Description
- Die vorliegende Erfindung betrifft modulare Komponenten eines prothetischen Gelenks und im Besonderen modulare Komponenten eines prothetischen Kniegelenks.
- Das Kniegelenk besteht im Grunde genommen aus der Knochenschnittstelle des distalen Endes des Femur und des proximalen Endes der Tibia. Die Patella, die ein Sesamknochen innerhalb der Sehne des langen Muskels (Quadrizeps) an der Vorderseite des Oberschenkels ist, scheint diese Schnittstelle zu bedecken oder zumindest zum Teil zu schützen. Diese Sehne insertiert in den Tuberositas tibiae, und die posteriore Fläche der Patella ist glatt und gleitet über den Femur.
- Der Femur ist mit zwei knopfartigen Fortsätzen (medialer Gelenkkopf und lateraler Gelenkkopf) versehen, die im wesentlichen glatt sind und mit dem medialen Plateau bzw. dem lateralen Plateau der Tibia gelenkig verbunden sind. Die Plateaus der Tibia sind im wesentlichen glatt und leicht ausgehöhlt, wodurch sie eine kleine Aufnahmeeinrichtung für die Aufnahme der Femur-Gelenkköpfe bereitstellen.
- Wenn das Kniegelenk verletzt wird, gleich ob im Ergebnis eines Unfalls oder einer Erkrankung, kann ein prothetischer Ersatz des geschädigten Gelenks erforderlich sein, um Schmerzen zu lindern und um den normalen Gebrauch des Gelenks wiederherzustellen. Typischerweise wird das gesamte Kniegelenk durch ein chirurgisches Verfahren ersetzt, welches das Entfernen der Enden der entsprechenden geschädigten Knochen und das Ersetzen dieser Enden mit prothetischen Implantaten involviert. Dieses Ersetzen eines nativen Gelenks durch ein prothetisches Gelenk wird als primäre Arthroplastik des gesamten Knies bezeichnet.
- Gelegentlich versagt die primäre Knieprothese. Das Versagen kann auf viele Ursachen zurückzuführen sein, einschließlich von Abnutzung, aseptischer Lockerung, Osteolyse, Bänderinstabilität, Arthrofibrose und patellofemoralen Komplikationen. Wenn das Versagen zur Schwächung führt, kann eine Revisions-Knieoperation erforderlich werden. Bei einer Revision wird die primäre Knieprothese entfernt und mit Komponenten eines prothetischen Revisions-Kniesystems ersetzt.
- Bei der Revisions-Knieoperation oder Arthroplastik kann Knochenverlust auf der proximalen Tibia es schwierig gestalten, die Tibiakomponente des Revisions-Systems in Bezug auf den intramedullären Kanal der Tibia ordnungsmäßig zu stabilisieren. Verschiedene Implantatgestaltungen stehen für die Immobilisierung und Stabilisierung der Tibiakomponente und für die Verteilung der Belastung am proximalen Ende der Tibia zur Verfügung.
- So ist zum Beispiel die Bereitstellung integraler Kiele bekannt, welche sich vom Schaft der Tibiakomponente zur distalen Fläche der Tibiawanne erstrecken. Ein solches prothetisches Implantat wird in
1 veranschaulicht. Wie dort gezeigt wird, schließt die Tibiakomponente10 eine Tibiawanne12 und einen Tibiaschaft14 ein, der sich nach außen von der distalen Seite16 der Tibiawanne12 und integral mit derselben erstreckt. Bei dieser Komponente des Standes der Technik erstrecken sich drei integrale Kiele18 ,20 ,22 zwischen dem Schaft14 und der distalen Fläche16 der Wanne12 . Der Schaft ist hohl und kann eine Schaftverlängerung (nicht gezeigt) an seinem distalen Ende24 aufnehmen. Die Kiele18 ,20 ,22 dienen dazu, die Stabilisierung des Implantats10 in der proximalen Tibia zu unterstützen. - Die Bereitstellung einer stufenförmigen oder terrassenförmigen Hülse für den Tibiaschaft ist ebenfalls bekannt. Diese Hülsen werden im Handel als "S-ROM"-Produkte von DePuy Orthopaedics, Inc. aus Warsaw, Indiana, verkauft. Ein Beispiel eines solchen Systems wird in
2 –5 der Zeichnungen veranschaulicht. Die Tibiakomponente30 dieses Systems des Standes der Technik weist ebenfalls eine Tibiawanne32 und einen integralen Schaft34 auf, der mit Schaftverlängerungen (nicht gezeigt), am distalen Ende38 verbunden werden kann. Bei diesem System des Standes der Technik gibt es jedoch keine Kiele auf der distalen Seite36 der Tibiawanne32 . Bei diesem System des Standes der Technik wird eine separate, modulare Hülse bereitgestellt. Ein Beispiel einer solchen modularen Hülse wird in2 –4 veranschaulicht. Die modulare Hülse wird bei40 in2 –4 gezeigt. Die modulare Hülse40 weist eine sich verjüngende Innenbohrung42 auf, in welcher ein Teil des Schaftes34 der Tibiakomponente40 von5 aufgenommen wird. Die sich verjüngende Innenbohrung42 ist am breitesten am proximalen Ende und am schmalsten am distalen Ende, was der Form des Schafts34 entspricht. Wie in2 und3 gezeigt wird, weist die proximale Fläche44 der modularen Hülse40 eine im allgemeinen elliptische Form auf, und sie ist im allgemeinen flach, um gegen die distale Fläche36 der Tibiawanne32 zu passen. Wie in4 gezeigt wird, weist die modulare Hülse eine stufenförmige oder terrassenförmige äußere Fläche46 auf. Die Hülsen unterstützen die Stabilisierung des Implantats, besonders im Falle von Knochenverlust, und sie verteilen Belastung. - Die
EP-A-904749 - Die vorliegende Erfindung stellt ein modulares prothetisches Implantatsystem bereit, welches dem Chirurgen und dem Patienten die Vorteile sowohl einer Tibiakomponente mit Kielen als auch einer modularen Hülsengestaltung bietet, was die Optionen des Chirurgen während des chirurgischen Eingriffs erhöht, so daß die optimale Implantatgestaltung für die Bedürfnisses des Patienten ausgewählt werden kann.
- Bei einer Ausführungsform stellt die vorliegende Erfindung eine modulare prothetische Tibiahülse bereit, die eine proximale Fläche umfaßt, welche eine im allgemeinen ovale Gestalt mit im Abstand angeordneten Aussparungen aufweist. Die Hülse weist ebenfalls ein distales Ende und eine terrassenförmige Außenfläche auf, die sich von der proximalen Fläche hin zum distalen Ende verjüngt. Die Hülse hat einen inneren Kanal, der sich von der proximalen Fläche zum distalen Ende hin erstreckt.
- Bei einer anderen Ausführungsform stellt die vorliegende Erfindung ein prothetisches Kniesystem bereit, das eine Tibiakomponente und eine modulare Hülse umfaßt. Die Tibiakomponente umfaßt eine Tibiawanne, einen Schaft und Kiele. Die modulare Hülse umfaßt eine proximale Fläche mit im Abstand angeordneten Aussparungen, ein distales Ende und einen inneren Kanal, der sich von der proximalen Fläche zum distalen Ende erstreckt. Die Aussparungen sind so dimensioniert, geformt und positioniert, daß die modulare Hülse auf der Tibiakomponente montierbar ist, wobei der Schaft der Tibiakomponente in dem inneren Kanal der Hülse aufgenommen ist und die Kiele der Tibiakomponente in den im Abstand angeordneten Aussparungen der modularen Hülse aufgenommen sind.
- Ausführungsformen der Erfindung werden jetzt beispielhaft unter Bezugnahme auf die beigefügten Zeichnungen beschrieben werden. Es zeigt:

6 bis22 ein prothetisches Knieimplantatsystem100 , welches eine Tibiakomponente102 und eine modulare Hülse104 einschließt. Die Tibiakomponente102 und die modulare Hülse104 können als Komponenten eines prothetischen Knieimplantatsystems bereitgestellt werden, das ebenfalls eine Femurkomponente und ein Tibialager einschließt. - Ein Beispiel einer Femurkomponente wird bei
103 in21 veranschaulicht, und ein Beispiel eines Tibialagereinsatzes wird bei105 in22 veranschaulicht. Es versteht sich, daß diese veranschaulichten Komponenten103 ,105 als Beispiele von Komponenten bereitgestellt werden, die mit der Tibiakomponente102 und modularen Hülse104 zum Einsatz kommen können, die in6 bis20 gezeigt werden. Die vorliegende Erfindung ist nicht beschränkt auf die veranschaulichten speziellen Femur- und Tibialagereinsatzkomponenten103 ,105 oder auf die veranschaulichten Merkmale dieser Komponenten, es sei denn, dies wird ausdrücklich in den Ansprüchen verlangt. - Die Tibiakomponente
102 des veranschaulichten Systems100 schließt eine Tibiawanne106 und einen integralen Schaft108 ein, der sich distal von der distalen Seite110 der Tibiawanne106 aus erstreckt. Zwei Kiele112 ,114 sind einstückig mit der Tibiawanne106 und dem Schaft108 und verbinden diese. Bei der veranschaulichten Ausführungsform erstrecken sich die zwei Kiele112 ,114 nach außen und hin zur posterioren Seite116 der Wanne106 und definieren einen Winkel von 130°, wie bei θ in9 gezeigt wird. Es versteht sich jedoch, daß diese spezielle Kiel-Konfiguration nur beispielhaft bereitgestellt wird; die Erfindung ist nicht beschränkt auf eine spezielle Anzahl oder ein geometrisches Verhältnis zwischen den Kielen, es sei denn, dies wird ausdrücklich in den Ansprüchen verlangt; die Tibiakomponente könnte zum Beispiel drei oder mehr Kiele haben. - Wie in
9 gezeigt wird, ist die veranschaulichte Tibiakomponente symmetrisch um eine zentrale Ebene121 , welche die Tibiawanne106 in mediale und laterale Abschnitte teilt. - Der Schaft
108 verjüngt sich von einem breitesten Abmaß an der Tibiawanne106 zu einem schmalsten Abmaß am distalen Ende109 des Schafts108 . Der Schaft hat eine anteriore Seite117 und eine posteriore Seite119 . Der zentrale Schaft108 hat einen Morsekonus. - Wie in
10 bis11 und in13 gezeigt wird, weist die veranschaulichte modulare Hülse104 eine abgestufte oder terrassenförmige äußere Fläche118 auf, die sich bei jeder Stufe vom proximalen Ende120 zum distalen Ende122 hin verjüngt. Diese terrassenförmige Fläche118 erfüllt die gleiche Funktion wie die terrassenförmige Fläche der modularen Hülse des Standes der Technik: sie trägt dazu bei, das Implantat in der Tibia zu stabilisieren, besonders im Falle von Knochenverlust, und Belastung in der Tibia zu verteilen. Die Erfindung ist jedoch nicht auf die veranschaulichte Gestaltungsform beschränkt, es sei denn, dies wird ausdrücklich durch die Ansprüche gefordert. - Wie in der Draufsicht von
12 gezeigt ist, hat die modulare Hülse104 der veranschaulichten Ausführungsform insgesamt einen elliptischen Umriß, mit einer Hauptachse124 und einer Nebenachse126 und einer zentralen longitudinalen Achse128 . Sie weist eine proximale Fläche129 mit einer nach hinten zeigenden Kante130 und einer nach vorn zeigenden Kante132 . Die Hauptachse124 definiert einen posterioren Abschnitt131 und einen anterioren Abschnitt133 der proximalen Fläche (in12 und in16 gezeigt). - Die proximale Fläche weist einen im allgemeinen ovalen Umriß in der Draufsicht auf. Wie in
12 und16 gezeigt wird, schließt die modulare Hülse104 der veranschaulichten Ausführungsform zwei Aussparungen134 ,136 in der proximalen Fläche129 ein, im allgemeinen im posterioren Abschnitt131 zwischen der Hauptachse124 und der nach hinten zeigenden Kante132 der modularen Hülse104 . Diese Aussparungen134 ,136 sind so geformt, daß sie die zwei Kiele112 ,114 der Tibiakomponente110 so aufnehmen, daß die modulare Hülse mit der mit Kielen versehenen Tibiakomponente genutzt werden kann. - Die zwei Aussparungen
134 ,136 sind so geformt, daß sie eine relative Drehung zwischen der Tibiakomponente110 und der Hülse104 ermöglichen, wenn die Tibiakomponente110 und die Hülse104 zusammengebaut sind.16 veranschaulicht die Winkelverhältnisse verschiedener Strukturen der Hülse104 . Bei der veranschaulichten Ausführungsform beträgt der Winkel α 56°, der Winkel β 25° und der Winkel θ 130°, um mit dem Winkel θ zwischen den Kielen112 ,114 zu korrespondieren, wie in9 gezeigt wird, und φ beträgt 25°. Mit diesen Winkelverhältnissen gestatten die Formen der Kiele112 ,114 und die Aussparungen134 ,136 in der veranschaulichten Ausführungsform eine relative Drehung von zirka plus oder minus 20° zwischen diesen Komponenten104 ,110 , wenn sie montiert sind. Es versteht sich jedoch, daß diese Winkel nur als Beispiele angegeben werden; die vorliegende Erfindung ist nicht auf ein spezielles Winkelverhältnis beschränkt, es sei denn, dies wird ausdrücklich in den Ansprüchen angegeben. Darüber hinaus ist die vorliegende Erfindung nicht auf einen speziellen Grad relativer Bewegung beschränkt, die zwischen den Komponenten gestattet ist, es sei denn, dies wird ausdrücklich in den Ansprüchen verlangt. Es versteht sich, daß die spezielle Form und das geometrische Verhältnis zwischen den Aussparungen134 ,136 nur als Beispiel angegeben werden; die vorliegende Erfindung ist nicht auf die beschriebene und veranschaulichte Struktur beschränkt, es sei denn, dies wird ausdrücklich in den Ansprüchen verlangt. - Die modulare Hülse
104 weist ebenfalls einen inneren Kanal140 auf, der sich von der proximalen Fläche129 zum distalen Ende122 der Hülse104 hin erstreckt. Der innere Kanal140 ist so geformt, daß er den Schaft108 der Tibiakomponente110 aufnimmt und reibschlüssig mit diesem gekoppelt ist, wenn die Hülse104 und die Tibiakomponente110 zusammengebaut und miteinander verkeilt sind. Der innere Kanal140 verjüngt sich von einem größten Durchmesser an der proximalen Fläche129 zu einem kleinsten Durchmesser am distalen Ende122 . Die Konen des inneren Kanals140 und des Schafts108 sind Morsekonen. Sobald der Schaft108 und die Hülse104 reibschlüssig miteinander gekoppelt sind, gibt es nicht länger eine relative Drehung oder longitudinale Bewegung zwischen dem Schaft108 und der Hülse104 . - Die Komponenten des Systems der vorliegenden Erfindung können aus Standardmaterialien für diese prothetischen Implantate hergestellt werden. Die modulare Hülse kann zum Beispiel aus einer Titanlegierung oder einer Kobalt-Chrom-Legierung hergestellt werden, und die Tibiakomponente kann ebenfalls aus einer Titan- oder Kobalt-Chrom-Legierung hergestellt werden.
- Für den Einsatz des Systems der vorliegenden Erfindung bereitet der Chirurg den Patienten vor, und er führt chirurgische Standardverfahren aus, um den Knochen für die Aufnahme der Tibiakomponente
110 vorzubereiten. Der Chirurg kann zum Beispiel den intramedullären Kanal der Tibia fortschreitend aufreiben, um den Schaft108 und eine Schaftverlängerung (nicht gezeigt) mit entsprechender Größe aufzunehmen. Eine Erprobung kann auf Standardweise vorgenommen werden. Während des Verfahrens bestimmt der Chirurg [...]110 allein ohne die Hülse104 . Wenn die proximale Tibia ausreichend gesundes Knochengewebe aufweist, kann der Chirurg die Tibiakomponente110 auf Standardweise implantieren, und das Zusammenwirken der Kiele112 ,114 und des Knochengewebes des Patienten sollte die Tibiakomponente110 im Knochen stabilisieren. Wenn der Chirurg feststellt, daß es einen Hohlraumdefekt oder ungenügendes gesundes Knochengewebe in der proximalen Tibia gibt, kann der Chirurg die proximale Tibia für die Aufnahme der Anordnung aus Tibiakomponente und Hülse unter Verwendung von Standard-Aufreibern und -aufräumern vorbereiten. Der Chirurg kann dann den Schaft108 durch den Kanal140 der Hülse104 einführen und sie gemeinsam bewegen, bis die Kiele112 ,114 in den Aussparungen134 ,136 der Hülse aufgenommen worden sind. Die modulare Anordnung der Tibiakomponente110 und der Hülse104 kann dann in einer Standardweise implantiert werden. Das Festkeilen der Anordnung110 ,104 sollte die Komponenten110 ,104 miteinander koppeln. Der Chirurg kann andere Standardverfahren ausführen, um die Vorbereitung des Femur abzuschließen. - Die modulare Anordnung aus Tibiakomponente
110 und Hülse104 wird in den17 –20 veranschaulicht. Die Gestaltung der Aussparungen134 ,136 bei der vorliegenden Erfindung läßt eine gewisse unabhängige Positionierung der Hülse104 und der Tibiakomponente110 zu, wie in den18 –20 gezeigt wird. Der Chirurg kann die Hülse104 in einer optimalen Position für die Stabilisierung des Implantats plazieren, während er ebenfalls die Tibiakomponente110 in der geeigneten Position für das Zusammenwirken mit der Femurkomponente103 des Implantatsystems plaziert. Die Hülse104 kann zum Beispiel, wie in18 gezeigt wird, auf der Tibiakomponente so plaziert werden, daß die Kiele112 ,114 im wesentlichen in den Aussparungen134 ,136 zentriert werden; bei dieser Gestaltung kann der Winkel λ zwischen der zentralen Ebene121 der Tibiawanne und einer Ebene durch die Hauptachse124 der Hülse104 hindurch variieren: bei18 beträgt der Winkel λ 90°. Die Kiele können in den Aussparungen auch außermittig sein, wie in19 und20 gezeigt wird: bei19 beträgt der Winkel λ 70°; und bei20 beträgt der Winkel λ 110°. Somit gestattet die veranschaulichte Ausführungsform der Erfindung dem Chirurgen, die Tibiakomponente und die Hülse um den inneren Kanal140 der Hülse zu drehen, mit einem Spielraum von +20°. Es versteht sich, daß diese Winkel nur als Beispiele angegeben werden; die vorliegende Erfindung ist nicht auf diese Winkel noch auf einen besonderen Freiheitsgrad beschränkt, es sei denn, dies wird in den Ansprüchen ausdrücklich verlangt. - Sobald die optimale relative Drehposition ausgewählt worden ist, kann der Chirurg die Tibiakomponente
102 und die Hülse104 in dieser optimalen Position durch das Festkeilen der Komponenten für die reibschlüssige Ineingriffnahme des Schafts108 und der inneren Fläche der Hülse104 , welche den inneren Kanal140 definiert, miteinander verkoppeln. Somit können die Tibiakomponente102 und die Hülse104 in einer Vielfalt von relativen Positionen verriegelt werden, wie zum Beispiel in den Positionen, welche in den18 bis20 veranschaulicht werden. - Es wird für die Fachleute selbstverständlich sein, daß unterschiedliche Größen von Tibiakomponenten
110 und Hülsen104 in einem einzigen Chirurgie-Kit für die Verwendung durch den Chirurgen bereitgestellt werden können. - Es wird für die Fachleute ebenfalls selbstverständlich sein, daß die veranschaulichte Tibiakomponente in Verbindung mit einem Tibiallagereinsatz verwendet würde, wie er bei
105 in22 veranschaulicht wird, und mit einer Femurkomponente, wie sie bei103 in21 veranschaulicht wird. - Obwohl die Erfindung für den Einsatz bei der Revisionschirurgie veranschaulicht worden ist, können die Grundgedanken der vorliegenden Erfindung ebenfalls bei der primären Gelenkarthroplastik Anwendung finden.
Claims (13)
- Prothetisches Kniesystem, umfassend: eine Tibiakomponente (
102 ) mit einer Tibiawanne (106 ) und einem Schaft (108 ); eine modulare Hülse (104 ) mit einer proximalen Fläche (129 ) mit einem distalen Ende (122 ), einem inneren Kanal (140 ), der sich von der proximalen Fläche zum distalen Ende erstreckt, und einer Außenfläche zum Kontaktieren der Fläche eines Hohlraums in der resezierten Tibia; wobei die Tibiakomponente einen Kiel (112 ,114 ) aufweist und die modulare Hülse (129 ) eine Aussparung (134 ,136 ) aufweist, die so dimensioniert, geformt und positioniert ist, daß die modulare Hülse auf der Tibiakomponente montierbar ist, wobei (a) der Schaft der Tibiakomponente in dem inneren Kanal der Hülse aufgenommen ist, (b) der Kiel der Tibiakomponente in der Aussparung der modularen Hülse aufgenommen ist und (c) die Unterseite der Tibiawanne um den Schaft zur teilweise exponierten resezierten Tibia zeigt. - Prothetisches Kniesystem nach Anspruch 1, dadurch gekennzeichnet, daß die proximale Fläche (
129 ) der Hülse (104 ) eine allgemein ovale Gestalt aufweist, wobei eine Hauptachse (124 ) posteriore und anteriore Abschnitte (131 ,133 ) der proximalen Fläche definiert, und daß die Aussparung (134 ,136 ) sich in dem posterioren Abschnitt der proximalen Fläche der Hülse befindet. - Prothetisches Kniesystem nach Anspruch 1, dadurch gekennzeichnet, daß der Kiel (
112 ,114 ) und die Aussparung (134 ,136 ) so dimensioniert, geformt und positioniert sind, daß eine relative Drehung zwischen der Tibiakomponente (102 ) und der Hülse (104 ) ermöglicht wird. - Prothetisches Kniesystem nach Anspruch 3, dadurch gekennzeichnet, daß der Kiel (
112 ,114 ) und die Aussparung (134 ,136 ) so dimensioniert, geformt und positioniert sind, daß eine Drehung um plus oder minus zwanzig Grad ermöglicht wird. - Prothetisches Kniesystem nach Anspruch 3, dadurch gekennzeichnet, daß die Tibiakomponente (
102 ) eine Vielzahl von im Abstand angeordneten Kielen (112 ,114 ) aufweist und die Hülse (104 ) eine Vielzahl von im Abstand angeordneten Aussparungen (134 ,136 ) in der proximalen Fläche (129 ) zur Aufnahme der Kiele aufweist. - Prothetisches Kniesystem nach Anspruch 3, dadurch gekennzeichnet, daß der Schaft (
108 ) der Tibiakomponente (102 ) und der innere Kanal (140 ) der modularen Hülse (104 ) so geformt sind, daß die Tibiakomponente und die modulare Hülse mit in einer vorab festgelegten Position befindlicher Tibiakomponente und modularer Hülse miteinander reibschlüssig gekoppelt werden. - Prothetisches Kniesystem nach Anspruch 6, dadurch gekennzeichnet, daß die Tibiakomponente (
102 ) eine zentrale Ebene (121 ) aufweist, die mediale und laterale Abschnitte definiert, und die proximale Fläche (129 ) der modularen Hülse (104 ) eine Hauptachse (124 ) aufweist und daß die Tibiakomponente und die modulare Hülse in einer Position, in der der Winkel zwischen der zentralen Ebene der Tibiakomponente und der Hauptachse der proximalen Fläche der modularen Hülse kleiner als 90° ist, miteinander reibschlüssig koppelbar sind. - Prothetisches Kniesystem nach Anspruch 6, dadurch gekennzeichnet, daß die Tibiakomponente (
102 ) eine zentrale Ebene (121 ) aufweist, die mediale und laterale Abschnitte definiert, und die proximale Fläche (129 ) der modularen Hülse (104 ) eine Hauptachse (124 ) aufweist, und daß die Tibiakomponente und die modulare Hülse in einer Position, in der der Winkel zwischen der zentralen Ebene der Tibiakomponente und der Hauptachse der proximalen Fläche der modularen Hülse 90° beträgt, miteinander reibschlüssig koppelbar sind. - Prothetisches Kniesystem nach Anspruch 8, dadurch gekennzeichnet, daß die Tibiakomponente (
102 ) eine zentrale Ebene (121 ) aufweist, die mediale und laterale Abschnitte definiert, und die proximale Fläche (129 ) der modularen Hülse (104 ) eine Hauptachse (124 ) aufweist, und daß die Tibiakomponente und die modulare Hülse in einer Position, in der der Winkel zwischen der zentralen Ebene der Tibiakomponente und der Hauptachse der proximalen Fläche der modularen Hülse kleiner als 90° ist, miteinander reibschlüssig koppelbar sind. - Prothetisches Kniesystem nach Anspruch 9, dadurch gekennzeichnet, daß die Tibiakomponente (
102 ) eine zentrale Ebene (121 ) aufweist, die mediale und laterale Abschnitte definiert, und die proximale Fläche (129 ) der modularen Hülse (104 ) eine Hauptachse (124 ) aufweist, und daß die Tibiakomponente und die modulare Hülse in einer Position, in der der Winkel zwischen der zentralen Ebene der Tibiakomponente und der Hauptachse der proximalen Fläche der modularen Hülse zwischen 70° und 110° beträgt, miteinander reibschlüssig koppelbar sind. - Prothetisches Kniesystem nach Anspruch 1, dadurch gekennzeichnet, daß es zusätzlich mindestens eine Femurkomponente (
103 ) oder eine Tibialagerkomponente (105 ) enthält. - Modulare prothetische Tibiahülse mit einer proximalen Fläche (
129 ) mit einem distalen Ende (122 ), einem inneren Kanal (140 ), der sich von der proximalen Fläche zum distalen Ende erstreckt und einer Vielzahl von Aussparungen (134 ,136 ), die so dimensioniert, geformt und positioniert sind, daß die modulare Hülse auf der Tibiakomponente montierbar ist, wobei der Schaft der Tibiakomponente in dem inneren Kanal der Hülse aufgenommen ist und der Kiel der Tibiakomponente in der Aussparung der modularen Hülse aufgenommen ist, und mit einer terrassenförmigen Außenfläche (118 ), die sich von der proximalen Fläche in Richtung auf das distale Ende verjüngt. - Modulare prothetische Tibiahülse nach Anspruch 12, dadurch gekennzeichnet, daß die proximale Fläche (
129 ) eine posteriore Kante (130 ), eine anteriore Kante (132 ) und eine Hauptachse (124 ) zwischen der posterioren Kante und der anterioren Kante aufweist, die einen posterioren Abschnitt (131 ) und einen anterioren Abschnitt (133 ) definiert, und daß die Aussparungen (134 ,136 ) an dem posterioren Abschnitte der proximalen Fläche positioniert sind.
