  • 1. Gebiet der Erfindung
  • Die vorliegende Erfindung bezieht sich grundsätzlich auf eine Vorrichtung zum Schaffen eines perkutanen Zugangs zu einer inneren Operationsstelle während eines chirurgischen Verfahrens. Insbesondere bezieht sich die vorliegende Erfindung auf ein Trokarsystem das perkutan eingeführt werden kann, während es sich in einem Zustand mit engem Durchmesser befindet, und das nach dem Einführen radial expandiert werden kann, um chirurgischen Instrumenten mit größerem Durchmesser Durchlass zu gewähren.
  • Minimalinvasive chirurgische Verfahren beruhen darauf, dass man zu einer inneren Operationsstelle unter Verwendung von Zugangsrohren mit kleinem Durchmesser (typischerweise zwischen 5 und 12 mm) perkutan Zugang erhält, die üblicherweise als Trokare bezeichnet werden, welche die Haut penetrieren und die gewünschte Operationsstelle eröffnen. Ein Sichtgerät wird durch einen solchen Trokar eingeführt und der Chirurg operiert unter Verwendung von Instrumenten, die durch andere entsprechend platzierte Trokare eingeführt wurden, während er die Operationsstelle auf einem Videomonitor, der mit dem Sichtgerät verbunden ist, betrachtet. Der Chirurg ist dadurch in der Lage, eine große Vielfalt von chirurgischen Verfahren durchzuführen, wobei lediglich einige 5 bis 12 mm Punktierungen an der Operationsstelle benötigt werden. Dadurch werden Trauma und Rekonvaleszenzzeit des Patienten deutlich reduziert.
  • Insbesondere minimalinvasive, chirurgische Verfahren werden oft basierend auf dem Typ des Sichtgeräts benannt, das verwendet wird, um den Bereich des Körpers, der die Operationsstelle bildet, zu betrachten. Daher werden Verfahren im abdominalen Raum, die auf einem Laparoskop zur Betrachtung beruhen, als laparoskopische Verfahren bezeichnet. Bei solchen laparoskopischen Verfahren wird der abdominale Bereich des Patienten üblicherweise aufgeblasen (mit unter Druck stehendem Kohlenstoffdioxid- oder Stickstoffgas gefüllt), um die abdominale Wand anzuheben und einen ausreichenden Operationsraum zu schaffen, um ein gewünschtes Verfahren durchzuführen. Die Trokare, die bei laparoskopischen Verfahren verwendet werden, müssen daher ein Ventil an ihrem proximalen Ende aufweisen, um einen Durchlass des Sichtgeräts oder der chirurgischen Instrumente zu ermöglichen, während ein Lecken des Insufflationsgases verhindert wird. Ferner wurde vorschlagen, laparoskopische Verfahren durch eine mechanische Ausdehnung des Abdomens anstatt der Verwendung der Insufflation durchzuführen.
  • Andere minimalinvasive, chirurgische Verfahren schließen torakoskopische Verfahren, die im Bereich der Brust durchgeführt werden, arthroskopische Verfahren, die in Körpergelenken, insbesondere dem Knie, durchgeführt werden, gynäkologische, laparoskopische Verfahren und endoskopische, chirurgische Verfahren, die typischerweise mit einem flexiblen Sichtgerät in einer Vielzahl von Bereichen des Körpers durchgeführt werden, ein. Die letzten Verfahren verwenden normalerweise keine Beaufschlagung mit Druck, und die verwendeten Trokare weisen grundsätzlich keine Druckventile an ihren proximalen Seiten auf.
  • Die Gestaltung geeigneter Trokare muss vielen Anforderungen gerecht werden, insbesondere für jene, die bei laparoskopischen Verfahren in einer mit Druck beaufschlagten Umgebung verwendet werden. Alle Trokare sollten in den Patienten mit minimalem Trauma und mit minimalem Risiko einer Verletzung innerer Organe eingeführt werden können. Die Trokare, die bei laparoskopischen Verfahren verwendet werden, sollten leicht abzudichten sein, um einen Lecken des Gases aus dem Abdomen zu verhindern, und sollten insbesondere dazu konstruiert sein, ein Lecken in dem Bereich zu verhindern, der den äußeren Umfang des Trokars, der durch die abdominale Wand hindurch reicht, umgibt. Weiter ist es wünschenswert, dass Trokare ein Mittel zum Verankern innerhalb des perkutanen Durchlasses aufweisen, und es wäre insbesondere wünschenswert, wenn ein einzelner Trokar Instrumente mit einer großen Vielzahl von Querschnittsformen und -größen aufnehmen könnte.
  • Bisher sind Trokarkonstruktionen den oben aufgeführten Anforderungen mit gemischten Resultaten nachgekommen. Die Aufnahme von verschieden bemessenen Instrumenten wurde durch Bereitstellung von Trokaren mit verschiedenen, fixen Durchmessern erreicht. Ein Chirurg wird oft den größten Trokar, mit normalerweise zwischen 10 und 12 mm, einführen, der dann die meisten oder alle Instrumente (die eine Größe innerhalb dieses Maximums aufweisen) unter Verwendung eines Gummiadapters aufnehmen kann. Dieser Ansatz ist zwar machbar, das Einführen von Trokaren mit größeren Abmessungen setzt den Patienten aber einem größeren Trauma und einem größeren Risiko einer Verletzung aus, als das der Fall wäre, wenn kleinere Trokare verwendet würden.
  • Mit der äußeren Dichtung um den Außenbereich des Trokars herum wurde sich noch nicht ausreichend beschäftigt. Bestimmte Trokarkonstruktionen setzen ein kegelförmiges, spiralförmiges Gewinde an ihrem Außenbereich ein. Das Gewinde ist zur Verankerung hilfreich, vergrößert aber die Trokarpunktierung, um das Lecken von unter Druck stehendem Insufflationsgas abzudichten. Andere Verankerungen, wie zum Beispiel Malecot-Strukturen, werden am distalen Ende der Trokare eingesetzt und sind wirksam zur Verankerung, schaffen aber keine Abdichtung des Druckverlustes.
  • Aus diesen Gründen wäre es wünschenswert, Trokare bereit zu stellen, die zur Verwendung bei laparoskopischen und anderen minimalinvasiven, chirurgischen Verfahren geeignet sind, die in den Patienten einfach über eine verhältnismäßig kleine Punktierung (vorzugsweise weniger als 5 mm) einzubringen sind, die dann radial expandiert werden können (und optional nachfolgend wieder verkleinert), um eine Mehrzahl von Instrumentengrößen aufzunehmen, und die für eine verbesserte Abdichtung um den Außenbereich des Trokärs herum und für eine Verankerung innerhalb der Zielstelle des Operationsverfahrens sorgen.
  • 2. Beschreibung des Standes der Technik
  • Das auf den Inhaber der vorliegenden Erfindung ausgestellte US-Patent Nr. 5,183,464 beschreibt einen radial expandierbaren Dilator, der ein längliches Dilatationsrohr aufweist, das ein längliches Expansionsrohr aufnimmt. Die Europäische Patentanmeldung 385 920 beschreibt eine Geflechtsstruktur mit variablem Durchmesser, die stenotisches Material aus Blutgefäßen einfangen und entfernen soll. Das US-Patent Nr. 5,122,122 beschreibt eine Trokarhülle mit einer Malecot-Struktur an ihrem distalen Ende. Trokare zum Gebrauch bei laparoskopischen Verfahren sind im Handel von Zulieferern erhältlich wie United States Surgical Corp., Norwalk, Connecticut; Endomed division of Cooper Surgical, Inc., Shelton, Connecticut; und Dexide Inc., Fort Worth, Texas.
  • Das US-Patent Nr. 4,738,666 beschreibt einen expandierbaren Katheter mit einer externen Hülle, die perforiert ist, um ein Entfernen zu erleichtern, wenn der Katheter expandiert wird. Das US-Patent Nr. 4,601,713 beschreibt einen Katheter mit variablem Durchmesser, der einen aufblasbaren Rückhalteballon an seinem distalen Ende aufweist. Der Katheter wird mit einem inneren Mandrin (engl.: stylet), das den Katheter in einem zusammengefalteten Zustand (mit verringertem Durchmesser) hält, eingeführt. Eine Entfernung des Mandrins ermöglicht es dem Katheter, zu expandieren. Das US-Patent Nr. 4,141,364 beschreibt ein expandierbares, endotracheales Rohr, das in einem zusammengefalteten Zustand eingeführt wird und in einen expandierten Zustand zurückfedert, wenn Luft eingeführt wird.
  • Eine aufblasbare Dilatationsvorrichtung wird in den US-Patenten Nr. 4,589,868 und 2,548,602 beschrieben. Katheter mit expandierbaren Strukturen werden in den US-Patenten Nr. 4,986,830, 4,955,895, 4,896,669, 4,479,497, und 3,902,492 beschrieben.
  • Das US-Patent Nr. 4,772,266 beschreibt eine Dilator-/Hüllenvorrichtung, die über einen eingebrachten Draht geschoben werden kann, um ein Zugangsloch zu vergrößern, wobei der Eintritt der Hülle das Loch weiter vergrößert. Das US- Patent Nr. 1,213,001 beschreibt eine Trokar- und Kanülenbaugruppe, welche ein Zwischenrohr aufweist, um eine dreistückige Struktur zu bilden. Das US-Patent Nr. 3,742,958 offenbart eine Kanüle mit einem axialen Schlitz, um es der Kanüle zu ermöglichen, von einem Arbeitskatheter getrennt zu werden, der durch die Kanüle eingeführt wurde. Die US-Patente Nr. 4,888,000, 4,865,593, 4,581,025, 3,545,443 und 1,248,492 beschreiben jeweils Vorrichtungen, die zur perkutanen Durchdringung einer Körperhöhle, eines Blutgefäßes oder massiven Gewebes geeignet sind.
  • Das US-Patent Nr. 4,899,729 beschreibt eine expandierbare Kanüle, welche eine gewendelte, konische Hülle aufweist, die perkutan eingeführt und danach durch Vorbringen eines inneren Zylinders expandiert werden kann. Das US-Patent Nr. 4,716,901 offenbart einen expandierbaren Trokar (der kein Trokarventil aufweist) mit einem Paar gegenüberliegender Komponenten mit geschärften distalen Endstücken und bedeckt durch eine elastische Hülle über einem proximalen Abschnitt davon. Das US-Patent Nr. 4,846,791 beschreibt einen Multilumen-Katheter, der einen elastischen äußeren Mantel und einen inneren Aufteiler aufweist, der, wenn er eingeführt wird, den Mantel expandiert. Siehe ebenfalls die US-Patente Nr. 668,879, 3,789,852, 4,411,655, 4,739,762, 4,798,193, 4,921,479, 4,972,827, 5,116,318 und 5,139,511, die im US-Patent Nr. 5,183,464 angeführt wurden, das auf den Inhaber der vorliegenden Erfindung ausgestellt ist.
  • Die WO-A-92/19312 offenbart eine Vorrichtung zum Einführen einer Kanüle in ein Blutgefäß, die ein rohrförmiges Geflecht aufweist, das durch eine Hülle auf einem verhältnismäßig kleinen Durchmesser zusammen gehalten wird; und das radial nach außen eine federnde Expansion durchführt, wenn die Hülle vom Geflecht abgeschält wird.
  • Eine Dilatorvorrichtung, die ein Führungselement mit einer Verankerung an seinem distalen Ende aufweist, wird in US-A-5,275,611 beschrieben, die auf den Inhaber der vorliegenden Erfindung ausgestellt ist. Eine abschälbare Hülle wird in US-A-5,250,333 beschrieben, die auf den Inhaber der vorliegenden Erfindung ausgestellt ist.
  • Zusammenfassung der Erfindung
  • Entsprechend der vorliegenden Erfindung wird eine verbesserte Vorrichtung, wie in Anspruch 1 unten definiert, zur Bildung und Vergrößerung von perkutanen Durchdringungen an einem Zielort im Körper eines Patienten vorgesehen. Die Vorrichtung umfasst ein längliches Dilatationselement mit einem radial expandierbaren, rohrförmigen Geflecht und ein längliches Expansionselement, das in einem axialen Lumen des länglichen Dilatationselementes aufgenommen ist, um das Geflecht radial zu expandieren, und, um durch dieses ein vergrößertes Zugangslumen vorzusehen. Das längliche Dilatationselement weist ferner ein Punktierungsmittel in der Nähe seines distalen Endes auf, das typischerweise ein längliches Durchdringungselement mit einem geschärften, distalen Endstück ist, das im axialen Lumen des Geflechts entfernbar aufgenommen ist und sich distal davon erstreckt. Das längliche Dilatationselement kann daher perkutan durch Punktieren der Haut eingeführt werden. Das Durchdringungselement wird dann entfernt, um ein Einführen des länglichen Expansionselementes zu ermöglichen. Das längliche Expansionselement weist ein rohrförmiges Element mit festem Radius auf mit einem axialen Lumen, das den erwünschten Zugangsweg definiert.
  • Das expandierte Geflecht des länglichen Dilatationselementes stellt sowohl eine Verankerungsfunktion bereit als auch eine Abdichtungsfunktion. Das rohrförmige Geflecht, das vorzugsweise die Form eines Geflechts aufweist, das aus nicht elastischen Polymeren, rostfreiem Stahl oder anderen Fäden gebildet ist, verkürzt sich, wenn es radial expandiert wird, was das Geflecht dazu bringt, das Gewebe, mit dem es sich in Kontakt befindet, einzuspannen oder festzuklemmen. Ein solches Spannen des Geflechts verhindert, wenn es mit der radialen Expansion kombiniert wird, eine Trennung der Gewebeschichten, stellt eine exzellente Verankerung des länglichen Dilatationselements innerhalb der perkutanen Durchdringung bereit und verbessert die Dichtung am Umfang, um einen Verlust des Insufflationsdruckes, der bei laparoskopischen Verfahren verwendet wird, zu verhindern. Ein weiterer Vorteil des rohrförmigen Geflechts in dem länglichen Dilatationselement ist, dass es am Platz bleibt, wenn das längliche Expansionselement daraus entnommen wird. Daher können längliche, Expansionselemente mit verschiedenen Querschnittsbereichn und Geometrien in das Lumen des rohrförmigen Geflechts eingeführt und entnommen werden, wodurch es dem Querschnittsbereich des Zugangslumens ermöglicht wird, wie gewünscht verändert zu werden. Einem Gewebe über dem Außenbereich des Geflechts wird es möglich sein, sich entsprechend zu straffen oder zu entspannen, um eine solche Änderung des Durchmessers des Geflechts aufzunehmen.
  • In einer bevorzugten Ausführungsform der vorliegenden Erfindung weist das längliche Dilatationselement ferner eine entfernbare Schutzhülle auf, die über dem Geflecht gebildet ist, um das anfängliche perkutane Einführen zu erleichtern. Nachdem das längliche Dilatationselement sich am Platz befindet, kann die Schutzhülle entweder vor, während oder nach der Expansion des rohrförmigen Geflechts entfernt werden. In einer beispielhaften Ausführungsform wird die entfernbare Hülle axial aufgespaltet, wenn das rohrförmige Geflecht radial expandiert und der aufgespaltete Abschnitt der Hülle nach der Expansion entfernt wird.
  • Bei einer zweiten bevorzugten Ausführungsform der vorliegenden Erfindung weist das längliche Expansionselement ferner einen Stab mit einem kegeligen, distalen Ende auf, der entfernbar im axialen Lumen des rohrförmigen Elementes mit festem Radius aufgenommen ist. Das kegelige Ende erstreckt sich distal vom rohrförmigen Element, wodurch ein Einführen des Expansionselementes durch das axiale Lumen des rohrförmigen Geflechts erleichtert wird. Der Stab kann aus dem rohrförmigen Element, nachdem er durch das Geflecht eingeführt wurde, entnommen werden, wobei das Lumen des rohrförmigen Elementes als der gewünschte perkutane Zugangsweg verwendbar zurück bleibt.
  • Bei einer weiteren bevorzugten Ausführungsform der vorliegenden Erfindung weist das längliche Expansionselement ein Mittel zum Anbringen eines Trokarventils an seinem proximalen Ende auf. Typischerweise wird das Trokarventil an dem Expansionselements entfernbar angebracht, wodurch es dem gleichen Trokarventil ermöglicht wird, an verschiedenen länglichen Expansionselementen mit verschiedenen Querschnittsbereichen verwendet zu werden, wenn diese während eines Verfahrens ausgetauscht werden.
  • Bei einer weiteren bevorzugten Ausführungsform der vorliegenden Erfindung ist das längliche Durchdringungselement eine Nadel mit einem zurückziehbaren Obturator an ihrem distalen Ende. Der Obturator zieht sich zurück, wenn die Nadel die Haut penetriert, und schiebt sich nach vorne, sobald die Nadel in den Körper des Patienten eingetreten ist, wodurch dabei geholfen wird, den Patienten vor einer versehentlichen Verletzung durch die Nadel zu schützen. Die als Beispiel dienende Nadel ist eine Veress-Nadel von dem Typ, der üblicherweise bei Insufflationsvorrichtungen Verwendung findet.
  • Die vorliegende Erfindung stellt ferner einen verbesserten, radial expandierbaren Dilator von dem Typ bereit, der ein radial expandierbares Dilatationselement und ein längliches Expansionselement aufweist, das durch ein axiales Lumen des Dilatationselementes eingeführt werden kann. Die Verbesserung umfasst ein expandierbares Dilatationselement mit einem radial expandierbaren, rohrförmigen Geflecht, das durch eine entfernbare Hülle bedeckt ist. Das rohrförmige Geflecht weist vorzugsweise ein offenes Gewebe auf, das aus nicht elastischen Polymer- oder Metallfäden besteht, wodurch sich das Geflecht verkürzt, wenn sein Radius vergrößert wird.
  • Die vorliegende Erfindung stellt weiterhin einen verbesserten Trokar von dem Typ bereit, der ein rohrförmiges Element mit festem Radius und ein Trokarventil, das an einem proximalen Ende des rohrförmigen Elementes angeordnet ist, aufweist. Die Verbesserung umfasst die Bereitstellung mehrerer rohrförmiger Elemente mit unterschiedlichen festen Durchmessern und austauschbaren Mitteln an ihren proximalen Enden zur Sicherung eines Trokarventil daran. Der Trokar weist ferner ein Mittel auf, das ein radial expandierbares, axiales Lumen definiert, um im Einzelnen das jeweilige der mehreren rohrförmigen Elemente aufzunehmen und diesem zu entsprechen, wobei das definierende Mittel perkutan eingeführt werden kann, um ein Zugangslumen zum Einführen und Austauschen von rohrförmigen Elementen bereitzustellen, wobei die Trokarventile im Einzelnen an dem rohrförmigen Element angebracht werden, nachdem dieses eingeführt wurde. Das definierende Mittel weist ein längliches, radial expandierbares, rohrförmiges Geflecht auf mit Mitteln an seinem distalen Ende zum Punktieren von Gewebe, wenn das Geflecht perkutan vorangebracht wird.
  • Ein Verfahren, das mit einer Vorrichtung entsprechend der vorliegenden Erfindung durchgeführt werden kann, umfasst das Durchdringen einer Körperoberfläche zu einer Zielstelle innerhalb des Körpers eines Patienten mit einem länglichen Dilatationselement: Optional kann eine Abdeckung von dem Dilatationselement entfernt werden, um ein radial expandierbares rohrförmiges Geflecht mit einem axialen Lumen, das sich von der Körperoberfläche zum Zielort erstreckt, freizulegen. Ein erstes rohrförmiges Element mit festem Radius wird dann durch das Lumen des rohrförmigen Geflechts eingeführt, wodurch das rohrförmige Geflecht expandiert wird, um sich innerhalb der perkutanen Durchdringung zu verankern. Das rohrförmige Element stellt dadurch ein Zugangslumen mit einem vergrößerten Querschnittsbereich bereit. Optional umfasst das Verfahren ferner die Entfernung des rohrförmigen Elements mit festem Radius und das Einführen eines zweiten rohrförmigen Elements mit festem Radius, das ein Lumen mit einem anderen Querschnittsbereich aufweist als der des ersten rohrförmigen Elements. Auf diese Weise können Zugangslumina mit verschiedenen Querschnittsbereichen an derselben Stelle eingeführt werden, wobei beim Patienten nur ein minimales Trauma verursacht wird. Eine Abnutzung des Gewebes wird reduziert oder eliminiert, indem das zweite rohrförmige Element durch das rohrförmige Geflecht geschoben wird, und neue perkutane Durchdringungen müssen nicht ausgebildet werden.
  • Kurze Beschreibung der Zeichnungen

  • Beschreibung spezifische Ausführungsformen
  • Die vorliegende Erfindung ist dienlich zum Bilden und Vergrößern perkutaner Durchdringungen in einer Vielzahl von Zielbereichen innerhalb eines Körpers eines Patienten für eine Vielzahl von Zwecken. Solche Zwecke umfassen Drainage, Medikamentation innerhalb von Organen, Zuführung, Infusion, Absaugung und ähnliches und schließen das Einführen von chirurgischen Instrumenten wie Sichtgeräten und chirurgischen Instrumenten zum Gebrauch bei minimalinvasiven, chirurgischen Verfahren, wie etwa laparoskopischen Verfahren, thorakoskopischen Verfahren, arthroskopischen Verfahren, endoskopischen Verfahren und ähnlichem ein.
  • Die Vorrichtungen der vorliegenden Erfindung sind insbesondere wertvoll, weil sie eine sehr kleine anfängliche Durchdringung erzeugen, üblicherweise unterhalb von 5 mm, noch gebräuchlicher unterhalb von ungefähr 4 mm, oft unterhalb von ungefähr 3,5 mm und vorzugsweise von 3 mm oder darunter. Die Durchdringung wird nachfolgend auf ein gewünschtes Endmaß vergrößert, das üblicherweise einen Enddurchmesser im Bereich von ungefähr 5 mm bis 15 mm aufweist, noch gebräuchlicher von ungefähr 5 mm bis 12 mm groß ist und typischerweise zwischen 5 mm und 10 mm liegt. Die vergrößerte Durchdringung wird ein Zugangslumen von außerhalb des Körpers des Patienten zum gewünschten inneren Ort definieren, und es ist ein besonderer Vorteil der vorliegenden Erfindung, dass der Durchmesser des Zugangslumens verändert werden kann, wie im weiteren detaillierter beschrieben wird.
  • Die Dilatationsvorrichtung entsprechend der vorliegenden Erfindung weist ein längliches Dilatationsrohr mit einem distalen Ende, einem proximalen Ende und einem axialen Lumen, das sich vom distalen Ende zum proximalen Ende erstreckt, auf. Das Dilatationsrohr weist ein expandierbares, rohrartiges Geflecht auf, das sich anfänglich in einem länglichen Zustand mit engem Durchmesser befindet. Das Geflecht kann frei liegen, wird aber oft mit einer Beschichtung oder einer Lage elastischem oder plastisch deformierbarem Material bedeckt sein, wie etwa Silikonkautschuk, LaTeX, Polyethylen C-Flex oder ähnlichem. Das rohrförmige Geflecht wird in seinem Zustand mit dem engen Durchmesser perkutan eingeführt und danach unter Verwendung eines länglichen Expansionselementes radial expandiert, wie im Weiteren detaillierter beschrieben. Das rohrförmige Geflecht ist vorzugsweise als ein Gewebe aus einzelnen nicht elastischen Fäden (zum Beispiel aus einer Polyamidfaser (Kevlar®, DuPont), rostfreiem Stahl oder ähnlichem) gebildet, so dass eine radiale Expansion eine axiale Verkürzung des Geflechts verursacht. Eine solche axiale Verkürzung hilft dabei, das Dilatationselement an seinem Ort innerhalb des Gewebes des Patienten zu verankern, und hilft dabei, den Außenbereich des Dilatationselementes gegenüber dem Gewebe abzudichten, weil die Fäden des Geflechts radial in das umgebende Gewebe eindringen. Eine solche fest verankerte und gasdichte Dichtung stellt einen besonderen Vorteil bei laparoskopischen Verfahren dar.
  • Das Geflecht kann eine übliche Gestaltung aufweisen einschließlich runden Fäden, flachen oder Gummifäden, quadratische Fäden oder Ähnlichem. Nicht runde Fäden können vorteilhaft die axiale Kraft reduzieren, die erforderlich ist, um eine radiale Expansion vorzusehen. Die Breite oder der Durchmesser der Fäden wird typischerweise zwischen ungefähr 0,05 mm (0,002 Inch) und 6,35 mm (0,25 inch) liegen, gewöhnlich zwischen 0,13 mm bis ungefähr 0,25 mm (0,005 bis 0,010 inch). Geeignete Geflechte können von einer Vielzahl von Zulieferern im Handel bezogen werden, wie etwa Bently Harris, Lionville, Pennsylvania; Manville Sealing Components, Denver, Colorado und 3M Ceramic Materials, St. Paul, Minnesota. Ein besonders geeignetes Geflecht ist unter der Marke Exando® PT von Bently-Harris erhältlich.
  • Das längliche Dilatationselement kann optional weiterhin eine entfernbare Hülle aufweisen, die das rohrförmige Geflecht bedeckt. Die Hülle besteht für gewöhnlich aus einem gleitfähigen Material, wie etwa einem dünnwandigen, flexiblen Kunststoff, wie zum Beispiel Polyethylen, Tetrafluorethylen, fluoriertes Ethylenpropylen oder ähnlichem. Die Hülle schützt das rohrförmige Geflecht während des anfänglichen Einführens des Dilatationselementes, wird aber vom Geflecht entfernt nachdem sich das Dilatationselement an seinem Platz befindet. Vorzugsweise wird die Hülle entlang einer axialen Linie geschwächt sein, um ein Aufspalten der Hülle an einem Punkt während des Verfahrens zu erleichtern. Solche entfernbaren Hüllen werden für gewöhnlich nur eingesetzt, wenn das rohrförmige Geflecht selber keine elastische oder deformierbare Schicht, die auf dem Material der Fäden des Geflechts gesichert ist, aufweist. Die Gestaltung einer geeigneten Hülle wird detailliert in der anhängigen, auf den Anmelder der vorliegenden Erfindung lautenden Anmeldung, 07/702,642 beschrieben.
  • Das längliche Dilatationselement wird ferner ein Mittel zur Punktierung des Gewebes, wenn das Dilatationselement perkutan vorgeschoben wird, an seinem distalen Ende aufweisen. Das Punktierungsmittel könnte entweder entfernbar oder permanent entweder am rohrförmigen Geflecht, optional an der Schutzhülle oder an beidem befestigt sein. In der beispielhaften Ausführungsform weist das Punktierungsmittel jedoch ein separates längliches Durchdringungselement auf, das in einem axialen Lumen des rohrförmigen Geflechts aufgenommen ist, während sich das Geflecht in seinem axial länglichen Zustand befindet. Das Durchdringungsmittel weist ein geschärftes distales Endstück auf, das sich distal über das distale Ende des rohrförmigen Geflechts und die optionale Abdeckungshülle hinaus erstreckt, wodurch ein Durchdringen des Gewebes durch das Dilatationselementes erleichtert wird.
  • In einer besonders bevorzugten Ausführungsform besteht das Durchdringungselement aus einer Nadel, die einen Obturator aufweist, der darin zurückziehbar angebracht ist, und der sich distal über das geschärfte, distale Endstück hinaus erstreckt. Der Obturator ist für gewöhnlich federbeladen und wird sich daher automatisch zurückziehen, wenn die Nadel durch das Gewebe vorangebracht wird und einem signifikanten Gewebewiderstand beim Punktieren ausgesetzt ist. Allerdings wird sich der Obturator distal über das geschärfte distale Endstück erstrecken, um den Patienten vor einer Verletzung, nachdem das Dilatationselement in den gewünschten Zielort eingedrungen ist, zu schützen, etwa innerhalb des befüllten Bereichs des Abdomens während eines laparoskopischen Verfahrens. Solche Nadeln sind als Veress-Nadeln bekannt und werden für gewöhnlich zur Befüllung bei laparoskopischen und anderen Verfahren eingesetzt.
  • Das längliche Dilatationselement kann ferner eine Verankerung an seinem distalen Ende aufweisen, die nach der Durchdringung aber vor der Expansion aufgeweitet wird. Die Verankerung würde dazu dienen, das Dilatationselement an seinem Platz zu halten und eine Abtrennung der Haut und der fascialen Schichten zu verhindern, wenn das Expansionselement durch das Dilatationselement eingeführt wird. Allerdings wird normalerweise die inhärente Verankerungseigenschaft des rohrförmigen Geflechts selber ausreichen, um das Dilatationselement an seinem Platz zu halten und ein Abtrennen des Gewebes zu verhindern, so dass die Verankerung nicht notwendig sein wird.
  • Das längliche Expansionselement weist ein rohrförmiges Element mit einem festen Radius auf; das ein distales Ende, ein proximales Ende und ein axiales Lumen, das sich dazwischen erstreckt, aufweist. Das axiale Lumen hat einen Querschnittsbereich, der größer als der des rohrförmigen Geflechts in seinem radial nicht expandierten Zustand ist. Daher wird, durch Einführen des Expansionselementes durch das Lumen des rohrförmigen Geflechts und durch Verursachen einer radialen Expansion des Geflechts, ein vergrößerter Zugangskanal durch das Lumen des rohrförmigen Elements mit festem Radius vorgesehen. Um ein Einführen des länglichen Expansionselementes durch das axiale Lumen des rohrförmigen Geflechts zu erleichtern, wird vorzugsweise ein Stab mit einem kegeligen, distalen Ende im Lumen des rohrförmigen Elements vorgesehen: Das kegelförmige Ende erstreckt sich distal des Rohrs und dient zum Auseinanderdrücken des rohrförmigen Geflechts, wenn das Expansionselement vorgebracht wird. Der Stab kann dann aus dem Rohr entfernt werden, um das Zugangslumen unversperrt zu hinterlassen, nachdem das Expansionselement vollständig durch das rohrförmige Geflecht nach vorne gebracht wurde. Trokarsysteme der vorliegenden Erfindung werden vorzugsweise zumindest zwei längliche Expansionselemente mit verschiedenen Querschnittsbereichen einschließen, so dass das gleiche Dilatationselement selektiv zur Einbringung von Zugangsöffnungen vieler verschiedener Größen als ein Kanal verwendet werden kann.
  • Das längliche Expansionselement kann an oder nahe seinem distalen Ende optional ein Verankerungsmittel aufweisen. Das Verankerungsmittel kann eine Vielzahl von Formen annehmen, einschließlich Malecot-Strukturen, Haken, aufblasbaren Ballonen und Ähnlichem, und dient dazu, das Expansionselement nach dem Erweitern an seinem Platz an der Zielstelle zu halten. Um die Verankerung weiter zu verbessern, kann ferner eine externe Klemme oder Verankerung am oder nahe des proximalen Endes des Expansionselementes vorgesehen werden, so dass Gewebe, das sich zwischen der Verankerung befindet, zum Beispiel die abdominale Wand, dazwischen gefangen werden kann. Eine Verankerung, die durch ein solches Einklemmen vorgesehen wird, liegt natürlich zusätzlich zu der inhärenten Verankerung vor, die durch eine Expansion des rohrförmigen Geflechts wie oben beschrieben erreicht wird.
  • Üblicherweise weist das längliche Expansionselement an seinem proximalen Ende ein Mittel zur entfernbaren Befestigung eines Trokarventils auf. Das Trokarventil ähnelt konventionellen Trokarventilen, die fest an Trokaren befestigt sind. Das Ventil weist eine innere Struktur auf, die das Einführen von Sichtgeräten und/oder chirurgischen Instrumenten mit verschiedenen Querschnittsbereichen ermöglicht, während sie entlang der Außenbereiche solcher Vorrichtungen abdichtet, um einen Verlust des Insufflationsdruckes bei laparoskopischen Verfahren zu verhindern. Die Ventile weisen typischerweise Absperrhähne auf, die ein Einführen oder eine Entlüftung von Insufflationsgas, eine Spülung, eine Absaugung oder Ähnliches ermöglichen. Gewöhnliche Trokarventile werden von verschiedenen Zulieferern hergestellt, wie etwa Ethicon, Inc., Somerville, New Jersey; United States Surgical Corporation, Norwalk, Connecticut; Dexide, Inc. Fort Worth, Texas; und andere.
  • Die Länge des länglichen Dilatationsrohrs variiert in Abhängigkeit von dem angestrebten Gebrauch, wird aber grundsätzlich im Bereich zwischen ungefähr 10 cm und 25 cm liegen. Die Länge des Dilatationsrohrs, das für ein laparoskopisches Verfahren bestimmt ist, wird grundsätzlich im Bereich von ungefähr 10 cm bis 20 cm liegen, typischerweise im Bereich von ungefähr 10 cm bis 15 cm. Die Länge von Dilatationsrohren, die in Trokarsystemen verwendet werden, die für einen torakoskopischen Gebrauch bestimmt sind, wird grundsätzlich kürzer sein, typischerweise werden sie eine Länge im Bereich von ungefähr 5 cm bis 10 cm aufweisen. Die Länge der länglichen Expansionselemente wird natürlich grundsätzlich etwas größer sein als die des länglichen Dilatationselementes, wodurch eine radiale Expansion der gesamten Länge des Dilatationselementes ermöglicht wird.
  • Ein beispielhaftes Trokarsystem, das in Übereinstimmung mit den Prinzipien der vorliegenden Erfindung konstruiert ist, wird mit Bezug auf die Figuren im Weiteren beschrieben werden. Die drei grundsätzlichen Komponenten des Trokarsystems sind ein längliches Dilatationselement 10, das am besten in 1 bis 3 dargestellt ist, ein oder mehrere längliche Expansionselemente 12 und 14, das am besten in 4 bis 8 dargestellt ist, und ein Trokarventil 16, das am besten in 8 dargestellt ist. Jede dieser Komponenten und der Gebrauch dieser Komponenten in der Methode der vorliegenden Erfindung wird nun detaillierter beschrieben.
  • Das längliche Dilatationselement 10 weist ein rohrförmiges Geflecht 20 mit einem axialen Lumen auf, das sich von einem Griff 22, der sich an seinem proximalen Ende befindet, zu einer Hülse 24 an der Nadel 40 erstreckt, die an seinem distalen Ende angeordnet ist. Das Geflecht 20 ist an dem proximalen Griff 22 auf eine Weise angebracht, die es ermöglicht, eine radiale Expansion des Geflechts, wenn das Expansionselements 12 oder 14 anschließend dort hindurch eingeführt wird, zu ermöglichen, wie detaillierter im weiteren beschriebenen werden wird. Das rohrförmige Geflecht weist einzelne Geflechtfäden auf, wie oben beschrieben wurde, und kann ferner eine laminierte, elastomerische oder plastisch verformbare Schicht aufweisen, wie oben beschrieben wurde. Ein Durchgang 26 ist in dem proximalen Griff 22 ausgebildet, um einen Durchlass des Expansionselementes in das axiale Lumen des rohrförmigen Geflechts 20 zu ermöglichen.
  • Das rohrförmige Geflecht 20 ist mit einer entfernbaren Hülle 30 (in der dargestellten Ausführungsform) bedeckt und weist einen Griff 32 an dem proximalen Ende auf. Die Hülle 30 erstreckt sich über die gesamte Länge des rohrförmigen Geflechts 20 und schließt an seinem proximalen Ende ab, grundsätzlich an dem Ort der Hülse 24, wie am besten in Figur 2 dargestellt wird. Die entfernbare Hülle 30 kann so, wie in der anhängigen Anmeldung Nr. 07/967,602 beschrieben wurde, gestaltet sein.
  • Eine Nadel 40 mit einem geschärften, distalen Endstück 42 und einem proximalen Griff 44 wird anfänglich innerhalb des axialen Lumens des rohrförmigen Geflechts 20 aufgenommen. Die Hülse 24 ist in der Nähe des geschärften distalen Endstücks 42 der Nadel 20 befestigt und weist eine nach vorne kegelige Fläche 46 (2) auf, die ein Durchdringen von Haut, fascialem Gewebe und Organwänden des Patienten durch das längliche Dilatationselement 10 erleichtert. Die Hülse 24 dient insbesondere als ein Übergang von der Nadel 40 mit einem engen Durchmesser auf die entfernbare Hülle 30 mit einem etwas größeren Durchmesser hin. Das rohrförmige Geflecht 20 wird innerhalb eines ringförmigen Lumens, das zwischen der äußeren Fläche der Nadel 40 und der inneren Fläche der Hülle 30 definiert wird, aufgenommen. Alternativ kann das distale Ende der Hülle 30 selber kegelförmig sein, um den gewünschten glatten Übergang zu schaffen. Daher wird es möglich sein, die Hülse 24 in bestimmten Ausführungsformen der Erfindung zu eliminieren.
  • Die Nadel 40 weist vorzugsweise die Form einer Insufflationsnadel mit einem schützenden Element an ihrer distalen Spitze 42 auf. Wie dargestellt, ist das schützende Element ein Obturator 48 mit einem stumpfen Ende 50, der in dem axialen Lumen der Nadel 40 hin und her beweglich aufgenommen ist. Der Obturator 48 ist federbeladen, so dass sich das stumpfe Ende 50 in der Lager- oder Ruhestellung distal über die scharfe distale Spitze 42 der Nadel 40 hinaus erstreckt. Wenn die Spitze 42 fest gegen die Haut oder das Gewebe des Patienten gedrückt wird, wird das stumpfe Ende 50 jedenfalls zurück in die Nadel 40 gezogen, so dass die scharfe Spitze 42 penetrieren kann. Üblicherweise ist der Obturator 48 hohl und weist eine Öffnung 52 an seinem distalen Ende auf. Durch Bereitstellung einer Ventilbaugruppe 54 an seinem proximalen Ende kann die Kombination aus Nadel 40 und Obturator 48 zum Einführen oder Entnehmen von Fluiden dienen, was insbesondere nützlich zur Durchführung der anfänglichen Schritte der Insufflation ist.
  • Insufflationsnadeln, die zum Gebrauch mit der vorliegenden Erfindung modifiziert werden können, sind im Handel von Zulieferern erhältlich, wie Ethicon, Inc., somerville, New Jersey, (unter der Marke Endopath® Ultra Veress Needle) und United Surgical Corporation, Norwalk, Connecticut (erhältlich unter der Marke Auto Suture® Surgineedle®).
  • Das erste längliche Expansionselements 12 wird mit Bezug auf die 4 bis 6 detailliert beschrieben werden. Das Expansionselement 12 weist ein rohrförmiges Element 60 mit festem Radius 60 mit einem distalen Ende 62 und einem proximalen Ende 64 auf. Ein inneres koaxiales Rohr 66 ist verschieblich innerhalb des axialen Lumens des rohrförmigen Elements 60 mit festem Radius aufgenommen, wobei es an dem rohrförmigen Element nur an dem distalen Ende 68 befestigt, ist. Ein proximaler Griff 70 weist eine distale Hälfte 70A auf, die an dem proximalen Ende des inneren koaxialen Rohres 66 befestigt ist, und eine proximale Hälfte 70B, die mit dem proximalen Ende des rohrförmigen Elementes 60 verbunden ist. Demnach wird durch Auseinanderziehen der Hälften 70A und 70B in axialer Richtung das innere koaxiale Rohr in proximaler Richtung relativ zu dem rohrförmigen Element 60 mit festem Radius gezogen (wie in 7 mit Bezug auf eine zweite Ausführungsform des Expansionselementes gezeigt ist). Auf diese Weise kann eine Malecot-Struktur 72 expandiert werden, die nahe des distalen Endes des rohrförmigen Elementes 60 mit festem Radius gebildet ist, wie durch eine gestrichelte Linie in 6 gezeigt ist.
  • Das erste längliche Expansionselement 12 weist ferner einen inneren Obturator oder einen Stab 74 mit einem Griff 76 an seinem proximalen Ende und einer kegeligen, konischen Fläche 78 an seinem distalen Ende auf. Die kegelige, konische Fläche 78 erstreckt sich distal vom distalen Ende 62 des rohrförmigen Elementes 60 mit festem Radius, wie am besten in 5 dargestellt ist. Die äußeren Umfangsflächen sowohl des rohrförmigen Elementes 60 mit festem Radius als auch des inneren, koaxialen Rohrs 66 sind abgeschrägt, um eine konische Fläche 80 zu bilden, die mit der kegelförmigen, konischen Fläche 78 fluchtet, wenn der Stab 74 gänzlich in das Expansionselement 12 eingeführt ist. Wie im Weiteren in Verbindung mit 9 gesehen wird, erleichtert die Kombination aus der kegeligen, konischen Fläche 78 und dem kegeligen Bereich 80 das Einführen des Expansionselementes 12 durch das rohrförmige Geflecht 20 des länglichen Dilatationselementes 10.
  • Ein zweites längliches Expansionselement 14 wird mit Bezug auf 7 dargestellt. Das zweite Expansionselement 14 ähnelt in jeglicher Hinsicht dem ersten Expansionselement 12 außer, dass der Durchmesser des rohrförmigen Elements 60' mit festem Radius größer ist als der des rohrförmigen Elements 60 des ersten expandierbaren Elements 12. Die einzelnen Durchmesser der Expansionselemente sind ohne große Bedeutung und liegen innerhalb der beispielhaften Bereiche, wie oben dargestellt. In der beispielhaften Ausführungsform liegt der Durchmesser des ersten länglichen Expansionselementes 12 üblicherweise zwischen ungefähr 6 mm und 8 mm, um ein Zugangslumen von ungefähr 5 mm bereitzustellen. Der Durchmesser des zweiten Expansionselementes 14 liegt ungefähr zwischen 12 mm und 14 mm, um ein Zugangslumen von ungefähr 10 mm bereitzustellen. Der Gebrauch von 5 mm und 10 mm Zugangslumina ist in der laparoskopischen Chirurgie üblich. Es ist jedoch ein besonderer Vorteil der vorliegenden Erfindung, dass viele Durchmesser der Zugangslumina bereitgestellt werden können. Daher ist anzunehmen, dass die Trokarsysteme zwei, drei, vier oder mehr Expansionselemente mit abgestuften, unterschiedlichen Durchmessern aufweisen werden, um annähernd jedes Sichtgerät oder Instrument aufzunehmen, das eingebracht werden kann.
  • 7 stellt das Expansionselement 14 mit dem inneren Stab daraus entfernt dar. Nachdem der innere Stab entfernt wurde können die zwei Hälften 70A' und 708' des Griffes 70' in axialer Richtung auseinander gezogen werden, wie in gestrichelter Linie dargestellt ist, um die Malecot-Struktur 72', wie ebenfalls in gestrichelter Linie dargestellt ist, zu expandieren.
  • Mit Bezug auf 8 wird der Griff 70 des ersten Expansionselementes 12 (genauso wie der Griff 70' des zweiten Expansionselementes 14) so konstruiert sein, dass er das Trokarventil 16 entfernbar aufnimmt. Das Trokarventil 16 weist eine herkömmliche Konstruktion auf, abgesehen davon, dass es ein Montagemittel an seinem distalen Ende aufweist. Wie in 8 dargestellt ist, weist das Montagemittel eine männliche Bajonettbefestigung 90 auf, die innerhalb einer weiblichen Bajonettaufnahme 92 in der proximale Hälfte 70B des Griffs 7O lösbar aufgenommen ist. Auf diese Art kann das Trokarventil 16 an verschiedenen Expansionselementen montiert werden, wenn diese innerhalb des rohrförmigen Geflechts 20 des Dilatationselementes 10 ausgewechselt werden. Dies ist ein besonderer Vorteil, weil dadurch die Kosten und die Komplexität, die mit der Aufrechterhaltung eines Inventars von Trokaren mit bestimmten Ventilen auf Hüllen mit jedem Durchmesser verbunden ist, reduziert werden.
  • Das Einführen eines länglichen Expansionselementes 12 durch das längliche Dilatationselement 10 wird mit Bezugnahme auf 9 und 10 detaillierter beschrieben werden. Vor dem Einführen des Expansionselementes 12 wird natürlich die Nadel 40 aus dem axialen Lumen des rohrförmigen Geflechts 20 des Dilatationselementes 10 entfernt worden sein. Das durch Flächen 78 und 80 definierte, kegelige, distale Ende wird durch den Durchlass 26 im Griff 22 eingeführt und tritt so in das axiale Lumen in dem rohrförmigen Geflecht 20 ein. Die Hülle 30 wird durch das Zurückziehen der Nadel 40 und das Passieren der Hülse 24 durch die Hülle optional gespalten. Andernfalls wird das Einführen des Expansionselementes 12 die Hülle 30 axial aufspalten, wie in 9 dargestellt ist. Wie gesehen werden kann, verursacht das Voranbringen des Dilatationselementes 12 eine radiale Expansion des rohrförmigen Geflechts 20, was schließlich zu einer vollständigen Expansion des Geflechts führt, wie in 10 dargestellt ist (in welcher der innere Stab 74 entfernt wurde).
  • Nachdem das Expansionselement 12 gänzlich eingesetzt wurde, erstreckt sich die (nicht aufgefalte) Malecot-Struktur 22 distal vom distalen Ende des rohrförmigen Geflechts, wie in 10 dargestellt ist. Dann wird die Hülle 30 (die durch das Passieren des Expansionselementes durch sie hindurch aufgespalten wurde) unter Verwendung des Griffs 32. zurückgezogen und der innere Stab 74 wird unter Verwendung. des Griffs 76 zurückgezogen. Die Hälften 70A und 70B des Griffs 70 werden dann in axialer Richtung auseinander gezogen, um die Malecot-Struktur 72 aufzufalten. Das Trokarventil 16 wird dann an der proximalen Hälfte 70b des Griffs 70 montiert und die Struktur wird im Wesentlichen der Darstellung in 10 entsprechen. Zu bemerken ist, dass es das rohrförmige Element 60 mit festem Radius ist, das für Unterstützung der Struktur innerhalb des expandierten rohrförmigen Geflechts 20 sorgt. Ferner ist es das axiale Lumen innerhalb des rohrförmigen Elements 60 mit festem Radius, das den gewünschten Zugangskanal vom Trokarventil 16 zum distalen Ende der Struktur bereitstellt.
  • Die Expansion des rohrförmigen Geflechts 20 von seinem anfänglichen Zustand mit engem Durchmesser, der in 11 dargestellt ist, auf seinen radial expandierten Zustand, der in 12 dargestellt ist, wird mit Bezugnahme auf 11 und 12 beschrieben werden. 12A stellt den prozentualen Verkürzungseffekt bei der radialen Expansion des z. B. Expando® PT-Geflechts von ungefähr 2,5 mm (0,1 inch) auf 11,4 mm (0, 45 inch) dar.
  • Der Gebrauch des beispielhaften Trokarsystems der vorliegenden Erfindung zur Durchführung einer perkutanen Durchdringung wird mit Bezugnahme auf 13 bis 20 beschrieben. Das längliche Dilatationselement 10 befindet sich anfänglich an einer Stelle auf der Haut S des Patienten, an der die Durchdringung gebildet werden soll. Das Dilatationselement 10 penetriert dann die Haut S durch Voranbringen der geschärften, distalen Spitze 42 der Nadel 40 (1) durch die Haut, wie in 14 dargestellt ist. Im Fall von laparoskopischen Verfahren wird sich das stumpfe Ende 50 des Obturators 48 automatisch ausdehnen, sobald die geschärfte Spitze 42 der Nadel 40 die Haut in das Abdomen hinein penetriert hat, um die inneren Organe des Patienten vor einer zufälligen Verletzung zu schützen. An diesem Punkt kann, wenn es gewünscht ist, das Lumen der Nadel 40 für die Insufflation verwendet werden.
  • Nachdem das Dilatationselement 10 zu seiner gewünschten Stellung vorgebracht wurde, wird die Nadel 40 unter Verwendung des Griffs 44 zurückgezogen, wobei die Hülle 30 (die durch das Zurückziehen der Nadel 40 und der angebrachten Hülse 24 aufgespalten worden sein kann) und das rohrförmige Geflecht darin mit dem Griff 22 an seinem proximalen Ende zurückgelassen werden, wie in 15 dargestellt ist.
  • Das längliche Expansionselement 12 wird als nächstes durch den Durchlass 26 im Griff 22 eingeführt, wodurch das rohrförmige Geflecht 20 expandiert und die Hülle 30 gespalten wird (wenn nicht bereits geschehen), wie in 16 dargestellt ist. Die Anwesenheit der Hülle 30 und des Geflechts 20 erleichtert die radiale Expansion der Durchdringung, die durch die Haut S gebildet wurde, wie in 16 dargestellt ist.
  • Nachdem das Expansionselement 12 gänzlich durch das Dilatationselement 10 eingeführt wurde, wird der innere koaxiale Stab 66 aus dem Rohr 60 mit festem Radius zurückgezogen, und die Hülle 30 wird aus der Position über dem expandierten, rohrförmigen Geflecht zurückgezogen, wie in 17 dargestellt. Die Malecot-Struktur 72 wird durch axiales Auseinanderziehen der Hälften 70A und 70B, wie in 17 dargestellt, expandiert. Das Trokarventil 16 kann dann durch die proximale Hälfte 70B des Griffs 70, wie in 18 dargestellt ist, gesichert werden.
  • Wenn es gewünscht ist, den Durchmesser des Zugangskanals des Expansionselementes zu verändern, kann das erste Expansionselement 12 aus dem rohrförmigen Geflecht zurückgezogen werden, wie in 19 dargestellt ist. Das rohrförmige Geflecht 20 wird zumindest teilweise zusammenfallen, wird aber immer noch ein inneres Lumen bereitstellen, um die Einführung eines zweiten Expansionselementes, wie etwa des zweiten Expansionselementes 14 mit dem größeren Durchmesser, zu erlauben. Das zweite Expansionselement 14 kann durch den Griff 22 in einer zu der Einführung des ersten Expansionselementes 12 analogen Weise eingeführt werden außer, dass sich die Hülle 30 nicht mehr an ihrer Stelle befindet. Das rohrförmige Geflecht 20 ist jedenfalls allein ausreichend, um den Durchlass des Expansionselementes 14 durch die perkutane Durchdringung durch die Haut S mit minimalem Trauma für den Patienten zu ermöglichen. Es wird ersichtlich sein, dass Expansionselemente mit kleineren oder größeren Durchmessern anstelle des anfänglich vorliegenden Expansionselements im rohrförmigen Geflecht 20 ausgetauscht werden können.
  • Schließlich wird eine Platzierung des zweiten Expansionselementes 14 in 20 dargestellt, in der die Malecot-Struktur 72' durch Ziehen an dem Griff 70' in proximaler Richtung relativ zum Rohr 60' mit festem Radius aufgefaltet wurde.
  • Obwohl die vorstehende Erfindung zum Zweck eines klaren Verständnisses durch Darstellungen und Beispiele ziemlich detailliert beschrieben wurde, ist es offensichtlich, dass bestimmte Änderungen und Modifikationen innerhalb des Schutzbereiches der angefügten Ansprüche durchgeführt werden können.

Claims (15)

  1. Vorrichtung zum Bilden und Vergrößern einer perkutanen Durchdringung in eine vergrößerte Durchdringung mit einem Enddurchmesser von zumindest 5 mm, wobei die Durchdringung ein Eingangslumen zur Durchführung chirurgischer Instrumente bestimmt, umfassend: ein längliches Dilatationselement (10), das ein radial expandierbares Element (20) umfasst und das ein proximales Ende, ein distales Ende und ein axiales Lumen mit einer ersten Querschnittsfläche aufweist; ein Mittel (40, 42) an dem distalen Ende des Dilatationselements (10) zum Punktieren von Gewebe beim perkutanen Bewegen des Dilatationselements (10); ein längliches Expansionselement (12, 14), das ein rohrförmiges Element (60) mit konstantem Radius umfasst, welches ein distales Ende (62), ein proximales Ende (64) und ein axiales Lumen mit einem zweiten, größeren Querschnittsbereich zur Aufnahme des chirurgischen Instruments aufweist; und ein Mittel (78) an dem distalen Ende des Expansionselements (12, 14) zum Erleichtern des Einführens des rohrförmigen Elements (60) durch das axiale Lumen des Dilatationselements (10); und dadurch gekennzeichnet, dass das radial expandierbare Element (20) ein rohrförmiges Geflecht ist, das bezüglich seines Durchmessers expandiert und bezüglich seiner Länge sich verkürzt, wenn das längliche Expansionselement entlang dem axialen Lumen des Dilatationselements vorwärts bewegt wird.
  2. Vorrichtung nach Anspruch 1, wobei ein Trokarventil (16), das auf entfernbare Weise an dem proximalen Ende des länglichen Expansionselements (12, 14) angebracht ist, an dem proximalen Ende des axialen Lumens des rohrförmigen Elements (60) mit konstantem Radius angeordnet ist.
  3. Vorrichtung nach Anspruch 1 oder 2, wobei das längliche Dilatationselement (10) ferner eine entfernbare Hülse (30) umfasst, die über dem expandierbaren, rohrförmigen Geflecht (20) gebildet ist, und wobei die Hülse (30) das Geflecht (20) bei der perkutanen Bewegung des Dilatationselements (10) bedeckt und anschließend entfernt werden kann, um das Geflecht (20) durchdrungendem Gewebe auszusetzen.
  4. Vorrichtung nach einem der vorhergehenden Ansprüche, wobei das Gewebepunktiermittel ein längliches Durchdringungselement (40) aufweist, das eine scharfe distale Spitze (42) besitzt und auf entfernbare Weise innerhalb des axialen Lumens des Dilatationselements (10) derart aufgenommen ist, dass sich die scharfe distale Spitze (42) in distaler Richtung hiervon erstreckt.
  5. Vorrichtung nach einem der vorhergehenden Ansprüche, wobei das längliche Expansionselement (12, 14) in der Nähe seines distalen Endes (62) ferner einen expandierbaren Anker (72) umfasst.
  6. Vorrichtung nach einem der vorhergehenden Ansprüche, wobei das Mittel zum Vereinfachen der Einführung einen Stab (74) umfasst, der ein konisches distales Ende (78) besitzt, das auf entfernbare Weise in dem axialen Lumen des Expansionselements (12, 14) aufgenommen ist, und wobei das konische Ende (78) sich in distaler Richtung von dem Expansionselement (12, 14) erstreckt, wenn der Stab (74) vollständig darin eingeführt ist.
  7. Vorrichtung nach einem der vorhergehenden Ansprüche, wobei das rohrförmige Geflecht (20) des Dilatationselements (10) ein laminierter Aufbau ist, der Geflechtdrähte in einer elastischen oder plastisch deformierbaren Matrixschicht aufweist.
  8. Vorrichtung nach einem der vorhergehenden Ansprüche, wobei das längliche Dilatationselement (10) ferner eine entfernbare Hülse (30) über dem rohrförmigen Geflecht (20) umfasst, und wobei das expandierbare rohrförmige Geflecht (20) an seinem proximalen Ende an einem ersten Griff (20) angebracht ist und die entfernbare Hülse (30) an seinem proximalen Ende an einem zweiten Griff (32) angebracht ist, und wobei der zweite Griff (32) in distaler Richtung von dem ersten Griff (22) sich befindet, um das Entfernen der Hülse (30) über das Geflecht (20) unter Verwendung des zweiten Griffes (32) zu ermöglichen.
  9. Vorrichtung nach Anspruch 8, wobei die entfernbare Hülse (30) ein zylindrisches Polymerrohr ist, das in axialer Richtung an gegenüberliegenden Seiten oder lediglich an einer Seite eingekerbt ist, und wobei der zweite Griff (32) an Stellen geschwächt ist, die mit den axialen Kerben in dem Polymerrohr ausgerichtet sind, um ein Spalten der Hülse (30) und des Griffes (32) zu ermöglichen, und um so das Entfernen der Hülse (30) zu vereinfachen.
  10. Vorrichtung nach einem der vorhergehenden Ansprüche, wobei das Punktiermittel (40) eine Nadel ist mit einem an ihrem distalen Ende angeordneten zurückziehbaren Obturator (48).
  11. Vorrichtung nach einem der vorhergehenden Ansprüche, wobei das längliche Expansionselement (12, 14) ferner einen Stab (74) umfasst, der ein konisches distales Ende besitzt, das auf entfernbare Weise in dem axialen Lumen des rohrförmigen Elements (60, 60') aufgenommen ist, und wobei das konische distale Ende sich in distaler Richtung von dem rohrförmigen Element (60, 60') erstreckt, wenn der Stab (74) vollständig darin aufgenommen ist.
  12. Vorrichtung nach Anspruch 11, wobei das rohrförmige Element mit konstantem Radius (60) ferner einen expandierbaren Anker (72') in der Nähe seines distalen Endes umfasst.
  13. Vorrichtung nach Anspruch 2 oder nach einem der Ansprüche 3 bis 12, die abhängig vom Anspruch 2 sind, wobei das längliche Expansionselement (12, 14) ferner an seinem proximalen Ende ein Verbindungselement (90, 92) umfasst, das zum Lumen des Expansionselements (12, 14) offen ist zum abnehmbaren Anbringen des Trokarventils (16).
  14. Vorrichtung nach Anspruch 13, wobei das Verbindungselement (90, 92) eine Bayonettfassung ist.
  15. Vorrichtung nach Anspruch 2 oder nach einem der Ansprüche 3 bis 14, die abhängig vom Anspruch 2 sind, wobei das Trokarventil (16) einen Ventilkörper, der einen Körper mit einer axialen Durchführung besitzt, ein Ventilmittel in der axialen Durchführung zum Vermeiden eines Druckverlustes durch die Durchführung und ein Mittel auf dem Körper aufweist, das mit der axialen Durchführung ausgerichtet ist zum entfernbaren Anbringen des Körpers an das proximale Ende des länglichen Expansionselements (12, 14).
