Description
Die vorliegende Erfindung betrifft chirurgische Vorrichtungen
zum Stabilisieren des Rückgrats, insbesondere Rückgratimplan
tate zum Zusammenschließen und Stabilisieren benachbarter
Rückenwirbel.
Chronische Rückenprobleme verursachen Schmerzen und Behinde
rungen für einen großen Teil der Bevölkerung. In vielen Fäl
len sind chronische Rückenprobleme auf eine Relativbewegung
zwischen Rückenwirbel und Wirbelsäule zurückzuführen. Die
Rückgratchirurgie beinhaltet Maßnahmen zur Stabilisierung be
nachbarter Rückenwirbel. Bekannte Stabilisierungstechniken
schließen das Zusammenschließen der Rückenwirbel mit ein.
Zusammenschließungstechniken beinhalten das Entfernen von
Scheibenmaterial, welches die Rückenwirbel trennt, und Ein
keilen von Knochen in das Scheibengebiet. Der eingekeilte
Knochen verschmilzt mit dem Knochenmaterial der Rückenwirbel,
wodurch die beiden Rückenwirbel miteinander zusammengeschlos
sen werden. Durch Weiterentwicklung des Standes der Technik
wurden Rückgratimplantate entwickelt, die die Wahrscheinlich
keit eines erfolgreichen Zusammenschließens verbessern. Ein
Beispiel für ein derartiges Rückgradimplantat wird in der US
5,489,308 offenbart, die ein mit einem Gewinde versehenes
Rückgratimplantat zeigt, in das Knochensplitter oder Knochen
schlamm plaziert werden können. Der Zylinder weist Öffnungen
auf, die sich radial zu diesem erstrecken. Das Knochenmateri
al wächst durch die Öffnungen, um mit dem Knochenmaterial der
Rückenwirbel zu verwachsen. Ähnliche mit Gewinden versehene
Implantate sind in der US 5,489,307; 5,268,953; 5,458,638 und
5,026,373 offenbart, deren Offenbarungsgehalt damit in die
vorliegende Beschreibung aufgenommen ist.
Die metallischen Oberflächen dieser Rückgratimplantate sind,
im Gegensatz zu den gewindeaufweisenden Strukturen mit großen
Poren, im wesentlichen glatt ohne Stützen, Mikrostützen oder
Oberflächenstruktur. Ohne derartige Oberflächenstruktur kön
nen die Rückgratimplantate sich in manchen Fällen nicht früh
zeitig mechanisch verankern oder in den benachbarten Knochen
im gewünschten Umfang befestigen und die Bildung von Faser
kapseln um das Implantat kann nicht wirkungsvoll minimiert
werden. Daher besteht ein Bedarf für ein Rückgratimplantat
mit Stützen, Mikrostützen, Finnen oder Oberflächenstruktur,
das auf diese Probleme gerichtet ist und Verbesserungen in
diesen Gebieten bietet; die vorliegende Erfindung ist ein
derartiges Implantat.
Ein Rückgratimplantat weist folgendes auf: Eine hohle, im
allgemeinen tubusförmige Schale mit einer äußeren Seitenflä
che, einem ersten Ende und einem zweiten Ende. Die Schale
weist ein schraubenförmiges Gewinde auf, das von deren Außen
fläche hervorsteht, wobei das Gewinde mehrere Sockel auf
weist. Jeder Sockel hat eine Höhe von 100-4500 µm und eine
Seitenausdehnung an der breitesten Stelle von 100-4500 µm.
Die Außenfläche weist mehrere Öffnungen auf, um Knochenwachs
tum dort hindurch zu erlauben. Alternativ können mehrere Soc
kel von der Außenfläche der Schale in einer regelmäßigen
nicht schraubenförmigen Aufstellung hervorstehen.
Die vorliegende Erfindung wird im folgenden anhand von Aus
führungsbeispielen im Zusammenhang mit den beigefügten Zeich
nungen näher erläutert. Darin zeigen:

Generell werden die gleichen Bezugsziffern für gleiche Teile
verwendet, die in verschiedenen Figuren erscheinen. In Fig.
1-2 ist eine Rückgradimplantat 10 aus einem konventionellen
Material, wie beispielsweise Titan oder einem anderen Metall
oder Metallegierung mit Gewinde oder Gewindesegmenten 12 dar
gestellt. Das Rückgratimplantat 10 weist vorzugsweise nur ein
Gewinde auf, das sich schraubenförmig um das Implantat ca.
achtmal schraubt, ein einfaches spiral- oder schraubenförmi
ges Gewinde mit einer Steigung von zwölf Gewindegängen pro
Zoll, wie es aus dem Stand der Technik bekannt ist. Das Ge
winde 12 schraubt sich schraubenförmig um das Implantat 10 in
bekannter Weise, es können aber auch mehrere miteinander ver
flochtene schraubenförmige Gewinde vorgesehen sein. Räume
oder Täler 15 sind zwischen den Gewindegängen vorgesehen,
vorzugsweise ist, wie aus dem Stand der Technik bekannt, le
diglich ein einziges Tal 15 vorgesehen, das sich schrauben
förmig um das Implantat ca. achtmal schraubt und das durch
die Gewindegänge des schraubenförmigen Gewindes definiert
ist. Das Tal 15 kann aber auch ein Flachgewinde anstelle ei
nes muldenförmigen sein, so daß dort keine Senkung ist, so
daß eine flache Oberfläche die Basisfläche 32 eines Gewindes
mit der Basisfläche 32 eines benachbarten Gewindes verbindet.
Ein Endabschnitt 14 des Gewindes ist an einem Ende des Im
plantats dargestellt. Das Implantat 10 weist eine hohle, im
allgemeinen tubulare (röhrenförmige), vorzugsweise im allge
meinen zylindrische Schale auf, wie sie in Fig. 1-2 gezeigt
ist, und weist eine äußere Seitenfläche mit einem schrauben
förmigen Gewinde 12 auf, das, wie in Fig. 1 gezeigt, davon
hervorsteht. Der Schnitt der zylindrischen Schale des Implan
tats 10 ist vorzugsweise kreisförmig, wie aus dem Stand der
Technik bekannt. Er kann aber auch, wie in Fig. 23 gezeigt,
oval sein, oder aber rechteckig oder rechteckig mit abgerun
deten Enden (wie ein zusammengedrücktes Oval), wie in Fig. 24
gezeigt. Das Implantat 10 hat ein vorderes oder erstes Ende 7
und ein hinteres oder zweites Ende 8. Die schraubenförmige
Ausbildung des Gewindes 12 erlaubt es dem Gewinde 12, mit
Rückenwirbelmaterial in Eingriff gebracht zu werden, wenn das
Implantat (mit dem vorderen Ende 7 den Weg bahnend) einge
schraubt wird und das Implantat in das Rückgrat hineinzuzie
hen. Das Implantat hat eine Längsachse entlang der Mitte des
Zylinders und das Gewinde 12 ist schraubenförmig um diese
Achse orientiert.
Wie aus dem Stand der Technik bekannt, ist ein Rückgratim
plantat im allgemeinen ein hohler, ein Gewinde aufweisender
Zylinder, der aus einer Titanlegierung hergestellt ist, und
ca. 12-18 mm Durchmesser und 20-80 mm Länge aufweist. Polye
thylen oder Kunststoffkappen können an den Enden des Implan
tats verwendet werden, um das gefangene (Käfig) Material innen
zu behalten. In dem zylindrischen Rückgratimplantat 10 sind
eine erste innere Kammer 16, eine zweite innere Kammer 18 und
Verstärkungsrippen 20, 24, 26 angeordnet. Die Öffnungen 22
sind radial durch die Seitenfläche des Implantats vorgesehen,
um Knochenwachstum dort hindurch zu ermöglichen. Größere Öff
nungen als die in Fig. 1 gezeigten, können auch vorgesehen
sein, wie aus dem Stand der Technik bekannt (siehe US
5,489,307, deren Offenbarungsgehalt hiermit als Bestandteil
aufgenommen werden soll). Die Gewinde 12 weisen mehrere Soc
kel oder Mikrosockel 6 auf. Vorzugsweise die Gesamtheit des
Gewindes 12 oder alle Gewindegänge des Gewindes 12 weisen
Sockel auf, es können aber auch mindestens 75%, 50% oder 25%
des Gewindes 12 Sockel aufweisen. Sockel, wie sie in den An
sprüchen verwendet werden, können zylindrische Sockel, qua
dratische Sockel, rechteckige Sockel und sichelförmige Sockel
sein.
In Fig. 3-4 ist ein Detail der Oberfläche des Implantats 10
gezeigt, das Sockel 6 zeigt, die sich von der Basisfläche 32
des Gewindes 12 erstrecken, wobei die Gewinde durch Täler 15
getrennt sind. Jeder Sockel 6 in Fig. 3-4 ist im wesentlichen
ein umgekehrter Kegelstumpf, wobei jeder Sockel 6 eine Kegel
stumpfseitenfläche 28 und eine runde Oberfläche 30 aufweist.
Wie in Fig. 4 gezeigt, ist die Seitenfläche 28 des Sockels 6
hinterschnitten mit einem Hinterschneidungswinkel A-A der
vorzugsweise 10° bis 20° beträgt.
In Fig. 5-6 ist eine alternative Ausführungsform gezeigt, die
einen quadratischen, vierseitigen Pyramidenstumpf als Sockel
34 aufweist, der sich von der Basisfläche 32 zwischen den Tä
lern 15 erstreckt, wobei jeder Sockel 34 mit einem Hinter
schneidungswinkel C-C vorzugsweise 10° bis 20° an allen vier
Seiten hinterschnitten ist, um vier Seitenflächen 36 zu erge
ben, wobei der Sockel eine quadratische Oberseite 38 auf
weist. Die Basisfläche 32 ist typischerweise ca. 900 µm
breit.
In Fig. 7 ist eine alternative Ausführungsform gezeigt, worin
sich von der Basisfläche 40 erstreckende Sockel 46, die wie
oben beschrieben, zylindrisch oder quadratisch sein können,
eine Oberfläche 44 aufweisen und, wie oben beschreiben, hin
terschnitten sind, was Seitenflächen 42 ergibt.
In Fig. 8 ist eine alternative Ausführungsform ähnlich Fig. 7
gezeigt, allerdings wobei jeder Sockel in Fig. 7 durch ein
Sockelpaar ersetzt ist. Die Sockel 48, 50, die sich von der
Basisfläche 56 erstrecken, sind die gleichen und sind neben
einander (alternativ versetzt) angeordnet, wobei jeder Sockel
quadratisch oder kreisförmig mit einer Oberseite 54 ist und
hinterschnitten, wie oben beschrieben, um eine Seitenfläche
52 zu ergeben. Wie in Fig. 8 gezeigt, definiert das Sockel
paar 48, 50 eine Linie, die quer zur Gewinderichtung ausge
richtet ist, d. h. die Schraubenlinie, die durch das Schrau
bengewinde definiert ist.
Fig. 9 ist eine alternative Ausführungsform ähnlich Fig. 8,
wobei die Sockel 58, 60, die in der Draufsicht kreisförmig
oder quadratisch sind und gleich und nebeneinander angeordnet
sind, sich von der Basisfläche 62 erstrecken, eine Oberfläche
66 aufweisen und hinterschnitten sind, wie oben beschrieben,
um eine Seitenfläche 64 zu ergeben.
Fig. 10 zeigt eine alternative Ausführungsform identisch zu
Fig. 7, außer daß ein Sockel 70 in jedem Sockel 68 benachbar
ten Tal angeordnet ist. Jeder Sockel 70 weist dabei die glei
chen Dimensionen wie Sockel 48 auf.
Fig. 11 zeigt eine alternative Ausführungsform ähnlich zu
Fig. 10, wobei Sockel 72 gleich den Sockeln 70 sind, wobei
aber die Gewinde 74, 76 massiv ohne Sockel sind, wodurch sie
das Erscheinungsbild eines konventionellen Gewindes aufweisen
und eine schräge Seitenfläche mit einem Winkel von 1° bis
10°, vorzugsweise ca. 5°, aufweisen, wie bei K-K gezeigt.
Fig. 12 ist eine Ansicht ähnlich Fig. 5, die eine alternative
Ausführungsform zeigt, wobei regelmäßig angeordnete Sockel
oder Finnen 78 rechteckig in der Draufsicht sind und sich von
der Basisfläche 79 erstrecken, um Gewinde zu formen, die
durch Täler 15 getrennt sind. Alternativ kann jeder zweite
Sockel 78 um 90° gedreht sein, so daß er um 90° quer und
nicht längs ausgerichtet ist.
Fig. 13 ist eine alternative Ausführungsform ähnlich Fig. 12,
außer daß regelmäßig angeordnete rechteckige Sockel 80, die
sich von der Basisfläche 81 erstrecken, quer und nicht längs
ausgerichtet sind.
Fig. 14 ist eine alternative Ausführungsform ähnlich Fig. 13,
außer daß jeder Sockel 82, der sich von der Basisfläche 83
erstreckt, versetzt zwischen den Sockeln in den benachbarten
Gewindegängen angeordnet ist und nicht direkt gegenüber von
den Sockeln in den benachbarten Gewindegängen.
Fig. 15 zeigt eine alternative Ausführungsform ähnlich Fig.
13, wobei jeder Sockel 84a, 84b und 84c, die sich von der Ba
sisfläche 85 erstrecken, winklig in Bezug auf die Längsachse
des Gewindes, auf dem er plaziert ist, angeordnet ist, wobei
der Winkel zwischen 30° bis 60°, vorzugsweise 45° beträgt und
wobei winklig die Bedeutung von anders als 0°, 90° und 180°
hat. Da das schraubenförmige Gewinde 12 sich schraubenförmig
um das Implantat 10 fortsetzt, bewegt es sich 360° jedesmal
oder jeden Gewindegang um das Implantat. Alle 360° oder eine
Gewindesteigung um das Implantat ist der Winkel der Sockel um
90° gedreht, wie in Fig. 15 gezeigt, so daß der Winkel der
Sockel in benachbarten Gewindegängen alterniert, um eine
Fischgrätenmusteranordnung zu ergeben, wie in Fig. 15 ge
zeigt. Die Öffnungen 71a, 71b sind in Tälern 15a, 15c vorge
sehen, um Knochensplitterplatzierung oder Ablagerung dort
hindurch zu erlauben, wie weiter unten beschrieben, und nach
folgendes Knochenwachstum dort hindurch zu erlauben. Wenn das
Implantat in Position in das Rückgrad des Patienten ge
schraubt wird, wird jede Finne oder jeder Sockel wie ein
kleines Schneepflugblatt, das Schnee zu der Straßenseite
schiebt, d. h., wenn das Implantat eingeschraubt wird, rasiert
jede Finne zusätzlichen Knochen von dem benachbarten Knochen
gewebe und drückt ihn in eine bestimmte Richtung. Falls das
in Fig. 15 gezeigte Implantatsegment nach unten gerecht wird
(korrespondierend zu dem eingeschraubten Implantat), dann
werden Finnen 84a Knochenspäne oder Fragmente abrasieren und
in Richtung der Öffnungen 71a drücken. Finnen 84b werden Kno
chenfragmente abrasieren und sie in die entgegengesetzte
Richtung auf die Öffnungen 71b zuschieben. Finnen 84c werden
Knochenfragmente abrasieren und auch in Richtung der Öffnun
gen 71b drücken. Auf diese Weise werden die Öffnungen in je
dem zweiten Tal angeordnet, nicht in jedem Tal, wobei die
Öffnungen in den Tälern angeordnet sind, die die Knochen
fragmente gedrückt werden. Es ist anzumerken, daß die Täler
15b und 15d keine Öffnungen haben und die alternierenden Tä
ler 15a und 15c Öffnungen aufweisen. Die Finnen können in
Richtung der Öffnungen stärker gekippt oder unterschnitten
sein oder mit einer konkaven Oberfläche an den Seiten oder
Abschnitten, die in Richtung der Öffnungen weisen, versehen
sein, um das Herunterdrücken der Knochenfragmente in Richtung
der Öffnungen zu unterstützen. Auf diese Weise wird zusätzli
ches knochiges Material in die Öffnungen und in das Implantat
heruntergetrieben, wenn es eingeschraubt wird, oder zumindest
in Richtung oder angrenzend an die Öffnungen getrieben, um
Knochenwachstum durch die Öffnungen zu erleichtern. Diese
Konzepte werden weiter durch Fig. 22 dargestellt, wo ein bo
gen- oder sichelförmiger Sockel oder Finne 100 mit Enden 101
und 102 in der Draufsicht gezeigt ist. Diese Finnen können
gegen die in Fig. 15 gezeigten Finnen ausgetauscht werden und
in einem Winkel ausgerichtet werden, um Knochenfragmente ab
zukratzen oder abzurasieren und in Richtung der Öffnungen
71a, 71b auszurichten oder zu drücken. Die Enden 101, 102
können alternativ flach sein, eine stumpfe Spitze oder einen
kleinen oder großen Kurvenradius aufweisen. Um wirkungsvoller
zu sein können diese Finnen zum Abkratzen weiter voneinander
entfernt beabstandet sein und insbesondere an dem vorderen
Ende 7 des Implantats 10 angeordnet sein. Alternativ kann der
Winkel der Sockel konstant über das Implantat angeordnet sein
und nicht periodisch verdreht, um ein Fischgrätenmuster zu
bilden, wie das in Fig. 15 dargestellte Fischgrätenmuster.
Die Sockel können versetzt oder nicht versetzt mit Bezug auf
die Sockel in benachbarten Gewindegängen angeordnet sein. Al
ternativ können die Öffnungen in allen Tälern oder versetzt
oder ganz weggelassen sein.
Fig. 16 zeigt eine alternative Ausführungsform ähnlich Fig.
15, außer daß jeder Sockel 86, der sich von der Basisfläche
87 erstreckt, um 90° gegenüber der Ausrichtung des im Gewinde
davor liegenden Sockels verdreht ist. Die Sockel 86 können
versetzt oder nicht versetzt in Bezug auf die Sockel in be
nachbarten Gewindegängen angeordnet sein.
Jegliche Kombination der obenbeschriebenen Anordnungen kann
auch vorgesehen werden.
Die Sockel haben vorzugsweise oder alternativ eine Oberseite,
die etwas gerundet und nicht flach ist und keine scharfen
Kanten aufweist, die ein Potential für Gewebeabsterben
(tissue necrosis) bilden können. Dies kann durch Sandstrahlen
der Sockel nach der Herstellung oder durch chemische Behand
lung derselben erzielt werden, um die Kanten zu entfernen.
Diese Ausführungsform ist in Fig. 8A dargestellt, welche Fig.
4 entspricht, außer daß jeder Sockel 6a eine Oberseite 30a
aufweist, die etwas abgerundet ist und bei der die scharfen
Kanten dort, wo die Oberseite 30a die Seitenfläche 28a
trifft, entfernt worden sind, wobei diese Änderungen durch
Sandstrahlen oder chemische Behandlung erreicht worden sind.
Dieses Merkmal ist ferner in Fig. 11A dargestellt, die Fig. 5
entspricht, außer daß die obere Hälfte jedes Sockels 34 ent
fernt worden ist, um die weggeschnittene obere Fläche 38a zu
zeigen und um darzustellen, daß jede der vier Seitenkanten 39
des Sockels 34 abgerundet ist (quasi durch einen Kurvenradius
ersetzt) und die scharfen Kanten beispielsweise durch Sand
strahlen oder chemische Behandlung entfernt worden sind. Fer
ner kann ein Kurvenradius oder ein Band an oder um die Basis
jedes Sockels vorgesehen sein, wo jener mit der Oberfläche
verbunden oder an dieser befestigt ist.
In den Figuren betragen die Dimensionen B-B in Fig. 4 vor
zugsweise 1000 µm und die folgenden Dimensionen sind vorzugs
weise die folgenden Längen: D-D ca. 1000 µm; E-E ca. 500 µm;
G-G ca. 500 µm; J-J ca. 500 µm. Die Höhe wird von der Basis
fläche zu der Oberseite gemessen, die Breite wird an der
Oberseite gemessen (die Breite eines Kreises entspricht dem
Durchmesser), und der Kantenabstand wird an der Oberseite ge
messen. In Fig. 8 ist der Kantenabstand F-F vorzugsweise 500
µm und in Fig. 9 beträgt der Kantenabstand H-H vorzugsweise
250 µm. In Fig. 3-6 weisen die Sockel vorzugsweise eine Brei
te von 1000 µm auf. In Fig. 7 beträgt die Sockelbreite 500 µm
und in Fig. 8-9 beträgt die jeweilige Sockelbreite 250 µm.
Die Winkel A-A und C-C betragen zwischen 5° bis 30°, vorzugs
weise 10° bis 20°. Sie können aber auch 0° betragen. Alterna
tiv können die Sockel am Boden breiter sein als an der Spit
ze, so daß die Seitenflächen um einen Betrag von 1° bis 30°,
vorzugsweise 1° bis 10° und noch bevorzugter 1° bis 5° nach
außen geneigt sind, wenn sie sich der Basis nähern. Die Täler
15 sind typischerweise 250-3000 µms vorzugsweise 500-2000 µms
noch bevorzugter 1000 µm breit.
Die Sockel an den Außenflächen des Rückgratimplantats helfen
frühzeitig eine wirkungsvollere mechanische Verankerung des
Implantats zu erzielen und reduzieren, minimieren oder unter
brechen wirkungsvoller die Bildung von Faserkapseln um das
Implantat.
Die Sockel des Implantats sind vorzugsweise umgekehrte Kegel
stumpfe oder Pyramidenstümpfe, wie dargestellt, vorzugsweise
mit den obengenannten Abmessungen. In Fig. 12-16 haben die
Sockel oder Finnen eine rechteckige Oberseite mit einer Brei
te von 100 bis 1000 µms vorzugsweise 200 bis 950 µms noch be
vorzugter 300 bis 900 µms noch bevorzugter 400 bis 825 µms
noch bevorzugter 500 bis 750 µms weniger bevorzugt ca. 250
µms weniger bevorzugt 200 bis 250 µms weniger bevorzugt 150
bis 300 µm und eine Länge von 400 bis 4500 µms bevorzugt 500
bis 3000 µms noch bevorzugter 600 bis 2000 µms noch bevorzug
ter 700 bis 1750 µms noch bevorzugter 1000 bis 1500 µm (wobei
die Länge größer als die Breite ist und vorzugsweise 2- bis
10mal die Breite, bevorzugt 2- bis 6mal die Breite, noch be
vorzugter 2- bis 4mal die Breite ist); darüber hinaus ent
sprechen diese Sockel von den Dimensionen den oben beschrie
benen Sockeln.
Die Dimensionen oder Abstände der hierin beschriebenen Sockel
können innerhalb eines Bereichs +50%, vorzugsweise +40%,
noch bevorzugter +30%, noch bevorzugter +20%, noch bevor
zugter +10%, am bevorzugtesten +5% liegen.
Die Höhe der Sockel beträgt vorzugsweise 100 bis 4500 µms
vorzugsweise 100 bis 2500 µm, noch bevorzugter 150 bis 2000
µm, noch bevorzugter 200 bis 1500 µms noch bevorzugter 400
bis 1200 µms am bevorzugtesten 500 bis 1000 µms alternativ
ca. 500 oder 1000 µm. Die Breite der Sockel beträgt vorzugs
weise 100 bis 4500 µms noch bevorzugter 100 bis 3000 µms
noch bevorzugter 150 bis 2000 µms noch bevorzugter 200 bis
1500 µms am bevorzugtesten 250 bis 1000 µm. Dort, wo ein Soc
kelpaar im gleichen Gewinde ist, wie in Fig. 8, beträgt der
Kantenabstand vorzugsweise 100 bis 3000 µms noch bevorzugter
100 bis 2000 µms noch bevorzugter 130 bis 1000, noch bevor
zugter 160 bis 800 µms noch bevorzugter 200 bis 600 µms am
bevorzugtesten 250 bis 500 µm. Die Querabmessung am breite
sten Punkt jedes Sockels (für kreisförmige und quadratische
Sockel die Breite und für rechteckige Sockel die Länge) ist
vorzugsweise 100 bis 4500 µms noch bevorzugter 150 bis 2500
µms am bevorzugtesten 200 bis 1500 µm.
Zwei benachbarte Sockel im selben Gewinde (d. h. ohne Überque
rung eines Tals), wie beispielsweise in Fig. 1, 18 und 19,
weisen vorzugsweise einen Kantenabstand auf, der der Breite
des Sockels entspricht oder alternativ 100 bis 4500 µm, vor
zugsweise 100 bis 2000 µms am bevorzugtesten 250 bis 1000 µm
oder 250, 500 oder 1000 µm beträgt. Für Finnen oder rechtec
kige Sockel, wie in Fig. 12 bis 16, beträgt der Kantenabstand
vorzugsweise ca. die Breite des Sockels oder 100 bis 4500 µms
noch bevorzugter 100 bis 2500 µm, am bevorzugtesten 250 bis
1000 µm oder, wie in Fig. 12 bis 16 gezeigt, 250, 500 oder
1000 µm.
Außerdem können Sockel, wie oben beschrieben, an der Innen
seite des Implantats angeordnet sein (um zu helfen, den Kno
chen an seinem Platz zu halten), aber vorzugsweise ohne Hin
terschneidungen.
Die Metalloberflächen des Implantats können optional mit
hydroxybevorzugenden (hydroxy appetite), knochenmorphogeni
schen Proteinen (bone morphogenic protein) oder anderen Kno
chenreaktionen hervorrufenden Mitteln beschichtet sein.
Herkömmliche Rückgradimplantate werden mit großen Öffnungen
vorgesehen, wie z. B. bei 22 in Fig. 1 bis 2 für dort hin
durchgehendes Knochenwachstum. Die vorliegende Erfindung bie
tet auch mehrere kleinere wirkungsvollere Öffnungen zum Kno
chenwachstum dort hindurch.
Fig. 17 ist eine alternative Ausführungsform ähnlich Fig. 5
und 12, die Täler 15, Sockel 88, die sich von der Basisfläche
32 erstrecken, und Öffnungen 90 zeigt, die zwischen den Soc
keln 88 vorgesehen sind.
Fig. 18 ist eine alternative Ausführungsform ähnlich Fig. 17,
die Sockel 91 auf den Gewindegängen und Öffnungen 92 in den
Tälern 15 zeigt.
Fig. 19 ist eine alternative Ausführungsform ähnlich Fig. 18,
die Sockel 93 auf den Gewindegängen und Finnenöffnungen oder
rechteckige Öffnungen 94 in den Tälern 15 zeigt.
Fig. 20 ist eine alternative Ausführungsform, bei der die zy
lindrische Fläche des Rückgratimplantats 10 eine glatte, kein
Gewindegänge aufweisende Oberfläche 95 hat, von der die Soc
kel 96, die regelmäßig und nicht schraubenförmig angeordnet
sind, direkt hervorstehen und die Öffnungen 97 zwischen den
Sockeln 96 aufweist.
Fig. 21 ist eine alternative Ausführungsform ähnlich Fig. 20,
die außerdem eine glatte, kein Gewinde aufweisende Fläche 89
hat, von der Finnen oder rechteckige Sockel 98 direkt vorste
hen, die in einem nicht schraubenförmigen fischgräten- oder
fischgrätenartigen Muster angeordnet sind, mit hier und da
eingestreuten Öffnungen 99.
In Fig. 17 bis 21 können die Sockel sämtliche der oben be
schriebenen sein. Die runden Öffnungen gehen durch die Wand
des Implantats in das Innere der Kammer und sind vorzugsweise
150 bis 1500 Mikrometer, noch bevorzugter 200 bis 1250 µms
noch bevorzugter 250 bis 1000 µms alternativ 375 bis 750 µms
alternativ 450 bis 575 µms alternativ ca. 500 µm im Durchmes
ser, wobei 250 µms 500 µm und 1000 µm typische Öffnungsdurch
messer sind. Die rechteckigen Öffnungen 94 haben die gleichen
Breiten- und Längendimensionen wie die Breiten- und Längendi
mensionen der Sockel oder Finnen in Fig. 12 bis 16. Rechtec
kige Öffnungen können durch runde Öffnungen und umgekehrt er
setzt werden. Rechteckige Öffnungen können in Längs- oder
Querrichtung oder in einem Winkel oder in einer Kombination
dieser angeordnet sein, wie beispielsweise alternierend längs
und quer ausgerichtet oder wie die Finnen in Fig. 12 bis 16
ausgerichtet. Die Öffnungen können versetzt oder nicht ver
setzt in Bezug aufeinander und mit Bezug auf die Sockel ange
ordnet sein. Die Sockel können versetzt oder nicht versetzt
mit Bezug aufeinander oder mit Bezug auf die Öffnungen ausge
richtet sein. Jede der vorgenannten Anordnungen kann an ein
und demselben Implantat kombiniert werden, wie beispielsweise
die Öffnungen in Fig. 17 und 18 in dem gleichen Implantat an
geordnet sein können.
Obwohl die bevorzugten Ausführungsformen der Erfindung darge
stellt und beschrieben wurden, stellen diese keine Einschrän
kung des Umfangs der offenbarten und beanspruchten Erfindung
dar.
Claims (19)
1. Rückgratimplantat mit einer hohlen im allgemeinen tubu
laren Schale mit einer äußeren Seitenfläche, einem er
sten Ende und einem zweiten Ende, wobei die Schale ein
schraubenförmiges Gewinde aufweist, das von der äußeren
Fläche hervorsteht, wobei das Gewinde mehrere Sockel
aufweist, wobei jeder Sockel eine Höhe von 100 bis 4500
µm und eine seitliche Abmessung am breitesten Punkt von
100 bis 4500 µm aufweist, wobei die Außenfläche mehrere
Öffnungen aufweist, um Knochenwachstum dort hindurch zu
ermöglichen.
2. Rückgratimplantat nach Anspruch 1, wobei jeder Sockel im
wesentlichen kreisförmig oder im wesentlichen quadra
tisch in der Draufsicht ist und eine Höhe von 150 bis
2000 µms eine Breite von 200 bis 1500 µm aufweist und
wobei benachbarte Sockel in dem gleichen Gewinde einen
Kantenabstand von 100 bis 4.500 µm aufweisen.
3. Rückgratimplantat nach Anspruch 1, wobei jeder Sockel im
wesentlichen rechteckig in der Draufsicht ist und eine
Breite von 100 bis 1000 µm und eine Länge von 400 bis
4500 µm und eine Höhe von 150 bis 2000 µm aufweist und
wobei benachbarte Sockel in dem gleichen Gewinde einen
Kantenabstand von 100 bis 4500 µm aufweisen.
4. Rückgratimplantat nach Anspruch 1, wobei mehrere dieser
Sockel eine Seitenfläche aufweisen, die unter einem Hin
terschneidungswinkel von 5° bis 30° hinterschnitten ist.
5. Rückgratimplantat nach Anspruch 1, wobei das Gewinde
mehrere Sockelpaare aufweist, wobei jedes Sockelpaar
zwei nebeneinander angeordnete Sockel aufweist, wobei
jedes Paar eine Linie definiert, die quer zu der Rich
tung des Gewindes ausgerichtet ist.
6. Rückgratimplantat nach Anspruch 1, wobei jeder Sockel im
wesentlichen keine scharfen Kanten aufweist.
7. Rückgratimplantat nach Anspruch 1, wobei das schrauben
förmige Gewinde ein schraubenförmiges Tal zwischen den
Gewindegängen des Gewindes definiert, wobei mehrere Soc
kel aus dem Tal hervorstehen.
8. Rückgratimplantat nach Anspruch 3, wobei mindestens ei
nige der rechteckigen Sockel quer zu dem Gewinde ausge
richtet sind.
9. Rückgratimplantat nach Anspruch 3, wobei das Gewinde ei
ne Längsachse aufweist und wobei zumindest einige der
rechteckigen Sockel winklig zu der Längsachse des Gewin
des angeordnet sind.
10. Rückgratimplantat nach Anspruch 3, wobei die rechtecki
gen Sockel in einem Fischgrätenmuster abgeordnet sind.
11. Rückgratimplantat nach Anspruch 10, wobei das schrauben
förmige Gewinde ein schraubenförmiges Tal zwischen den
Gewindegängen des Gewindes definiert und wobei mehrere
Öffnungen durch die Schale in dem Tal vorgesehen sind.
12. Rückgratimplantat nach Anspruch 1, wobei eine Öffnung
durch die Schale zwischen jedem Sockelpaar vorgesehen
ist, die in dem Gewinde aneinander angrenzen.
13. Rückgratimplantat nach Anspruch 1, wobei das schrauben
förmige Gewinde ein schraubenförmiges Tal zwischen den
Gewindegängen des Gewindes definiert und wobei mehrere
Öffnungen durch die Schale in dem Tal vorgesehen sind.
14. Rückgratimplantat nach Anspruch 1, wobei das Implantat
mit einem Knochenreaktionen hervorrufenden Mittel über
schichtet ist.
15. Rückgratimplantat nach Anspruch 1, wobei die im allge
meinen tubulare Schale zylindrisch ist.
16. Rückgratimplantat mit einer hohlen, im allgemeinen tubu
laren Schale mit einer äußeren Seitenfläche, einem er
sten Ende und einem zweiten Ende, mehreren Sockeln, die
von der Außenfläche in einer regelmäßigen nicht schrau
benförmigen Aufstellung hervorstehen, wobei jeder Sockel
eine Höhe von 100 bis 4500 µm und eine Querabmessung am
breitesten Punkt von 100 bis 4500 µm aufweist, wobei die
Außenfläche mehrere Öffnungen aufweist, um Knochenwachs
tum dort hindurch zu ermöglichen.
17. Rückgratimplantat nach Anspruch 16, wobei jeder Sockel
in der Draufsicht im wesentlichen rechteckig ist und ei
ne Breite von 100 bis 1000 µm und eine Länge von 400 bis
4500 µm und eine Höhe von 150 bis 2000 µm aufweist.
18. Rückgratimplantat nach Anspruch 17, wobei der rechtecki
ge Sockel in einem nicht schraubenförmigen Fischgräten
muster angeordnet ist.
19. Rückgratimplantat nach Anspruch 16, wobei die im allge
meinen tubulare Schale zylindrisch ist.
