技术领域
本发明涉及包含与类美坦素(maytansinoid)化学偶联的抗体的缀合物和其使用方法。
背景技术
随着更有效靶向和杀死癌细胞的药物的开发,癌症的治疗已经取得显著的进展。为此,研究人员利用被癌细胞选择性表达的细胞表面受体和抗原来开发基于结合肿瘤-特异性或肿瘤-相关性抗原的抗体的药物。在这一点上,已经使细胞毒性分子、例如细菌与植物毒素、放射性核素和某些化疗药物在化学上与结合肿瘤-特异性或肿瘤-相关性细胞表面抗原的单克隆抗体连接(例如参见国际(PCT)专利申请No.WO00/02587、WO02/060955和WO02/092127,美国专利5,475,092、6,340,701、6,171,586,美国专利申请公报No.2003/0004210Al,和Ghetie等人,J.Immunol.Methods,112,267-277(1988))。这类化合物通常分别被称为毒素、放射性核素和药物“缀合物(conjugate)”。经常,它们也被称为免疫缀合物、放射性免疫缀合物和免疫毒素。肿瘤细胞的杀死发生在药物缀合物与肿瘤细胞的结合和类美坦素细胞毒性活性的活化之后。由药物缀合物所提供的选择性减少对正常细胞的毒性,由此增强药物在患者中的耐受性。

因而,鉴于上述,仍然需要含有高度细胞毒性药物的药物缀合物组合物,它们比目前可用的药物缀合物组合物更稳定。也仍然需要使用这类药物缀合物组合物治疗与细胞增殖有关的人类疾病、例如癌症的方法。

发明内容
本发明提供组合物,包含(i)治疗有效量的缀合物,包含与类美坦素化学偶联的抗体,(ii)缓冲剂,(iii)张力调节量(渗透压调节量,tonicifying amount)的氯化钠,(iv)水,和可选的(v)表面活性剂,其中该组合物具有约5-6的pH。本发明也提供冻干组合物,包含(i)治疗有效量的缀合物,包含与类美坦素化学偶联的抗体,(ii)缓冲剂,(iii)低温保护剂(冷冻保护剂,cryoprotectant),(iv)填充剂,和可选的(v)表面活性剂,其中当用水重构时该组合物具有约5-6的pH。本发明进一步提供杀死人体细胞的方法,包含对人体给予两种上述组合物之一,以便抗体与细胞表面结合,激活类美坦素的细胞毒性,由此杀死细胞。
因此,本发明具体地涉及:

本发明提供组合物,包含(i)治疗有效量的缀合物,包含与类美坦素化学偶联的抗体,(ii)缓冲剂,(iii)张力调节量的氯化钠,(iv)水,和可选的(v)表面活性剂,其中该组合物具有约5-6的pH。
本发明组合物含有缀合物,它包含与类美坦素化学偶联的抗体。本文所用的术语“抗体”表示任意免疫球蛋白,任意免疫球蛋白片段,例如Fab、F(ab′)2、dsFv、sFv、二体(diabodies)和三体(triabodies),或免疫球蛋白嵌合体,它们能够与细胞表面上的抗原结合(例如含有互补性决定区(CDR))。在本发明组合物中可以使用任意适合的抗体。本领域普通技术人员将领会到,适当抗体的选择将依赖于所要靶向的细胞群。在这一点上,在特定细胞群(通常和优选为患病细胞群)中被选择性表达的细胞表面分子(即抗原)的类型和数量将支配适用于本发明组合物的抗体的选择。多种细胞类型的细胞表面表达方式都是已知的,包括肿瘤细胞类型,或者如果未知,可以利用常规分子生物学和组织化学技术来测定。
抗体可以是单克隆的或多克隆的,但是最优选单克隆抗体。本文所用的“多克隆”抗体表示抗体的杂合群,通常包含在致免疫动物的血清中。“单克隆”抗体表示抗体分子的纯合群,它们是特定抗原特异性的。单克隆抗体通常是由B淋巴细胞(“B细胞”)的单一克隆体产生的。可以利用本领域技术人员已知的多种技术得到单克隆抗体,包括标准杂交瘤技术(例如参见Kohler and Milstein,Eur.J.ImmuRol.,5,511-519(1976),Harlow and Lane(Eds.),AntibodiEs:A Laboratory Manual,CSH Press(1988)和C.A.Janeway等人(eds.),Immunobiology,5th Ed.,Garland Publishing,New York,NY(2001))。简而言之,生产单克隆抗体的杂交瘤法通常牵涉向任意适合的动物、通常和优选为小鼠注射抗原(即“免疫原”)。随后处死动物,从其脾脏分离的B细胞与人骨髓瘤细胞融合。产生杂合细胞(即“杂交瘤”),它无限地增殖,连续分泌具有所需体外特异性的高效价抗体。可以利用本领域已知的任意适当方法来鉴定产生具有所需特异性的抗体的杂交瘤细胞。这类方法例如包括酶联免疫吸附测定(ELISA)、蛋白质印迹(Western blot)分析和放射性免疫测定。筛选杂交瘤群,以分离单独的克隆体,它们各自分泌单一的抗体品种针对抗原。因为每种杂交瘤是从与单一B细胞融合衍生的克隆体,它产生的所有抗体分子在结构上都是相同的,包括它们的抗原结合位点和同种型。也可以利用其他适合的技术生成单克隆抗体,包括EBV-杂交瘤技术(例如参见Haskard和Archer,J.Immunol.Methods,74(2),361-67(1984)和Roder等人,Methods Enzymol.,121,140-67(1986)),或者噬菌体载体表达系统(例如参见Huse等人,Science,246,1275-81(1989))。为了制备单克隆抗体片段,通常采用重组方法。
单克隆抗体可以是从任意适合的动物中分离的或者是在任意适合的动物中产生的,但是优选地是在哺乳动物中产生的,更优选小鼠,最优选人。在小鼠中产生抗体的方法是本领域技术人员熟知的,本文有所描述。关于人抗体,本领域普通技术人员将领会到,可以从用适当抗原接种疫苗或者致免疫的人类受试者血清中分离多克隆抗体。或者可以通过采用在非人类动物、例如小鼠中产生人抗体的已知技术,生成人抗体(例如参见美国专利5,545,806、5,569,825和5,714,352,和美国专利申请公报No.2002/0197266Al)。
尽管是人类治疗性应用的理想选择,人抗体、特别是人单克隆抗体通常比小鼠单克隆抗体更加难以生成。不过,当对人类给药时小鼠单克隆抗体诱导迅速的宿主抗体应答,这会降低抗体-药物缀合物的治疗或诊断潜力。为了回避这些复杂情况,单克隆抗体优选地不被人免疫系统识别为“外来物”。为此,可以利用噬菌体展示生成抗体。在这一点上,可以利用标准分子生物学和重组DNA技术生成编码抗体的抗原-结合性可变(V)结构域的噬菌体文库(例如参见Sambrook等人(eds.),Molecular Cloning,A Laboratory Manual,3d Edition,Cold Spring Harbor Laboratory Press,New York(2001))。选择编码具有所需特异性的可变区域的噬菌体与所需抗原特异性结合,重建包含所选择的可变结构域的完整人抗体。向适合的细胞系引入编码所重建的抗体的核酸序列,例如用于杂交瘤产生的骨髓瘤细胞,以便由该细胞分泌具有单克隆抗体特征的人抗体(例如参见J aneway等人,见上,Huse等人,见上和美国专利6,265,150)。或者,可以从小鼠生成单克隆抗体,所述小鼠是特异性人重链与轻链免疫球蛋白基因转基因的。这类方法是本领域已知的,例如描述在美国专利5,545,806和5,569,825,和Janeway等人,见上。最优选地,抗体是人源化抗体。本文所用的“人源化”抗体是这样的,其中构成抗体的抗原结合环的、小鼠单克隆抗体的互补性决定区(CDR)被移植到人抗体分子的构架上。由于小鼠与人抗体框架的相似性,一般为本领域所接受的是这种方法产生这样一种单克隆抗体,它在抗原性上等同于人抗体,但是与从中衍生出CDR序列的小鼠单克隆抗体结合相同的抗原。生成人源化抗体的方法是本领域熟知的,例如详细描述在Janeway等人,见上,美国专利5,225,539,5,585,089和5,693,761,欧洲专利No.0239400B1和英国专利No.2188638中。也可以利用抗体表面重整技术生成人源化抗体,如美国专利5,639,641和Pedersen等人,J.Mol.Biol.,235,959-973(1994)所述。尽管用在本发明组合物的缀合物中的抗体最优选地是人源化单克隆抗体,不过如上所述的人单克隆抗体或小鼠单克隆抗体也属于本发明的范围。
具有至少一个抗原结合位点、从而识别和结合至少一种存在于靶细胞表面上的抗原或受体的抗体片段也属于本发明的范围。在这方面,完整抗体分子的蛋白水解性裂解能够产生多种保留识别和结合抗原的能力的抗体片段。例如,用木瓜蛋白酶限制性消化抗体分子通常产生三个片段,其中两个是等同的,被称为Fab片段,因为它们保留母体抗体分子的抗原结合活性。用胃蛋白酶裂解抗体分子在正常情况下产生两个抗体片段,其中一个保留抗体分子的两支抗原结合臂,因而被称为F(ab’)2片段。利用常规重组DNA技术可以生成单链可变区片段(sFv)的抗体片段,它由截短的Fab片段组成,Fab片段包含经由合成肽与抗体轻链可变(V)结构域连接的抗体重链V结构域(例如参见Janeway等人,见上)。类似地,借助重组DNA技术可以制备二硫化物-稳定化的可变区片段(dsFv)(例如参见Reiter等人,ProteinEngineering,7,697-704(1994))。不过,本发明的抗体片段不限于这些示范类型的抗体片段。可以采用任意适合的识别和结合所需细胞表面受体或抗原的抗体片段。利用任意适合的本领域已知的方法可以测定抗体-抗原结合,例如放射性免疫测定(RIA)、ELISA、蛋白质印迹、免疫沉淀和竞争性抑制测定(例如参见Janeway等人,见上和美国专利申请公报No.2002/0197266Al)。
另外,抗体可以是嵌合抗体。“嵌合”意味着抗体包含从至少两种不同品种获得或衍生的至少两种免疫球蛋白或其片段(例如两种不同的免疫球蛋白,人免疫球蛋白恒定区与鼠免疫球蛋白可变区结合)。

按照本发明,上述抗体与任意适合的细胞毒性剂化学偶联,特别是诱导肿瘤细胞的细胞毒性的细胞毒性剂,生成如上所述的缀合物。作为正常药理廓清(清除)机理的结果,用在药物缀合物中的抗体仅以有限的量接触和结合靶细胞。因此,用在缀合物中的细胞毒性剂必须是高度细胞毒性的,以便充分地杀死细胞,引发治疗效果。这类细胞毒性剂的实例包括新颖的紫杉烷(例如参见国际(PCT)专利申请No.W001/38318和PCT/US03/02675)、DNA-烷基化剂(例如CC-1065类似物)、蒽环素(anthracyclines)、tubulysin类似物、duocarmycin类似物、auristatinE和包含反应性聚乙二醇部分的细胞毒性剂(例如参见Sasse等人,J.Antibiot.(Tokyo),53,879-85(2000),Suzawa等人,Bioorg.Med.Chem.,8,2175-84(2000),Ichimura等人,J.Antibiot.(Tokyo),44,1045-53(1991),Francisco等人,Blood(2003)(先于印刷出版物的电子出版物),美国专利5,475,092,6,340,701,6,372,738和6,436,931,美国专利申请公报No.2001/0036923A1,未决美国专利申请No.10/024,290和10/116,053和国际(PCT)专利申请No.WO 01/49698)。或者和最优选地,抗体与类美坦素化学偶联,生成本发明组合物的缀合物。
类美坦素最初是从东部非洲Maytenus属灌木分离的,但是随后也在土壤细菌的代谢产物中被发现,例如珍贵束丝放线菌(Actinosynnema pretiosum)(例如参见美国专利3,896,111)。类美坦素通过抑制有丝分裂诱导细胞毒性。实验证据显示,类美坦素通过抑制微管蛋白聚合、由此防止微管生成来抑制有丝分裂(例如参见美国专利6,441,163和Remillard等人,Science,189,1002-1005(1975))。体外利用细胞培养模型和体内利用实验室动物系统已经显示类美坦素抑制肿瘤细胞生长。而且,类美坦素的细胞毒性比常规化疗剂大1,000倍,例如甲氨蝶呤、柔红霉素和长春新碱(例如参见美国专利5,208,020)。本领域已知类美坦素包括美坦素、美坦醇、美坦醇的C-3酯和其他美坦醇类似物与衍生物(例如参见美国专利5,208,020和6,441,163)。美坦醇的C-3酯可以是天然存在的或合成衍生的。而且,天然存在的和合成的C-3美坦醇酯可以分为简单羧酸的C-3酯或者N-甲基-L-丙氨酸衍生物的C-3酯,后者比前者有更多细胞毒性。合成的类美坦素类似物也是本领域已知的,例如描述在Kupchan等人,J.Med.Chem.,21,31-37(1978)中。生成美坦醇和其类似物与衍生物的方法例如描述在美国专利4,151,042中。
适合用在本发明组合物中的类美坦素可以是利用本领域已知的方法从天然来源分离的、合成生产的或半合成生产的。而且,可以按任意适合的方式修饰类美坦素,只要在最终的缀合物分子中保存充分的细胞毒性。在这一点上,类美坦素缺乏适合与抗体连接的官能团。需要利用连接部分以连接类美坦素与抗体,生成缀合物。连接部分含有化学键,允许类美坦素细胞毒性在特定部位的活化。适合的化学键是本领域熟知的,包括二硫键、酸不稳定键、光不稳定键、肽酶不稳定键、在巯基与马来酰亚胺基团之间所生成的硫醚键和酯酶不稳定键。最优选地,连接部分包含二硫键或硫醚键。按照本发明,连接部分优选地包含反应性化学基团。特别优选的反应性化学基团是N-琥珀酰亚氨基酯和N-磺基琥珀酰亚氨基酯。在优选的实施方式中,反应性化学基团可以经由硫醇基团之间的二硫键与类美坦素共价结合。因而,如本文所述经过修饰的类美坦素优选地包含硫醇基团。本领域普通技术人员将领会到,硫醇基团含有与氢原子键合的硫原予,通常在本领域中也被称为巯基,可以表示为“-SH”或“RSH”。

具有其他化学键的连接部分也能用在本发明的背景下,正如其他类美坦素也是如此。其他化学键的具体实例包括酸不稳定键、硫醚键、光不稳定键、肽酶不稳定键和酯酶不稳定键。生产具有连接部分的类美坦素的方法例如描述在美国专利5,208,020、5,416,064和6,333,410中。
类美坦素的连接部分通常和优选地是用于连接抗体与类美坦素的更大连接剂分子的一部分。任意适合的连接剂分子可以用于本发明,只要连接剂分子分别提供细胞毒性的保持和类美坦素与抗体的靶向特征。连接剂分子通过化学键(如上所述)连接类美坦素与抗体,以便类美坦素和抗体彼此化学偶联(例如共价键合)。合乎需要的是,连接剂分子通过二硫键或硫醚键化学偶联类美坦素与抗体。最优选地,抗体经由二硫键与类美坦素化学偶联。

本发明组合物包含治疗有效量的缀合物,所述缀合物包含与类美坦素化学偶联的抗体。“治疗有效量”表示足以在个体中显示有意义的益处的量,例如促进至少一个方面的肿瘤细胞细胞毒性,或者治疗、治愈、预防或改善其他与特定癌症有关的相关医学状态。治疗有效量可以因在个体中所需的生物学效果、所要治疗的病症和/或缀合物的具体特征和个体而异。因而,按照本文所述的方法,主治医师(或其他负责给予组合物的医药专业人员)将通常决定用以治疗每一个别患者的量。合乎需要的是,缀合物在本发明组合物中的浓度是约0.1mg/mL至约5mg/mL(例如约0.5mg/mL、约2mg/mL或约5mg/mL)。在优选的实施方式中,缀合物在本发明组合物中的浓度是约1mg/mL或更高(例如约2mg/mL或更高、约3mg/mL或更高或者约4mg/mL或更高)。最优选地,缀合物在本发明组合物中的浓度是约5mg/mL。尽管包含至少1mg/mL缀合物的组合物是特别优选的,不过浓度小于1mg/mL(例如浓度为约0.1mg/mL至约0.9mg/mL)的缀合物也能在本发明组合物中保持稳定,因而属于本发明的范围。包含大于1mg/mL缀合物分子的组合物有利于临床和商业应用之处在于这类浓度能够在更适宜(即更小)的给药体积中制备单一剂量的组合物。
合乎需要的是,将本发明组合物配制成药学应用可接受的制剂,例如对需要的人类宿主给药的制剂。为此,优选地将缀合物分子配制成包含生理学上可接受的载体(例如赋形剂或稀释剂)的组合物。生理学上可接受的载体是熟知的和容易获得的,包括缓冲剂、抗氧化剂、制菌剂、盐和赋予制剂与人类患者血液或其他体液等渗的溶质,和水性与非水性无菌悬液,其中可以包括悬浮剂、增溶剂、增稠剂、稳定剂(例如表面活性剂)和防腐剂。载体的选择至少在部分程度上将取决于靶组织和/或细胞的位置,和用于给予组合物的特定方法。适合用在药物缀合物制剂中的载体和赋形剂的实例例如公开在国际(PCT)专利申请No.W000/02587、W002/060955和W002/092127和Ghetie等人,J.Immunol.Methods,112,267-277(1988)中。最优选地,本发明组合物包含缓冲剂、表面活性剂、张力调节量的氯化钠和水。
任意适合的药学上可接受的缓冲剂都可以用于本发明组合物。特别优选的缓冲剂的实例包括柠檬酸盐、乙酸盐、琥珀酸盐、磷酸盐和组氨酸。不过,本发明组合物不限于这些示范性缓冲剂。缓冲剂可以以任意适合的浓度存在于本发明组合物中,只要组合物在所需条件下达到充分的稳定性。在这一点上,缓冲剂在组合物中的浓度优选地是约2mM至约50mM(例如约2-10mM、约10-20mM、约20-30mM、约30-40mM或约40-50mM)。最优选地,缓冲剂在组合物中的浓度是约5-15mM(例如约10mM)。合乎需要的缓冲剂是琥珀酸钠或乙酸钠,但是最优选地是柠檬酸钠。缓冲剂在本发明组合物中的含量通常使pH维持在所需的范围内。在这方面,本发明组合物优选地具有约5-6的pH(例如约5、5.5或6)。据信pH较高(例如约pH6或更高)的组合物不如pH较低(也就是约pH6或以下)的组合物稳定。因而,本发明组合物最优选地具有约5.5的pH。
除了如上讨论的缓冲剂以外,本发明组合物还可选地含有表面活性剂。任意适合的表面活性剂都可以用于本发明。适合的表面活性剂是本领域技术人员熟知的。按照本发明组合物,合乎需要的表面活性剂是聚山梨醇酯,优选地是聚山梨醇酯20或聚山梨醇酯80。最优选地,表面活性剂是聚山梨醇酯20。表面活性剂可以以任意适合的浓度存在于本发明组合物中,只要组合物在所需条件下达到充分的稳定性。在这一点上,表面活性剂在组合物中的浓度优选地占组合物总体积的约0.002%至约0.1%wt./vol.(例如约0.002-0.01%、约0.005-0.02%或约0.01-0.1%wt./vol.)。最优选地,表面活性剂在组合物中的浓度占组合物总体积的约0.005-0.02%wt./vol.(例如约0.01%wt./vol.)。尽管用表面活性剂配制的组合物是优选的,不过不用表面活性剂配制的组合物也属于本发明的范围。
作为另外的稳定剂,还向本发明组合物加入氯化钠。在这一点上,本发明组合物包含适量、优选张力调节量的氯化钠(NaCl)。措辞“张力调节量的氯化钠”意味着NaCl在组合物中的浓度是这样的,组合物的张性与人血液的张性是相同的(即等渗的)。在这一点上,NaCl可以以任意适合的浓度存在于本发明组合物中,只要在本发明组合物中达到充分的张性和稳定性。合乎需要的是,氯化钠在组合物中的浓度是约50mM至约500mM(例如约50-100mM、约100-150mM、约150-250mM、约250-350mM或约350-450mM)。尽管较高浓度的氯化钠(例如约150mM或以上)可能赋予本发明组合物高渗性而非等渗性,不过在对人给药之前用任意适合的等渗溶剂、例如优选5%葡萄糖的水溶液(″D5W″)或生理盐水(″NS″)稀释这类组合物将赋予这类组合物仅仅轻微的高渗性,适合用在本发明中。优选地,氯化钠在组合物中的浓度是约100mM至约200mM(例如约100-140mM、约130-170mM或约160-200mM)。最优选地,氯化钠在组合物中的浓度是约110-150mM(例如约110mM-130mM或约120mM)。
在特别优选的发明实施方式中,组合物包含(i)约5mg/mL缀合物,该缀合物包含与DM1化学偶联的huN901,(ii)约10mM柠檬酸钠缓冲剂,(iii)约0.01%聚山梨醇酯20,(iv)约120mM氯化钠,和(v)水(优选注射用水(WFI)),其中组合物的pH是约5.5。在另一种优选的实施方式中,组合物包含(i)约1mg/mL或以上(例如约1mg/mL、约2mg/mL、3mg/mL、约5mg/mL或者它们之间的范围)缀合物,包含与DM1化学偶联的huMy9-6,(ii)约10mM柠檬酸钠缓冲剂,(iii)可选的约0.01%聚山梨醇酯20,(iv)约135mM氯化钠,和(v)水,其中组合物的pH是约5.5。在另一种优选的实施方式中,组合物包含(i)约1mg/mL或以上(例如约1mg/mL、约2mg/mL、3mg/mL、约5mg/mL或者它们之间的范围)缀合物,包含与DM4化学偶联的huMy9-6,(ii)约10mM柠檬酸钠缓冲剂,(iii)可选的约0.01%聚山梨醇酯20,(iv)约135mM氯化钠,和(v)水,其中组合物的pH是约5.5。在另外优选的实施方式中,组合物包含(i)约1mg/mL或以上(例如约1mg/mL、约2mg/mL、3mg/mL、约5mg/mL或者之间的范围)缀合物,包含经由SMCC连接剂与DM1化学偶联的huN901,(ii)约10mM柠檬酸钠缓冲剂,(iii)可选的约0.01%聚山梨醇酯20,(iv)约130mM氯化钠,和(v)水,其中组合物的pH是约5.5。
含有抗体(或一般而言蛋白质)的组合物因氧化作用而不稳定。因而,在另一种本发明实施方式中,组合物进一步包含抗氧化剂。任意适合的抗氧化剂都可以用在本发明组合物中。适合的抗氧化剂是本领域已知的,例如包括超氧化物歧化酶、谷胱甘肽过氧化物酶、生育三烯酚、多元酚、锌、锰、硒、维生素C、维生素E、β胡萝卜素、半胱氨酸和甲硫氨酸。用于本发明组合物的抗氧化剂最优选地是甲硫氨酸。抗氧化剂可以以任意适合的浓度存在于组合物中。合乎需要的是,抗氧化剂在组合物中的浓度是约100μM至约100mM(例如约0.25-1mM、约0.5-2mM、约5-15mM、约20-70mM或约60-90mM)。最优选地,抗氧化剂在组合物中的浓度是约5-15mM(例如约10mM)。
除了抗氧化剂以外,还可以借助蔗糖的加入进一步使本发明组合物稳定。蔗糖使抗体制剂稳定的应用是本领域技术人员已知的。任意适量蔗糖都可以用在本发明组合物中,但是合乎需要的是蔗糖在组合物中的浓度占组合物总体积的约0.1%至约10%wt./vol.(例如约0.1-1%、约2-5%或约7-10%wt./vol.)。最优选地,蔗糖在组合物中的浓度占组合物总体积的约4-6%wt./vol.(例如约5%wt./vol.)。
本发明进一步提供带包装的组合物,包含密封的容器,其中分散有本发明组合物,和惰性气体覆盖层。带包装的组合物可以用任意适合的惰性气体覆盖,只要本发明组合物在带包装的组合物内保持稳定。惰性气体优选地是氮或氩。带包装的组合物可以呈现为在单剂或多剂密封容器中,例如安瓿或小瓶。
除了本文所述的含水组合物(本文也称为“液体”或“水性”(含水)组合物)以外,本发明还提供冻干组合物,包含(i)治疗有效量的缀合物,该缀合物包含与类美坦素化学偶联的抗体,(ii)缓冲剂,(iii)表面活性剂,(iv)低温保护剂,和(v)填充剂,其中当用水重构时该组合物具有约5-6的pH。“冻干”意味着组合物已在真空下冷冻干燥。冻干通常是这样完成的,冷冻特定的制剂,以致溶质从溶剂中分离出来。然后升华除去溶剂(即初级干燥),其次解吸(即次级干燥)。上述与其他本发明实施方式有关的缀合物(也就是与类美坦素化学偶联的抗体)、缓冲剂、表面活性剂及其组分的说明也适用于上述冻干组合物的相同方面。在冻干组合物的重构之前,构成本发明冻干组合物的每种组分的相对量可以用每mg缀合物所用的赋形剂(例如缓冲剂、表面活性剂、填充剂、低温保护剂)的mg数来描述。
尽管本文所述任意适合的缓冲剂都可以用于本发明冻干组合物,不过本发明冻干组合物优选地包含琥珀酸钠缓冲剂。缓冲剂可以以任意适合的量存在于本发明冻干组合物中。确切而言,合乎需要的是,冻干组合物包含约0.1mg至约2mg缓冲剂每mg缀合物(例如约0.1mg至约0.5mg缓冲剂每mg缀合物、约0.5mg至约1mg缓冲剂每mg缀合物或约1mg至约2mg缓冲剂每mg缀合物)。最优选地,冻干组合物包含约0.3mg琥珀酸钠缓冲剂每mg缀合物。
尽管本文所述任意适合的表面活性剂都可以用于本发明冻干组合物,不过合乎需要的表面活性剂是聚山梨醇酯,优选地是聚山梨醇酯20或聚山梨醇酯80。最优选地,表面活性剂是聚山梨醇酯20。表面活性剂可以以任意适合的量存在于本发明冻干组合物中,只要冻干组合物在所需条件下达到充分的稳定性。在这一点上,合乎需要的冻干组合物包含约0.005mg至约0.1mg表面活性剂每mg缀合物(例如约0.005mg至约0.01mg表面活性剂每mg缀合物、约0.01mg至约0.05mg表面活性剂每mg缀合物或约0.05mg至约0.1mg表面活性剂每mg缀合物)。当表面活性剂是聚山梨醇酯20时,冻干组合物优选地包含约0.02mg聚山梨醇酯20每mg缀合物。
为了防止组合物活性成分在冷冻和干燥期间降解,本发明冻干组合物进一步包含低温保护剂,优选无定形低温保护剂。本文所用的术语“低温保护剂”表示在冷冻期间保护不稳定分子的赋形剂。适合用在本发明组合物中的低温保护剂是本领域技术人员已知的,例如包括甘油、二甲基亚砜(DMS0)、聚乙二醇(PEG)、葡聚糖、葡萄糖、海藻糖和蔗糖。最优选地,低温保护剂是蔗糖。低温保护剂可以以任意适合的量存在于本发明冻干组合物中。合乎需要的是,冻干组合物包含约0.5mg至约5mg(例如约0.5mg至约2mg)低温保护剂每mg缀合物(例如约0.8mg低温保护剂每mg缀合物、约2mg低温保护剂每mg缀合物或约4mg低温保护剂每mg缀合物)。当低温保护剂是蔗糖时,冻干组合物优选地包含约0.5mg至约2mg(例如约1mg)蔗糖每mg缀合物。
本发明冻干组合物可以进一步含有填充剂,优选可结晶的填充剂。填充剂通常在本领域中用于为作为冻干结果所形成的“饼”提供结构和重量。本领域已知的任意适合的填充剂都可以用于本发明冻干组合物。适合的填充剂例如包括甘露糖醇、葡聚糖和甘氨酸。用在本发明组合物中的填充剂最优选地是甘氨酸。冻干组合物可以含有任意适量的填充剂,但是优选地冻干组合物包含约2mg至约20mg填充剂每mg缀合物(例如约2mg至约10mg填充剂每mg缀合物、约5mg至约10mg填充剂每mg缀合物、约10mg至约15mg填充剂每mg缀合物或约15mg至约20mg填充剂每mg缀合物)。当填充剂是甘氨酸时,冻干组合物优选地包含约3.8mg甘氨酸每mg缀合物。
因而,按照本发明,所要重构成含有5mg/mL缀合物(例如优选包含与DM1化学偶联的huN901的缀合物)的冻干组合物成分优选地包含(i)约0.3mg琥珀酸钠缓冲剂每mg缀合物,(ii)约0.02mg聚山梨醇酯20每mg缀合物,(iii)约1mg蔗糖每mg缀合物,和(iv)约3.8mg甘氨酸每mg缀合物。一旦用水重构,这样一种冻干组合物优选地具有约5.5的pH。而且,当将冻干组合物用水重构时,上述与本发明液体组合物有关的缀合物、缓冲剂和表面活性剂的相对浓度的说明也适用于上述冻干组合物。
冻干方法是本领域熟知的,例如描述在Wang,W.,Int.J.Pharm.,203,1-60(2000)中。例如,可以利用包含下列步骤的冻干循环生产本发明冻干组合物:(1)在4℃贮藏温度和环境室压力下预冷却2.5小时,(2)在-50℃贮藏温度和环境室压力下冷冻14小时,(3)在-20℃贮藏温度和环境室压力下甘氨酸重结晶6小时,(4)在-50℃贮藏温度和环境室压力下再冷冻16小时,(5)在-13℃贮藏温度和100mTorr压力下初级干燥24小时,(6)在24℃贮藏温度和100mTorr压力下次级干燥10小时,和(7)在24℃贮藏温度和环境室压力下的终止期。不过,本发明冻干组合物不限于由上述方法所生产的组合物。事实上,任意适合的冻干方法都可以用于生产本发明冻干组合物,对本领域技术人员将显而易见的是所选择的冻干参数(例如干燥时间)将因多种因素而异,包括所要冻干的溶液的体积。
除了本文所述优选的实施方式以外,本发明组合物(无论液体还是冻干形式)可以包含另外的治疗剂或生物活性剂。例如,可以含有可用于治疗特定适应症(例如癌症)的治疗因子。控制炎症的因子、例如布洛芬或类固醇可以作为组合物的一部分以减少与组合物体内给药有关的肿胀和炎症以及生理痛苦。在组合物中可以包括免疫增强剂,以增量调节机体对疾病的天然防御。可以与组合物共同给予维生素与矿物质、抗氧化剂和微量营养素。可以含有抗生素,即杀微生物剂和杀真菌剂,以减少涉及与组合物给药有关的程序的感染和其他障碍的危险。
本发明进一步提供杀死人体细胞的方法,包含对人体给予一种组合物,该组合物包含(i)治疗有效量的缀合物,该缀合物包含与类美坦素化学偶联的抗体,(ii)缓冲剂,(iii)表面活性剂,(iv)张力调节量的氯化钠,和(v)水,其中该组合物具有约5-6的pH,以便抗体与细胞表面结合,激活类美坦素的细胞毒性,由此杀死细胞。上述与其他本发明实施方式有关的缀合物(即与类美坦素化学偶联的抗体)、赋形剂(例如缓冲剂、表面活性剂、氯化钠等)及其组分的说明也适用于上述本发明方法的相同方面。
本发明方法牵涉时人体给予本发明组合物。理想地,本发明方法用于靶向和杀死受疾病影响的细胞,特别是与细胞增殖水平升高有关的疾病,例如癌症。因而在这一点上,本发明方法优选地用于杀死人体肿瘤细胞,由此导致癌症的预防、改善和/或治愈。
尽管任意适合的对人体给予组合物的手段都可以用在本发明的背景中,不过通常和优选地经由注射对人体给予本发明组合物,最优选经由输注。术语“注射”意味着向人体靶组织强行引入组合物。术语“输注”意味着向人体组织、通常和优选为静脉引入组合物。可以借助任意适合的途径对人体给予组合物,但是优选地对人体静脉内或腹膜内给药。不过当本发明方法用于杀死肿瘤细胞时,本发明组合物的肿瘤内给药是特别优选的。当通过注射给予本发明组合物时,可以利用任意适合的注射装置向肿瘤直接给予组合物。例如,可以利用普通医用注射器直接注射组合物至皮下肿瘤中。或者,使肿瘤浸渍在本发明液体组合物中,可以将组合物局部应用至肿瘤。同样,利用任意适合的递送装置,例如导管,可以用本发明组合物灌注肿瘤历经一段时间。尽管不太优选,其他给药途径也可以用于向人体递送组合物。事实上,尽管一种以上途径可以用于给予本发明组合物,不过一种特定途径可以比另一种途径提供更及时的和更有效的反应。例如,尽管不是特别优选的,可以将本发明组合物应用或滴注到体腔内、吸收进入皮肤、吸入或者例如经由肌内或动脉内给药而肠胃外给予。优选地,对人体肠胃外给予的本发明组合物明确靶向特定的细胞,例如癌细胞。
如本文所述,缀合物包含抗体,它优选地是人源化单克隆抗体,例如huN901、huMy9-6、huB4或huC242。其他适合的抗体例如包括trastuzumab、bivatuzumab、sibrotuzumab和rituximab。当在本发明方法中采用包含这类缀合物的组合物时,抗体通过与在细胞(例如肿瘤细胞)表面上表达的抗原(例如肿瘤-特异性抗原)的相互作用使缀合物靶向所需细胞(例如肿瘤细胞)。现有技术已经对多种肿瘤广泛描述过肿瘤-特异性抗原,例如包括上皮癌(例如MUC1)和乳腺与卵巢癌(例如HER2/neu)(例如参见Bartnes,Tidsskr.Nor.Laegeforen.,121,2941-5(2001)和Von Mensdorff-Pouilly等人,Int.J.Biol.Markers,15,343-356(2000))。
在优选的发明实施方式中,抗体(例如huMy9-6)与CD33抗原结合,该抗原例如被急性骨髓性白血病细胞表达。在另一种优选的实施方式中,抗体(例如huB4)与CD19抗原结合,该抗原例如被人B-细胞淋巴瘤细胞表达。或者,抗体(例如huC242)与CanAg抗原结合,该抗原被一些癌细胞类型表达,例如包括结肠直肠、胰腺、胃和其他胃肠癌,和大多数非小细胞肺癌。最优选地,抗体(例如huN901)与NCAM/CD56抗原结合,该抗原例如被小细胞肺癌(SCLC)细胞和其他神经内分泌来源的癌症所表达。其他优选的能够与抗体结合的抗原包括GD3抗原、PSMA、α-叶酸受体、Her2/neu、CD44v6、胎儿腺泡胰腺(FAP)抗原、Cripto-1抗原、CA6抗原、CD20、CA55.1、MN/CA IX和硫酸软骨素蛋白聚糖(例如参见Chang等人,Cancer Res.,59,3192-98(1999),Miotti等人,Int.J.CanCer,39,297-303,(1987),Colomer等人,见上,Heider等人,见上,Welt等人,见上,LePage等人,American Assn.For Cancer Research(AACR),2003AnuualMEEting,Poster Abstact No.749,Kearse等人,Int.J.Cancer,88,866-72(2000),MalonEy等人,见上,Opavsky等人,Genomics,33,480-87(1996),Behm等人,Blood,87,1134-39(1996)和美国专利5,665,357)。在缀合物经由本文所述任意肿瘤特异性抗原或受体与靶(即肿瘤)结合后,激活类美坦素的细胞毒性。可以激活类美坦素细胞毒性的机理的实例包括经由抗体与类美坦素之间二硫键的裂解向细胞内部释放游离类美坦素、细胞内的抗体降解和类美坦素细胞毒性在细胞表面的活化。不过,本发明方法不限于这些示范性的类美坦素活化模式。事实上,任意激活类美坦素细胞毒性的机理都属于本发明方法的范围。
出于人体给药的目的,本文所述本发明液体组合物可以直接对人体给药(例如输注),或者在给药前不久用适合的稀释剂稀释。适合的稀释剂是本领域已知的,包括D5W和生理盐水(NS)。与适合的稀释剂的稀释比例有可能是1∶1、1∶2、1∶3或以上(例如1∶5、1∶10或者甚至1∶50)。合乎需要的是,本发明组合物的稀释不降低缀合物分子在组合物中的浓度低于约0.1mg/mL。在稀释本发明液体组合物后,组合物每一组分(例如缓冲剂、表面活性剂和氯化钠)的前述浓度相应地降低了。
当本文所述本发明冻干组合物对人体给药时,必须首先使组合物重构,在使用前不久加入无菌液体赋形剂,例如注射用水、DSW或NS。因而,本发明进一步提供杀死人体细胞的方法,包含(a)提供如本文所述的冻干组合物,(b)向冻干组合物加入水,提供重构组合物,和(c)对人体给予重构组合物,以便抗体与细胞表面结合,类美坦素被细胞内在化(internalized),由此杀死细胞。上述与其他本发明实施方式有关的冻干组合物、给药途径、肿瘤特异性抗原和其组分也适用于上述本发明方法的相同方面。而且,正如本文所讨论的,在用水重构生本发明冻干组合物之后,上述与本发明液体组合物有关的缀合物和赋形剂(例如缓冲剂、表面活性剂、低温保护剂和填充剂)的相对浓度的说明也适用于上述本发明方法的相同方面。
正如本文所讨论的,本发明方法无论采用液体组合物还是冻干组合物,都优选地用于治疗癌症。本发明方法可以用于治疗任意类型的癌症,例如包括肺、乳腺、结肠、前列腺、肾、胰腺、卵巢、血液和淋巴器官的癌症。尽管不太优选,本发明组合物还可以用于治疗其他与细胞增殖有关的疾病,包括自体免疫疾病(例如系统性狼疮、类风湿性关节炎和多发性硬化)、移植排斥(例如肾移植排斥、肝移植排斥、肺移植排斥、心脏移植排斥和骨髓移植排斥)、移植物对宿主的疾病、病毒感染(例如CMV感染、HIV感染、AIDS等)和寄生虫感染(例如贾第虫病、阿米巴病、血吸虫病)等等。

实施例1
本实施例证明组合物的生产,该组合物包含缀合物、缓冲剂、表面活性剂、张力调节量的氯化钠和水,该缀合物包含与类美坦素化学偶联的抗体。

为了证实上述制剂关于也是不稳定性指标的二聚物生成的稳定性,将huN901:DM1缀合物浓缩,配制在磷酸盐缓冲盐水(PBS),pH6.5(制剂1A-1C)或者10mM柠檬酸钠、0.01%聚山梨醇酯20、60mM NaCl,pH5.5(制剂1D)中。在4℃或25C下6个月后,用SEC-HPLC测定样品。这种分析的结果如表1所述。

实施例2
本实施例证明组合物的生产,该组合物包含缀合物、缓冲剂、表面活性剂或蔗糖、张力调节量的氯化钠和水,该缀合物包含与类美坦素化学偶联的抗体。

实施例3

利用本文所述和本领域已知的方法(例如参见美国专利6,441,163)制备包含与类美坦素DM1经由N-琥珀酰亚氨基4-(N-马来酰亚氨基甲基)环己烷-1-羧酸酯(SMCC)连接剂化学偶联的huN901单克隆抗体(“huN901-SMCC-DM1”)的缀合物。将1mg/mL huN901-SMCC-DM1缀合物配制在(a)磷酸盐缓冲盐水(PBS),pH6.5(制剂3A),(b)10mM柠檬酸钠、0.01%聚山梨醇酯20、130mM NaCl,pH5.5(制剂3B),或者(c)10mM柠檬酸钠、0.01%聚山梨醇酯20、130mM NaCl、10mM甲硫氨酸,pH5.5(制剂3C)中。在25℃和37℃下培育3.5个月后,借助色谱测定法测试制剂中缀合物二聚物的存在。这些分析的结果如表3所述。

实施例6
本实施例证明包含缀合物的冻干组合物的生产,该缀合物包含与类美坦素DM1化学偶联的单克隆抗体huN901。
包含与类美坦素DM1经由二硫键化学偶联的huN901单克隆抗体(“huN901:DM1”)的缀合物的生成以前已有描述(例如参见美国专利6,441,163)。制备四种制剂,称为制剂6A-6D。每一制剂含有(a)1mg/mL huN901:DM1,(b)10mM柠檬酸钠或10mM琥珀酸钠,(c)0.5%wt./vol.蔗糖,(d)250mM甘氨酸,和(e)水,还有或没有0.01%wt./vol.聚山梨醇酯20,pH5.5,如表7所述。

冻干后,每一制剂6A-6D样品呈现固体、均匀白色的饼,当用蒸馏水重构时所有制剂样品迅速再悬浮(也就是不到20秒完全溶解)。观察重构样品的外观,借助HPLC体积排阻色谱(SEC-HPLC)分析高分子量种类。微粒和/或高分子量种类的存在说明不稳定,因此被视为不可取的,而澄清溶液说明是稳定的制剂。这种分析的结果如表9所述。
基于这些结果,本发明的冻干组合物(即制剂6D)是唯一有效防止在冻干后生成微粒和高分子量品种的组合物。本发明的冻干组合物在所测试的制剂中是最稳定的。
实施例7
本实施例证明包含缀合物的冻干组合物的稳定性,该缀合物包含与类美坦素DM1化学偶联的单克隆抗体huN901。
如本文所述制备包含与类美坦素DM1经由N-琥珀酰亚氨基4-(2-吡啶基二硫代)戊酸酯(SPP)连接剂化学偶联的huN901单克隆抗体的缀合物(“huN901-SPP-DM1”)。将5mg/mL huN901-SPP-DM1缀合物配制在(a)PBS,pH6.5中,以液体形式贮存,或者(b)10mM琥珀酸钠、0.5%蔗糖、0.01%聚山梨醇酯20、250mM甘氨酸,pH5.5中,如实施例6所述冻干。将样品在4℃和25℃下培育6个月,然后如本文所述测试颗粒、缀合物二聚物和游离药物品种的存在。这些分析的结果如表10所述。

Claims (69)
