Description
- HINTERGRUND DER ERFINDUNG
- Die vorliegende Erfindung betrifft Wirbelsäulenimplantate und insbesondere ein transplantatloses Wirbelsäulenimplantat, welches eine bioaktive Beschichtung, wie beispielsweise Hydroxylapatit, zum Fördern der Befestigung des Implantates an benachbarten Knochenflächen verwendet.
- Das US-Patent 6,241,769 mit dem Titel „Implant for Spinal Fusion" und das US-Patent 6,096,080 mit dem Titel „Apparatus for Spinal Fusion Using Implanted Devices" offenbaren Wirbelsäulenersatzvorrichtungen. Beide Patente erörtern detailliert die mit den Vorrichtungen des Standes der Technik assoziierten Schwierigkeiten.
- Die in den US-Patenten 6,241,769 und 6,096,080 offenbarten Vorrichtungen verwenden einen mittleren Transplantatraum zum Fördern der Langzeitbefestigung auf benachbarten Knochenflächen. Die Vorrichtung kann aus einem humanen Knochenallotransplantatmaterial aus einem Knochenersatzmaterial, wie beispielsweise Korallen oder Kalziumphosphat, hergestellt werden. Eine Schwierigkeit mit den in diesen Patenten offenbarten Vorrichtungen ist, dass sie ein Knochentransplantat erfordern und die Fehlerquote dieser Vorrichtungsarten ziemlich hoch ist. Zudem besteht eine unannehmbar hohe Morbiditätsrate (beispielsweise 30%).
- Ein Wirbelsäulenimplantat gemäß dem Oberbegriff des Anspruchs 1 ist in
US 5,709,683 offenbart. - Daher besteht Bedarf an einer Wirbelsäulenimplantatvorrichtung, welche in einem Ansatz die Befestigung der Implantatvorrichtung an benachbarten Wirbelknochenflächen zu ermöglichen und dadurch den Wirbelsäulenbewegungsabschnitt zu stabilisieren kein Knochentransplantat verwendet.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Kurz sind nach der vorliegenden Erfindung eine implantierbare Wirbelsäulenfusionsvorrichtung, die in Anspruch 1 dargelegt ist, und ein Verfahren zum Bilden eines Wirbelsäulenimplantates, das in Anspruch 7 dargelegt ist, geliefert.
- Das transplantatlose Wirbelsäulenimplantat zum mechanischen Verbinden von zwei benachbarten Wirbeln enthält einen Körper mit ersten und zweiten gegenüberliegenden Flächen. Die ersten und zweiten gegenüberliegenden Flächen enthalten hervorstehende Teile zum Befestigen des Körpers an benachbarten Wirbeln, wobei die äußeren Flächen bzw. Außenflächen des Körpers und die ersten und zweiten hervorstehenden Teile eine bioaktive Beschichtung beinhalten.
- Die bioaktive Beschichtung kann aus Beschichtungen ausgewählt werden, wie z.B. Hydroxylapatit, Bioglas, Bioverbundstoffe, Hydroxypatit-Trikalziumphosphat, knochenmorphogenes Protein oder andere knochenproduzierende Proteine.
- Die äußere bioaktive Beschichtung ermöglicht den Einbau/Durchbau (ongrowth/ingrowth) des Wirbelknochens zwischen der implantierbaren Wirbelsäulenfusionsvorrichtung und benachbarten Wirbeln. Dies ermöglicht der Vorrichtung vorteilhaft die Verwendung eines Knochentransplantats und/oder Knochenwachstumsagens zu vermeiden.
- Diese und andere Aufgaben, Merkmale und Vorteile der vorliegenden Erfindung werden angesichts der folgenden detaillierten Beschreibung der bevorzugten Ausführungsformen derselben detailliert beschrieben, die in den beiliegenden Zeichnungen veranschaulicht ist.
- KURZE BESCHREIBUNG DER ZEICHNUNGEN

7 veranschaulicht noch eine andere Ausführungsform eines transplantatlosen Wirbelsäulenimplantates. - DETAILLIERTE BESCHREIBUNG DER ERFINDUNG

1 veranschaulicht eine transplantatlose, anteriore Wirbelsäulenfusionsvorrichtung10 . Die Vorrichtung10 enthält einen Hauptkörper12 , welcher in dieser Ausführungsform im Wesentlichen rechteckig geformt ist. Jemand mit gewöhnlichen Fähigkeiten wird jedoch erkennen, dass der Hauptkörper12 in anderen Formen als rechteckig vorgesehen sein kann. Der Hauptkörper12 enthält auch erste bzw. zweite gegenüberliegende Flächen14 ,16 . In dieser Ausführungsform sind die ersten und zweiten Flächen14 ,16 im Wesentlichen parallel und beinhalten hervorstehende Teile18 –21 . Der Hauptkörper und die hervorstehenden Teile bestehen vorzugsweise aus einer einteiligen (d.h. einheitlichen) Struktur. Die Vorrichtung10 kann auch eine leichte Rundung/Verjüngung aufweisen. -
2 ist eine vereinfachte Veranschaulichung der transplantatlosen, anterioren Wirbelsäulenfusionsvorrichtung10 , welche innerhalb einer Wirbelsäule24 funktionsfähig implantiert ist. Die Wirbelsäule enthält Wirbel26 ,28 und natürliche Scheiben30 ,32 . Die Wirbelsäule24 enthält auch eine erkrankte/defekte Scheibe34 , die teilweise entfernt und durch die transplantatlose, anteriore Wirbelsäulenfusionsvorrichtung10 der vorliegenden Erfindung ersetzt wurde. In Vorbereitung auf das chirurgische Einsetzen der transplantatlosen, anterioren Wirbelsäulenfusionsvorrichtung10 werden Nuten bzw. Keilnuten36 –39 , die zum festen Aufnehmen eines zugeordneten, hervorstehenden Teils (z.B.18 –21 ) größenmäßig zugeschnitten sind, in jeden Wirbel26 ,28 gebildet, welcher sich an die defekte/erkrankte Scheibe angrenzend befindet. -
3 veranschaulicht eine Seitenansicht des in2 gezeigten, transplantatlosen Wirbelsäulenimplantates10 . Insbesondere werden die Streckung und Drehkräfte durch die hervorstehenden Teile neutralisiert. - Die Vorrichtung
10 ist mit einer bioaktiven Beschichtung beschichtet, wie z.B. Hydroxylapatit, Bioglas, Bioverbundstoffe, Hydroxypatit-Trikalziumphosphat, knochenmorphogenes Protein oder andere knochenproduzierende Proteine. Die bioaktive Beschichtung ermöglicht bezeichnenderweise den Einbau/Durchbau des Wirbelknochens. -
4 veranschaulicht eine Perspektivansicht einer Ausführungsform eines transplantatlosen Wirbelsäulenimplantates70 . Diese Ausführungsform enthält auch einen im Wesentlichen rechteckigen Hauptkörper72 und mindestens einen Vorsprung (z.B.74 –77 ), welcher sich aus jeder der ersten und zweiten gegenüberliegenden Flächen80 ,82 erstreckt. Insbesondere sind die Vorsprünge74 –77 im Wesentlichen zylinderförmig (hufeisenförmig). Natürlich sind die in angrenzenden Wirbeln gebildeten Keilnuten (nicht gezeigt) geformt, um die Vorsprünge74 –77 aufzunehmen. Zudem sind die Außenflächen dieses transplantatlosen Wirbelsäulenimplantates70 strukturiert/aufgeraut und enthalten eine Vielzahl von Vertiefungen79 , welche die Mantelfläche des Implantates vergrößert. D.h., die Außenflächen des transplantatlosen Wirbelsäulenimplantates70 enthalten vorzugsweise eine Vielzahl von flachen Vertiefungen79 , um die Fusion des Implantates und angrenzenden Knochens zu ermöglichen. Die strukturiere Fläche kann Wülste, Bogenablagerungen und/oder synthetische Schaumstoffe enthalten. Die Außenflächen des Implantates70 sind mit einer bioaktiven Beschichtung beschichtet, wie beispielsweise Hydroxylapatit. Diese Beschichtung ermöglicht den Einbau/Durchbau des Wirbelknochens. Die Außenschichten der in den1 –4 veranschaulichten Vorrichtung in einer bevorzugten Ausführungsform sind vorzugsweise ähnlich aufgeraut und enthalten eine Vielzahl von Vertiefungen. - In einer alternativen Ausführungsform kann das Implantat
70 eine Öffnung84 enthalten, in welche Knochenwachstumsagenzien gegeben werden können, wie in den oben bezeichneten US-Patenten offenbart ist, die durch Verweis hierin enthalten sind. Die sich ergebende Vorrichtung kann aufgrund des Einschlusses der Knochentransplantatmaterialien, welche in die Öffnung gegeben werden, nicht länger als rein transplantatlose Vorrichtung betrachtet werden. Diese Vorrichtung kombiniert bezeichnenderweise die Vorteile der oben erörterten, transplantatlosen Vorrichtung und die Merkmale der Transplantattechniken des Standes der Technik. Die Öffnung84 kann sich durch den Hauptkörper72 erstrecken (d.h. sie ist eine Durchgangsöffnung) oder sich nur teilweise durch den Hauptkörper72 erstrecken. Die Flächen, die die Öffnung definieren, können vorzugsweise auch strukturiert/aufgeraut sein und flache Vertiefungen/Öffnungen beinhalten. - Die
5 und6 sind zusätzliche Perspektivansichten des transplantatlosen Wirbelsäulenimplantates70 . -
7 veranschaulicht eine Perspektivansicht noch einer anderen Ausführungsform eines transplantatlosen Wirbelsäulenimplantates100 . Dieses Wirbelsäulenimplantat gleicht mit der Ausnahme, dass diese Ausführungsform zugespitzte bzw. verjüngte Kanten102 auf mindestens einer Seite der Vorrichtung enthält, im Wesentlichen dem in den4 –6 veranschaulichten Implantat. Die verjüngten Kanten102 ermöglichen das Einführen des Implantates in die Keilnuten (nicht gezeigt), die in die Wirbel geschnitten sind. In dieser Ausführungsform ist das Implantat100 auch mit einer bioaktiven Beschichtung beschichtet, wie beispielsweise Hydoxylapatit. - Das Implantat selbst kann eine Vielfalt von derzeit akzeptablen, biokompatiblen Materialien enthalten, wie z.B. Titan, Polyphenolsulfon, Polyaryletherketon, (PEEK), Polysulfon, Azetal (Delrin), UHMW-Polyethylen und Verbundstoffe aus diesen Materialien, welche hochfeste Kohlenstofffasern oder REM-Glasfilamente involvieren, um Zugfestigkeit und Scherfestigkeit hinzuzufügen. Das Implantat kann auch aus einem humanen Knochenallotransplantatmaterial, Autotransplantatmaterial oder Knochenergänzungsmnaterial hergestellt werden, wie beispielsweise Korallen oder Kalziumphosphat. Der Körper des Implantates kann optional eine kleine Verjüngung aufweisen.
- Zwar wurde die vorliegende Erfindung in Bezug auf mehrere bevorzugte Ausführungsformen derselben gezeigt und beschrieben, aber verschiedene Änderungen, Auslassungen und Hinzufügungen zur Form und zum Detail derselben können in derselben vorgenommen werden, ohne vom Bereich der Erfindung abzuweichen.
Claims (8)
- Wirbelsäulenimplantat zum mechanischen Verbinden von zwei benachbarten Wirbeln, umfassend: einen Körper (
12 ,72 ) mit ersten und zweiten gegenüberliegenden Flächen (14 ,16 ,80 ,82 ); hervorstehende Teile (18 ,19 ,20 ,21 ,74 ,75 ,76 ,77 ), die sich von den ersten und zweiten gegenüberliegenden Flächen (14 ,16 ,80 ,82 ) zum Befestigen des Körpers an benachbarte Wirbel erstrecken; wobei die äußeren Flächen des Körpers und die hervorstehenden Teile eine bioaktive Beschichtung beinhalten; dadurch gekennzeichnet, dass zwei hervorstehende Teile (18 ,19 ,20 ,21 ,74 ,75 ,76 ,77 ) sich entlang einer jeden der ersten und zweiten gegenüberliegenden Flächen (14 ,16 ,80 ,82 ) erstrecken und aus ihnen hervorstehen, wobei die hervorstehenden Teile hufeisenförmige Zungenvorsprünge zum Eingreifen in Nuten, die in Wirbeln ausgebildet sind, sind. - Wirbelsäulenimplantat gemäß Anspruch 1, wobei die bioaktive Beschichtung Hydroxylapatit umfasst.
- Wirbelsäulenimplantat gemäß Anspruch 2, wobei der Körper im Wesentlichen rechteckig ist.
- Implantat gemäß Anspruch 1, des weiteren gekennzeichnet durch eine Öffnung in dem Körper, in die Knochentransplantatmaterialien eingebracht werden können.
- Implantat gemäß Anspruch 4, wobei die Öffnung ein Durchgangsloch ist.
- Implantat gemäß Anspruch 4, wobei die Wände, die die Öffnungen definieren, strukturiert sind.
- Verfahren zum Herstellen eines Wirbelsäulenimplantates gemäß Anspruch 1, umfassend: Bereitstellen eines Wirbelsäulenimplantates; und Beschichten äußerer Flächen des Wirbelsäulenimplantates mit einer bioaktiven Beschichtung; gekennzeichnet durch Formen des Wirbelsäulenimplantates, so dass es hervorstehende Teile, die sich von gegenüberliegenden Flächen des Wirbelsäulenimplantates erstrecken, besitzt, wobei die hervorstehenden Teile hufeisenförmige Zungenvorsprünge sind.
- Verfahren gemäß Anspruch 7, wobei die bioaktive Beschichtung Hydroxylapatit umfasst.
