Abstract

To measure the haemoglobin concentration, an amount of about 0.1 ml of blood per min is continuously taken under sterile conditions, and without altering the proportionate volume of the erythrocytes, from the extracorporeal circulation. A double peristaltic pump is used to dilute the blood in a fixed ratio. Blood and dilution medium are combined via a specially designed connector in order to avoid sedimentation of the erythrocytes. The erythrocytes are completely haemolysed by the dilution medium (0.5% NH3) and converted into oxyhaemoglobin. The haemolysate is homogenised in a stirred cell using a magnetic stirrer and is then transferred into a microcuvette in a photometer. The oxyhaemoglobin concentration is determined by measuring the extinction at a wavelength of 415 nm (SORET band). The method makes it possible to monitor the blood volume in an existing extracorporeal circulation, e.g. in artificial kidney treatment. It is also possible on the basis of this method to measure the concentration of other constituents of blood continuously over several hours.

Description

The invention assumes that the hemoglobin concentration in the extracorporeal circulation through the continuous removal so Low blood levels result in blood loss for the patient is also irrelevant for a measurement over several hours.

Changes in hemoglobin concentration reflect changes in Blood volume again. The hemoglobin concentration can therefore Control of blood volume in the treatment of terminal kidney failure be used. The same applies to the hematocrit. In the literature there are various methods for continuous Determination of the hematocrit in the extracorporeal circuit described (1 to 7). A method for the continuous measurement of the hemoglobin concentration was also given in (8).

From the determination of the hematocrit can be done in a similar way as by determining the hemoglobin concentration for changes of the blood volume are closed. The relationship between the Hemotocrit and changes in blood volume is straightforward since the Hematocrit due to changes in electrolyte composition or a redistribution of the plasma volume between the large ones Vessels and the capillary system of the organism even without changes the blood volume shows certain fluctuations.

A method for the continuous determination of the hemoglobin concentration in the extracorporeal with the decrease of very small amounts of blood is not known to me. The control of ultrafiltration in haemofiltration based on continuous hematocrit measurement has already been described (9). Measurement of the hematocrit was done by determining the electrical conductivity in whole blood and in hemofiltrate.

Changes in the relative blood volume are an important quantity too Control of ultrafiltration in processes for treating the terminal kidney failure (hemodialysis, hemofiltration, hemodiafiltration, continuous arterio-venous hemofiltration). By Ultrafiltration is treated by the patient Withdrawn fluid that is not due to the lack of kidney function is eliminated. The liquid immediately goes to the blood  withdrawn. This depends on the filtration rate and the ability of the organism to replenish Decrease in blood volume.

The decrease in blood volume is with a tendency to drop blood pressure connected. Drops in blood pressure can reduce well-being and health Significantly affect the patient's condition. That is why Knowing to what extent the blood volume during the course of treatment decreases, of great clinical importance.

When it comes to extracorporeal blood purification (see above) it often depends on a certain one in the shortest possible time To withdraw the amount of liquid! With the method described here the fluid withdrawal can be controlled so that the percentage Decrease in blood volume is limited to an advance amount becomes. This ensures optimal fluid withdrawal at the same time gentle treatment possible.

Beschreibung der MethodeDescription of the method

Die Methode verlangt die Lösung von 4 Problemen:The method requires 4 problems to be solved:

  • - die kontinuierliche Abnahme einer sehr geringen Blutmenge unter sterilen Bedingungen- the continuous withdrawal of a very small amount of blood under sterile conditions
  • - die kontinuierliche Verdünnung in einem festen Verhältnis über einen Zeitraum von mehreren Stunden- the continuous dilution in a fixed ratio over a period of several hours
  • - das Erreichen der vollständigen Hämolyse der Erythrozyten und Oxygenierung des Hämoglobins- the achievement of complete hemolysis of the erythrocytes and oxygenation of hemoglobin
  • - die kontinuierliche Messung der Extinktion- the continuous measurement of the absorbance

A schematic representation of the method is shown in Fig. 1.

Blood is drawn through a capillary PVC tube (inner Diameter 0.127 mm), which on the arterial part before the Blood pump and in front of the entrances for infusions on the extracorporeal  Circuit is connected. The position is chosen so the measurement is not falsified by any infused solutions becomes.

Access to the extracorporeal circuit is via a short one Stainless steel cannula 27 G through a silicone septum in the blood tube is stabbed. The tip of the cannula is near the wall with the opening directed against the current. As long as none Access septa integrated into the blood tube system a piece of silicone tubing is attached to a special clamp pressed as a septum on the blood tube and the cannula through the Pierced the silicone tube and the wall of the blood tube. Sterility is achieved by having the entire withdrawal system up to the stirring cell (see below) is designed as a disposable system.

The blood is carried through this tube as described under 1. a peristaltic pump. About the same pump head Peristaltic pump is using a second hose made of silicone rubber an inner diameter of 2.0 mm. Be behind the pump the two hoses are brought together using a special connection technology. The required dilution is thus achieved. The Peristaltic pump is designed so that the direction of promotion is clearly defined. This is achieved in that the Hoses can only be inserted in the correct way, and that the pump only regardless of the correct power supply can start in one direction.

The mixture of blood and dilution medium drips behind the merge in a stirred cell. This creates a separation between the sterile disposable system (consisting of a blood collection tube, Dilution hose, connector and mixture hose) and the remaining non-sterile measuring section (consisting of a stirring cell, Cell with inlet and outlet hoses and collecting container) reached.

The sterile hypotonic medium is fed in from a bag with welded hose line.

To inadvertently connect the hypotonic medium to one Exclude access to the patient's bloodstream under all circumstances, the hose connectors are designed so that a Connection with the LUER-female- used exclusively here Cone is impossible. An easy to implement measure for this is the non-detachable connection between the supply hose of the dilution medium and dilution tube.

The diluted blood is then passed through a mixing zone, in the blood and hypotonic medium through mechanical support of a magnetic stirrer. The pH of the medium is selected so that the partial pressure of oxygen present in it sufficient to reach a final state in which more than 99% of the Hemoglobins are present in an oxygenated state (abnormalities present Hemoglobin derivatives, e.g. B. Met hemoglobin excluded). The The volume between the mixing zone and the extinction measuring section is selected that the blood is complete when entering the measuring section is hemolyzed.

The absorbance is measured in a cuvette, which is continuous from which hemolysate flows. The temperature of the The cuvette is selected so that there are no failures due to failing Air bubbles appear. The wavelength of 415 nm (Soret band) is by a band filter from the radiation of a tungsten lamp filtered out or generated by a monochromator.
