Abstract
用于超声和/或磁共振(MR)成像中的造影剂,是含有气体或气体前体的蛋白质微泡,其中蛋白基质的交联是通过在水介质中,实际中性pH下,与双功能醛(例如象戊二醛这样的二醛或者是烯醛这样的α,β-不饱和醛)发生交联反应。此种造影剂提高了其体内稳定性和贮藏稳定性,尤其是当蛋白基质也与氢硼化物之类席夫碱还原剂发生反应后。这些交联的含气蛋白质造影剂,在进行大小分布改善后,减小了微泡的平均大小,进一步提高了微泡的稳定性,并能制备出微泡大小分布范围特别狭小的新颖造影剂。
Description
本发明是关于新型造影剂,尤其是应用于诊断成像的新型含气或产气造影剂。
众所周知,在血管系统的研究中,尤其是在心动描记术和组织微脉管系统的研究中,超声成像是具有潜在应用价值的诊断工具。人们已提出了各种各样能增强所获声像的造影剂,包括固体颗粒是浮液、乳化液滴、气泡和包气胶囊或包液胶囊。人们普遍认为易于压缩的低密度造影剂能特别有效地产生声反射,因此对含气和产气体系作为超声造影剂给予特别的关注。
含气造影剂在磁共振(MR)成像中也是有效的,例如用于减弱MR信号强度的灵敏性造影剂即是含气造影剂。含氧造景影剂也是具有潜在应用价值的顺磁共振造影剂。
此外,像二氧化碳这样的气体可能在X-射线成像领域中用作负性口服型造影剂。
最初的研究是向心内注射生理可受的物质,它们在体内产生自由气泡,这种气泡作为超声心动描记术的造影剂相当有效。可是由于游离气泡的寿命短,此技术在实际应用中受到严格限制。因此在超声心动描记术和其它超声研究中,使气泡稳定的方法显得很重要,例如使用乳化剂、油、增粘剂或糖。
人们意识到为使左边心脏和心肌在静脉注射造影剂后更容易超声显像,这种应用于超声心动描记术中的气泡体系最好直径不超过8-10微米,以使其能自由地通过肺毛细管床到达左心房和左心室。很明显,要使左边心脏有效显像,要求这种气泡体系(下文中将称作“微泡”)有足够的体内稳定性,其寿命最好能超过一个循环周期。
微泡造影剂体系同样需具有好的贮存稳定性,例如,其生产、销售以及使用前在医院中的贮存,都要求造影剂能稳定存在。
人们对制备蛋白微气泡胶囊用作超声造影剂领域进行了大量的研究工作。正如US-A-4718433和US-A-4774958中所述,这种造影剂可通过如下方法制备:声处理—粘性蛋白溶液以形成微泡体系,至少使部分胶囊化的蛋白质发生热变性或化学变性以稳定微泡。热变性可能过加热或者简单地通过声处理中产生的热来实现,化学变性可通过在PH为4.5的水介质中,蛋白质与甲醛或戊二醛反应来进行。较好的蛋白质原料是5%的人血清白蛋白(HSA),据说用它能促进形成直径为2-4微米为主的微泡。用该技术生产的微球能稳定贮存48小时或更长的时间。
EP-A-0324938叙述了一种改进型的胶囊蛋白质微泡体系的制备方法。这一方法包括两步声处理。首先将声波发生器浸入蛋白质溶液中产生微泡体系,然后将该发生器移至液面上贴近液面的位置,使蛋白质溶液产生泡沫和烟雾。据说这样制得微泡浓分散体系,其微泡直径大多小于10微米,且在室温下,即20-25℃,能够稳定保存4-8周或者更第的时间。EP-A-0359246中叙述了适于工业上连续生产这种胶囊蛋白质微泡的过程。
HSA又是上述步骤中较好的一种蛋白质,可应用的浓度为0.5-25%W/W,工业上用5%或者更稀的水溶液更方便,比如用0.5-3%W/W。实验性产品AlbunexR就是用HSA按这种方法制备的,它的微球由空气微泡中心和不溶性(即变性)的HSA膜组成。
EP-A-0324938和EP-A-0359246中的这种产品具有适当的体外贮存稳定性,同时发现能够提高受试对象给药后其体内稳定性。例如Shapiro等人在J.Am.Coll.Cardilo.16(7)(1990),pp.1603-1607中报道了声处理过的HSA超声造影剂,在人体静脉注射后心肌浑浊化难以定量且结装不能重复。他们还观察到在心脏收缩初期,左心室底部和顶部的造影强度快速减弱,心脏收缩后造影强度几乎全部消失,并提出这可能是由于左心室的高收缩压破坏了白蛋白包覆的微泡。
Wo92/05806中报道了类似的观察结果,根据EP-A-0324938所说的方法制成比较稳定的胶囊膜,在血流中,由于心脏搏动时的压力变化导致膜的破裂。这一文献建议用下法制备超声膜:向一成膜蛋白质水溶液鼓泡并通过切割(如:研磨,声或者超声振动)使气泡大小减小到期望值范围(例如:0.5-10微米,最好4-10微米),然后通过热变性或者用能与蛋白质反应的交联剂与蛋白质反应,使气泡悬浮液稳定。这些交联剂有醛和巯基丙氨酸类硫化物,甲醛水溶液和戊二醛水溶液的应用已列举过。然而所得产品除非冻干,否则稳定性差。而对冻干剂,在注射时需用水或其它生理可受的液体再生,人们将意识到免去用前的重新配制,在实用中有很多优越性。
具有更好的体内稳定性和贮存稳定性是蛋白质胶囊包气微泡或者产生气体微泡体系造影剂不断发展的需要。
本发明除上述之外是以我们的研究结果为基础的,即蛋白质与能使其发生交联的双功能醛在水介质中,实际中性PH下发生反应能增强蛋白质型造影剂的稳定性。尤其是我们将惊奇地发现,当交联反应在中性缓冲液的水介质中进行时,稳定性提高的程度可大大超过上面US-A-4718433和US-A-4774958中的酸性甲醛水溶液以及酸性戊二醛水溶方法稳定性。
这里所用的“实际中性PH值”一词是指既无明显酸性,也无明显碱性的中间PH区域,例如PH为5-9。
本发明的侧重点之一是提供具有蛋白质包裹的微气泡或气体前体的微泡造影剂,蛋白质在实际中性PH的水溶液中与能使蛋白质发生交联的双功能醛反应而交联。
蛋白质可以是含有聚氨基酸的任何蛋白质及其衍生物。典型的蛋白质是白蛋白,动物胶以及r-球蛋白。这些蛋白质可从生物来源中获得,例如从人或动物的血液中得到,或者由低级生物体采用重组技术制得。发酵是制备人血清白蛋白的典型方法,详见Wo90/02808。
热处理(例如可以通过声处理直接发生)和本发明中的交联至少有助于胶囊的部分蛋白变性,从而使胶囊蛋白的性能更为优越,这样使预先经声处理的蛋白基造影剂(例如声处理的白蛋白Albunex)通过交联很容易获得本发明的造影剂。
本发明中用以稳定超声造影剂的双功能醛包括二醛和α,β-不饱和醛。典型的二醛包括10个碳原子以内的脂肪族二醛,例如戊二醛、已二醛、1,8-辛二醛及其象2-羟基-己二醛之类的取代衍生物。典型的α,β-不饱和醛包括10个碳原子以内的α,β-不饱和脂肪醛,例如3-8个碳原子的2位链烯醛,象丙烯醛(即2-丙烯醛),甲基丙烯醛(即2-甲基-2-丙烯醛),丁烯醛(即2-丁烯醛),2-戊烯醛和2-己烯醛及其象3-二甲基-氨基丙烯醛之类的取代衍生物。
另外,根据本发明,造影剂与能还原席夫碱中双链的还原剂(例如:象氢硼化钠和氰基氢硼化钠三类的氢硼化物试剂)反应有利于造影剂的稳定。
另一方面,本发明可提供制备微泡造影剂的方法。这包括在实际中性PH的水介质中,蛋白质与能够使蛋白质交联的双功能醛发生交联反应,及在交联进行之前,或者在交联进行中,或者在交联之后把气体或者气体前体囊包在该蛋白质中的方法。
任何生物体可接受的气体都可用于此过程及本发明的造影剂中。例如:空气、氮气、氧气、氢气、一氧化二氮、二氧化碳、氦气、氩气、六氟化硫以及可任意选择的低分子量的氟化烃,象甲烷、乙炔、四氟化碳等。气体可以自由地存在于微泡中,或者被截留、或者被夹带在某种所含的物质中。这里的“气体”包括37℃时以气态存在的任何物质。
气体前体包括碳酸盐、碳酸氢盐,例如碳酸氢钠、碳酸氢铵和氨基丙二酸酯。
在象超声心动描记述的超声应用中,制备和使用平均大小为0.1-10微米的微泡,比如1-7微米,可顺利地通过肺部系统,并在约0.1-15兆赫这个较佳的成象频率下获得共振信号。在实际中,平均大小大至500微米的较大气泡可用在胃肠成像、子宫以及输卵管的研究等其它方面。
可以通过任何适当的方法制备微泡体系,象前面提到的任何一种合适的方法都可。例如在气体存在下,通过剧烈振荡蛋白质溶液,就可将气体夹带在蛋白质溶液中,即产生US-A-4684479所描述的液包气乳液。另一个人所共知的方法是用注射器把气体注入到蛋白质溶液中,如US-A-3900420所述,用仪器快速向高流速液体中注入气体,在液体中产生低压区,此时的液体是蛋白质溶液。把气体注入蛋白质溶液中,如此而得到的液包气乳液从制备仪中泵出。
在装有蛋白质溶液的容器中,可能把电解产生的气体直接夹带在溶液中。此外,电解所需的电解质有助于稳定蛋白胶囊微泡。电解含有一种或多种电解质的水溶液,一般在阴极产生氢气,在阳极产生氧气。当加入肼时,在阳极可产生氮气。电极可用盐桥隔开。用科尔柏(Klobe)反应通电解羧酸也可产生二氧化碳。
制备微泡体系的较好方法是前面提到的US-A-47184333、US-A-4774958,EP-A-0324938,EP-A-0359246中所述的方法。即在气体存在下(例如在大气下)声处理蛋白质溶液。这种声处理技术的优点之一是除了产生所需微泡之外,还能起到控制胶囊蛋白变性质的作用,因此增强了最终产物的稳定性。
本发明中蛋白质的交联是在蛋白质的水溶液或悬浮体中进行的,在水缓冲液中进行更好。所用的醛可以是纯液态,或者是醛的水溶液,或者是象乙醇之类可与水混溶的低级链烷醇与水形成混合溶剂的醛溶液,醛的浓度为50%,所加醛的量大约是相应蛋白质的2-50倍。
交联反应可以在室温或加热下进行,不过不应使反应温度超过60℃,否则会引起过多的没必要的蛋白质变性。最好轻轻搅动反应介质,比如反应在滚动烧瓶中进行。反应时间一般在1分钟到24小时之间。
交联反应可在PH为5-9下进行,6-8之间更好,例如PH为7。反应在水缓冲液中进行,一般来说所有合适的缓冲体系都可以使用,象本发明中提到的。例如:柠檬酸/氢氧化钠体系PH为5或6,磷酸盐缓冲体系PH约为7,硼酸盐/盐酸体系PH约为8,硼酸盐/氯化钾/氢氧化钠体系PH约为9。
在我们不希望受理论根据约束的同时,我们认为应用α,β-不饱和醛,其交联反应包含快速的米切尔(Michael)类型的加成,此加成反应是在蛋白质分子的伯胺基与α、β-双键上的β碳原子之间进行的,醛基和蛋白质分子上另外的伯胺基之间发生慢反应,形成含有-CH=N-键的席夫碱和/或含有-CH(OH)-NH-键的产物(参见Polymer 31(1990)PP130-134Sebenik等人的研究结果)。用二醛作交联剂主要是通过生成-CH=N-和-CH(OH)-NH-键而交联。
如前所述:交联蛋白质又能有效地与象氢硼化钠、氰基氢硼化钠之类的还原剂反应,还原席夫碱中的双键,还原剂的用量应为相应用于交联反应的醛的2-5倍。此反应可在任一种PH为5-9的缓冲液中进行,PH6-8更好,比如PH为7。如果必要的话,此反应可与交联反应同时进行,这样需在加醛以前将还原剂加到蛋白质溶液或悬浮体中。
交联反应和任何还原反应最好在气体或气体前体囊包在蛋白质中以后进行,尤其是交联前通过控制囊包蛋白的变性进行初步稳定化处理时。交联反应可用的原料是象Albunex之类的声处理过的白蛋白产品等蛋白基超声造影剂。
后文将这种预先成形的蛋白质材料称作“微球”,交联前要洗涤球,除去自由的不囊包的蛋白质,否则它将参与不必要的交联反应。例如水洗微球3-4次,每欠洗涤后使微球漂浮,用移液管取走漂浮层下面的洗涤水。然后在旋转的烧瓶中使悬浮于水或者适当的缓冲液中的微球与醛反应,必要的话可与还原剂反应,再用水洗以除去未反应的醛和任何还原剂。
交联反应产物其微泡表面具有醛基(参见PharmaceuticalResearch9(6)(1992),P776和J.Pharm.Sci.7(12)(1982),P1323)。此醛基在发生还原反应时被还原成羟甲基。另外当胺大大过量时,此醛基会与胺反应生成席夫碱,如果需要,此席夫碱可象前述那样与还原剂反应,还原其中的-CH=N-双键。用双功能胺允许选择适当的功能基以代替醛基,例如:与氨基酸反应将引入羧基,与氨基醇反应将引入羟基,与二胺反应将引入氨基。
另外,除交联和其它化学反应外,对微泡大小分布的改善会进一步增强本发明造影剂的体内、体外稳定性。这里的“减小体积”一词,意味着减小微泡的平均大小。这种大小分布改善包括减小微泡的平均大小,和/或缩小微泡的大小范围,在交联和任何化学还原反应进行之前,之中,最好是之后进行这种改善。进行这种改善使本发明更具特色。
一种大小分布的改良技术是对微泡,最好是对微泡水悬浮液,或悬浮于磷酸盐之类缓冲液中的微泡施加外部气压(例如用空气或氧气)。例如:对充气微泡悬浮体施加20-100千帕的气压,30-80千帕较有利,60-70千帕更好;此操作可方便地在室温下的压力容器中进行时间为30秒-5分钟。大约1分钟较好。
另一种大小分布改良技术是向充气微泡中加入一种能溶解该气体的液体,例如对充空气微泡加水或者象磷酸盐之类的盐水缓冲液,在此液体中慢慢搅动微泡,比如滚动烧瓶。大小分布改良程度受两个因素的控制,其一是所加液体与微泡中所含气体的相对量,其二是气体在液体中的溶解度,改良程度随前者的增加而增加,随后者的增加而减小;通过控制这两个因素,可产生预期的结果。
我们发现:通过大小分布改良,使微泡的平均体积减小了40-60%,比如50%左右。其稳定性大大增强,并用作超声造影剂时其声频衰减性能优越。例如:用这种改良型造影剂大大增强了兔子动脉和静脉的多普勒信号。
高体外稳定性也是本发明中改良型造影剂的特征。例如:在标准的体外超声造影效应测试中,超过90秒钟其造影效应不减弱。
应用上述大小分布改良技术,我们可制得大多数微泡在4微米以下的交联蛋白质造影剂,例如至少85%,常常是至少90%的微泡大小在4微米以下,其余的微泡大小在4-10微米范围内。这可与US-A-4718433列举的造影剂相比较,在US-A-4718433中列举的造影剂,大约有20%的微泡其大小超过了4微米。具有上述大小分布的交联蛋白囊包气体微泡造影剂是新产品,它在象超声心动描记术之类超声技术中特别有用,并使本更具特色。这些产品可用交联了的蛋白基的微泡造影剂来制备,而不用本发明中双功能醛交联而稳定的造影剂来制备。例如用WO92/17213中所述的生物可降解的交联蛋白囊状微泡造影剂来制备。
总的来说,按本制得产品其微泡的数目和大小分布可通过库利特计分析来确定。据此可以公式化地制备微球的标准悬浮液,其单位悬浮液所含的气体总体积是预先确定的。
本发明的造影剂可应用于各种诊断成象技术中,包括超声成像、MR成像,X-射线成像。本发明的最佳特点是其产品作为敏感性造影剂应用于诊断超声成像和磁共振成像。
下列非限制性的例子用以说明本发明,所有温度都采用摄氏温标,用作起始材料的Abbunex(阿巴尼克斯)悬浮液中微球浓度一般在5-9×108个/毫升范围内。

以上标准化的悬浮液的体外回声性能(echogenicity)和稳定性是通过用7毫升的埃斯通(Iaotln)II(英国,拉坦(Luton)库利特电子有限公司)稀释每份悬浮液而测得的,埃斯通II的气容量与人的静脉血近似。每份悬浮液的声传导是作为时间的函数而测得的,稀释后立即用3.5兆赫的能量转换器开始测量。
信号稳定后,测试的所有产品的回声性能均大于未处理的阿巴尼克斯微球参比悬浮液。
对比实验是用33%的甲醛水溶液代替丙烯醛乙醇溶液,重复例13的步骤。所得产物在信号稳定后,体外的回声性能最小。例27:
用PH为7的磷酸盐缓冲液洗涤阿巴尼克斯微球三次,然后再悬浮于装有磷酸盐缓冲液(30毫升)的玻璃瓶中,加入25%的戊二醛水溶液(300微升),置瓶于滚动混合器上在室温下滚动20小时。这样得到的交联微球可通过下列任一种方法进行大小分布改良。(i)加入被空气饱和的磷酸盐缓冲液(制备方法:将磷酸盐缓冲液置于37℃水浴上,保持氧气分压Po2(注:原文中写成了PO2)约为21千帕)慢慢搅拌过夜以除去过量气泡;(ii)加入部分脱除空气的磷酸盐缓冲液(制备方法:37℃时将(i)中制备的饱和溶液在氧气分压Po2大约为10千帕时脱气1小时),并将其置于滚动器上,室温下滚动3小时;(iii)将微球悬浮液放高压容器中,空气加压1分钟,然后将微球再悬浮于装有磷酸盐缓冲液的瓶中,并将瓶置于滚动混合器上滚动5分钟,得到均匀的悬浮液。
交联微球大小分布改良时所加溶液的体积(相对于微球悬浮液的体积)、施加的空气压力以及微球的气体体积浓度详见下表:

对标准化的、含有相同气体体积的微球悬浮液进行回声性能测量,发现微球体积约减少50%是大小分布改良的最佳程度,即产生稳定的尽可能大的声衰减。这可以通过加入约5体积的饱和磷酸盐缓冲液、或者加入约2体积的部分脱气磷酸盐缓冲液,以及应用66千帕的空气超压来得到。加入更大量的磷酸盐缓冲液,或者施加更高的压力得到的微球虽然具有高稳定性,但声衰减减小了。应用过大的体积/压力导致微球皱缩,必然减小声衰减。例28:
根据例27中方法(iii)制备大小分布改良的微球,即施加66千帕的空气压力。用磷酸盐缓冲液洗涤所得的微球三遍,通过漂浮/取除下面的溶液使微球浓缩。下表中列出了各阶段所得微球的物理特性:(a)大小分布改良前;(b)大小分布改良后;(c)洗涤以后;(d)浓缩以后。

向500毫升带盖烧瓶中加入埃斯通II(250毫升)将烧瓶置于37℃水浴中,在电磁搅拌下空气饱和过夜。然后以250-300转/分的转速在室温水浴下进行搅拌。5分钟内将烧瓶从水浴中取出,用注射器和针头通过瓶盖将戊二醛交联白蛋白微球悬浮液(30毫升)注射到埃斯坦中,继续搅拌4小时。所加戊二醛交联白蛋白微球悬浮液是用300微升醛试剂、按例24中描述的方法制备的,含气量为25%。然后将其放置过夜使球漂浮。用注射器和针头从瓶底吸除埃斯通,吸除时要在瓶盖上另插一针头进气。此后加入磷酸盐缓冲液(15毫升),将烧瓶置滚动混合器上滚动20分钟,得到均匀的微球悬浮液。此悬浮液通过库利特计分析以及回声测量来表征,结果如下表所示:

Claims (22)
1.包含蛋白质外膜囊包的气体微泡或蛋白质外膜囊包的气体前体微泡的造影剂,所述蛋白质外膜是通过在水介质中,在pH5-9的条件下,用能与蛋白质发生交联的双功能醛与蛋白质进行交联反应形成的,其特征在于,交联蛋白质已与还原剂反应,用以还原席夫碱中的双键。
2.根据权利要求1的造影剂,其中的蛋白质是白蛋白、动物胶或者r-球蛋白。
3.根据权利要求2的造影剂,其中的蛋白质是人血清白蛋白。
4.根据权利要求1-3中任何一项的造影剂,其中的醛是二醛或α、β-不饱和醛。
5.根据权利要求4的造影剂,其中的醛是选自戊二醛、己二醛、2-羟基己二醛,1,8-辛二醛、丙烯醛、3-二甲氨基丙烯醛、甲基丙烯醛、丁烯醛、2-戊烯醛、2-己烯醛。
6.根据权利要求1的造影剂,其中所述气体微泡已被降低了微泡的平均大小和/或缩小了微泡的大小范围。
7.根据权利要求6的造影剂,其特征在于在造影剂的体外跟踪测试中,其造影效果在超过90秒钟时仍不减弱。
8.根据权利要求1的造影剂,其特征在于至少85%的微泡大小小于4微米,其余微泡大小在4-10微米之间。
9.根据权利要求8的造影剂,其中至少有90%的微泡大小小于4微米。
10.根据权利要求1的造影剂,其中的微泡中包含的气体选自空气、氮气、氧气、氢气、一氧化氮、二氧化碳、氦气、氩气、六氟化硫以及可选择性氟化的低分子量烃。
11.一种制备权利要求1中所述的微泡造影剂的方法,该方法包括在水介质中,在pH5-9的条件下,用能与蛋白质发生交联的双功能醛与蛋白质进行交联反应,在上述交联反应之前、期间或之后,将气体或气体前体包囊于所述蛋白质中,并且其中交联蛋白再与还原剂反应,用以还原席夫碱中的双键,或者其中使还原剂存在于交联反应的过程中进行还原。
12.根据权利要求11的方法,该方法包括预先声处理蛋白质微球,再进行交联。
13.根据权利要求11或12的方法,其中的交联过程在pH6-8范围内进行。
14.根据权利要求13的方法,其中的交联过程是在pH7时进行的。
15.根据权利要求11的方法,其中的交联过程是在水缓冲体系中进行的。
16.根据权利要求15的方法,其中的缓冲体系是磷酸盐缓冲液。
17.根据权利要求11的方法,其中的还原剂是氢硼化钠或氰基氢硼化钠。
18.根据权利要求11的方法,其中对微泡进行处理以降低它们的平均大小或缩小它们的大小范围。
19.根据权利要求18的方法,其中进行的降低微泡平均大小或缩小微泡的大小范围的处理是在蛋白质交联后进行的。
20.根据权利要示18或19的方法,其中进行的降低微泡平均大小或缩小微泡的大小范围的处理是通过对微泡或其悬浮液施加外气压实现的。
21.根据权利要求18或19的方法,其中进行的降低微泡平均大小或缩小微泡的大小范围的处理是通过用可溶气液体处理微球来实现的。
22.根据权利要求18的方法,其中微泡的平均体积被减小了40-60%。
