Abstract

Description
本发明涉及医疗领域,特别是粘膜感染领域。更具体而言,本发明涉及新的、用于治疗局部粘膜感染或粘膜炎和念珠菌病的延缓释放的生物粘附治疗系统。本发明还涉及用于治疗这种症状的方法以及新的、用于提高它们功效的释放形式。本发明的生物粘附治疗系统可用于治疗或预防这种类型的疾病。
念珠菌病是一种由真菌(念珠菌属)局部增殖引起的疾病。这种增殖可有很多原因,特别是与例如服用抗生素,甾类或其它物理治疗(放疗,化疗,免疫抑制等)有关的局部物理-化学失衡(pH值改变等)相关。免疫抑制情况的增加(与利用化疗治疗癌症或AIDS传染病后,连续的免疫抑制有关)是口腔念珠菌病和几种严重牙旁疾病复发的原因(Hermant等,1997,Med.Mal.Infect.
27:715-718)。口腔念珠菌病甚至常可在明显处于良好健康状态的HIV血清反应阳性患者中检测到。它们通常是HIV感染的最初表现。有超过90%的AIDS患者会发展为口腔念珠菌病(Vasquez,1999,Pharmacotherapy,19(1):76-87)。在特定群体中的流行接近20%;它随着CD4数的减少而增加(Greenspan& Greenspan,1996,Lancet 348:729-733)。此外,念珠菌病是免疫缺陷病毒HIV感染和癌症共有的特征。造成那些念珠菌病的真菌是念珠菌属,如白色念珠菌,glabrata念珠菌,热带念珠菌或克鲁斯氏念珠菌。
与念珠菌病有关的特殊症状是口干,摄食疼痛,味觉丧失,烧灼等。对于健康情况一般的患者而言,口腔状况的改变会有严重的隐患。没有得到很好治疗的口腔感染可能是吞咽痛和咽下困难的原因,会妨碍说话、咀嚼和吞咽。此外,由那些感染引起的疼痛导致患者减少摄食;如此导致体重减轻,脱水和营养不良。因此,口腔念珠菌病的预防和治疗在维持生活质量,预防那些患者更严重的并发症方面是必要的考虑(Weinert等,1996,Amn.Intern.Med.
125:485-496)。
念珠菌病还是抗癌治疗中常见的并发症。特别是,化疗、骨髓移植或局部放疗都是促进局部念珠菌病感染发展的因素。化疗的继发性口腔效应是癌症发病的主要原因。在27种临床试验:14种粘膜炎(945个随机病例)和15种口腔念珠菌病(1164个随机病例)中,随着癌症位置的不同,粘膜炎的发病率为50%-80%,念珠菌病的发病率为30%-70%。鉴于上述分析,就预防而言,局部吸收的治疗似乎比完全由肠道吸收的那些更为有效(Clarkson等,2000,Cochrane Database Syst Rev.(2):CD000978)。
促进念珠菌病发展的其它因素是由例如局部或弥漫性脱落导致的粘膜完整性改变。粘膜炎属于这种情况的其中一种:它涉及红斑,然后是局部脱落。粘膜(通常是口腔粘膜)的改变是形成感染层,特别是AIDS或癌症(化疗,骨髓移植,对头颈部肿瘤的局部放疗等)中真菌感染的早期阶段。对于局部患有唾液分泌机能减退的患者而言,唾液分泌机能减退本身是造成口腔卫生改变的原因(Greenspan等,TheLancet 348(1996)729)。
目前,念珠菌感染首先以局部方式进行治疗,主要使用抗真菌的:处方制剂,锭剂,漱口剂,唑类,多烯(Greenspan等,Lancet 348:29-733)。第二,是通过全身途径对食管念珠菌病患者施用唑类衍生物(酮康唑,氟康唑,伊曲康唑)。这种治疗对于预防有效,但却存在着危险(与药物相互作用,抗性,不耐受性)。这种全身给药的唑类仍被保留用于短期治疗经证实的念珠菌病(Kovacs等,2000,The NewEngland Journal of Medicine,May 11,1416-1429)。
这些疗法的其它缺点在于常常复发。一些研究已经观察到,在治疗之后的3个月内有60%复发(Imam等,Amer.J.of Medicine 89(1990)142)。
考虑念珠菌病的病理生理学,人们认为白色念珠菌和克鲁斯氏念珠菌,热带念珠菌及glabrata念珠菌是造成局部(口腔,食管,皮肤,指甲,阴道)感染的原因。它程度的深浅取决于宿主的防御。它是一种皮肤-粘膜疾病,可诱导并发症,并发展为全身性的。有效的局部治疗或预防口腔念珠菌病可避免全身性念珠菌病以及抗性菌株的出现。
真菌与粘膜的粘连是其致病的重要因素。抗真菌产品在口腔中的存留时间可能是其即时及长期功效的主要因素。
已经描述过的或现有的制剂或组合物都是以全身给药的形式为基础的,其中包括它们的盖仑制剂形式及活性成分。
本发明提供一种缓释生物粘附治疗系统,与通常所用具有短暂效果的局部形式(漱口剂,凝胶,芳香薰剂,锭剂)相比,它对于在感染位置产生较长的存留时间较为重要。
作为其中一个例子,局部使用的基于咪康唑的口腔凝胶由Janssen-cilag(92787 Issy-les-Moulineaux,France)销售,商品名为Daktarin口腔凝胶,用于治疗口腔真菌病。咪康唑是来源于咪唑家族的抗真菌剂。它在使用后于原位发挥作用。因为它只有非常少量被再次吸收,因此可被很好地耐受。治疗成人口腔真菌病的剂量是125mg咪康唑(2匙),每天4次,共7-15天。咪康唑的唾液浓度是产品功效的良好反映。在使用凝胶后的30分钟-3小时内,它从5μg/ml降至0.4μg/ml。浓度非常迅速的降低可由凝胶在口腔中极短的存留时间解释。此外,咪康唑抗白色念珠菌的MIC(最小抑制浓度)为1-10μg/ml(Daktarin专题论文,来源于Swiss Compendium ofMedicinal Products的医学信息)。这种浓度只能在使用凝胶后的短期内获得。因此,使用口腔凝胶获得的抗真菌程度较为普通。
专利EP-0 542 824 B1已经描述过生物粘附形式及它们的制备方法。它们被设计用于全身流通,但不能用于局部发挥作用。它们不能令人满意地在体外溶解,而体外溶解是决定局部有效性的标准。因此,它们不能与如抗真菌剂的活性成分相兼容而后者必须局部作用和/或进行有限的系统流通。
本发明的目的在于通过提供新的延缓释放的生物粘附治疗系统而克服上述所有缺点,确保活性成分的溶解而确保其局部功效。事实上,延缓释放的形式可减少剂量次数,并在较长时间内产生更稳定的活性成分水平。
在本文中,术语“生物粘附”或“粘膜粘附”可同等使用,本发明的治疗系统尤其适于粘膜给药。
同样,术语“治疗”同时包括对上述不同病理,特别是粘膜炎和念珠菌病的治疗和预防。
生物粘附治疗系统可以是生物粘附片剂,微球或毫微球(nanosphere)的形式。
本发明还提供一种制备片剂形式的所述生物粘附治疗系统的方法,该系统具有在所有类型粘膜上使用的理想品质。
更准确地说,本发明的生物粘附片剂和方法适于治疗免疫抑制(上了年纪的,营养不良的,抗生物制剂疗法,癌症,AIDS,放疗,化疗,移植)患者的粘膜炎和念珠菌病。
所述片剂还适于粘膜施用活性成分,对于该活性成分粘膜给药比口服,透皮,或全身给药在治疗或舒适方面具有很多优点。例如,它可以是抗病毒剂,如阿昔洛韦,伐昔洛韦,更昔洛韦,齐多夫定或不溶性镇痛剂,如芬太尼碱。
它们有利于不溶性或微溶性活性成分的局部使用。它们还可使多种活性成分,如具有不同菌谱的其它抗真菌剂,镇痛剂,唾液分泌促进剂等在片剂中组合,增加治疗的适应性及可接受性。
本发明的片剂还可减少给药剂量,从而降低正常使用这种药物时伴发的不良副作用。此外,本发明还描述了在临床上特别有益的组合物,因为它们以每天给药一次(或两次)为基础发挥作用。最后,它们的新制剂不会改变味觉和食欲,而气味和食欲是维持免疫抑制患者良好健康状况的重要因素。
本发明还提供一种克服现有技术缺点的方法,从而更完全且更易忍受的对粘膜感染及相关症状(连续或非-短暂覆盖)和某些病毒疾病,疼痛或其它疾病的控制。
首先,本发明提供一种延缓释放的生物粘附粘膜治疗系统,它含有至少一种活性成分,在8小时内的活性成分溶解试验高于70%,含有占活性成分重量至少50%、占所述生物粘附治疗系统重量至少20%的天然蛋白,10%-20%的亲水性聚合物,用于加强生物粘附治疗系统硬度的压缩赋形剂,并含有4%-10%的碱金属烷基硫酸盐和任选的0.1%-1%的一水合糖。
本发明的粘膜生物粘附治疗系统的具体实施方案由粘膜粘附片剂构成。
加入碱金属烷基硫酸盐的主要作用是使微溶性或不溶性活性成分溶解,并促进其局部有效性:所述碱金属烷基硫酸盐可促进活性成分的全身流通,这与其浓度有关(Martin-Algarra,1994,Pharmaceutical Research,(11),7:1042-1047)。在本发明的制剂中,通过形成微胶粒,它可起助溶剂的作用,但不会促进吸收。它还可促进溶胀,并使活性成分在8小时内持续释放。
更具体而言,本发明的生物粘附治疗系统适于预防或治疗口腔,食管,或阴道感染。
更具体而言,它们适于预防或治疗免疫抑制患者的口腔念珠菌感染。本发明的片剂可以治疗或预防的方式使用。此外,本发明的片剂更适于口腔给药,而且可通过改变盖仑制剂形式而适于其它形式的给药。
总之,本发明的生物粘附治疗系统具有很多重要的性质和优点:
·它是一种可获得活性成分现成剂量(与预防或维持性治疗相一致)及最佳活性的制剂;
·可将一种或多种其它活性成分和/或赋形剂与第一种活性成分结合,从而确保水合以及组合物良好的局部接受性;
·大大低于在其它现有盖仑制剂形式中使用的活性成分剂量可减轻或避免不利的作用;
·在延长的时间内,活性成分的唾液浓度高于其最小抑制浓度(MIC)。
本发明的生物粘附治疗系统含有至少一种活性成分。
当生物粘附治疗系统用于预防和治疗真菌感染时,优选的活性成分是来源于广谱唑类家族的化合物,优选自咪康唑,克霉唑,酮康唑,氟康唑,伊曲康唑,异康唑,益康唑,沙泊那唑,genaconazole,曲康唑,布康唑,噻康唑,奥昔康唑,必佛那唑,芬康唑,双醚康唑,舍他康唑,沃利康唑和硫康唑。优选三唑,如氟康唑,伊曲康唑或沙泊那唑;或优选自咪康唑,克霉唑和酮康唑的咪唑。
本发明特别优选的唑类化合物是咪康唑,酮康唑和伊曲康唑,每片的单位剂量为10-150mg。
更优选的化合物是咪康唑,它的化学名称为(RS)-1-[-2-(2,4-二氯苄氧基)-2-(2,4-二氯苯基)-乙基]-1H-咪唑,它以10-150mg/片,优选25-75mg/片,更优选50mg/片的剂量存在。
已知唑类化合物可对真菌膜成分:麦角甾醇的合成起作用。
本发明含有约50mg/片的生物粘附治疗系统可在没有全身流通时,以每天给药一次的方式,在作用位点确保高于MIC的活性成分持续并长时间地存在,同时确保上述各种优点。
主要含有50mg咪康唑的生物粘附治疗系统的其它优点是活性成分极好的耐受性,这是因为50mg制剂中的每日有效剂量要比对照制剂,Daktarin口腔凝胶制剂(500mg/天)中相同产品的平常剂量低10倍,同时令人吃惊的是,局部浓度显著增加(7-10倍)。其中一个重要优点是降低导致真菌抗性的危险。事实上,如果局部浓度小于MIC,致使酵母菌再次生长,则常常会产生对化合物的抗性。本发明片剂中的有用剂量还可降低甚或消除由500mg/天剂量产生的不利作用,如肠道问题(恶性,呕吐,腹泻)或变态反应。很少见到转氨酶升高。
从主要描述药物形式和对比临床试验结果的实施例中可以很清楚地看出本发明生物粘附治疗系统的各种优点。
咪康唑型化合物可与其它活性成分,例如,具有不同菌谱的如多烯型抗真菌剂,镇痛剂,唾液分泌促进剂,唾液代用品,防腐剂,抗炎剂(肾上腺皮质激素),沙利度胺,或其混合物结合。
多烯具有不同于唑类的作用机理。它们结合真菌膜中存在的甾醇基,主要是麦角甾醇,并诱导孔和管的出现。这些孔和管可大大增加细胞的渗透性和小分子的损耗,导致细胞死亡。
用于本发明组合物中的优选多烯是广谱多烯,优选四烯。特别引用制霉素和两性霉素B。
优选在本发明的生物粘附治疗系统中使用制霉素。
优选使用10-100mg,优选20-90mg剂量的多烯。作为举例说明,优选使用20-60mg剂量的两性霉素。制霉素使用10mg-100mg,20mg-40mg,更优选约25mg量。
所述剂量还可以单位的形式提出,已知1mg相当于4400个单位。本发明优选的组合物含有50-400mg的咪唑或三唑化合物以及20-90mg或100 000-200 000个单位的多烯。更优选,唑类化合物是咪康唑,多烯是制霉素。本发明的组合物通常每天给药一次或两次。优选它们是间隔给药,每天服用20-200mg量的唑类和50 000-500 000个单位的多烯(10-110mg)。
此外,为了提高治疗功效,用于粘膜给药的本发明的生物粘附治疗系统还可含有一种或多种其它活性成分。具体而言,本发明的生物粘附治疗系统可含有一种或多种确保水合以及局部接受性的活性成分,表明这类治疗的适应性。活性成分的这类使用在紧急应用和维持治疗方面特别有益。
相关的赋形剂和其它活性成分可,例如,有利地选自单独或组合使用的一种或多种麻醉化合物,唾液分泌促进剂,防腐剂,抗炎剂,沙利度胺,抗生素,唾液代用品或掩味剂。
活性成分与麻醉性的结合使用特别有益,这是因为如上所述,念珠菌病常常伴有严重的疼痛,特别是在摄食过程中。用于本发明中的适宜麻醉剂的例子是利多卡因或丁卡因。优选,使用占本发明组合物活性成分的总重量0.1%-10%,更优选1%-7%剂量的麻醉剂。
本发明的典型组合物含有约1%-5%的麻醉剂,例如利多卡因。
结合使用唾液分泌促进剂对于治疗念珠菌病也是有益的,这是因为念珠菌病常常引起口干。另一方面,本发明涉及含有至少一种活性成分及唾液分泌促进剂的生物粘附治疗系统或组合物。
所述活性成分可以是下述抗真菌化合物,用于治疗HIV感染(人免疫缺陷病毒),EBV(Epstein-Barr病毒,传染性单核细胞增多症,多毛粘膜白斑病),CMV(巨细胞病毒),单纯性疱疹病毒(HSV)或水痘带状疱疹病毒(VZV)的抗病毒剂。包含在生物粘附治疗系统中的活性成分是齐多夫定,阿昔洛韦,伐昔洛韦或更昔洛韦。它还可以是不溶性或微溶性镇痛剂,而其它给药方式则恰恰会造成那些溶解性问题。可以提到的其中一个例子是微溶性芬太尼碱,它对于治疗严重的抗性疼痛,特别是与癌症有关的严重抗性疼痛较为重要。
用于本发明中的适宜的唾液分泌促进剂的例子是毛果芸香碱。目前,当治疗头颈部癌症时,毛果芸香碱用于治疗接受放疗患者的口腔干燥。其它唾液分泌促进剂的例子是氨基甲酰甲基胆碱。
优选使用占本发明组合物活性成分总重量0.1%-10%,更优选0.1%-5%剂量的唾液分泌促进剂。
本发明类型的组合物含有约0.5%-3%的唾液分泌促进剂,例如毛果芸香碱。
更优选,本发明的特定组合含有至少一种抗真菌化合物,一种麻醉剂和一种唾液分泌促进剂。更优选,本发明的有益组合物含有:
·25-75mg的咪唑或三唑化合物;
·1%-7%重量的麻醉剂;和
·0.1%-5%重量的唾液分泌促进剂。
此外,为了提高治疗效果,如果需要,还可将掩味剂与抗真菌剂一起使用。然而,使用50mg剂量的咪康唑时,生物粘附片剂并没有令人不快的气味。这种药剂优选没有糖,从而避免可能的口腔并发症,特别是牙齿问题(龋齿)。在另一实施方案中,本发明的生物粘附治疗系统含有至少一种抗真菌化合物及掩味剂。后者包括例如薄荷型的衍生物。优选所述掩味剂与上述麻醉剂和唾液分泌促进剂组合使用。
此外,如上所述,防腐剂的组合使用也具有很多优点,特别是在用于预防性治疗或维持性治疗时。这种类型化合物的使用可确保念珠菌病患者良好的口腔卫生。特别是,这种类型药剂的使用可减少口腔中存在的细菌种群。适宜的防腐剂是单独,或与洗必泰(0-5%),抗生素(镰刀霉素,500mg,例如)或抗炎剂,或肾上腺皮质激素组合使用的碱金属烷基硫酸盐。
所述对症活性成分的组合使用可赋予本发明组合物补充性质,特别是:
·粘膜与口腔水合;
·掩味;
·使用舒适(控制疼痛);
·良好的口腔卫生。
因此,本发明显示,它可将具有补充性质的不同药剂在活性组合物中组合,确保更有效治疗患者的念珠菌感染。具体而言,与使用唑类进行的、功效很高但可引起抗性的早期或局部系统治疗相比,本发明组合物可全面治疗这些疾病。
可将本发明所用的活性成分在具有两相、两种释放速率的同一均质或异质生物粘附治疗系统中调节,或根据给药是同时还是间隔进行而分开调节。此外,还可根据化合物的性质和剂量,按不同方式配制它们。所述制剂可以是均质或双层片剂的形式,或是微球或毫微球的形式。通常,包装和配制可获得有关产品的相容性,减少或增加给药频率(每天一次或两次),使释放系统更加简单(优选口腔生物粘附形式),如果需要还可以掩味,使局部水合,不进行全身流通,并获得良好的接受性。
本发明生物粘附治疗系统的其它方面可在赋形剂和填料方面发现。粘附性是由天然蛋白赋予的,它占活性成分重量的至少50%。可以使用的天然蛋白类型是在EP 0 542 824中描述的那些。具体的例子是滴定最少85%蛋白的乳蛋白浓缩物,如Prosobel L85,LR85F或,优选Armor Protéines销售的Promilk 852A,或来源于NZMP的Alaplex系列(4850,1180,1380或1395)。本发明生物粘附片剂中天然蛋白的相对浓度为15%-50%,优选20%-30%。
本发明的生物粘附治疗系统还含有赋形剂,它们常常存在于生物粘附系统中,并在下面的实施例中给出。
本发明组合物的特有性质是它们含有3.5%-10%的金属烷基硫酸盐。它优选选自十二烷基硫酸钠和二乙基磺化琥珀酸盐。更优选,它是占片剂总重量4%-6%浓度的十二烷基硫酸钠。
如下面实施例所示,超过3.5%剂量的碱金属烷基硫酸盐,特别是十二烷基硫酸钠的存在可增加体外咪康唑从片剂的释放,即与无碱金属烷基硫酸盐存在时活性成分不能于短期内溶解相反,有超过80%的都在8小时内溶解。因此,它可增加不溶性或微溶性活性成分的溶解度。这一点对于活性成分的局部有效性很重要。此外,它还可使片剂较好地溶胀,产生活性成分向致病剂作用位点的持续释放。值得注意的是,与粘附片剂结合的这种浓度的十二烷基硫酸钠对于片剂而言很不寻常,它在这种产品中的浓度几乎不超过3%。
因此,在本发明中,十二烷基硫酸钠对微溶活性成分的助溶功能对于其在原位释放有重要作用。下面的实施例表明体外溶解试验与体内一定时间内测量的累积唾液浓度之间的良好相关性。
因此,4%-6%浓度的十二烷基硫酸钠的存在是片剂组合的重要因素,并对上述片剂的性质重要:良好的有效性,每天给药一次等。
3.5%最小浓度,优选3.5%-10%浓度的十二烷基硫酸钠本身还具有防腐性。因此,它不仅起赋形剂的作用,而且它还可作为活性成分起作用,特别是当治疗粘膜炎时。
最后,本发明的生物粘附治疗系统可含有0.1%-1%的糖一水合物,优选乳糖一水合物或蔗糖。只要乳蛋白的浓度较高,那么乳糖的存在就并非首要(priori)或本身不可缺少。然而,乳糖一水合物或蔗糖的存在可导致EP 0 542 824-B1中所述制备方法步骤的重大改变,包括如下所述,取消醇存在下的成粒步骤,并使用由乳糖或蔗糖的纯净水溶液组成的润湿液代替它。
无需乙醇存在时,具有在不使用防爆器的情况下大规模生产本发明生物粘附系统的特定优点,而当工业过程的任一步骤中存在醇时,防爆器都是必须的。
本发明的生物粘附系统属于延缓释放的系统,并且可用即时释放、含有相同活性成分的可溶层覆盖;这一点对于镇痛治疗特别重要,因为镇痛需要双效—即时和延续作用。
本发明还涉及一种制备粘膜控释生物粘附治疗系统的方法,该系统含有至少一种活性成分,在8小时内的活性成分溶解百分比超过70%,所述方法至少包括:
·将活性成分与占活性成分重量至少50%的天然蛋白、赋形剂和含有至少一种亲水性聚合物的填料混合的步骤;以及
·与占生物粘附系统重量3.5%-10%,优选4%-6%浓度的碱金属烷基硫酸盐混合的步骤。
在其中一个实施方案中,生物粘附系统是一种延缓释放的粘膜生物粘附片剂,它的制备包括下列步骤:
a)将活性成分与占活性成分重量至少50%的天然蛋白,赋形剂和含有至少一种亲水性聚合物的填料混合的步骤;以及
b)用一水合糖或多元醇型粘附剂润湿步骤a)所得混合物的步骤;
c)干燥混合物并筛分所得颗粒的步骤;
d)用占片剂重量3.5%-10%,优选4%-6%浓度的碱金属烷基硫酸盐混合所得颗粒的步骤。
在本发明的方法中,如果需要,使用孔隙0.4-1mm的筛子将与甲基羟丙基纤维素或metolose(MHPC)混合的活性成分过筛,然后与至少50%重量的天然蛋白,如乳蛋白混合。所得粉末是均质的。然后进行成粒步骤中的润湿步骤,其中将粉末与乳糖或蔗糖的蒸馏水溶液混合。然后干燥,使残余水分含量约为3%。随后进行校准步骤,接着与剩余的赋形剂混合,然后是压片步骤。润湿液,即乳糖水溶液的特有特征是在批量生产过程中引入的量不固定,引入的量应作为颗粒外观的函数而定。有三种可能:
·或者按照下面实施例所述,引入所有润湿液,并通过加入赋形剂而按常规进行随后的片剂生产阶段;
·或者使用一部分润湿液,加入metolose和制剂赋形剂,达到终浓度(占最终片剂的0.1%-1%,优选2%-4%)。典型的生产图在举例说明实施例2的图1中表示;
·或者将所有乳糖或蔗糖溶解在一半理论量的纯净水中。将所有润湿液都引入到混合物中。当最优化润湿时,可将剩下那半纯净水用作储备,并根据批量生产的多少而调节。
后者的可能性避免了用metolose补偿一水合乳糖。当按比例增加时,由于引入量可利用已知批次的多少而计算,无需知道颗粒的表观,这一点特别有益。下面按比例增加的实验表明,该方法适于用乳糖和蔗糖大规模生产。实施例3组合物的大规模溶解试验可证实这一点。
制备本发明片剂方法的另一重要特征是在上述d)步骤中,加入占片剂重量3.5%-10%,优选4%-6%浓度的碱金属烷基硫酸盐。事实上,它与制剂的赋形剂如滑石粉和硬脂酸镁同时加入,从而赋予片剂上述功能,特别是溶解速率。
本发明还涉及一种制备除片剂之外,如微球形式的生物粘附治疗系统的方法,其中可在制备时加上用于加入如上所述相同浓度碱金属烷基硫酸盐的步骤。
所述碱金属烷基硫酸盐优选十二烷基硫酸钠或二乙基磺化琥珀酸钠。下面实施例中描述的试制批次的生产使用了4.5%浓度的十二烷基硫酸钠。
本发明的方法可生产延缓释放的生物粘附片剂,它特别适合用在粘膜,更优选口腔粘膜上。
实施例涉及含有治疗口腔念珠菌病的抗真菌物质,抗-感染物质(AZT,阿昔洛韦)作为活性成分的片剂,含有治疗疼痛(芬太尼)或恶心(甲氧氯普胺)的活性成分的片剂。技术人员能够使用其它活性成分改变生物粘附治疗系统的生产,从而用于其它类型的粘膜和/或用于其它类型的疾病。
对于活性成分的吸收而言,有两种作用机理,局部和全身。例如:属于口腔感染的口疮,用于阴道感染、炎症的抗感染剂,用于疼痛,干燥的局部镇痛剂或类阿片镇痛剂,用于恶心的甲氧氯普胺,抗溃疡剂和局部细菌感染。
最后,本发明涉及溶解活性成分、并允许药物延续释放的生物粘附治疗系统用于预防性,治愈性或维持性疾病治疗,特别是治疗粘膜疾病的用途。更具体而言,本发明生物粘附治疗系统的优点是它们用于预防或治疗口腔粘膜念珠菌感染的用途,所述生物粘附治疗系统含有作为活性成分的唑类,剂量为10-150mg/片,优选25-75mg/片。
为了治疗其它口腔感染,阴道或全面感染,特别是HIV感染(人免疫缺陷病毒),EBV(Epstein-Bar病毒,传染性单核细胞增多症,多毛粘膜白斑病),CMV(巨细胞病毒),单纯性疱疹病毒(HSV)或水痘带状疱疹病毒(VZV),生物粘附治疗系统中所含的活性成分是齐多夫定,阿昔洛韦,伐昔洛韦或更昔洛韦。阿昔洛韦和伐昔洛韦的剂量为20-100mg,优选50mg。齐多夫定和更昔洛韦的剂量为10-2000mg,优选500-1500mg。理想的作用是局部的,特别是在感染剂进入处,但由于这些病毒神经节向性的特定作用方式,作用也可以是全身的。
生物粘附系统还可以是用于治疗口疮或疼痛的药物。当治疗疼痛时,由局部释放引起的全身作用更为有利,这是凭借生物粘附治疗系统的释放方式获得的。芬太尼较低的生物有效性可由生物粘附形式提高,生物粘附形式可使活性成分溶解并释放到口腔粘膜中。用于芬太尼的这种生物粘附系统可将即时作用与延续作用结合起来,这一点对于疼痛,特别是对于剧烈、抗性疼痛,尤其是与癌症有关的疼痛特别有益。
当活性成分是芬太尼碱时,本发明的生物粘附系统特别有益,芬太尼碱是不溶性分子,它的溶解度可用十二烷基硫酸盐提高。该治疗系统中芬太尼的剂量可为50-1600μg,优选200-1200μg,并可根据所治疗的疼痛而调节。
本发明生物粘附治疗系统的应用对于患者特别有吸引力,这是因为延续释放可每天只给予一次生物粘附治疗系统,并使活性成分在局部流通,降低全身流通。生物粘附治疗系统,特别是使用上述方法制备的本发明片剂的各种优点可从在附图中举例说明的下述实施例很明显地看出。
图1表示本发明片剂的典型生产图。
图2表示含有4.5%浓度十二烷基硫酸钠的两种不同剂量(50mg和100mg)片剂的体外溶解试验。
图3表示咪康唑随时间改变而得到的平均唾液浓度,其中比较了含50mg咪康唑的片剂(黑三角),含100mg咪康唑的片剂(空白方块)以及商品名为Daktarin的含咪康唑的凝胶(空白圆圈)。

实施例1-十二烷基硫酸钠对生物粘附片剂溶解试验的影响:
咪康唑在缓释粘附形式中的溶解性可利用溶解试验顺序检测,试验条件分别为EP 0 542 824所述的常规配制条件,作了改变但没有十二烷基硫酸盐,然后向最初的颗粒中加入十二烷基硫酸钠的条件,及下面实施例2和图1的条件。

没有十二烷基硫酸钠的配方:
在纯水中进行的溶解试验在早期(1和3小时)产生非常低的咪康唑溶解百分比(<5%)。

没有十二烷基硫酸钠的配方并改变试验条件:
因为溶解试验不可能在纯净水中进行,考虑到EMEA(对改进型释放产品质量的指导)的建议,可改变溶解试验的条件,从而在体外试验中获得最大程度的差别。该试验仍然保持满足PharmacopéeEuropéenne(Essai de dissolution des formes solides,Phée Eur1997,2.9.3)的要求,其特点在于溶解介质含有0.05%十二烷基硫酸钠。

含有十二烷基硫酸钠的配方1.3不仅能使咪康唑延续释放,而且可产生新的性质(润湿,溶胀并使活性成分以恒定的局部浓度在原位溶解)。
实施例2-含50mg咪康唑的片剂的制备:
a)
1.9kg批次:
该表的制备图在图1中表示。将50mg咪康唑碱(或,对100mg的片剂而言为100mg)与9.2mg玉米淀粉和20.5mg甲基羟丙基纤维素混合。
然后利用过筛使混合物均质,加入27.43mg的LR85F乳蛋白,并与最初的混合物混合。然后用乳糖一水合物的混合物(溶解在纯净水中的,0.39mg量的200目乳糖)将后者润湿。在批量生产过程中,引入所有润湿液,即0.39mg/115mg片,占全部成分重量的0.34%。润湿步骤后,是在常规条件下进行的成粒,干燥和筛分步骤,从而获得图1所指的最初颗粒。
将赋形剂的混合物加入到该最初颗粒中:滑石粉,硬脂酸镁,甲基羟丙基纤维素,然后在筛分后加入4.5%浓度的十二烷基硫酸钠。
接着使终混合物经过常规的压片和包装步骤。
50mg剂量片剂的组合物和100mg剂量片剂的组成在下表中表示。

此外,在与下面实施例7相同的条件下,可使用蔗糖替代乳糖。

工业上的按比例增加参考上面表II的相同组成进行,生产10kg一批。
大规模生产时,可改变并简化图1的方法:
·引入固定量的乳糖,无需用metolose补偿;
·计算引入的水量,忽略颗粒表观,使该方法适于密闭容器。
对下面实施例3的组合物进行的溶解试验表明大规模生产方法的工业可行性,这是因为所得结果与1.9kg批次的片剂的结果没有差异。
实施例3-体外溶解试验
a)
50mg/片和100mg/片剂量片剂的比较:
咪康唑从其盖仑制剂形式的释放和片剂的崩解速率利用溶解试验体外测量。该试验按照Pharmacopée Européenne的建议,在浆式旋转器中,60rpm,37℃,以及于适宜的介质中进行。使用HPLC技术测定咪康唑。
BA001-50和BA001-100这两种类型片剂的结果在下面的表III中表示。表中只给出了释放出来的咪康唑百分比;事实上,片剂的崩解百分比结果完全重叠。

这些结果表明,对于50mg片剂而言,8小时后,几乎所有咪康唑都已经从其盖仑制剂形式中释放出来,而且这种释放是持续的。100mg片剂的结果具有相同的量级。这些释放时间较长,在体内可延续释放到口腔中,产生延续的抗菌效果。这种假设可通过下面的实施例4中,对健康志愿者进行的I期临床研究加以证实。该研究是用两种剂量的咪康唑(50和100mg)进行的。通过检测各自在体外获得的各释放时间以及最小的所需唾液浓度,可选择两种剂量中的一种,推断其给药频率。

实施例4-稠度测定仪生物粘附试验的结果:
本试验用于比较片剂的体外粘附性。
仪器:
本试验所用的仪器是稠度测定仪(texturometer)(TEC 025型)。它由下列元件组成:
·升高或降低探针的水平臂;
·200N力的传感器,用于测量使片剂碎裂所需的力;
·上面固定片剂的一次性塑料探针;
·顶部覆盖有直径4cm的塑料结晶器的钢性组件,它的基底由不锈钢板构成。该结晶器含有润湿片剂所需的介质。
操作过程:
·用氰基丙烯酸粘附剂将所要研究的片剂固定在塑料探针上;
·将探针旋在稠度测定仪的水平臂上;
·将去离子水放在结晶器中(约4ml);

由力传感器测量的数据用稠度测定仪TEC v6.0 ETIA 1996软件分析,并以力(N)/时间(s)曲线的形式表示。该软件还可描绘力/距离(mm)曲线。片剂的体外粘附性通过在片剂碎裂阶段测量的正功(J)值评估。该功由力/距离曲线下的面积表示。
值越高,片剂的生物粘附性越好。
在条件相同且片剂直径相同时获得的结果可进行不同制剂之间的比较。
实施例5-本发明片剂的体内缓释研究:
该研究借助18名健康志愿者和用两种剂量(50和100mg)的咪康唑,利用唾液动力学进行。此外还评估了这种盖仑制剂形式的耐受性。
将本发明的片剂与含有咪康唑的凝胶进行比较。因为凝胶不能在口腔中存留,因此不得不每天多次使用它们。咪康唑的最大唾液浓度是在使用之后立即观察到的,但活性成分迅速从口腔消除(Odds,1981,Clean Raise Rev.1:231-232)。使用相当于125mg咪康唑的6.25g口腔凝胶后,唾液浓度从使用后30分钟时的5μg/ml降至3小时后的0.4μg/ml。给药频率为每天4次。
5.1
研究目标:
·主要目标:
本研究的主要目标是在18名健康志愿者中,测定50和100mg两种类型的缓释(PR)片剂中咪康唑的唾液动力学,并将它们与使用口腔凝胶(125mg即时释放的咪康唑)获得的结果进行比较。
·次要目标:
本研究的次要目标如下:
○测定50mg和100mg两种类型缓释(PR)片剂中咪康唑的血液药动学;
○评估生物粘附性咪康唑口腔片剂的临床耐受性;
○测定患者自身的可接受性和耐受性。
5.2
研究方案:
在健康志愿者中进行单中心随机交叉I期研究。18位被试者中,每位以不同顺序接受3种类型的产品,每次给药之间的等待时间为8天。
所给予的3种产品为50mg的生物粘附咪康唑片,100mg的生物粘附咪康唑片和每次给予125mg的即时释放的咪康唑凝胶。将治疗组分别标为1-18。在各治疗组中,在不知情的条件下,将产品标为“周1”,“周2”或“周3”(50mg片或100mg片或凝胶)。
咪康唑的唾液和血液测定是在给予产品之前进行的。唾液和血液样品在第30分钟,第1小时采集,之后每1小时采集1次,唾液采集直到12小时,血液样品采集直到4小时,然后在第8和第12小时采集血液样品;最后在第24小时采集唾液和血液样品。
·样品:
○唾液样品:为了避免异常高的咪康唑水平,在除去样品之前,应避免舌头与片剂接触达10分钟。此外,在给药后30分钟除去样品时,小心清洗被试者的嘴唇;
在2分钟内将2ml唾液收集在硼硅酸盐试管中(规定时间之前1分钟和之后1分钟)。
将标记过的试管在-20℃保存,直到进行分析。
○血液样品:将收集有血液的标记过的硼硅酸盐试管在-20℃保存,直到进行分析。
·生物粘附片剂的给药方式及粘附:
○给药方式:将片剂从计量烧瓶中移出,放在犬齿窝的下唇下。可用手指或一次性装置将片剂放置在犬齿窝的中心,并促进其粘附。
○粘附时间:注意片剂的粘附时间。粘附时间是指在该时间后,口腔前庭中或牙龈上再也看不到片剂,在该时间还要对每份样品进行灵敏的外部对照。详细说明粘附结束时的状况(片剂的磨耗或脱离)。记录发生脱离的时间,之后将片剂吞咽。
·给予口腔凝胶的方式(Daktarin2%口腔凝胶):
凝胶管的剂量是2匙。在吞咽凝胶前,必需使凝胶在口腔中保持足够长的时间(2-3分钟)。给药必需在进餐或喝水之间进行,或至少在此后10分钟进行。
·口腔检查:
这是在10小时后,由在给予产品前就进行口腔检查的同一个人进行。
5.3评估:
a)主要评估标准:
·咪康唑的唾液药动学:
咪康唑是使用高性能的色谱技术,在不知道具体内容的情况下测定的。结果用标准药动学标准表示(Cmax,Tmax,AUC),并对产品进行了比较。结果在下面的表IV中给出。
b)次要标准:
·咪康唑的血液测定:
咪康唑是使用高性能的色谱技术,在不知道具体内容的情况下测定的。结果用标准药动学标准表示(Cmax,Tmax,AUC),并对产品进行了比较。结果在下面的表IV中给出。
·粘附片剂的局部临床耐受性:
这由同一研究人员,在给药10小时后,对口腔粘膜进行临床检查而评估。
测定每个产品的耐受性,并由志愿者本人使用调查表,在每次给药当天进行(4种可能的选择:良好,可接受,适度或令人不快:如果令人不快,还要提供详情)。
·生物粘附片剂的可接受性:
志愿者使用调查表确定每个产品的可接受性,以及每次给药当天的可接受性(4种可能的选择:良好,可接受,适度或令人不快:如果令人不快,还要提供详情)。
5.4结果:
a)体内唾液浓度:
随时间改变获得的体内唾液浓度在图3和下面的表IV中给出。

*平均值;#相当于第2次给药后0.5小时。
这些结果表明,50mg剂量的片剂具有15.07μg/ml的最大唾液浓度,而口腔凝胶为1.61μg/ml。在图3中观察到的3个峰与T0,T4和T9后凝胶的三次给药相对应,而片剂只给药一次。因此,与口腔凝胶相比,片剂的唾液浓度非常高。
咪康唑对白色念珠菌的MIC(最小抑制浓度)为1-10μg/ml。高于1μg/ml的唾液浓度表明凝胶几乎不会超过该阈值,而50mg的生物粘附片剂常常高于该阈值(与凝胶组0个相比,18名被试者中有16名高于阈值)。50mg生物粘附片剂具有高于1μg/ml唾液浓度的平均时间为7.22小时(凝胶为0.61小时)。这意味着浓度可确保局部对任一病菌的充分抑制,并因此降低抗性菌株出现的危险。
b)其它结果:
50mg及100mg片剂的平均片剂粘附时间为15小时。
凝胶的平均血液浓度为0.035μg/ml,50mg片剂为0.020μg/ml,100mg片剂为0.013μg/ml。
局部和全身耐受性良好;优选生物粘附片剂,18名中有17名的耐受性良好(凝胶为18名中有1名耐受性良好)。
对于凝胶而言,气味被证实是自发调查表中特别重要的标准(18名中有13名认为气味较差,100mg的片剂为2/18,50mg的片剂为1/18)。
实施例6-体外/体内的相互关系
图4表示咪康唑的累积唾液量(μg/ml)在8小时内随体外溶解试验而改变。体外溶解试验按照横坐标表示,横坐标为溶解的咪康唑百分比与片剂中咪康唑百分比的比值。该图清楚地表明从片剂释放的每种物质都能在唾液中找到。吞咽被认为是持续的,全身流通为0。在曲线的第二部分,即斜率增加的曲线中,在唾液中测定到的咪康唑要多于从片剂中释放的。
这个结果特别重要。事实上,在没有全身流通的情况下,可支持这些结果的最可能假设为:活性成分是从口腔粘膜释放的。在第一阶段,吞咽会损失活性成分,同时活性成分在口腔粘膜中的储存。这种储存可通过由十二烷基硫酸盐形成的微胶粒中活性成分的平衡来解释,之后活性成分沉淀出来,并与粘膜粘连;然后随着唾液的分泌而释放。
然后,本发明的片剂不仅具有不全身流通的优点,而且口腔粘膜可为活性成分形成一个贮器。这一事实可解释为什么在24小时内一次服用50mg活性成分就非常充足,而Daktarin口腔凝胶每天则要以500mg/天的量给药。
实施例7-从本发明生物粘附系统产生的不同组合物
7.1
改变活性成分:
在下列条件下,使用其它活性成分重复实施例2中表II的组成1:

使用蔗糖替代乳糖:
在所有组合物中,每片0.39g的乳糖可用29.59g±3的蔗糖替代。
溶解试验和粘附性试验的结果相似。
7.3
迅速和延续释放片剂的生产:
当治疗需要相同活性成分的迅速释放层时,可将上述组合物包衣。
这些两层-迅速释放和延续释放片剂的几何形状并不重要,可使用技术人员已知的任何技术获得,只要能够保持本发明片剂的生物粘附量。
结论:
本发明提供一种制备方法和缓释生物粘附片剂,它可以24小时内次低于现有对照盖仑制剂形式近10倍的剂量给予活性成分。此外,溶解试验,即8小时内超过70%,可改变给药频率。对于咪康唑而言,不仅观察到较低的全身流通,而且似乎原位浓度可在较长时间内确保局部抗真菌作用。该形式长时间粘附,而且可较好耐受,气味和可接受性也得到改善,使得本发明在临床上有用并外延用于很多活性成分。
此外,本发明还适于其它粘膜粘附性微球或毫微球系统。
Claims (34)
1.一种延缓释放的生物粘附粘膜治疗系统,它含有至少一种活性成分,在8小时内具有超过70%的活性成分溶解百分比,含有占活性成分重量至少50%、占所述生物粘附治疗系统重量至少20%的天然蛋白,10%-20%的亲水性聚合物,压缩赋形剂,并含有3.5%-10%的碱金属烷基硫酸盐和0.1%-1%的一水合糖。
2.根据权利要求1所述的生物粘附治疗系统,其特征在于它是一种粘膜粘附片剂。
3.根据权利要求1或2所述的生物粘附治疗系统,其特征在于所述碱金属烷基硫酸盐是十二烷基硫酸钠或二乙基磺化琥珀酸盐。
4.根据权利要求3所述的片剂,其中所述烷基硫酸盐是占片剂中化合物总重量3.5%-10%,且优选4%-6%浓度的十二烷基硫酸钠。
5.根据权利要求1所述的生物粘附治疗系统,其中所述糖一水合物是乳糖一水合物或蔗糖。
6.根据权利要求1所述的片剂,其特征在于所述压缩赋形剂含有玉米淀粉。
7.根据前述权利要求任何一项所述的生物粘附治疗系统,其中一种活性成分是来源于广谱唑类家族的抗真菌剂,优选选自咪康唑,克霉唑,酮康唑,氟康唑,伊曲康唑,异康唑,益康唑,沙泊那唑,genaconazole,曲康唑,布康唑,噻康唑,奥昔康唑,必佛那唑,芬康唑,双醚康唑,舍他康唑和硫康唑。
8.根据权利要求7所述的生物粘附治疗系统,特别是片剂,其中所述唑类是以10-150mg/片,优选25-75mg,且更优选约50mg/生物粘附治疗系统的剂量存在的咪康唑。
9.根据权利要求7所述的生物粘附治疗系统,其中所述活性成分是与其它活性成分结合的广谱唑类,其它活性成分选自:
·具有不同菌谱的多烯型抗真菌剂;
·镇痛剂;
·唾液分泌促进剂;
·防腐剂;
·唾液代用品;
·抗炎剂(肾上腺皮质激素);
·抗生素;
·沙利度胺;
或其混合物。
10.根据权利要求9所述的生物粘附治疗系统,其中所述第二种活性成分是10-100mg/片,优选20-90mg/片剂量的多烯。
11.根据权利要求9所述的生物粘附治疗系统,它还含有0.1%-10%重量的麻醉剂和/或0.1%-10%重量的唾液分泌促进剂。
12.根据权利要求1或权利要求9所述的生物粘附治疗剂,其中所述活性成分是防腐剂。
13.根据权利要求12所述的生物粘附治疗系统,其中所述防腐剂是最小浓度3.5%重量的十二烷基硫酸钠。
14.根据权利要求1所述的生物粘附治疗系统,其中所述活性成分是对HSV病毒(疱疹),VZV(水痘带状疱疹病毒),Epstein-Barr病毒(传染性单核细胞增多症,多毛粘膜白斑病),巨细胞病毒有效的抗病毒剂,优选选自阿昔洛韦,伐昔洛韦和更昔洛韦。
15.根据权利要求14所述的生物粘附治疗系统,特别是片剂,其中所述活性成分是以20-100mg,优选50mg量存在的阿昔洛韦或伐昔洛韦,或者是以10-2000mg,优选500-1500mg量存在的齐多夫定和更昔洛韦。
16.根据权利要求1所述的生物粘附治疗系统,其中所述活性成分是抗HIV病毒(人免疫缺陷病毒)的抗病毒活性成分,优选齐多夫定(AZT)。
17.根据权利要求16所述的生物粘附治疗系统,特别是阴道用系统,其中所述活性成分以10-2000mg/生物粘附治疗系统,优选500-1500mg/生物粘附治疗系统的量存在。
18.根据权利要求1所述的生物粘附治疗系统,其中所述活性成分是来自类阿片家族的不溶性镇痛剂,优选芬太尼。
19.根据权利要求18所述的生物粘附治疗系统,其中所述芬太尼,优选不溶性碱,以50-1600微克/生物粘附治疗系统,优选200-1200微克/生物粘附治疗系统的量存在。
20.一种用于制备延缓释放的粘膜生物粘附治疗系统的方法,该系统含有至少一种活性成分,在8小时内该活性成分具有超过70%的溶解百分比,所述方法至少包括:
·将活性成分与占活性成分重量至少50%的天然蛋白、赋形剂和含有至少一种亲水性聚合物的填料混合的步骤;以及
·与占片剂重量3.5%-10%,优选4%-6%浓度的碱金属烷基硫酸盐混合的步骤。
21.根据权利要求20所述的方法,其中所述生物粘附治疗系统是一种片剂,所述方法包括下列步骤:
a)将活性成分与占活性成分重量至少50%的天然蛋白、赋形剂和含有至少一种亲水性聚合物的填料混合的步骤;以及
b)用一水合糖或多元醇润湿步骤a)所得混合物的步骤;
c)干燥混合物并筛分所得颗粒的步骤;
d)与占片剂重量3.5%-10%,优选4%-6%浓度的碱金属烷基硫酸盐混合的步骤。
22.根据权利要求21所述的方法,其中所述润湿步骤b)之后是在无醇条件下进行的成粒步骤。
23.根据权利要求21或权利要求22所述的方法,其中所述糖一水合物是浓度为0.1%-1%的乳糖一水合物。
24.根据权利要求21-23中任一项所述的方法,其中所述碱金属烷基硫酸盐是十二烷基硫酸钠或二乙基磺化琥珀酸钠。
25.根据权利要求21-23中任一项所述的方法,其中如果需要,可将所述活性成分与其它活性成分结合,其它活性成分选自:
·具有不同菌谱的多烯型抗真菌剂;
·镇痛剂;
·唾液分泌促进剂;
·防腐剂;
·唾液代用品;
·抗炎剂(肾上腺皮质激素);
·抗生素;
·沙利度胺;
或其混合物。
26.根据权利要求1-13任何一项所述的生物粘附治疗系统作为预防,治疗或维持粘膜,特别是口腔粘膜或阴道粘膜疾病的药物的用途。
27.根据权利要求26所述的用途,其中所述粘膜疾病是口腔粘膜的念珠菌感染,且所述生物粘附治疗系统含有唑类作为活性成分。
28.根据权利要求27所述的用途,其中所述唑类是以10-150mg/生物粘附治疗系统,优选25-75mg,且优选约50mg/生物粘附治疗系统的剂量存在的咪康唑。
29.根据权利要求26所述的用途,用于局部治疗口疮。
30.根据权利要求26所述的用途,用于局部治疗疼痛,特别是与癌症有关的剧烈疼痛,用于局部治疗炎症或感染,特别是与癌症有关的粘膜炎。
31.根据权利要求14-19中任一项所述的生物粘附治疗系统用于预防,治疗,或维持病毒感染的用途。
32.根据权利要求31所述的用途,其中所述活性成分是对HSV病毒(疱疹),VZV(水痘带状疱疹病毒),Epstein-Barr病毒(传染性单核细胞增多症,多毛粘膜白斑病),巨细胞病毒有效的抗病毒抗感染剂,它优选选自阿昔洛韦或伐昔洛韦,以20-100mg/片的量存在的阿昔洛韦或甲氧氯普胺,或以10-2000mg,优选500-1500mg的量存在的AZT。
33.根据权利要求31所述的用途,其中所述活性成分是有效抗HIV病毒(人免疫缺陷病毒)的抗病毒抗感染剂,优选齐多夫定(AZT),其中所述活性成分以10-2000mg/生物粘附治疗系统,优选500-1500mg/生物粘附治疗系统的量存在。
34.根据权利要求30所述的用途,其中所述活性成分是剂量为0.05-1.6mg/片的芬太尼,特别是不溶性芬太尼碱,用于治疗剧烈疼痛和抗性疼痛。
