技术领域

背景技术
自二十世纪八十年代早期以来,可调节胃带就为胃路和其它一些用于病态肥胖者的不可逆的外科减肥方式提供了一种有效的选择。该胃带环绕在患者胃部的上部周围,形成一比胃的正常内径要小的人造入口,以限制食物从胃的上部通过而进入胃下部的消化部分。当该人造入口的尺寸合适时,食物便停留在胃的上部区域以形成一种饱的感觉,这样便阻止了吃得过多。
除设定胃带的直径的锁定位置以外,胃带的可调节性一般都是通过一向内膨胀的可充气球来实现,与血压套相似,例如盐水的流体通过一流体注射通道注射到该可充气球中,以获得一希望的直径。该气球通常为可调节的或部分膨胀的,当其首次植入体内时要考虑到身体的调节且能使新束带的周围部分愈合。由于可调节胃带可能会留在病人体内很长一段时间,所以流体注射通道通常为皮下植入的,以避免感染,例如植入在胸骨前。在最初的植入之后,外科医生可根据病人的需要而将胃带放松或加紧。可通过将一休伯特(Huber)针尖通过皮肤插入注射通道的硅树脂隔膜中的方法来调节可调节胃带内的流体量。一旦将针头抽出后,隔膜就又会依靠其自身产生的压缩载荷而密封住针孔。一柔性的管道连接在注射通道和可调节的胃带之间。
一种用于可调节胃带的连接机构提供了胃部人造入口的初始尺寸。一种公知的连接就是可调节胃带的缝合端部。另一种公知的连接包括在一柔性管道中终止的胃带的一端,该柔性管道具有一扩展部分,该扩展部分穿过胃带第二端部中的通孔,然后缝合到环绕带部—确保束带附着在胃上。当该结构设置到合适的位置上之后,再将注射通道锚定在一方便的位置上。

发明内容
本发明通过提供一种可调节胃带装置解决了现有技术中的上述和其它一些问题,该束带可以较小的力量来加以连接,因此使得用手术器具来进行植入的操作更容易,而所述连接机构能安全使用很长的时间。
本发明的一个主要目的是提供一种可调节胃带,其中将第二端部插入第一端部时所需的力量较小,且通过一锁紧机构加以接合,使得要将这两端部分开所需的力量很大。
本发明的另一目的是提供一种容易可逆调节的胃带,可将其加紧或松开,而不会减小该连接机构的紧固力量。
本发明的另一目的是提供一种具有纵向力的可调节胃带,该纵向力通过连接机构传输,其相当大但不会导致分开,尽管结合力相当小。
为了取得上述和其它一些目的,且根据本发明此处所描述的目的,描述了一种具有相对面连接机构的可调节胃带,该连接机构具有一从其中一端部横向延伸出的构件,这一端部设置用以和另一端部接合。根据一个方面,该横向构件包括一用以接收另一端的一部分的通道。另一方面,该横向构件被另一端部中的一个通孔所接收。

具体实施方式
在下面的描述中,贯串于附图中,相同的附图标记代表相同或相应部件。而且,在下面的描述中,应该理解,像前方、后方、内部、外部和其它的类似术语都是为了方便的用词,不解释为限定术语。该申请中所采用的命名法并不意味着只将其限定为此处描述的装置或部件,而是可以固定或用于其它方面。下面将结合附图对本发明进行更加详细的描述。
参考图1,其示出了一种环绕在胃部12的上部周围的可调节胃带10,该胃带通过以下方式保持在适当的位置上,即将其两端连接在一起并使胃12的一部分14采取缝合到胃上的方式延伸越过可调节胃带10。参考图2,可调节胃带10包括一不可伸缩的衬带(strap)16和用医用硅树脂合成材料或任意其它合适的材料制成的且由衬带16的内表面20支撑的可充气球18。气球18可以任何已知的方式安装在内表面20上,或者甚至还可和衬带16做成一个整体结构,虽然衬带16通常可由一种不同材料制成。
柔性管道22的一端和气球18的内腔24以流体可以通过的方式连接在一起,另一端则和一远端的注射通道26的内腔(图中未示出)以流体可以通过的方式连接在一起。远端的注射通道26包括一硅树脂隔膜28。在将可调节胃带10植入到胃部周围时,也将远端的注射通道26植入到一个合适的位置上,通常是在直肌鞘膜中,以通过休伯特针头形成一个经皮的进入方式。
众所周知,内腔24、柔性管道22和远端注射通道26的内腔都优选为至少充有部分病理生理学的兼容流体,例如盐水溶液。手术后的由气球18闭合形成的周长的调节,以及因此而形成的人造入口的尺寸调节,都可采用将一休伯特针头经过皮肤插入所述注射通道18的硅树脂隔膜28中来从气球18的内腔24中增加或去掉一部分流体的方式来实现。
如本领域中已知的一样,可调节胃带10可由任何合适的医用兼容材料制成,该材料具有所需的足够强度能用于特殊的腹腔镜外科手术或特殊的病人。
如上所述,可调节胃带10的两端连接在一起(图1中未示出其详细的连接机构的结构)。本发明给出了多种用于将两端连接在一起的连接机构的实施例,其中需要用两步来将其两端连接在一起。图1和2所示的以及如前所述的可调节胃带10的普通结构与在图3-14中描述的实施例是相同的,各个实施例的不同点在于其具体连接机构。应该注意,可以采用各种束带来实施本发明,并非只局限于用一种具有如上所述或如下所述的精确特征的可调节胃带。
现在参考图3,可调节胃带30包括一细长衬带(elongated strap)32,其沿此处被参考作为纵向的方向延伸,即使植入的可调节胃带30具有一弧形的轮廓。衬带32包括一内表面34和一外表面36,气球38从相邻的内表面34向内延伸。可调节胃带30包括一与第二端部42交搭的第一端部40,在第一端部40处的可调节胃带30的内表面34设置成邻近并在第二端部42处的所述可调节胃带30的外表面36的外侧。
参考图4A,,所述第一和第二端部40、42借助于一个两级膨胀连接机构连接在一起。所述衬带32的第一端部40终止于一细长构件44中。该细长构件44包括一圆环形扩大部46,该部具有一通孔48。扩大部46具有切口部分50,该切口部分设置为器械可以夹持该扩大部46来进行所需的操作。
细长构件44包括两个扩大的、间隔分布的球形部分52a、52b,其设置在细长构件46的末端处,形成为一种杠铃形状,细长构件44的一个部分44a、44b分别在所述球形部分52a和52b之间以及在球形部分52b和扩大部46之间延伸。应当理解,所述部分52a和52b并不局限于是球形,可以是任何合适的形状。
至少一个横向延伸构件54从所述衬带32的第二端部42的外表面36上延伸而出,其设置为用以和第一端部40相配合。该横向延伸构件54限定了一个通道56,此处也称为通孔,第一端部40设置为通过该通道沿纵向延伸。通道56具有一和细长构件44的横截面形状和尺寸互补的横截面形状和尺寸。至少通道56的某一方向上的尺寸要小于扩大部46的相应尺寸,在详细实施例中,通道56在宽度和高度上都要小于扩大部的相应尺寸。通道56还被限定为其高度和宽度都比球形部分52a和52b小,尽管通道56可以选择为足够大以使球形部分52a和52b毫无阻碍地通过。
第二端部42的末端具有两个间隔分布的短小突出部58和60,其分别具有倾斜面58a、60a和横截表面58b、60b,从上部看时,它们一起形成槽62。第一端部40包括相应表面64a、64b、66a和66b,它们成形为当两端部40、42如下所述结合时与槽62互补并相配合。第二端部42还包括一在外表面36中形成的凹进部分68。该凹进部分68成型为与球形端部52a互补,且用以接收该部分,如下所述。
将所述两级连接机构连接在一起的第一步是,拉动包括球形部分52a、52b和扩大部46的细长部分44,使其通过通道56。虽然相对于部分44a、44b和球形部分52a、52b的通道56的尺寸可以允许细长构件44的末端通过所述通道56,但在所述实施例中,还是采用一种器械例如夹钳将细长构件44的末端拉入通过通道56,以结合至少球形部分52a。
参考图4B,已将球形部分52a、52b完全拉过通道56,扩大部46部分进入通道中。如图所示,该扩大部46和横向构件54在此第一步中发生弹性变形。
图4C示出了处于连接过程的第一步和第二步之间的两级连接机构的设置,扩大部46几乎已被完全拉过通道56,表面64a、64b、66a和66b和槽62得以结合,以提供横向稳定性。或者,可以这样来设置细长构件44的部分44c的尺寸,即使扩大部46完全留在通道56之外,但是优选为迫使表面64a、64b、66a和66b结合槽62。
将所述两级连接机构连接在一起的第二步是将第一端部40限定在适当的位置上,或者是与第二端部42接合在一起,或者是像本实施例一样和所述第一端部40接合在一起。参考图4C、4D和图5,球形部分52a穿过通孔48插入如图4D和图5所示的位置中,球形部分52a、52b的相应局部延伸通过上下表面46a、46b,球形部分52a的局部位于凹进部分68中。将通孔48的尺寸设定为这样,即结合扩大部46的弹性将球形部分52a、52b限定到图5所示的位置上。因此,球形部分52a、52b和部分44a包括一第一端部40的限定部,其和第一端部40的另一部分即通孔48配合连接。
在图5所示的实施例中,所述两级连接结构会借助于由所述第一端部40的限定部的横向方向以及由比通道56大的扩大部46所产生的纵向力,来防止两个端部40、42的脱离。
图6示出了具有两级连接机构的可调节胃带的另一种实施例。可调节胃带70包括一具有内表面74和外表面76的细长衬带72,气球78向内延伸。可调节胃带70包括一与第二端部82交搭的第一端部80,在第一端部80处的外表面76与第二端部82处的外表面76相邻设置。
参考图7,该图示出了处于分开状态的第一和第二端80、82,第一端80包括一横向通道84,此处也称为通孔,其设置为可使第二端部82插入通过其中。第一端部80还包括两个横向通孔86、88。第二端部82包括固定部件(retainingpart)90,该固定部件包括两个间隔分布的横向延伸构件92、94,其分别在各自的末端上具有一扩大部92a、94a,两个侧向通孔86,88的尺寸和形状设计成与构件92,94配合,并在扩大部92a、94a推过之后保持它们,如图6所示。
将所述两个端部80、82连接在一起的第一步是,将第二端部82通过横向构件94插入穿过通孔84。第二步是通过将固定部件90和第一端部80配合连接,将构件92、94插入到对应的通孔86、88中,将第二端部82固定在适当的位置上。应当理解,可以使用一个或两个以上的横向构件和相应的配合安装通孔,且可使用能保持彼此配合连接的不同形状。
图8示出了一具有两级连接机构的可调节胃带的另一实施例。图8中,只示出了安装连接在一起的第一和第二端部96、98,第一端部96处的内表面100设置为邻近第二端部98处的内表面100。第二端部98包括至少一个横向延伸的构件102,该构件设置用于连接第一端部96。该横向延伸构件102限定了一通道104,也称为通孔,第一端部96设置为纵向延伸通过该通道。
第二端部98包括一横向通孔106和一固定构件108。固定构件108如图所示和第二端部98是一整体结构,虽然其可单独形成且可以任何适当的方式加以连接。固定构件108的末端包括一只臂110,该臂终止于具有一扩张顶部114的销112。臂110及其和第二端部98的连接可允许该臂旋转移动销112,使其和通孔106对齐并进入该通孔。臂110和第二端部98之间的连接可以任何合适的可使臂110按如前所述的方式进行旋转的方式来设置,例如包括使用一活动铰链(living hinge)。
第一端部96的尺寸被设置为在纵向方向上延伸穿过通道104,如图所示。第一端部还包括一横向通孔116。
将所述两个端部96、98连接在一起的第一步是,将第二端部96插入穿过通孔104,使其插入到通孔116和通孔106对齐的位置上。第二步是通过移动固定构件108以使销112进入通孔106、116中的方式将第一端部96固定在适当的位置上,因此扩展(相对通孔116而言)顶部114能阻止销112从通孔116和相随的106通孔中向后脱出。
或者,固定构件108可以由第一端部96支撑,该第一端部延伸穿过构件102并延伸穿过通孔116、106,且扩展顶部114的尺寸设置为防止销112从通孔106和相随的通孔116中向后脱出。而且,可通过缩短第二端部98的方式而将通孔106省略,以清楚可见销112和扩展顶部114。可以在不带有固定件108的端部上形成多个纵向间隔分布的孔,以调整可调节胃带的尺寸。
图9A、9B和10示出了具有两级连接结构可调节胃带的另一种实施例。图9A中只示出了第一和第二端部118、120,两者按照所述第一步连接步骤部分连接在一起。第一端部118包括一细长构件122,其终止在固定件124中,该固定件具有一带有宽底边124a的箭头形状,导向边缘(leading edges)124b、124c从该底边处纵向延伸汇聚到箭头尖端124d处。如图10所示,底边124a具有一弧形的横向轮廓,表面124e、124f从该处延伸汇聚到尖端124d。虽然第一端部118上构件122延伸的部分被图解为弧形,但可以为任何合适的形状。
第二端部120包括一在其末端上形成的通孔126和一对间隔分布横向延伸构件128、130,其从通孔126往下间隔分布。每个横向延伸构件128、130分别包括一横向延伸部分128a、130a,其连接到纵向延伸构件128b上,该构件与可调节胃带的上表面132一起形成孔134、136。孔134、136设置为用以固定固定件124的底边124a,该孔可以包括具有向外表面132倾斜的下表面的纵向延伸部分128b、130b。
将两个端部118、120连接到一起的第一步是,将第一端118插入穿过通孔126,使其向前直到使边缘138和第二端部120邻近通孔126处的下表面140接触到一起为止,且第二端部120的末端142稍微地翘起,如图10所示。第二步是采用将固定件124和第二端部120配合安装在一起的方式将第一端部固定在适当位置上,将固定件124插入相应通孔134、136中,如图10所示定位底边124a。构件122的长度使末端142朝向于图中所示的弯曲位置。
应当指出,固定件124并不局限于箭头形状,而可以是任何能插入穿过第二端部中的通孔且能起到固定件作用的形状。
图11示出了具有两级连接结构的可调节胃带的另一实施例。图11中只示出了未连接在一起的第一和第二端部144、146。第一端部144终止于一横向延伸构件148,该构件借助于穿过活动铰链152的带150连接在第一端部144的终端处。一固定件154从该带部150处延伸出,与构件148成一定角度,如图11所示。固定件154包括一钥匙孔形状的通孔156。
第二端部146的末端158包括两个纵向延伸间隔分布的构件160、162,其限定出了两个间隔分布且互相对齐的通孔164、166。如图12A所示,通孔164、166被成型为与构件148互补配合,以便可旋转地容纳构件148,这两个孔具有相应的进口边168、170,其分别具有比构件148的相应尺寸稍小的尺寸。
第二端部146包括一横向延伸构件172,可将其描述为铆钉形状,其具有一头部174,该头部大于基底部176。构件172和通孔156设置用以彼此配合。
参考图12A,将两个端部144、146连接在一起的第一步是,将构件148插入通孔164、166中,这可以通过以下这种方法来实现,即使构件148和入口边168、170靠近并对其施加一足够大的纵向力以临时扩大入口直到构件148进入到其中合适的位置为止。图12A示出了两级连接机构
第二步是将固定件154和第二端部172连接在一起。通过使固定件154朝第二端部146移动使圆柱形构件148在通孔164、166中旋转,引起带150围绕活动铰链152转动。将头部174插入穿过通孔156的较大部分156a,且基底部176滑入较小的部分156b中,借此固定固定件154。
本领域技术人员应当理解的是,如上所述的本发明和其它一些类型的可植入带具有相同的适用性。例如,用于治疗排便失禁的束带。美国专利US6461292中描述了这样一种束带,其可以在此引入作为参考。束带还可用于治疗小便失禁。美国专利申请2003/0105385中就描述了这样一种束带,其可以在此引入作为参考。束带还可用于治疗胃痛或胃酸。美国专利申请US6470892中就描述了这样一种束带,其可以在此引入作为参考。束带还可用于治疗阳痿。美国专利申请2003/0114729中就描述了这样一种束带,其可以在此引入作为参考。
因此,正如此处和权利要求中所述的一样,一种可植入束带是一种可植入到适当位置以堵塞例如食物或体液流经生理通道例如胃或管腔的束带。
总之,已描述了使用本发明可带来的许多好处。出于解释和说明的目的,前面已给出了本发明的一个或多个实施例的说明。但并不意味着要将本发明限定在前面所披露的精确形式中。根据以上技术,做出明显的变化和改变是可能的。选择描述其中的一个或多个实施例是为了能更好地解释本发明的原理和其实际应用,以使本领域内的普通技术人员能在多种实施例中以及结合各种适合特殊用途的改进中来最好地实施本发明。本发明的范围由所附的权利要求来加以限定。
Claims (10)
1、一种用于医学治疗的可植入束带,该束带包括:
一设置用以环绕一生理通道的衬带,所述衬带限定出了一环绕在其上的圆周方向,所述衬带具有一内表面和一外表面;
分布在所述衬带两端处的第一和第二端部,所述第一和第二端部设置用以彼此连接,以贴近生理通道固定所述衬带,所述第一和第二端部分别包括内表面和外表面,其与所述衬带的内外表面相对应;
与所述衬带形成为整体的第一装置,其用于将所述第一端部连接到所述第二端部上;
与所述衬带形成为整体的第二装置,如果所述第一装置发生失效时,该第二装置用以保持第一端部连接在第二端部上。
2、如权利要求1的束带,其中所述第一装置包括一装置,当所述第一端部借助于所述第一装置连接到第二端部上时,该装置用以形成所述束带的最终圆周尺寸。
3、如权利要求2的束带,其中所述第一装置包括一分布在第一端部中的横向通孔,和第二端部的一部分,该部分具有比所述横向通孔的相应尺寸更大的尺寸,因此当所述第二端部穿过横向通孔时,所述第二端部的这一部分就可防止第二端部从横向通孔中脱出。
4、如权利要求3的束带,其中所述第二装置包括一从第二端部延伸出的固定件和一分布在第二端部中的横向通孔,该通孔用以容纳固定所述固定件。
5、如权利要求4的束带,其中所述分布在第二端部中的横向通孔具有钥匙孔结构,其包括一通孔头部和一通孔狭缝部,且其中所述固定件包括一从第二端部延伸出的茎部和一固定头部,所述固定头部设置为可插入所述通孔头部且不会从所述通孔狭缝部横向脱出,所述茎部设置为当所述固定头部插入通孔头部之后其可沿开孔狭缝部前进。
6、如权利要求1的束带,其中所述第二装置包括一分布在第二端部中的横向延伸通孔。
7、如权利要求6的束带,其中所述第二装置包括一分布在第一端部中的横向延伸通孔。
8、如权利要求6的束带,其中所述第一装置包括一从第一端部横向延伸出的横向构件,所述横向构件限定出了一设置用以接收第二端部的周向延伸通孔。
9、如权利要求8的束带,其中所述第二装置包括一分布在第一端部中的横向延伸通孔,和一由第一端部或第二端部支撑的构件,其设置用以当所述第二端部的横向延伸通孔和第一端部的横向延伸通孔对齐时,插入到这两个通孔中。
10、如权利要求9的束带,其中所述第二端部包括当第二端部设置在所述周向延伸的通孔中时,用以与所述横向构件邻接的台阶。
