Description

  • Die vorliegende Erfindung betrifft eine Vorrichtung zur Knochenreparatur durch Arthrodese, die zur Wiederherstellung eines geschädigten Knochens bestimmt ist, insbesondere für die Halswirbel.
  • Im Rahmen von Operationen, die durchgeführt werden, um Knochenerkrankungen in der Wirbelsäule zu behandeln, nimmt der Chirurg eine vollständige oder teilweise Abtragung eines oder mehrerer Wirbelkörper vor. Die entfernten Knochen bilden nun keinerlei mechanische Stütze mehr. Um diesem Mangel abzuhelfen, ist es bekannt, Stäbe, im Allgemeinen aus Titan, zu verwenden und die durch die Abtragungen frei gewordenen Räume mit einem neutralen und biokompatiblen Füllmaterial zu füllen. Ein Stab wird zwischen zwei gesunde Wirbel eingeführt, um für eine erste Versteifung zu sorgen, die es ermöglicht, die Aushärtung des injizierten teigigen Materials abzuwarten. Man kann auch ein einstückiges autologes Transplantat oder einen Knochen aus einer Knochenbank oder einen Knochenersatz verwenden.
  • Eine andere Technik besteht darin, den Stab und das biokompatible teigige Material durch eine starre Gitterstruktur ersetzen, die Wirbelkörper oder „vertebral body" genannt wird und in die ein Knochentransplantat eingefügt wird, um den durch die Abtragung hinterlassenen Freiraum zu füllen und eine Knochenstütze zu bilden. Das Einsetzen des starren Käfigs zwingt jedoch dazu, die angrenzenden Wirbel wegzuziehen, was zu Komplikationen bei der Operation führt. Der Käfig ist darüber hinaus undurchlässig gegenüber Röntgenstrahlen.
  • Ein erster Zweck der Erfindung ist es, diesem Mangel abzuhelfen.
  • Das Dokument WO-A-98/28 725 offenbart eine Vorrichtung zur Knochenreparatur durch Transplantation, umfassend einen mit biologisch resorbierbarem Material beschichteten Schlauch, in den ein Transplantat eingeführt werden kann, wobei der Schlauch aus einem Metallgeflecht gebildet ist.
  • Gemäß der Erfindung ist die Vorrichtung zur Reparatur eines geschädigten Knochens durch Transplantation dadurch gekennzeichnet, dass sie einen breitmaschigen, biokompatiblen Gewebeschlauch umfasst, der mit einem biologisch resorbierbaren Material beschichtet ist und in den ein Transplantat eingeführt werden kann.
  • Unter Gewebe versteht man sowohl Gewirke als auch gewebte Textilien oder Vliese. Es kann biologisch resorbierbar oder nicht biologisch resorbierbar sein. Der Schlauch wird in eine Knochenkavität eingeführt, um sie zu füllen. Vorzugsweise wird der Schlauch jedoch durch eine Platte gehalten, die durch Schrauben an gesunden Knochenteilen befestigt ist, wobei im Fall von Wirbeln der Schlauch durch die Wirbelplatten des nächsthöheren bzw. nächsttieferen Wirbels in Position gehalten wird.
  • Der Schlauch wird durch eine biokompatible, resorbierbare oder nicht resorbierbare, breitmaschige textile Hülle gebildet. Das Beschichtungsmaterial kann eine Zusammensetzung aus Poly-L-Lactid, Polyglycolsäure oder ihren Copolymeren sein. Poly-L-Lactid wird mit der Zeit resorbiert, was ein Wachstum des Transplantats durch die Maschen in Richtung der gesunden, durch die Abtragung freigelegten Knochenteile ermöglicht. Der Gewebeschlauch alleine wäre zu biegsam, um einen guten Kontakt des Implantats mit den gesunden Knochenwänden des geschnittenen Teils zu gewährleisten.
  • Die Beschichtung mit dem biologisch resorbierbaren Material wie z.B. PLLA ermöglicht es,
    • – den Schlauch während einer gewissen Zeit zu versteifen;
    • – ihm eine geeignete anatomische Form zu verleihen;
    • – den Schlauch in einer ersten Zeit undurchdringlich für die Transplantate zu machen, um ihre Verstreuung zu vermeiden.
  • Gemäß einem anderen Merkmal der Erfindung dient im Fall von Wirbeln die Platte auch dazu, das Implantat mit Hilfe von mindestens einer Schraube in Position zu halten, deren Ende in das Knochentransplantat versenkt ist.
  • Im Fall von Wirbeln ist es bekannt, die vollständige oder teilweise Abtragung des Wirbelkörpers von vorne her vorzunehmen, wodurch der nötige Platz zum Einsetzen des Schlauchs geschaffen wird, der mit Transplantaten oder komprimierten Knochenfragmenten gefüllt ist. Zweck der Platte ist es, dem Gerüst eine gute mechanische Festigkeit zu verleihen und den Schlauch, der ein Füllprodukt enthält, während der Wartezeit bis zur Verfestigung der Knochen in einem genau abgegrenzten Bereich zwischen den Knochen zu halten.
  • Auf Grund seiner Zusammensetzung ist der Schlauch durchlässig gegenüber Röntgenstrahlen, was es erlaubt, die Entwicklung des Knochentransplantats mittels Radiographie zu beobachten und den Verlauf der Verfestigung des Transplantats zu überwachen. Der Gewebeschlauch kann durch einfaches Zuschneiden mittels einer Schere an den verfügbaren Zwischenwirbelraum angepasst werden. Nach der Resorption des PLLA können die Transplantate zu den Rippenwänden hin wachsen und eine Verschmelzung mit den Letztgenannten hervorrufen, wodurch die gewünschte Arthrodese erzeugt und der Schlauch überflüssig wird.
  • Weitere Merkmale und Vorteile der Erfindung werden aus der folgenden Beschreibung einer speziellen, als nicht erschöpfen des Beispiel angeführten Ausführungsform ersichtlich, bei der auf die beiliegenden Zeichnungen Bezug genommen wird, wobei:
  • 1 eine Seitenansicht mit teilweisem Vertikalschnitt des Implantats in Position in der Kavität eines Wirbels darstellt, der einer Wirbelkörperentfernung unterzogen wurde, und zwar zwischen zwei gesunden Wirbeln;
  • 2 in Draufsicht einen Horizontalschnitt eines Wirbels darstellt, der einer teilweisen Wirbelkörperentfernung unterzogen wurde.
  • In den Figuren bezeichnen die gleichen Bezugszeichen die gleichen Elemente. In 1 ist zu sehen, dass das Implantat aus einem im Wesentlichen zylindrischen Schlauch 1 gebildet wird, der Knochentransplantate 2 enthält und zwischen zwei gesunden Wirbeln V eingeschlossen ist. Eine Platte 3 wird entlang dem Schlauch 1 angebracht, wobei die Platte 3 an den Wirbel geschraubt wird, der über bzw. unter dem Wirbel liegt, der einer teilweisen Abtragung des Körpers unterzogen wurde. Die Platte 3 wird durch Halteschrauben 4 an ihren beiden Enden in Position gehalten, wobei die Schrauben 4 in den beiden gesunden Wirbeln V fixiert werden. Das Implantat ist mit der Platte 3 durch mindestens eine Schraube 5 verbunden, die sich in dem dargestellten Beispiel in der Mitte befindet. Gemäß der Erfindung lösen sich die breiten Maschen 6 nach einer gewissen Zeit auf, um das Wachstum der Transplantate zu ermöglichen. In dieser Figur sind auch die Quer- und Dornfortsätze (nicht mit Bezugszeichen versehen) der Wirbel zu erkennen.
  • 2 zeigt eine Draufsicht des Implantats in Position im Inneren eines aufgeschnittenen Wirbelkörpers V1. In dieser Figur wurde der Markkanal schematisch dargestellt (nicht mit Bezugszeichen versehen). In der Mitte der Platte 3 ist die Schraube 5 zu erkennen, die durch den Schlauch 1 dringt und in der Masse der Knochentransplantate fixiert wird, um dem Schlauch 1 sicheren Halt zu verleihen.
  • Obwohl die vorstehende Beschreibung die Wirbel betrifft, kann die Erfindung in allen Fällen angewendet werden, in denen es darum geht, Kavitäten zwischen zwei Knochen, insbesondere im Fall einer Abtragung, zu füllen.
  • Selbstverständlich können zahlreiche Änderungen vorgenommen werden, insbesondere durch eine ersatzweise Anwendung gleichwertiger technischer Mittel, ohne deswegen den Rahmen der Erfindung zu überschreiten.

Claims (7)

  1. Vorrichtung zur Knochenreparatur durch Transplantation und Arthrodese eines verletzten Knochens, dadurch gekennzeichnet, dass diese einen breitmaschigen, biokompatiblen, biologisch resorbierbaren oder nicht biologisch resorbierbaren Gewebeschlauch (1) umfasst, der mit einem biologisch resorbierbaren Material beschichtet ist und in den ein Transplantat (2) einführbar ist.
  2. Reparaturvorrichtung nach Anspruch 1, dadurch gekennzeichnet, dass diese eine Platte (3) aufweist, um den Schlauch (1) in Position zu halten, wobei die Platte (3) mit Schrauben (4) auf gesunden Knochenteilen (V) fixierbar ist.
  3. Reparaturvorrichtung nach Anspruch 2, dadurch gekennzeichnet, dass die Vorrichtung eine Schraube (5) zur Befestigung des Schlauchs (1) an der Platte (3) aufweist, deren Ende in das Knochentransplantat versenkbar ist.
  4. Reparaturvorrichtung nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass das Knochentransplantat (2) aus komprimierten Knochenfragmenten und/oder aus Knochenersatzmitteln gebildet ist.
  5. Reparaturvorrichtung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass das biologisch resorbierbare Beschichtungsmaterial auf Poly-L-Lactid und/oder Polyglycolsäure und/oder ihren Copolymeren basiert.
  6. Reparaturvorrichtung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass das den Schlauch (1) bildende Gewebe ein gewobenes Textil oder Vlies oder ein Gewirk ist.
  7. Reparaturvorrichtung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass diese in eine Kavität einsetzbar ist, welche Ergebnis der Abtragung eines Teils des Knochens ist.
