Description
- Diese Erfindung betrifft pharmazeutische Aerosolformulierungen und insbesondere pharmazeutische Suspensions-Aerosolformulierungen, die Albuterolsulfat enthalten.
- Pharmazeutische Suspensions-Aerosolformulierungen verwenden gegenwärtig ein Gemisch aus flüssigen Chlorfluorkohlenwasserstoffen als Treibmittel. Fluortrichlormethan, Dichlordifluormethan und Dichlortetrafluorethan sind die in Aerosolformulierungen zur Verabreichung durch Inhalation am häufigsten verwendeten Treibmittel.
- Chlorfluorkohlenwasserstoffe (CFCs) werden jedoch mit der Zerstörung der Ozonschicht in Zusammenhang gebracht und ihre Herstellung ist im Auslaufen. Der Fluorkohlenwasserstoff 134a (HFC 134a, 1,1,1,2-Tetrafluorethan) und der Fluorkohlenwasserstoff 227 (HFC 227, 1,1,1,2,3,3,3-Heptafluorpropan) werden als ozonfreundlicher angesehen als viele Chlorfluorkohlenwasserstofftreibmittel; darüber hinaus haben sie eine geringe Toxizität und Dampfdrucke, die für die Verwendung in Aerosolen geeignet sind.
- Die Patentanmeldungen WO 91/11495 und WO 91/11496 (beide von Weil) beschreiben pharmazeutische Suspensions-Aerosolformulierungen, welche einen medizinischen Wirkstoff, gegebenenfalls ein Tensid und ein Treibmittelgemisch, enthaltend 1,1,1,2,3,3,3-Heptafluorpropan und eine oder mehrere zusätzliche Komponenten, z. B. Pentan, Butan, das Treibmittel 134a, das Treibmittel 11, das Treibmittel 125 oder das Treibmittel 152a, umfassen.
- Die Offenlegungsschrift des Europäischen Patentamts 0 384 371 (Heiskel) beschreibt Lösungsaerosole, in denen 1,1,1,2,3,3,3-Heptafluorpropan oder sein Gemisch mit Propan, Butan, Isobutan, Dimethylether oder 1,1-Difluorethan als Treibmittel dient. Die Anmeldung offenbart jedoch nicht Suspensions-Aerosole oder pharmazeutische Aerosolformulierungen.
- Die europäische Patentanmeldung 89.312270.5 (Purewal et al.) offenbart unter anderem Aerosolformulierungen, welche ein Medikament, 1,1,1,2-Tetrafluorethan, ein oberflächenaktives Mittel und mindestens eine Verbindung mit höherer Polarität als 1,1,1,2-Tetrafluorethan umfassen.
- Die US-Patentschrift 2,868,691 (Porush et al.) offenbart Aerosolformulierungen, welche ein Medikament, ein halogeniertes niederes Alkan als Treibmittel und ein Colösungsmittel, das die Auflösung des Medikaments in dem Treibmittel unterstützt, umfassen. Die chemische Formel für das Treibmittel, die in Spalte 2, Zeile 6–16, angegeben ist, umschließt HFC 134a und HFC 227. Beispiele offenbarter Colösungsmittel schließen Ethanol und Diethylether ein.
- Die US-Patentschrift 3,014,844 (Thiel et al.) offenbart Aerosolformulierungen, welche ein feinstzerkleinertes Medikament, ein halogeniertes niederes Alkan als Treibmittel und ein oberflächenaktives Mittel zur Unterstützung der Suspendierung des Medikaments in dem Treibmittel umfassen. Die chemische Formel für das Treibmittel, die in Spalte 4, Zeile 17–28 angegeben ist, umschließt HFC 134a und HFC 227.
- Die Patentanmeldung WO 1990/GB01454 (Greenleaf et al.) offenbart Aerosolzusammensetzungen, die HFC 134a als Treibmittel aufweisen und die ein Medikament umfassen, das mit einem nicht perfluorierten, oberflächenaktiven dispergierenden Mittel beschichtet ist. Diese Anmeldung beschreibt Kontrollformulierungen, die nur HFC 134a und 0,866 Gew.-% eines Arzneimittels enthalten.
- Albuterolsulfat ist ein relativ selektiver beta-2-adrenerger Bronchodilatator. Es ist in einer Vielfalt von Dosierungsformen, einschließlich Tabletten, Sirupen und Formulierungen, die zur Inhalation geeignet sind, verfügbar. Zum Beispiel ist VENTOLINTM Inhalation Aerosol (im Handel erhältlich von Allen & Hansburys) eine Aerosol-Dosierungseinheit, die eine mikrokristalline Suspension von Albuterol (freie Base) in einem Treibmittel (ein Gemisch aus Trichlormonofluormethan und Dichlordifluormethan) mit Ölsäure enthält. VENTOLIN ROTOCAPSTM for Inhalation (im Handel erhältlich von Allen & Hansburys) enthalten ein Gemisch von mikrofeinem Albuterolsulfat mit Lactose und sind zur Verwendung mit einem speziell entworfenen Gerät zum Inhalieren von Pulver gedacht. VENTOLINTM Solution for Inhalation (im Handel erhältlich von Allen & Hansburys) ist eine wäßrige Lösung von Albuterolsulfat, die für die Verwendung mit einem Zerstäuber gedacht ist.
- Diese Erfindung stellt pharmazeutische Suspensions-Aerosolformulierungen bereit, welche eine therapeutisch wirksame Menge mikronisiertes Albuterolsulfat, ein Treibmittel ausgewählt aus HFC 134a, HFC 227 und Gemischen davon und Ethanol in einer Menge von 5 bis 15 Gew.-% der Aerosolformulierung, umfassen.
- Diese Erfindung stellt auch einen Aerosolbehälter bereit, welcher eine wie vorstehend beschriebene Formulierung in einer Menge umfaßt, die ausreicht, um eine Vielzahl von therapeutisch wirksamen Dosierungseinheiten des Arzneistoffs bereitzustellen. Es wird auch ein Verfahren zur Herstellung einer wie vorstehend beschriebenen Formulierung bereitgestellt, umfassend die Schritte: (i) Vereinigen einer Menge des Arzneistoffs, die ausreicht, um eine Vielzahl von therapeutisch wirksamen Dosierungseinheiten bereitzustellen, und des Treibmittels in einer Menge, die ausreicht, um eine Vielzahl von therapeutisch wirksamen Dosierungseinheiten des Arzneistoffs aus einem Aerosolbehälter zu treiben; und (ii) Dispergieren des Arzneistoffs in dem Treibmittel. Diese Erfindung stellt weiterhin ein Verfahren zur Behandlung eines Säugers, der eine Erkrankung aufweist, die zur Behandlung durch Inhalation geeignet ist, umfassend den Schritt der Verabreichung einer wie vorstehend beschriebenen Formulierung an den Säuger durch Inhalation, bereit.
- Diese Erfindung stellt ebenfalls ein Verfahren zur Herbeiführung einer Bronchodilatation bei einem Säuger bereit, welches die Verabreichung einer wie vorstehend beschriebenen Formulierung durch Inhalation an den Säuger umfaßt.
- Der Begriff „Suspensions-Aerosolformulierung", wie er hier gebraucht wird, bezeichnet eine Formulierung, in der der Arzneistoff in Teilchenform vorliegt und im wesentlichen in dem Treibmittel unlöslich ist.
- Mengen, die hier in Form von Prozent ausgedrückt sind, bezeichnen Gew.-%, bezogen auf das Gesamtgewicht der Formulierung.
- Die Formulierungen der Erfindung enthalten einen Arzneistoff in einer therapeutisch wirksamen Menge, das heißt, einer derartigen Menge, daß der Arzneistoff als Aerosol (z. B. örtlich oder durch orale oder nasale Inhalation) verabreicht werden kann und seine erwünschte therapeutische Wirkung mit einer Dosierungseinheit, oder weniger bevorzugt mehreren Dosierungseinheiten, aus einem herkömmlichen Ventil, z. B. ein Dosierungsventil, hervorrufen kann. „Menge", wie sie hier gebraucht wird, bezeichnet je nach gegebenem Zusammenhang Menge oder Konzentration. Die Menge eines Arzneistoffs, die eine therapeutisch wirksame Menge bildet, ist abhängig von Faktoren wie der Wirkungsstärke, Wirksamkeit und dergleichen des speziellen Arzneistoffs, dem Wege der Verabreichung der Formulierung und dem Gerät, das zum Verabreichen der Formulierung verwendet wird. Eine therapeutisch wirksame Menge eines speziellen Arzneistoffs kann von einem Fachmann unter gebührender Berücksichtigung derartiger Faktoren ausgewählt werden. Besonders bei Formulierungen der Erfindung, die für orale Inhalation in die Lungen vorgesehen sind, ist der Arzneistoff vorzugsweise mikronisiert, d. h. etwa 90 Prozent der Teilchen oder mehr haben einen Durchmesser von weniger als etwa 10 μm, um sicherzustellen, daß die Teilchen in die Lungen inhaliert werden können.
- Die spezielle Menge von Arzneistoff, die in einer Formulierung der Erfindung für eine Zeit, die ausreicht, um eine reproduzierbare Dosierung des Arzneistoffs zu erlauben, suspendiert bleibt, hängt in gewissem Maße von den Eigenschaften des speziellen Arzneistoffs ab, z. B. seiner Dichte, und von dem speziellen Treibmittel, das in der Formulierung verwendet wird. Im allgemeinen wurde jedoch gefunden, daß, wenn Konzentrationen des Arzneistoffs von weniger als etwa 0,1 Prozent in einer Formulierung der Erfindung verwendet werden, der Arzneistoff in bestimmten Maße ausflockt, sich aber im allgemeinen nicht in dem Maße absetzt oder aufrahmt, daß die Suspension zur Verwendung als Aerosolformulierung, z. B. in einem Dosierungsinhalator, ungeeignet wird. Daher sind solche Formulierungen hinsichtlich der Arzneistoffkonzentration annehmbar homogen.
- Wenn Arzneistoffkonzentrationen, die größer als etwa 0,1 Prozent, aber kleiner als etwa 0,5 Prozent sind, in einer Formulierung der Erfindung verwendet werden, sieht man manchmal, daß das Arzneimittel in der Formulierung beträchtlich ausflockt und deshalb eine erhöhte Neigung haben könnte, sich aufzurahmen oder abzusetzen. Wie nachstehend im Zusammenhang mit der Treibmittelkomponente der Formulierungen der Erfindung diskutiert wird, ist es in diesen Fällen vorzuziehen, das Treibmittel in einer Weise auszuwählen, die Aufrahmen und Absetzen des Arzneistoffs minimiert, um sicherzustellen, daß die Formulierung hinsichtlich der Arzneistoffkonzentration annehmbar homogen ist.
- Wenn die Arzneistoffkonzentration anwächst, z. B. über etwa 0,5 Prozent hinaus, nimmt im allgemeinen die Neigung des Arzneistoffs zum Ausflocken ebenfalls zu. Jedoch nimmt das Volumen, das von dem ausgeflockten Arzneistoff eingenommen wird, ebenfalls zu, und der ausgeflockte Arzneistoff beginnt, im wesentlichen das gesamte Volumen der Formulierung auszufüllen. In solchen Fällen zeigt der ausgeflockte Arzneistoff oft eine geringere Neigung sich aufzurahmen oder abzusetzen. Hinsichtlich der Arzneistoffkonzentration sind derartige Formulierungen annehmbar homogen.
- Im allgemeinen ist die Konzentration des Arzneistoffs in einer Formulierung der Erfindung vorzugsweise kleiner als etwa 0,1 Prozent, stärker bevorzugt kleiner als etwa 0,08 Prozent und am meisten bevorzugt kleiner als etwa 0,05 Prozent. Dementsprechend wird gemäß dieser Erfindung bevorzugt, daß der Arzneistoff eine solche Wirkungsstärke hat, daß Konzentrationen von weniger als etwa 0,1 Prozent, stärker bevorzugt weniger als etwa 0,08 Prozent und am meisten bevorzugt weniger als etwa 0,05 Prozent therapeutisch wirksam sind.
- Der Arzneistoff, der in den Formulierungen der Erfindung verwendet wird, ist Albuterolsulfat.
- Das Treibmittel in einer Formulierung der Erfindung kann HFC 134a, HFC 227 oder ein Gemisch davon in jedem Verhältnis sein. Das Treibmittel ist in einer Menge vorhanden, die ausreicht, um eine Vielzahl von Dosierungseinheiten aus einem Dosierungsinhalator zu treiben.
- Die Dichte von HFC 134a unterscheidet sich von der Dichte von HFC 227. Deswegen kann die Dichte des Treibmittels innerhalb eines bestimmten Rahmens festgelegt werden, indem Gemische von HFC 134a und HFC 227 verwendet werden, um an die Dichte des Arzneistoffs anzupassen. Manchmal wird es bevorzugt, dass das Treibmittel so gewählt wird, dass die Treibmitteldichte so exakt wie möglich der Arzneistoffdichte entspricht, um Tendenzen zu minimieren, dass sich der Arzneistoff absetzt oder aufrahmt, speziell dann, wenn die Arzneistoffkonzentration größer als 0,1% oder wenn die Arzneistoffkonzentration zwischen ca. 0,1% und ca. 0,5% liegt.
- Die Albuterolsulfatformulierungen der Erfindung enthalten eine therapeutisch wirksame Menge mikronisiertes Albuterolsulfat. Vorzugsweise bildet mikronisiertes Albuterolsulfat etwa 0,2 bis etwa 0,5 Gew.-%, stärker bevorzugt etwa 0,35 bis etwa 0,42 Gew.-%, der Aerosolformulierung.
- Ethanol bildet von 5 bis 15 Gew.-% der Formulierung.
- Bestimmte bevorzugte Albuterolsulfatsuspensions-Aerosolformulierungen der Erfindung umfassen HFC 227 als im wesentlichen einziges Treibmittel. Typischerweise macht das Treibmittel den Rest des Gewichts der Formulierung aus, sobald das Albuterolsulfat und gegebenenfalls das Tensid und/oder das Ethanol in Rechnung gestellt sind. Dementsprechend ist das Treibmittel im allgemeinen in einer Menge von mindestens etwa 75 Gew.-%, bezogen auf das Gesamtgewicht der Formulierung, vorhanden.
- Bevorzugte Albuterolsulfatformulierungen der Erfindung zeigen im wesentlichen kein Wachstum in der Teilchengröße oder Veränderung in der Kristallmorphologie des Albuterolsulfats über einen ausgedehnten Zeitraum, sind im wesentlichen und leicht wiederdispergierbar und flocken nach dem Wiederdispergieren nicht so schnell aus, um reproduzierbares Dosieren von Albuterolsulfat zu verhindern.
- Im allgemeinen können die Formulierungen der Erfindung hergestellt werden durch Vereinigen (i) des Arzneistoffs in einer Menge, die ausreicht, um eine Vielzahl von therapeutisch wirksamen Dosierungseinheiten bereitzustellen; und (ii) des Treibmittels in einer Menge, die ausreicht, um eine Vielzahl von Dosierungseinheiten aus einem Aerosolbehälter zu treiben, und Dispergieren des Arzneistoffs in dem Treibmittel. Der Arzneistoff kann unter Verwendung eines herkömmlicher Mischers oder Homogenisierers, durch Schütteln oder durch Ultraschallenergie dispergiert werden. Eine Formulierung in großer Menge kann unter Verwendung von Ventil-zu-Ventil-Überführungsverfahren oder herkömmlichen Kälteabfüllverfahren in kleinere einzelne Aerosolampullen überführt werden.
- Die Albuterolsulfatsuspensions-Aerosolformulierungen dieser Erfindung können hergestellt werden, indem das Albuterolsulfat und das Treibmittel vereinigt werden und das Albuterolsulfat in dem Treibmittel unter Verwendung eines herkömmlichen Mischers oder Homogenisierers dispergiert wird. Ethanol kann zusammen mit dem Albuterolsulfat zu dem Treibmittel hinzugegeben werden.
- Aerosolbehälter, die mit herkömmlichen Ventilen, vorzugsweise Dosierungsventilen, ausgestattet sind, können verwendet werden, um die Formulierungen der Erfindung abzugeben. Es wurde jedoch gefunden, daß die Auswahl geeigneter Ventilsysteme zur Verwendung mit Aerosolformulierungen abhängig ist von den verwendeten speziellen Tensiden oder Hilfsstoffen (wenn solche verwendet wurden), von dem Treibmittel und von dem speziellen Arzneistoff, der verwendet wird. Herkömmliche Ventilgummis aus Neopren und Buna, die in Dosierungsventilen für die Abgabe herkömmlicher CFC-Formulierungen verwendet werden, haben oftmals nicht optimale Eigenschaften der Abgabe aus dem Ventil und der Leichtigkeit der Handhabung, wenn sie mit Formulierungen verwendet werden, die HFC 134a oder HFC 227 enthalten. Darüber hinaus enthalten herkömmliche CFC-Formulierungen im allgemeinen ein Tensid, zum Teil als Schmiermittel für den Ventilschaft. Einige Formulierungen der Erfindung enthalten jedoch kein Tensid oder Schmiermittel. Deshalb werden bestimmte Formulierungen der Erfindung vorzugsweise über ein Ventilsystem verteilt, in dem das Diaphragma geformt ist durch Extrusions-, Spritzguß- oder Kompressionsformung aus einem thermoplastischen elastomeren Material wie FLEXOMERTM DFDA 1137 NT7 Polyolefin, FLEXOMERTM DFDA 1138 NT Polyolefin, FLEXOMERTM DEFD 8923 NT Polyolefin, FLEXOMERTM GERS 1085 NT Polyolefin, FLEXOMERTM DFDA 1163 NT7 Polyolefin, FLEXOMERTM 1491 NT7 Polyolefin, FLEXOMERTM 9020 NT7 Polyolefin, FLEXOMERTM 9042 NT Polyolefin (Union Carbide), C-FLEXTM thermoplastisches Elastomer R70-001, C-FLEXTM thermoplastisches Elastomer R70-051, C-FLEXTM thermoplastisches Elastomer R70-041, C-FLEXTM thermoplastisches Elastomer R70-085, C-FLEXTM thermoplastisches Elastomer R70-003 oder C-FLEXTM thermoplastisches Elastomer R70-026 (Concept Polymer Technologies) oder ein Gemisch aus zwei oder mehreren von ihnen.
- Herkömmliche Aerosolbehälter, z. B. diejenigen aus Aluminium, Glas, Edelstahl oder Polyethylenterephthalat, können verwendet werden, um eine Formulierung der Erfindung aufzunehmen.
- Die Formulierungen der Erfindung können der Lunge durch orale Inhalation zugeführt werden, um Bronchodilatation zu bewirken oder um eine Erkrankung zu behandeln, die eine Behandlung durch Inhalation zuläßt, z. B. Asthma, chronisch-obstruktive Lungenerkrankung.
Claims (4)
- Pharmazeutische Suspensions-Aerosolformulierung, die aus einer therapeutisch wirksamen Menge mikronisiertem Albuterolsulfat, Treibmittel, ausgewählt aus HFC 134a, HFC 227 und Gemischen davon und Ethanol in einer Menge von 5 bis 15 Gew.-% der Aerosolformulierung besteht.
- Pharmazeutische Suspensions-Aerosolformulierung gemäß Anspruch 1, in der Albuterolsulfat in einer Menge von 0,2 bis 0,5 Gew.-% der Aerosolformulierung vorhanden ist.
- Pharmazeutische Suspensions-Aerosolformulierung gemäß Anspruch 2, in der Albuterolsulfat in einer Menge von 0,35 bis 0,42 Gew.-% der Aerosolformulierung vorhanden ist.
- Aerosol-Behälter mit festgelegter Dosierung, der eine Formulierung nach einem vorhergehenden Anspruch in einer Menge enthält, die ausreicht, um eine Vielzahl von therapeutisch wirksamen Dosen des Arzneistoffs bereitzustellen.
