Description
Die Erfindung bezieht sich auf eine neue Art von isotonischen
Arzneimitteln zur ophthalmologischen Anwendung, die besonders
nützlich sind bei der Behandlung und Linderung der Symptome
des Augentrockenheit-Syndroms. Der Hauptbestandteil solcher
Augentropfen ist ein Feuchthaltemittel. Geeignete Feuchthal
temittel sind isotonisches Glycerin, Sorbit oder ähnliche
Substanzen. Es können auch Salze der Milchsäure und ähnliches
zur Anwendung kommen.
Die isotonischen Lösungen können als Augentropfen im Falle
des Augentrockenheit-Syndroms verwendet werden. Sie können
auch als Träger für pharmazeutisch aktive Bestandteile bzw.
Komponenten eingesetzt werden.
Zahlreiche Augenkrankheiten erfordern Behandlungen durch An
wendung von flüssigen Präparaten bzw. Arzneimitteln, die
verabreicht werden, um das Auge zu berühren bzw. zu benetzen.
Augentropfen werden auch häufig von Trägern von Kontaktlinsen
benötigt. Die Augentrockenheit-Krankheit (Augenxerose) ist
ein ernsthaftes Leiden, das die wiederholte Anwendung von
Augentropfen je Tag erfordert. Gebräuchliche Augentropfen
beruhen auf isotonischen Lösungen von verschiedenen anorgani
schen Salzen mit oder ohne "nichtionische" Substanzen (siehe
US-PS 44 09 205), häufig mit Zusatz einer Substanz von hohem
Molekulargewicht, die die Viskosität der Tropfen erhöht. Zu
den häufig verwendeten polymeren Substanzen gehören Methyl
cellulose, Hydroxyethylcellulose, Polyvinylalkohol, Poly
vinylpyrrolidon und Hyaluronsäure. Die Viskosität beträgt
allgemein 1 bis 30 mPa · s. Die isotonischen Lösungen, die an
organische Salze enthalten, weisen bestimmte Nachteile auf,
die mit ihrer physiologischen und Viskositäts-Leistung zu
sammenhängen, vor allem, wenn sie kontinuierlich während einer
längeren Zeitspanne angewandt werden. Mit Hilfe der vorlie
genden Erfindung werden diese Nachteile überwunden.
Es werden isotonische Augentropfen für die Behandlung und
Linderung der Symptome des Augentrockenheit-Syndroms bereit
gestellt. Die neuen Augentropfen haben vorteilhafte Eigen
schaften im Vergleich mit den gebräuchlichen Augentropfen.
Die neuen Augentropfen beruhen auf der Verwendung eines Feucht
haltemittels, wie Glycerin, Sorbit oder anderen physiologisch
annehmbaren feuchthaltenden bzw. befeuchtenden Polyolen. Eine
andere Gruppe von Feuchthaltemitteln umfaßt Natriumpyrroli
doncarboxylat, Salze der DL-Milchsäure, der L-Milchsäure,
der D-Milchsäure sowie von anderen niederen α-Hydroxyfett
säuren. Diese können als solche für die Behandlung der Augen
trockenheit-Krankheiten verwendet werden. Solche isotonischen
Lösungen können als Träger für verschiedene Arten von in der
Augenheilkunde verwendeten Medikamente eingesetzt werden.
Sie können unter anderem als Träger für zahlreiche antimikro
bielle Mittel, wie Antobiotika, Antivirusmittel und ähnliches
Verwendung finden. Das Lactat und/oder Glycerin sowie die
anderen Substanzen, die erfindungsgemäß verwendet werden,
sind selbst Feuchthaltemittel, d. h. sie können Wasser halten
bzw. zurückhalten.
Die Arzneimittel zur ophthalmologischen Anwendung nach der
Erfindung sollen gebräuchliche Zusätze und Hilfsstoffe ent
halten, um zu verhindern, daß sie beim Lagern verderben. Diese
Zusätze und Hilfsstoffe sind allgemein bekannt und werden
in den einzelnen Beispielen nicht angegeben. Außerdem muß
ein entsprechendes Additiv verwendet werden, um die gewünschte
notwendige Viskosität einzustellen. Mittel, die die Visko
sität erhöhen, sind allgemein Verbindungen mit hohem Molekular
gewicht, die physiologisch verträglich sind. Die Viskosität
wird auf einen Wert im Bereich von 1 bis 30 mPa · s eingestellt;
der bevorzugte Bereich liegt bei etwa 3 bis etwa 20 mPa · s.
Zusätze dieser Art sind allgemein Cellulosederivate, wie
Methylcellulose, Hydroxyethylcellulose usw., Polyvinylalkohol,
Polyvinylpyrrolidon und ähnliches. Besonders vorteilhafte
Ergebnisse werden mit Hyaluronsäure oder Polyacrylat erzielt,
entweder alleine oder in Kombination mit anderen physiologisch
verträglichen hochmolekularen Substanzen.
Die Augentropfen beruhen auf physiologisch annehmbarem bzw.
verträglichem Feuchthaltemittel. Feuchthaltemittel der Wahl
sind Glycerin, Sorbit und andere verträgliche, feuchthaltende
Polyole. Diese sind organische, nichtionische Substanzen,
und sehr gute Ergebnisse wurden durch Anwendung dieser Sub
stanzen erzielt. Es können auch geeignete Salze von Milch
säure in der DL-, D- oder L-Form Verwendung finden. Weiterhin
können andere α-Hydroxyfettsäuren in Form ihrer Salze einge
setzt werden. Salze der Wahl sind Alkalisalze, wie Natrium
salze, obwohl auch andere physiologisch annehmbare organische
oder anorganische Salze Verwendung finden können. Die Feucht
haltemittel werden stets in Form einer im wesentlichen iso
tonischen Lösung verwendet. Die Augentropfen werden sterili
siert und, wenn erforderlich, werden geeignete Mittel zuge
setzt, um nachteilige Veränderungen durch Bakterien oder Fungi
zu verhindern.
Es können beliebige andere wasserbindende, physiologisch ver
trägliche, wasserlösliche Salze einer organischen Säure oder
einer geeigneten zuckerartigen Verbindung, wie Sorbit, verwen
det werden.
Es wurden Lösungen dieser Art zur Behandlung der Augentrocken
heit-Krankheit getestet. Es wurden auch Arzneimittel zur
ophthalmologischen Anwendung hergestellt, die antimikrobielle
Mittel und Antivirusmittel enthielten, bei denen der Träger
eine wie oben definierte isotonische Lösung war.
Die Erfindung wird in den folgenden Ausführungsbeispielen
näher erläutert.

Isotonische Antivirus-Lösung zur ophthalmologischen Anwendung:

0,1 g Idoxuridin.
Isotonische Antiglaucom-Lösung:

2 g Pilocarpin.
Isotonisches antibakterielles Präparat zur ophthalmologi
schen Anwendung:
Zu 100 ml der Lösung der Beispiele 1, 2, 3 oder 4 wurde zu gegeben:

0,5 g Chloramphenicol.
Entzündungshemmende Lösung zur ophthalmologischen Anwendung:
Zu 100 ml einer Lösung der Beispiele 1, 2, 3 oder 4 wurde zugegeben:

0,05 g Dexamethason-natriumphosphat.
In allen Beispielen wurde die Lösung isoton und auf pH-Wert
7 eingestellt.
Geeignete gebräuchliche Stabilisatoren und Konservierungsmit
tel wurden zugegeben.
Eine Gruppe von 20 Patienten, die am Augentrockenheit-Syndrom
litten, wurde mit der Augenlösung des Beispiels 1 behandelt.
Bei gebräuchlichen Präparaten mußten die Augentropfen mehr
als viermal täglich angewandt werden. Bei Einsatz des Mittels
des Beispiels 1 nahm die benötigte durchschnittliche Menge
auf 2,8 Anwendungen täglich ab. Ein anderer Durchschnitt wur
de mit dem Mittel des Beispiels 3 getestet. Hier wurden nur
2 Anwendungen täglich benötigt. Bei einem anderen Versuch
wurde eine gleiche Gruppe von Patienten mit dem Mittel des
Beispiels 2 behandelt. Die erforderliche Anzahl der Anwen
dungen nahm von 4 bis 3,2 täglich ab.
Augentropfen, die eine entzündungshemmende Lösung gemäß Bei
spiel 8 enthielten, wurden zur Behandlung von Augenentzündun
gen verwendet. Die Tropfen wurden gut vertragen, und es wur
den gute Ergebnisse ohne irgendwelche Irritationen der Augen
erzielt.
Linderung trat sehr schnell ein.
Die feuchthaltenden Eigenschaften der erfindungsgemäßen Prä
parate bzw. Arzneimittel werden leicht nachgewiesen bzw. ge
zeigt durch Aufbringen einer isotonischen Lösung nach der
Erfindung auf die Haut des Unterarms und durch Messen der
elektrischen Leitfähigkeit oder Kapazitanz der Hornhautschicht
eine halbe Stunde nach dem Aufbringen. Eine Zunahme der
Leitfähigkeit oder Kapazitanz ist ein Anzeichen für einen
Feuchthalte-Effekt.
Claims (8)
1. Augentropfen, bestehend aus einer im wesentlichen iso
tonischen Lösung eines physiologisch verträglichen, organi
schen Feuchthaltemittels in Kombination mit erforderlichen
Hilfs- oder Zusatzstoffen, enthaltend weniger als 1,5 Milli
mol/Liter anorganisches Salz.
2. Augentropfen, bestehend aus einer im wesentlichen iso
tonischen Lösung eines physiologisch verträglichen, nicht
ionischen, organischen Feuchthaltemittels in Kombination mit
erforderlichen Hilfs- oder Zusatzstoffen, enthaltend weniger
als 1,5 Millimol/Liter Salz, nicht eingeschlossen das Salz
von ionischen, viskositätsverstärkenden Mitteln.
3. Arzneimittel nach Anspruch 2, dadurch gekenn
zeichnet, daß das Feuchthaltemittel ausgewählt wird
aus Glycerin oder Sorbit oder anderen niedrigmolekularen,
nichtpolymeren feuchthaltenden Polyolen.
4. Arzneimittel nach Anspruch 1, dadurch gekenn
zeichnet, daß das Feuchthaltemittel ein Salz von
Pyrrolidoncarbonsäure, ein Salz der DL-Milchsäure, L-Milch
säure oder D-Milchsäure oder ein Salz einer niederen, (C2-C8)-α-
Hydroxyfettsäure ist.
5. Arzneimittel nach einem der Ansprüche 1 bis 4, dadurch
gekennzeichnet, daß das zur Erhöhung der Visko
sität zugesetzte Mittel ein in der Augenheilkunde akzeptier
tes viskositätsverstärkendes Mittel ist.
6. Arzneimittel nach einem der Ansprüche 1 bis 4, dadurch
gekennzeichnet, daß das Mittel zur Erhöhung
der Viskosität ein Polymer mit Carbonsäuregruppen, wie Hyalu
ronsäure oder Polyacrylsäure oder Carbomere ist.
7. Arzneimittel nach einem der Ansprüche 1 bis 6, dadurch
gekennzeichnet, daß sie auch in angemessener
Konzentration ein antibakterielles Mittel, ein Antivirusmit
tel, ein Antifungimittel, ein Antiglaucommittel oder ein ent
zündungshemmendes Mittel enthalten.
8. Arzneimittel nach einem der Ansprüche 1 bis 6 für die
Behandlung und Linderung der Symptome der Augentrockenheit.
