统包括:
超吸收性液体受控的垫枕,其用于放置在患者的表皮上且依尺寸制造以覆盖所述伤口,所述超吸收性液体受控的垫枕可操作来输送减压;
其中所述超吸收性液体受控的垫枕包括:
透气的流体受限的干燥层,其具有第一表面和面向内的第二表面;
超吸收性层,其具有第一表面和面向内的第二表面,所述超吸收性层的面向内的第二表面邻近所述透气的流体受限的干燥层的第一表面布置;和
不透气层,其具有第一表面和面向内的第二表面,所述不透气层的面向内的第二表面邻近所述超吸收性层的第一表面布置;密封子系统,其用于提供所述超吸收性液体受控的垫枕与患者之间的流体密封;
减压子系统,其用于将减压传送至所述密封子系统;
流体传送接驳体,其用于将流体传送至所述超吸收性层,其中所述超吸收性层适于随着所述流体的加入而变得更坚硬;
其中所述密封子系统和所述减压子系统可操作来将减压传送至所述伤口;以及
其中所述超吸收性液体受控的垫枕、所述密封子系统和所述减压子系统可操作来产生压缩力。
7.如权利要求6所述的系统,其中所述超吸收性液体受控的垫枕包括:超吸收性材料,所述超吸收性材料在减压下可操作来产生压缩力,在流体加入所述超吸收性材料时,该压缩力较大。
8.如权利要求6所述的系统,其中所述超吸收性层的面向内的第二表面至少部分地连接到所述透气的流体受限的干燥层的第一表面。
9.如权利要求6所述的系统,其中所述超吸收性层的面向内的第二表面至少部分地连接到所述透气的流体受限的干燥层的第一表面,并且所述不透气层上的面向内的第二表面和所述超吸收性层的第一表面至少部分地连接。
10.如权利要求6所述的系统,其中所述密封子系统包括:盖布,该盖布在所述超吸收性液体受控的垫枕之上延伸。
11.如权利要求6所述的系统,其中所述密封子系统包括:
盖布,其在所述超吸收性液体受控的垫枕之上延伸;以及
密封装置,其用于提供在患者的表皮与所述盖布之间的流体密封。
12.如权利要求6所述的系统,其中所述减压子系统包括:
减压源,其用于提供减压;
减压接驳体,其连接到所述密封子系统;以及
减压传送导管,其用于从所述减压源向所述减压接驳体提供减压。
13.如权利要求6所述的系统,其中所述减压子系统包括:
减压源,其可操作来提供在环境压力以下的可变压力;
减压接驳体,其连接到所述密封子系统;
减压传送导管,其用于从所述减压源向所述减压接驳体提供减压。
14.如权利要求6所述的系统,其中所述减压子系统包括:
便携式减压源;
减压接驳体,其连接到所述密封子系统;
减压传送导管,其用于从所述便携式减压源向所述减压接驳体提供减压;以及
其中所述减压接驳体将减压传输至所述密封子系统。
15.一种用于帮助患者上的伤口的愈合的系统,所述系统包括:
超吸收性垫枕组件,其用于放置在患者的伤口上,所述超吸收性垫枕组件包括:
透气的流体受限的干燥层,其具有第一表面和面向内的第二表面;
超吸收性层,其具有第一表面和面向内的第二表面,所述超吸收性层的面向内的第二表面邻近所述透气的流体受限的干燥层的第一表面布置;和
不透气层,其具有第一表面和面向内的第二表面,所述不透气层的面向内的第二表面邻近所述超吸收性层的第一表面布置;
密封子系统,其用于在所述超吸收性垫枕组件和患者之上提供流体密封;
减压子系统,其用于将减压传送至所述密封子系统;以及
其中所述超吸收性垫枕组件、所述密封子系统和所述减压子系统可操作来将减压传送至所述伤口,将压缩力施加至所述伤口并从所述伤口除去渗出液;
其中所述超吸收性垫枕组件可操作来容纳多于250毫升的流体,同时呈现干燥的面向内的表面;以及
其中所述压缩力响应于所述超吸收性垫枕组件吸收流体而增加。
16.如权利要求15所述的系统,还包括减压接驳体,所述减压接驳体连接在所述不透气层的第一表面上且与所述密封子系统一起可操作来接收减压。
17.如权利要求15所述的系统,还包括流体传送接驳体,所述流体传送接驳体用于将流体传送至所述超吸收性层,所述流体传送接驳体连接到所述不透气层的第一表面且流体地连接到所述超吸收性层。
