Description
Die Erfindung betrifft Implantationsmaterial zur Wiederherstellung
von Gelenkknorpel- und Knochendefekten. Das Implantationsmaterial kann entweder in Form eines Gels vorliegen oder
zusammen mit natürlichen oder künstlichen Knochen als Trägermaterial. Das Implantationsmaterial der Erfindung eignet
sich vor allem zur Wiederherstellung defekter Gelenkknorpel, da diese besonders schwierige biologische Strukturen
darstellen. Das Implantationsmaterial der Erfindung enthält eine Kombination embryonaler Chondrocyten oder bestimmter
Mesenchymzellen, die in Knorpelzellen unter dem Einfluß von chondrogenen Induktionsfaktoren überführbar
sind, sowie geeignete Mengen Fibrinogen,Thrombin und eines Proteaseinhibitors (Antiprotease) als biologischen Leim.
Die erhaltenen Gele lassen sich längere Zeit lagern, wenn sie in gleicher Weise gelagert werden wie Gewebekulturen.
Sie lassen sich transportieren und leicht handhaben.
Gemäß einer bevorzugten Ausführungsform kann den Gelen eine
extrazelluläre Matrix (ECM) oder bestimmte Wachstumsfaktoren, wie SM, FGG, CGF, BDGF oder eine Kombination dieser
Wachstumsfaktoren zugegeben werden. Zur Wiederherstellung von Knochendefekten können sie als Irnplantationsmaterial eingesetzt
werden, das geeignet geformte Knochenteile eingebettet in diesen Gelen enthält.
Bei der Beschädigung von Gelenkknorpeln durch Verletzungen, Infektionen oder degenerative Prozesse erfolgt im allgemeinen
keine Heilung oder Besserung. Es wurden bereits Versuche unternommen, osteochondrale Verpflanzungen durchzuführen,
und verschiedene Prothesen zu entwickeln, doch waren
die Ergebnisse schlecht und entmutigend. Es sind auch Versuche zur Verwendung gezüchteter Chondrocyten als Quel-

Ie für Knorpeltransplantate bekannt geworden, doch war die
Integration der Transplantate in die benachbarten Knorpel im allgemeinen unbefriedigend.
Der Erfindung liegt die Aufgabe zugrunde, Implantationsmaterial zur Wiederherstellung von Gelenkknorpel- und Knochendefekten zur Verfügung zu stellen, das bestimmte Zellen, nämlich
embryonale Chondrocyten oder aus Mesenchym stammende
Zellen enthält, die sich in vitro oder in vivo in Knorpelzellen unter dem Einfluß von chondrogenen Induktionsfaktoren
überführen lassen. Dieses Implantationsmaterial liegt in einem geeigneten biologischen Milieu und als Gel vor. Eine weitere
Aufgabe der Erfindung ist es, ein Verfahren zur Herstellung des Implantationsmaterial zu entwickeln.
Das Implantationsmaterial wird folgendermaßen hergestellt: Embryonale Chondrocyten (junge Chondrocyten) werden aus
Embryonalgewebe durch Trypsinbehandlung und mechanische Zerlegung gewonnen und in einem geeigneten Medium gezüchtet. Die
Zellen werden danach geerntet und mit Fibrinogen, einem Proteaseinhibitor (Antiprotease) und Thrombin vermischt. Es wird ein
Gel erhalten, das in einem Inkubator eine gewisse Zeit aufbewahrt werden kann. Die geernteten Zellen können auch längere
Zeit tiefgefroren aufbewahrt werden. Unmittelbar vor der Verwendung
wird das Material aufgetaut. Das Gel wird vor der Implantation in eine Lösung von Fibrinogen getaucht. Nun wird
der Defekt mit einerThrombinlösung besprüht und dann mit dem
Implantationsmaterial aufgefüllt. Durch Versuche an Vögeln und Säugern wurde nachgewiesen, daß Gelenkknorpel- und Knochendefekte
sich gut auffüllen. Dies zeigte eine Untersuchung nach 2 bis 12 Monaten nach der Implantation.

Das Implantationsmaterial der Erfindung eignet sich zur Wiederherstellung
defekter humaner Gelenkknorpel oder Knochen. Die Ursache des Defektes kann Trauma oder Altersverschleiß
sein. Das Verfahren kann zur Wiederherstellung der verschiedensten
Knorpel- und Knochendefekte in Frage kommen, einschließlich degenerativer Veränderungen und Brüche im
Gelenkbereich·
Gemäß einer bevorzugten Ausführungsform enthält das Implantationsmaterial
eine bestimmte Menge einer extrazellulären Matrix (ECM) von embryonalen Chondrocyten/ die auf eine bestimmte,
nachstehend geschilderte Weise gewonnen werden. Die extrazelluläre Matrix fördert das Wachstum der Chondrocyten
nach Implantation des Materials. 15
Die Chondrocyten werden aus artspezifischem embryonalem Epiohysengewebe erhalten, mit anderen Worten, das embryonale
Epiphysengewebe wird der gleichen Art entnommen, der
später das Material der Erfindung implantiert werden soll. Allogene Zellen ergeben befriedigende Ergebnisse, ohne daß
man auf HLA-Typisieren angewiesen ist. Für die Humanmedizin werden humane embryonale Chondrocyten gezüchtet.
Es wurde festgestellt, daß die Zahl der Chondrocyten pro Volumeneinheit einen bestimmten Wert nicht übersteigen soll,
da sonst Zellnekrosen erfolgen und schlechtere Ergebnisse erhalten werden. Typische Werte für Chondrocytenkonzentrationen
sind etwa 100 000 bis 500 000 pro ml des Gels. Es werden etwa 5 bis 50 Einheiten Thrombin pro ml und etwa 25 bis
80 mg/ml Fibrinogen verwendet. Im allgemeinen wird der Proteaseinhibitor dem Implantationsmaterial zugesetzt, um
eine rasche Lysis des Gels zu verhindern oder zu unterdrücken. Es können natürliche oder synthetische Proteaseinhibitoren
verwendet werden. Beispiele für geeignete chemische Proteaseinhibitoren sind £-Aminocapronsäure, die in
einer Menge von 10 bis 20 mg/ml Gel verwendet wird, und
Tranexemsäure, die in einer Menge von 1 bis 2 mg/ml Gel ver-

wendet wird. Es können auch natürliche Proteaseinhibitoren verwendet werden, wie Antitrypsin (Hühnereiweiß, Sigma,
Typ III) in einer Konzentration von 50 bis 75 μg/:ml. Sofern
eine langsame Bildung des Gels erwünscht ist, werden 5 bis 10 Einheiten/ml Thrombin verwendet. Wenn das Gel rasch fest
werden soll, werden etwa 20 bis 50 Einheiten/ml Thrombin verwendet.
Außerdem ist es vorteilhaft, dem Gel einen oder mehrere Wachstumsfaktoren
des lokalen Typs, wie ECM oder BDGF, oder des hormoneilen Typs, wie Somatomedin (SM)-ähnliche Peptide,
Knorpelwachstumsfaktor (CGF) und dergl. zuzufügen. Die.
Wachstumsfaktoren werden in folgenden Mengen verwendet: ECM einige mg/ml, BDGF etwa 100 μg/ml, CGF einige Nanogramm/ml,
SM einige Nanogramm/ml.
-Medium Das Implantationsmaterial kann überschichtet mit F-I2 /sowie
10 % fetalem Kälberserum einige Wochen in einem CC· -Inkubator bei 370C aufbewahrt werden. Das gelartige Implantationsmaterial
ist sofort verwendbar und kann unmittelbar auf den
wiederherzustellenden Bereich aufgebracht werden.
Für bestimmte Wiederherstellungsmaßnahmen, ist es zweckmäßig,
Implantationsmaterial zu verwenden, das aus einer Knochenbesteht struktur (natürlich oder künstlich) /eingebettet in ein Gel
der Erfindung, d.h. ein Gemisch aus Thrombin, Antiprotease, Fibrinogen und einem oder mehreren Wachstumsfaktoren und/
oder-hormonen sowie einer geringen Konzentration der vorgenannten Arten von Zellen oder sogar ohne diese Zellen. Im
letztgenannten Fall dringen Zellen aus der Umgebung langsam in das Implantat ein und bilden eine zusammenhängende
Masse zwischen Umgebung und Implantat.

Herstellung eines Implantationsmaterial zur Wiederherstellung von Knorpeldefekten

Als Ausgangsmaterial werden die Epiphysen von langen Knochen
(Tibia, Femur) verwendet.
Zur Isolierung embryonaler Chondrocyten (junger Chondrocyten)
werden die Epiphysen einer energischen Trypsinbehandlung unterworfen, d.h. 45 Minuten auf einer Drehschüttelmaschine
in einem Wasserbad mit einer Trypsinkonzentration von 1 % behandelt und gleichzeitig mit einem Homogenisator mechanisch
aufaebrochen. Dann wird das Trypsin durch Zugabe von Serum inaktiviert,
das antiproteolytische Substanzen enthält. Die erhaltene Suspension von Einzelzellen wird sodann mehrere Tage
(6 bis 10 Tage) in einem flüssigen Harn F-12 Medium auf
Platten überimpft, die mit Weichagar (0,6 % Agar, enthaltend Ham F-12 Medium) beschichtet sind. Während dieser Wachstumsperiode
sterben die meisten . Fibroblasten ab und es erfolgt eine Anreicherung an Chondrocyten. Die Zellen von den Weichagarplatten
werden sodann in Spinner-Flaschen überführt und in F-12 Medium in Suspension weitere 3 bis 10 Tage gezüchtet.
Die Wachstumsbedingungen in Spinner-Flaschen fördern die bevorzugte Bildung von Chondrocyten gegenüber Fibroblasten. Sodann
werden die Zellen aus den Spinner-Flaschen abgeschleudert und unmittelbar verwendet oder in 20prozentigem fetalem
Kälberserum mit 10 % Dimethylsulfoxid und 70 % F-12 Medium
bei der Temperatur des flüssigen Stickstoffs eingefroren. Das beim Zentrifugieren der Chondrocyten erhaltene Sediment wird
in einem geringen Volumen Phosphat-gepufferter Kochsalzlösung mit 50 mg/ml Fibrinogen oder einem anderen serumpräzipitieretiden
Protein und einem Trypsininhibitor (50 |ig/ml, Sigma Typ III) oder einer anderen Antiprotease suspendiert.

Ι Die jüngste Lösung, die Zellen (je nach Älter in der Kultur
und der Größe in einer Konzentration von 1 bis 5x10 /ml)
sowie Fibrinogen und Trypsininhibitor enthält, wird als Lösung A bezeichnet. Eine Mikrotestplatte (96 F Nunc Dänemark
mit Ausguß) oder eine Zelle mit vielen Näpfchen wird mit 30 μΐ Thrombinlösung (Lösung B) (1U/30 ml 40 mM CaCl^) in jedes
Näpfchen versetzt und gleichmäßig am Boden und an der Seitenwand verteilt. Sodann werden 90 μΐ der Lösung A zugegeben.
Hierauf läßt man das Gemisch gelieren.
Erforderlichenfalls kann die Endmenge geändert werden, wobei das Volumenverhältnis von Lösung A zu Lösung B auf einen Wert
von 3 : 1 eingestellt wird. Das mit 0,2 ml F-12 Medium übersnhichtete
Gel läßt sich etwa 14 Tage in einem Inkubator mit 5 bis 10 % CO2 enthaltender Luft oder in tiefgefrorenem
Zustand aufbewahren.
Vor der Transplantation wird der defekte Bereich vorsichtig mit
einer Thrombinlösung besprüht. Das Gel wird vor der Transplantation
in eine Lösung getaucht, die Fibrinogen und Trypsininhibitor (50 mg/ml bzw. 50 μg/ml) enthält, sodann wird das
Material in den defekten Bereich eingedrückt und dieser Bereich aufgefüllt.
Anstelle von embryonalen Chondrocyten können auch embryonale Mesenchymzellen (Stufe 24) oder Knochenmark-Stammzellen verwendet
werden. Außerdem kann jedes erwachsene Bindegewebe mit darin enthaltenen undifferenzierten Mesenchymzellen verwendet
werden, die in Kultur Zellen bilden und schließlich durch Selbstdifferenzierung oder gesteuert durch chondrogene
Faktoren in Chondrocyten überführt werden.
Für genetisch identische (syngene) Transplantate können hoirogenedsche
Mesenchymzellen verwendet werden, die in erwachsenem Gewebe des Empfängers enthalten sind, z.B. Hautfibroblasten,
Knochenmarkzellen, die durch Biopsie erhalten werden, und die
Zellen in Kultur bilden, die in Chondrocyten überführbar
sind.
Beispiel 2 5
Herstellung eines Gels, das ECM (extrazelluläre Matrix)
enthält
a) Herstellung von ECM
Embryonale Hühnerchondrocyten, die 14 bis 21 Tage in Spinner-Flaschen
kultiviert worden sind, werden in mit Fibronectin beschichtete Petrischalen in einer Anfangsdichte von
2 χ 10 Zellen/35 mm Petrischale in ein F-12 Medium überimpft,
das 50 μg/ml Ascorbinsäure enthält. Die Zellkulturen
fließen innerhalb 6 Tagen zusammen. Sodann wird das Medium erneuert, und die Kulturen werden weitere 6 Tage inkubiert.
Drei Kontrollkulturen werden mit Trypsin behandelt und danach werden die Zellen mit einem Coulter Zähler Modell D
gezählt. Die Zelldichte beträgt etwa 1x10 Zellen pro Petrischale.
Sämtliche anderen Petrischalen werden sodann mit Phosphat-gepufferter Kochsalzlösung (PBS) gewaschen, 20 Minuten
mit 20 mM wäßriger Ammoniaklösung behandelt und erneut
dreimal mit PBS und sodann zweimal mit destilliertem Wasser gewaschen, so daß keine Cytoskelette oder Kerne zusammen
mit dem intakten ECM beobachtet werden können. Das ECM wird mit einem Gummischaber gesammelt und gefriergetrocknet.
Die Ausbeute an getrocknetem ECM-Pulver beträgt etwa 0,3 bis 1,0 mg pro Petrischale.
Das ECM wird in einer Menge von 5 bis 20 mg/ml, insbesondere 10 mg/ml verwendet.
Versuche mit Chondrocytengelen bei Vögeln und Säugern haben nach makroskopischer Untersuchung, i.m histologischen· Schnitt
und durch biochemische Tests ergeben, daß die Transpl'antationsstelle
innerhalb 2 bi's 3 Monaten gut ausgefüllt und in die Umgebung

1 einschließlich der Oberflächen der Gelenkknorpel eingefügt ist. Im Transplantat·, zeigen sich aktive proliferierende Knorpelzellen,
die den typischen Stoffwechsel zeigen, und die gut verbunden sind (ohne Faserknorpel oder anderes weiches
5 Gewebematerial an den Kanten) mit dem ursprünglichen Knorpel gewebe
Claims (10)
1. Implantationsmaterial zur Wiederherstellung von Gelenkknorpel-
und Knochendefekten, gekennzeichnet
durch einen Gehalt artspezifischer embryonaler Chondrocyten oder von Mesenchymzellen, die durch
Induktionsfaktoren in Knorpelzellen überführbar sind, zusammen mit einem biologischen Leim aus Fribrinogen,
Thrombin sowie einem natürlichen oder chemischen Proteaseinhibitor.
2. Implantationsmaterial nach Anspruch 1, gekennzeichnet durch einen Gehalt von 100 000 bis 500 000 Chondrocyten
oder Mesenchymzellen pro ml.
3. Implantationsmaterial nach Anspruch 1, gekennzeichnet durch einen Gehalt von 5 bis 20 mg/ml einer extrazellulären
Matrix (ECM).
4. Implantationsmaterial nach Anspruch 1, gekennzeichnet durch einen Gehalt an einem lokalen Wachstumsfaktor oder
einem hormonellen Wachstumsfaktor.
5. Implantationsmaterial nach Anspruch 4, dadurch gekennzeichnet,
daß der lokale Wachsturnsfaktor aus Knorpel
(CDF) oder Knochen (BDGF) stammt.
6. Implantationsmaterial nach Anspruch 4, dadurch gekenn-
zeichnet, daß der hormonelle Wachsturnsfaktor Somatomedin
aus Serum oder der Hypophyse stammt.
7. Implantationsmaterial nach Anspruch 1, dadurch gekennzeichnet,
daß die Zellen Chondrocyten oder Zellen sind, die sich von Bindegewebe mit undifferenzierten
Mesenchymzellen ableiten, die in Kulturen proliferieren
und in Chondrocyten überführbar sind.
8. Implantationsmaterial nach Anspruch 7, dadurch gekennzeichnet,
daß das Material in geformten, gefriergetrockneten (Kiel) oder künstlichen Knochen eingebettet
ist.
9. Verwendung des Implantationsmaterials nach Anspruch 1
bis 8 zur Wiederherstellung von Epiphysenknorpel
und Knochen.
10. Implantationsmaterial zur Wiederherstellung von Gelenkknorpel-
und Knochendefekten, gekennzeichnet durch
einen Gehalt an geeignet geformtem Knochenmaterial, das mit einem Gel aus Thrombin, einer Antiprotease und
Fibrinogen imprägniert ist, zusammen mit einer extrazellulären Matrix (ECM) oder mit einem lokalen oder
hormonellen Wachstumsfaktor und gegebenenfalls einer
geringen Menge artspezifischer embryonaler Chondrocyten
oder Mesenchymzellen, die durch Induktionsfaktoren in Knorpelzellen überführbar sind.
