Abstract

The invention relates to a preparation for transdermal administration of galanthamine, characterized in that galanthamine or one of its pharmaceutically acceptable salts is brought in a non-oil emulsion for transdermal absorption.

Description

The The present invention relates to a preparation in the form of a cream, a gel, a lotion, a suppository, an ointment or a transdermal therapeutic system (TTS) for transdermal administration the active substance galanthamine or one of its pharmaceutically active Salts for the treatment of Alzheimer's dementia.

The invention relates to a preparation which inhibits the permeation of the active ingredient galanthamine (4a, 5, 9, 10, 11, 12-hexahydro-3-methoxy-11-methyl-6H-benzofuro (3a, 3, 2-e, f) ( 2) benzazepine-6-ol) or one of its pharmaceutically-acceptable salts through the skin to such an extent as to enable the treatment of Alzheimer's dementia by transdermal administration of galanthamine. Due to its pharmacological properties, galanthamine belongs to the group of reversible cholinesterase inhibitors and, in addition to the inhibition of AChE, has a further mode of action with specific modulation of the nicotinic part of cholinergic neurotransmission. It is similar in its effects to physostigmine and neostigmine, but also has specific properties. Galanthamine has a 3 to 6 times greater therapeutic index, as it does not have the toxicity of physostigmine or neostigmine. This advantage outweighs its dose-covered somewhat lower cholinesterase inhibitory activity. The main applications of galanthamine are the treatment of narrow-angle glaucoma and its use as an antidote to curare applications. Galanthamine has also recently been used to treat alcohol dependence ( DE 40 10 079 C2 ) and nicotine dependence ( DE 43 01 782 C1 ) known.

The late nineties galanthamine was used under the trade name Reminyl ® in the form of tablets approved and for the treatment of Alzheimer's dementia. However, for the treatment of this disease, not the tablet or other oral dosage form is the medication of choice, but the Transdermal Therapeutic System (TTS). This was the reason that such TTSs were developed and patented (eg DE 43 01 783 ). Although TTSs according to this teaching are suitable for transdermal absorption of galanthamine, the dose administered is insufficient. The already Reminyl ®, mentioned possible by oral administration and three-time administration of tablets containing 8 mg galanthamine hydrobromide delaying Procreants of Alzheimer's disease. A daily dose of 24 mg galanthamine hydrobromide corresponds to about 22 mg galanthamine base. Although a TTS represents the preferred dosage form, it has still not been able to transdermal to bring galanthamine in the required amount for absorption. Since the skin is not a resorptive organ, but primarily has the function of preventing the ingress of foreign bodies, such a high dose is currently not transdermally absorbable with a TTS of acceptable size. There is therefore a need for formulations in the transdermal therapy, with which galanthamine can be administered in these daily doses of up to 22 mg per day, without having to cover in the application skin of 100 cm 2 and more.

Als Vorstufe von Acetylcholin ist Lezithin besonders geeignet, um somit die benötigte Menge an Galanthamin senken können zu.task The invention was therefore a preparation for transdermal application to provide galanthamine that meets these requirements becomes. This task comes in surprising solved in a simple manner with a preparation according to claim 1. These solution is even more amazing than by the gift of lecithin, oleic acid and Cholesterol had by no means been tried for transdermal absorption of galanthamine. The starting point of the development was rather that Alzheimer's dementia is known to be characterized by a deficiency of acetylcholine. The use of galanthamine for the treatment of Alzheimer's dementia follows the consideration, galanthamine inhibits acetylcholinesterase. The lack of Acetylcholine is to be compensated by the fact that the degradation of the Enzyme's acetylcholinesterase is inhibited. The invention was so the consideration underlying that one not only the lack of acetylcholine thereby compensates that one prevents its dismantling, but also that Reinforced acetylcholine is produced. This is achieved by adding lecithin is given. As a precursor of acetylcholine, lecithin is especially suitable, thus the required Can reduce the amount of galanthamine to.

This Not only has the particular benefit of the side effects of galanthamine to reduce, but allows Also, the now reduced dose of galanthamine transdermally by a Apply plaster system of acceptable size to be able to. In addition, the preparation is relatively easy and inexpensive to produce. The preparation according to the invention contains thus as essential ingredients lecithin and galanthamine. Based on the In the following example, the composition was under in vitro Conditions on excised human skin in those known in the art Franz's diffusion cell tested. Surprisingly, it turns out that one Combination of lecithin, oleic acid and Cholesterol is suitable to absorb galanthamine to an extent to bring about the achievement of 22 mg of galanthamine over the Skin is possible through a plaster system of acceptable size. The invention is explained by the following example:

Herstellung der transdermalen AnwendungsformProduction of the transdermal application form

A siliconized polyester film is coated with a solution of a pressure-sensitive adhesive medically acceptable polyacrylate in organic solvent in such a way that, after evaporation of the solvents, a surface weight of about 100 g / m 2 results. After removal of the solvents, the two-layer laminate consisting of siliconized polyester film and solvent-free pressure-sensitive adhesive layer is covered with a non-siliconized 15 μm thick polyester film. The three-layer laminate, consisting of siliconized polyester film, polyacrylate adhesive layer and non-siliconized polyester film, is cut into 6 cm wide rolls. To prepare a system for the transdermal administration of galanthamine, the siliconized polyester film is removed, and a sheet of 30 cm 2 tea towel paper is placed on the polyacrylate pressure-sensitive adhesive layer. About 500 mg of the described non-oil galanthamine-containing emulsion is pressed onto the round of tea filter paper. In order to produce a ready-to-use dosage form, the round plate is now covered with the preparation on a pressure-sensitive adhesive-coated carrier with a removable foil. Subsequently, the drug form is punched into a format that the Ronde is surmounted on all sides by pressure-sensitive adhesive. After removing the removable protective layer, the non-oil emulsion soaked Ronde is glued and fixed on the skin of a patient.

The preparation must be in one second step to patients with Alzheimer's dementia in a clinical Study to be tested. With this system, it should even be possible Galanthamine over 20 mg in 24 hours with patches of acceptable size for absorption to be able to bring.
