Description
- Die vorliegende Erfindung bezieht sich auf eine neue Vorrichtung zum Durchstechen von Körpergeweben bis zu einem Zielgewebe zum Zweck der Entnahme einer Biopsieprobe eines Gewebes und gegebenenfalls zu anderen medizinischen Zwecken, wie z. B. der Zerstörung von Gewebe und der Zufuhr von Fluidsubstanzen. Die Vorrichtung durchsticht das Gewebe bis zum genauen Ziel, das so ausgewählt wurde, um die Probe zu erhalten und gegebenenfalls Energie dem Gewebe und/oder Substanzen zuzuführen. Dadurch wird die Aktivität der Vorrichtung auf die genaue, vorausgewählte Stelle beschränkt, wodurch das Trauma auf das normale umgebende Gewebe minimiert und ein größerer medizinischer Vorteil erreicht wird. Diese Vorrichtung ist eine Katheter-ähnliche Vorrichtung zum Positionieren einer Biopsie- und gegebenenfalls Behandlungsanordnung in dem Bereich oder Organ, der/das zur Untersuchung und medizinischen Behandlung mit einem oder mehreren Stiletten im Katheter ausgewählt wurde, die so angebracht sind, dass sie von einer Stilettöffnung in der Seite des Katheters durch das umgebende Gewebe bis zum Gewebe, das für eine medizinische Behandlung ausgewählt wurde, vorgeschoben werden können.
- Hintergrund der Technik
- Die Behandlung von Zellgewebe erfordert gewöhnlich einen direkten Kontakt des Zielgewebes mit einem medizinischen Instrument, gewöhnlich mittels eines chirurgischen Verfahrens, in welchem sowohl das Ziel- als auch das Interventionsgewebe einem wesentlichen Trauma ausgesetzt sind. Oftmals ist die genaue Positionierung einer Behandlungssonde aufgrund der Position des Zielgewebes im Körper oder aufgrund der Nähe des Zielgewebes zu leicht zu beschädigenden wichtigen Körperorganen, Nerven oder anderen Komponenten schwierig.
- Gutartige Hypertrophie oder Hyperplasie der Prostata (BPH) stellt bei Männern über 50 das häufigste medizinische Problem dar. Eine Verengung der Harnröhre aufgrund von Hyperplasie der Prostata ist seit den frühesten Anfängen der Medizin bekannt. Die hyperplastische Vergrößerung der Prostatadrüse führt oftmals zu einem Zusammendrücken der Harnröhre, was dazu führt, dass die Harnröhre verstopft wird und sich nachfolgend Symptome wie häufiges Urinieren, abnehmender Urinfluss, Nykturie, Schmerz, Unbehagen und Tröpfeln des Harns entwickeln. Ein Zusammenhang zwischen BPH und dem Alterungsprozess wurde durch die Tatsache bewiesen, dass BPH bei Männern über 50 eine Häufigkeit von mehr als 50% ausmacht und bei Männern mit über 80 Jahren auf mehr als 75% ansteigt. Die Symptome von Harnröhrenverengung treten am häufigsten im Alter zwischen 65 und 70 Jahren auf, wenn etwa 65% der Männer dieser Altersgruppe eine Vergrößerung der Prostata zeigen.
- Im Moment gibt es kein nicht-chirurgisches Verfahren zur Behandlung von BPH, das sich als effektiv erwiesen hat. Zusätzlich dazu sind die zur Verfügung stehenden chirurgischen Eingriffe nicht zur Gänze zufriedenstellend. Gegenwärtig bieten sich Patienten, die an den Verstopfungssymptomen dieser Erkrankung leiden, folgende wenige Möglichkeiten: weiter mit den Symptomen umgehen (d. h. konservative Behandlung), sich im frühen Stadium einer medikamentösen Behandlung unterziehen oder sich einem chirurgischen Eingriff unterziehen. Mehr als 430.000 Patienten unterziehen sich pro Jahr in den USA jährlich einer chirurgischen Entfernung von Prostatagewebe. Diese Zahl stellt weniger als 5 Prozent der Männer mit signifikanten klinischen Symptomen dar.
- Oftmals leiden ältere Männer, viele mit zusätzlichen Gesundheitsproblemen, an BPH, wodurch das Risiko von chirurgischen Eingriffen noch zusätzlich erhöht wird. Chirurgische Eingriffe zum Entfernen von Prostatagewebe sind mit einer Reihe von Gefahren, einschließlich dabei Tod in Folge von Anästhesie, Hämorrhagie, Koagulopathien, Lungenembolien und Ungleichgewicht des Elektrolythaushalts, verbunden. Diese gegenwärtig durchgeführten Eingriffe können auch zu Herzkomplikationen, Perforation der Blase, Inkontinenz, Infektion, Verengung der Harnröhre oder des Blasenhalses, Retention von Prostatastückchen, retrograder Ejakulation und Unfruchtbarkeit führen. Aufgrund der extensiven invasiven Natur der gegenwärtigen Behandlungsmöglichkeiten für die obstruktive Uropathie schiebt die Mehrzahl der Patienten eine definitive Behandlung ihres Zustands weiter auf. Diese Umstände können aber zu ernsthaften Schädigungen an Strukturen führen, die eine Folge der obstruktiven Läsion in der Prostata (Blasenhypertrophie, Hydronephrose, Erweiterung der Nierenbecken, chronische Infektion, Erweiterung der Harnleiter etc.) sind, jedoch nicht ohne bedeutende Konsequenzen. Zusätzlich dazu ist eine bemerkenswerte Anzahl von Patienten mit Symptomen, die ausreichend ernsthaft sind, um einen chirurgischen Eingriff zu rechtfertigen, Risikopatienten und somit schlechte Kandidaten für eine Prostatektomie. Darüber hinaus sind jüngere Männern, die an BPH leiden und keine Komplikationen wie Unfruchtbarkeit in Kauf nehmen wollen, oftmals gezwungen, einen chirurgischen Eingriff außer Betracht zu lassen. Somit ist der Bedarf an verbesserten chirurgischen und nicht-chirurgischen Verfahren zur Behandlung von BPH sowie deren Wichtigkeit und Wert unbestreitbar.
- In Elektrokauterisierverfahren zum Schneiden menschlichen Gewebes, insbesondere wenn ein unblutiger Schnitt erwünscht ist, oder wenn die Operationsstelle mit einem normalen Skalpell nicht erreichbar ist, sondern nur einen Zugang für ein dünnes Instrument, so z. B. natürliche Körperöffnungen wie Esophagus, Darm oder Harnröhre, bietet, wird Hochfrequenzstrom verwendet. Beispiele dafür umfassen die Entfernung von Adenomen in der Prostata, Blasentumoren oder Darmpolypen. In solchen Fällen wird Hochfrequenzstrom über eine chirurgische Sonde in das zu schneidende Gewebe zugeleitet. Die sich ergebende umgewandelte Hitze führt dazu, dass die Zellflüssigkeit zu sieden und zu verdampfen beginnt, woraufhin die Zellwände aufbrechen und das Gewebe getrennt wird.
- Die Zerstörung von Zellgeweben in situ wird in der Behandlung von vielen Erkrankungen und medizinischen Zuständen allein oder zusätzlich zu chirurgischen Eingriffen, in denen Gewebe entfernt wird, eingesetzt. Sie ist oftmals weniger traumatisch als chirurgische Eingriffe und kann die einzige Alternative darstellen, wenn andere Verfahren sich als unsicher erwiesen haben. Ablative Behandlungsvorrichtungen haben den Vorteil, dass sie elektromagnetische Energie verwenden, die durch die Leitungs- und Konrektionskräfte der zirkulierenden Flüssigkeiten und durch andere natürliche Vorgänge im Körper schnell verbraucht und auf ein nicht-zerstörerisch wirkendes Niveau reduziert wird.
- Mikrowellen-, Hochfrequenz-, akustische (Ultraschall) und Lichtenergie-(Laser-)Vorrichtungen sowie gewebezerstörende Substanzen wurden zur Zerstörung bösartiger, gutartiger und anderen Zelltypen und Gewebe aus einer großen Vielzahl an anatomischen Stellen und Organen verwendet. Behandelte Gewebe umfassen isolierte Krebsmassen und insbesondere Organe wie Prostata, Drüsen- und Stromaknötchen, die ein charakteristisches Merkmal für gutartige Hyperplasie der Prostata sind. Diese Vorrichtungen umfassen typischerweise einen Katheter oder eine Kanüle, die verwendet wird, um eine Hochfrequenzelektrode oder eine Mikrowellenantenne durch eine Durchführung zur Behandlungszone zu führen und Energie diffus durch die Durchführungswand in das umgebende Gewebe in alle Richtungen anzulegen. Während des Vorgangs der Zellzerstörung erleidet die Durchführungswand oftmals ein schweres Trauma, und einige Vorrichtungen kombinieren auch Kühlsysteme mit Mikrowellenantennen, um das Trauma auf die Durchführungswände zu reduzieren. Um die Prostata mit diesen Vorrichtungen behandeln zu können, wird z. B. Wärmeenergie durch die Wände der Harnröhre in die umgebenden Prostatazellen eingeleitet, um dadurch das die Harnröhre zusammendrückende Gewebe zu zerstören. Lichtenergie, gewöhnlich von einem Laser, wird mittels Laser zu den Zielstellen des Prostatagewebes zugeführt, indem durch die Wand der Harnröhre "durchgebrannt" wird. Gesunde Zellen der Durchführungswand wie auch das gesunde Gewebe zwischen den Knötchen und der Durchführungswand werden ebenso ohne Unterschied in diesem Vorgang zerstört, was zu einem nicht notwendigen Verlust einer gewissen Prostatafunktion führen kann. Weiters verkompliziert die zusätzliche Kühlfunktion einiger Mikrowellenvorrichtungen die Apparatur und es ist erforderlich, dass die Vorrichtung ausreichend groß ist, um dieses Kühlsystem aufnehmen zu können.
- Die Anwendung von Fluiden auf spezifisches Gewebe zu medizinischen Zwecken ist durch die Fähigkeit eingeschränkt, eine Zufuhr ohne Traumatisierung des Interventionsgewebes zu erreichen und eine Zufuhr zu bewirken, die auf das spezifische Zielgewebe beschränkt ist. Lokalisierte Chemotherapie, medikamentöse Infusionen, Kollageninjektionen oder Injektionen von Mitteln, die danach durch Licht, Wärme oder Chemikalien aktiviert werden, würden durch eine Vorrichtung, die herkömmlich und genau einen Katheter zur Flüssigkeitszufuhr, der sich am spezifischen Zielgewebe öffnet, platziert, in großem Ausmaß erleichtert.
- Möchte der operierende Chirurg eine Probe des ausgewählten Gewebes entnehmen, so wird eine Biopsievorrichtung verwendet. Biopsievorrichtungen sind allgemein bekannt. U.S. P. 3.595.217 (Rheinfrank), erteilt am 27. Juli 1971, beschreibt ein Biopsieverfahren für die Prostata, worin der Finger des Arztes in das Rektum geführt wird, woraufhin eine Biopsienadel danach durch die Führung und transrektal in die Prostatadrüse eingeschoben wird. U.S. P. 3.598.108 (Jamishidi), erteilt am 10. August 1971, beschreibt eine Biopsienadel mit einem inneren entfernbaren Hüllenelement und einem Stilett. U.S. P. 4.396.021 (Baumgartner), erteilt am 2. August 1983, beschreibt eine Prostatabiopsievorrichtung unter Verwendung einer Umhüllung, woraufhin eine Probenentnahmeeinheit mit einer Harpunenspitze verwendet wird. U.S. P. 4.600.014 (Beraha), erteilt am 15. Juli 1986, offenbart eine Kernproben-Biopsienadelanordnung, die einen Griff und eine Führungsröhre umfasst. U.S. P. 4.989.614 (Dejter), erteilt am 9. Februar 1991 offenbart eine Aspirationsbiopsievorrichtung mit feiner Nadel. U.S. P. 5.014.717 (Lohrmann), erteilt am 14. Mai 1991, beschreibt eine Lochbiopsietechnik, umfassend ein hüllenförmiges Messer und ein Stilett, das in das Messer hineingleitet. Das Dokument DE-A-11 60 573 offenbart eine Vorrichtung, die als nächstliegender Stand der Technik angesehen wird. Alle diese Patente sowie andere Patente offenbaren typische Techniken und Vorrichtungen nach dem Stand der Technik zum Einsammeln und Entfernen von Biopsieprobegewebe für eine weitere Untersuchung und Überprüfung durch geschultes Personal.
- Offenbarung der Erfindung
- Es ist ein Ziel dieser Erfindung, eine Vorrichtung zum Durchstechen von Gewebe, durch Interventionsgewebe bis zum genauen Zielgewebe, das für einen medizinischen Vorgang wie Gewebeablation und gegebenenfalls Zufuhr einer Substanz ausgewählt wurde, bereitzustellen, wobei dieser Vorgang auf die genaue vorbestimmte Stelle beschränkt ist, wodurch das Trauma minimiert wird und ein größerer medizinischer Nutzen daraus gezogen werden kann.
- Ein weiteres Ziel der Erfindung besteht darin, eine medizinische Sondenvorrichtung mit einer chirurgischen Biopsievorrichtung direkt am distalen Ende dieser bereitzustellen, die selektiv Gewebe aus den Zielgeweben einsammelt.
- Noch ein weiteres Ziel der Erfindung besteht darin, eine medizinische Sondenvorrichtung mit einer chirurgischen Biopsievorrichtung bereitzustellen, die eine Gewebeprobe aus dem Zielgewebe entnehmen kann.
- Ein anderes Ziel der vorliegenden Erfindung besteht darin, medizinische Sondenvorrichtungen mit einer chirurgischen Biopsievorrichtung bereitzustellen, die eine Gewebeprobe aus Zielgeweben in Verbindung mit einer integrierten Schaltkreis- oder Halbleitervorrichtung entnehmen kann, welche wiederum direkte Mikrowellenenergie für selektive Ablationsanwendungen zuleiten kann.
- Zusammengefasst ist die Vorrichtung dieser Erfindung eine medizinische Sondenvorrichtung, umfassend einen Katheter mit einem Stilettführungsgehäuse mit zumindest einer Stilettöffnung in einer Seite davon und einem Stilettführungsmittel, um ein biegsames Stilett nach außen durch zumindest eine Stilettöffnung und durch Interventionsgewebe bis zu den Zielgeweben zu lenken. Die Stilettführung besitzt an ihrem distalen Ende eine Biopsieprobenvorrichtung oder eine andere Gewebemanifestationsvorrichtung zum Einsammeln von Gewebe oder zum Durchführen eines anderen Vorgangs am erwünschten Gewebe, so z. B. dem Abgeben von elektromagnetischer Energie als ablative Leistung an die Zielgewebe. Die Stilette umfassen dünne längliche Haltemittel, wobei jedes Haltemittel in einer nicht-leitenden Hülle eingeschlossen ist, umfassend dabei Mittel an den distalen Enden dieser Stilette zum selektiven Abgeben von elektromagnetischer Energie oder zum Einsammeln einer Probe des Zielgewebes, oder beides, wobei die elektromagnetische Energie als ablative Leistung den Zielgeweben zugeleitet wird und wobei die elektromagnetische Energie Mikrowellenenergie ist. Die nicht-leitenden Hüllen sind für eine Bewegung in die Längsrichtung auf den jeweiligen Haltemitteln montiert, um einen ausgewählten Abschnitt der Stilettfläche im Zielbereich auszusetzen.
- In einer anderen Ausführungsform ist die medizinische Sondenvorrichtung so offenbart, dass sie einen Katheter mit einem Stilettführungsgehäuse mit zumindest einer an das distale Ende derselben angrenzenden Stilettöffnung umfasst, wobei Stilettführungsmittel zum Lenken eines biegsamen Stiletts nach außen durch zumindest eine Stilettöffnung und durch das Interventionsgewebe bis zu den Zielgeweben umfasst sind, worin ein Stilett in zumindest einem der Stilettführungsmittel positioniert ist, wobei das Stylett ein dünnes längliches Haltemittel umfasst sowie ein Mittel am distalen Ende des Stiletts zum Einsammeln einer Probe des Zielgewebes. Das Einsammelmittel umfasst einen Biopsiegewebe-Sampler, wobei dieser Biospiegewebe-Sampler ein Biopsienadelmittel zum Einsammeln von Gewebezellen der Proben umfasst, oder ein zugespitztes Stilett mit Widerhaken zum Schneiden und Einsammeln der Gewebeprobe, oder einen Zweischalengreifer oder ein Spannstilett zum Einsammeln der Gewebeprobe. Umfasst der Biopsiegewebe-Sampler einen Zweischalengreifer oder eine Alligatorklemmvorrichtung zum Einsammeln der Gewebeprobe, so wird die Biopsievorrichtung geöffnet, wenn sie durch die Bewegung des Haltemittels in die Längsrichtung ausgesetzt wird, und sie wird auf dem Zielgewebe geschlossen, um dieses dadurch zu definieren, wenn die Biopsievorrichtung in die Isolierungshülle zurückgezogen wird.
- Auch kann der Biopsiegewebe-Sampler eine Vorrichtung mit einer Harpunenspitze zum Einsammeln von Gewebeproben umfassen. Der Biopsiegewebe-Sampler umfasst eine Kernprobenvorrichtung zum Einsammeln der Gewebeprobe, wobei die Kernprobenvorrichtung eine hohle Röhre mit einem zugespitzten Ende zum Erleichtern der Penetration in die Gewebeprobe und zum Einsammeln dieser umfasst. Zusätzlich dazu wird, wenn der Biopsiegewebe-Sampler eine hohle Röhre mit einer Öffnung entlang der lateralen Seite davon angrenzend an das distale Ende dieser Röhre umfasst, die Gewebeprobe durch einen negativen Druck, der auf die hohle Röhre angelegt wird, in die hohle Röhre eingezogen und vom Zielgewebe abgetrennt, wenn die hohle Röhre in die Isolierungshülle zurückgezogen wird. Die hohle Röhre kann die Form einer Harpune mit einer Öffnung darin und einer zugespitzten Spitze aufweisen, um die Penetration des Zielgewebes zu erleichtern.
- Weiters umfasst der Biopsiegewebe-Sampler einen Zweischalengreifer oder eine Alligator-Klemmvorrichtung zum Einsammeln der Gewebeprobe, wobei diese Biopsievorrichtung geöffnet wird, wenn sie durch die Bewegung des Haltemittels in die Längsrichtung ausgesetzt wird, und sie wird auf dem Zielgewebe geschlossen, um dieses dadurch zu definieren, wenn die Biopsievorrichtung in die Isolierungshülle zurückgezogen wird.
- Eine weitere Ausführungsform umfasst auch ein Mittel am distalen Ende des Stiletts zum Abgeben von elektromagnetischer Energie, wobei das Emissionsmittel selektiv mit dem Einsammelmittel oder unabhängig davon mit Energie versorgt wird. Hier umfasst das Emissionsmittel für die elektromagnetische Energie eine Mikrowellenantenne, und das Einsammelmittel umfasst einen Biopsiegewebe-Sampler, wobei das Stilett selektiv mit dem Biopsiegewebe-Sampler oder unabhängig davon elektromagnetische Energie abgibt. Der Antennendraht ist um die Isolierungshülle herum vom proximalen Ende bis zum distalen Ende der Isolierungshülle gewunden.

- Die Vorrichtung dieser Erfindung stellt ein genaues, gesteuertes Positionieren eines Behandlungsstiletts in einem Gewebe bereit, auf das eine Behandlung, Ablation oder Probenentnahme durch einen Katheter, der in der Nähe der Zielgewebe positioniert ist, abzielt.
- Der hierin verwendete Ausdruck "Stilett" ist so definiert, dass er sowohl massive als auch hohle Sonden umfasst, die so adaptiert sind, dass sie von einer Katheteröffnung durch das normale Gewebe zu den Zielgeweben geführt werden können. Das Stilett ist so geformt, dass es einen leichten Durchgang durch das Gewebe erleichtert. Es kann ein fester Draht, ein dünner Stab oder von anderer fester Gestalt sein, es kann aber auch eine dünne hohle Röhre oder eine andere Form mit einem Längslumen zum Einführen von Fluiden zur oder zum Entfernen von Materialen von einer Stelle sein. Das Stilett kann auch eine dünne hohle Röhre oder eine andere hohle Gestalt sein, wobei das hohle Lumen davon einen Verstärkungs- oder Funktionsstab oder eine Verstärkungs- oder Funktionsröhre wie einer Laser-Glasfaser enthält. Das Stilett besitzt vorzugsweise ein zugespitztes Ende, um Widerstand und Traumata zu verringern, wenn es durch das Gewebe zu einer Zielstelle vorgeschoben wird. Wie hierin nachfolgend beschrieben ist, kann das Stilett auch eine Biopsieprobeneinsammel-, Laserlichtemissions- und Mikrowellenvorrichtung umfassen.
- Das Stilett kann auch so konzipiert sein, dass es eine Vielzahl medizinisch erwünschter Behandlungen eines ausgewählten Gewebes bereitstellt. Es kann als Widerstandsheizung, Hochfrequenzelektrode, Laserlichtemissionsvorrichtung oder Mikrowellenantenne verwendet werden, um die Zielgewebe abzutragen oder zu zerstören. Als hohle Röhre kann es verwendet werden, um Behandlungsfluida wie eine Flüssigkeit den Zielgeweben zuzuführen. Die Flüssigkeit kann als einfache Lösung oder als Suspension von Feststoffen, z. B. kolloiden Teilchen, in einer Flüssigkeit vorhanden sein. Da das Stilett sehr dünn ist, kann es vom Katheter durch das normale Interventionsgewebe mit einem Minimum an Traumata für das normale Gewebe gelenkt werden. Als Biopsieprobeneinsammler kann ein Probenstück des Zielgewebes genommen und für eine weitere extern durchgeführte histologische Analyse und Untersuchung entfernt werden.
- Die Vorrichtung dieser Erfindung stellt eine noch genauere, kontrollierte medizinische Behandlung bereit, die zum Einsammeln oder Zerstören von Zellen aus medizinischen Zielgeweben im ganzen Körper, sowohl in als auch außerhalb von Körperorganen, geeignet ist. Die Vorrichtung ist insbesondere für die Behandlung von gutartiger Prostata-Hyperplasie (BPH) geeignet, und die Vorrichtung sowie deren Verwendung sind nachfolgend in Bezug auf BPH zum Zwecke der Vereinfachung ihrer Beschreibung erklärt. Für Fachleute auf dem Gebiet der Technik ist leicht erkennbar, dass die Vorrichtung zum Einsammeln und Zerstören von Körpergeweben in allen Körperhöhlen oder Gewebepositionen, die durch perkutane oder endoskopische Katheter erreicht werden können, verwendet werden kann, und dass diese nicht auf die Prostata allein beschränkt ist.
- BPH ist ein Zustand, der sich aus der gutartigen Reproduktion und dem Wachstum von Zellen in der Prostata ergibt, wodurch Drüsen- und Stromaknötchen gebildet werden, welche die Prostata ausdehnen und die Öffnung der Harnröhre in die Prostata verengen. Drüsenknötchen sind vorrangig in der Übergangszone positioniert, während Stromaknötchen in der Periurethralregion zu finden sind. Traditionelle Behandlungsformen dieses Zustands umfassen die chirurgische Entfernung der gesamten Prostatadrüse, die digitale Entfernung des Adenoms sowie die transurethrale Resektion des Harnröhrenkanals und der Prostata, um Gewebe zu entfernen und den Durchgang zu erweitern. Eine wichtige und ernste Komplikation, die mit diesen Verfahren in Zusammenhang gebracht wird, ist die iatrogene Sterilität. In letzter Zeit wurde die Laserbehandlung eingesetzt, um Gewebe zu entfernen, wobei dabei der Blutfluss und der Verlust von Körperflüssigkeiten beschränkt wurde. Es wurden auch Ballone innerhalb der Harnröhre aufgeblasen, um ihren Durchmesser zu vergrößern, wobei in diesem Zusammenhang die Verwendung von Wärme fakultativ ist, es zeigte sich aber, dass auch dabei beträchtliche Einschränkungen erkennbar sind.
- Die Mikrowellentherapie wurde mit einigem Erfolg verwendet, wobei hier eine Mikrowellenantenne in der Harnröhre der Prostata positioniert und Hitze im die Harnröhre umgebenden Gewebe mit Hilfe eines Mikrowellenfelds erzeugt wurde. Manchmal werden Kühlmittel innerhalb des Katheterschafts angewendet, um die Temperatur der Harnröhrenwand zu verringern. Dies erfordert aber komplizierte Mechanismen, um sowohl die unmittelbar daran angrenzenden Gewebe zu kühlen und gleichzeitig Hitze in den weiter entfernten Geweben der Prostata zu erzeugen. Diese Technik ist der Mikrowellen-Hyperthermie ähnlich. Auf ähnliche Weise setzt die Hochfrequenz-Gewebeablation mittels in der Harnröhre positionierter Elektroden die Harnröhrenwand zerstörerischen Temperaturen aus. Um dies zu vermeiden, muss die Temperatureinstellung, die erforderlich ist, um die Harnröhre zu schützen, so niedrig sein, dass die Behandlungszeit, die erforderlich ist, um eine nützliche Wirkung zu erzielen, so ungebührlich lang ausgedehnt wird, d. h. es ergibt sich eine bis zu drei Stunden lange Energieanwendung.
- Eine Ausführungsform der Vorrichtung dieser Erfindung, die zuvor in einer Vorgängeranmeldung offenbart wurde, verwendet die Harnröhre, um zur Prostata zu gelangen, und positioniert HF-Elektrodenstilette direkt in den zu zerstörenden Geweben oder Knötchen. Der Abschnitt des Stilettleiters, der sich von der Harnröhre zu den Zielgeweben erstreckt, ist in einer in Längsrichtung einstellbaren Hüllenabschirmung eingeschlossen, die verhindert, dass das an die Hülle angrenzende Gewebe dem HF-Strom ausgesetzt wird. Die Bewegung der Hülle wird auch dazu verwendet, die Menge an zugeführter Energie pro Einheitsfläche zu regulieren, indem das Ausmaß der ausgesetzten Elektrode reguliert wird. Auf diese Weise wird die Ablation auf für eine Ablation bestimmte Zielgewebe beschränkt, nämlich auf jene, welche die mechanische Verengung hervorrufen. Andere Aspekte der Erfindung sind aus den Zeichnungen und den begleitenden Beschreibungen der Vorrichtung dieser Erfindung besser verständlich. Für Fachleute auf dem Gebiet der Technik ist es offensichtlich, dass diese Verfahren in vielen Bereichen des Körpers verwendet werden können, um einen perkutanen Zugang und einen Zugang durch Körperöffnungen zu schaffen.

1 ist eine isometrische Ansicht einer Ausführungsform eines Ablationskatheters dieser Erfindung mit einem faseroptischen Betrachtungshilfsmittel. Der biegsame Katheter2 , der am Griff4 befestigt ist, weist eine Endstilettführung6 mit zwei Stiletten auf. Der Griff besitzt zwei Stiletthüllenstreifen10 und11 sowie Elektrodenstreifen12 und13 , die nachfolgend hierin detaillierter beschrieben sind. Der Griff4 ist auch mit einer optischen Betrachtungsanordnung14 und einem HF-Stromanschluss16 , einem Transponderanschluss16 und einem Anschluss für Thermoelemente20 verbunden. Die Abschnitte des Katheters2 , die vom Griff4 zur Stilettführungsspitze6 führen, können gegebenenfalls von abgestufter Steifigkeit sein. So kann der Katheter z. B. so konzipiert sein, dass er nahe des Griffs steifer und nahe der Spitze biegsamer ist, er kann aber auch andere Steifigkeitsprofile zeigen. Der Katheter kann aus einer geschlitzten Innenröhre aus Edelstahl mit einer biegsamen Außenhülle, wie sie z. B. in der ebenfalls anhängigen Anmeldung Nr. 790.648, eingereicht am 11. August 1991 (entspricht der Australischen Patentanmeldung Nr. 9210858) beschrieben ist, zusammengesetzt sein. Er kann auch aus einem gewendelten oder geflochtenen Draht bestehen, an dem eine Außenhülle befestigt ist. - Die faseroptische Betrachtungsanordnung in dieser Ausführungsform umfasst eine Linsen-Fokussieranordnung
22 und einen Halterungsanschluss für die Linsen-Betrachtungsanordnung24 , die über einen biegsamen Kunststoffschlauch mit einem schnell trennbaren Stecker26 verbunden ist. -
2 ist eine Teilquerschnittsansicht einer bevorzugten Katheterspitze und des Stilettführungsgehäuses dieser Erfindung. Die feste Katheterspitze106 besitzt eine seitliche Vertiefung oder einen Sattel108 darin, wobei eine Mittelachse etwa orthogonal auf eine Ebene durch die Mittelachse der Spitze liegt. Die Vertiefung108 besitzt eine proximate Wand110 . Die Vertiefung108 kann sich bis zu etwa der Hälfte der Dicke des Gehäuses erstrecken, zumindest aber ausreichend so weit, dass die Betrachtungsfläche der Betrachtungsspitze112 der Glasfaser114 unblockiert ist. Die faseroptische Betrachtungsspitze112 stellt, wenn sie an der Öffnung in der Wand110 positioniert ist, ein Betrachtungsfeld mit seitlichen Grenzen116 und einer Endgrenze118 bereit. Dies umfasst den Weg der Stilette, die sich durch die Öffnungen120 nach außen erstrecken. -
3 ist eine distale Endansicht der Katheterspitze und des Stilettführungsgehäuses, die in2 dargestellt sind. Das proximate Ende der Vertiefung108 ist in zwei Vorkragungen oder Ohren122 und124 geteilt, die eine längsgerichtete oder axiale Rille oder einen längsgerichteten oder axialen Sattel126 definieren, die/der sich von der Vertiefung108 zur Endspitze128 des Katheters106 erstreckt. Die Rille126 öffnet für die Betrachtungsspitze112 das Betrachtungsfeld, wenn sie sich in der in2 mit durchgezogenen Linien dargestellten Position befindet, und ermöglicht dann eine Ausdehnung der Glasfaser und seiner Spitze durch die längsgerichtete Rille zu den Positionen114' und112' , die in strichliierten Linien dargestellt sind. In der letzteren Position besitzt das Sichtfeld Seitengrenzen130 und eine Endgrenze132 . Dies ermöglicht dem Benutzer, die Flächen der inneren Durchführung vor der Katheterspitze zu untersuchen. In einer alternativen Ausführungsform kann die Rille126 durch ein Loch im Ende der Spitze mit einer solchen Größe und Position ersetzt werden, so dass sich die Glasfaser114 hindurch erstrecken kann. - Die Glasfaser
114 wird in einem Durchlass134 positioniert, der ausreichend größer als die Glasfaser ist, um auf diese Weise den Fluss von Spülflüssigkeit um die Glasfaser herum zum Ausgang in der Wand110 zu ermöglichen. Die Spülflüssigkeit wäscht Verunreinigungen aus der Betrachtungsspitze aus. Die Innenwände der Durchführung (nicht dargestellt), welche die Katheterspitze106 während der Verwendung umgeben, begrenzen den Fluss der Flüssigkeit, so dass die Flüssigkeit weiterhin über die Spitze der Glasfaser fließt, selbst wenn diese in die mit gepunkteten Linien dargestellte Position vorgeschoben wird. Lumina136 und138 für die Rückspülflüssigkeit erstrecken sich durch die Wand110 , um verunreinigte Spülflüssigkeit konstant zu entfernen. -
4 ist eine proximate Endansicht der auseinandergenommenen Katheterspitze und des Stilettführungsgehäuses, die in2 dargestellt sind, wobei die Lumina der Komponenten davon veranschaulicht sind. Die Stilette werden durch die Stilettlumina140 und142 zu den Stilettöffnungen120 vorgeschoben und zurückgezogen. Die Glasfaser wird durch das Glasfaserlumen134 vorgeschoben und zurückgezogen. Die verunreinigte Spülflüssigkeit wird durch die Spülflüssigkeits-Rücklumina136 und138 entfernt. Das Temperatursensor-Lumen144 wird verwendet, um die Leitungen eines Temperatursensors (nicht dargestellt) aufzunehmen. -
5 ist eine schematische Ansicht eines einzelnes Stiletts der1 oder2 , das ausgesetzt wird, um einen Abschnitt einer Prostata, die in die Harnblase vorragt, zu behandeln. Die feste Katheterspitze196 wird am Ende der Harnröhre220 positioniert. Durch die Zellproliferation im oberen Ende222 der Prostata224 ist diese in den Raum vorgeschoben worden, der normalerweise von der Harnblase eingenommen wird, wodurch ein Abschnitt der Blasenwand226 in den Hohlraum gedrückt wird und eine Verengung225 über das Ende der Harnröhre hinaus ausbildet. Die Stiletthülle206 und die Elektrode204 sind in einem Winkel von etwa 30° durch die Harnröhrenwand in einen Abschnitt der vorragenden Prostata erstreckt, und HF-Strom wird angelegt, um die Läsion228 auszubilden. Dadurch wird die vorragende Prostata reduziert, ihr Rückzug aus der Harnröhrenwand wird gefördert und die Verengung am Auslassende der Harnröhre wird geöffnet. Der Katheter mit einem erwünschten Winkel kann aus Kathetern mit vorbestimmten Winkeln ausgewählt werden, um das Stilett genau auszurichten und eine genaue Penetration des z. B. Prostatagewebes, das sich über das Ende der Harnröhre hinaus erstreckt, durchzuführen. -
6 ist eine Explosionsansicht der Ablationskatheteranordnung der1 . Die obere Griffplatte276 weist zwei Mittelschlitze278 und280 auf, durch welche die Elektrodensteuerschieber10 am jeweils linken bzw. rechten Elektrodenschieberblock282 bzw.284 befestigt sind. Hüllensteuer-Schieber12 und13 sind durch die äußeren Schlitze286 und288 an einem entsprechenden linken Hüllenschieber-Anschlag290 und einem entsprechenden rechten Hüllenschieber-Anschlag292 angebracht. Ein Glasfaser-Aufnahmegehäuse30 ist auf der proximalen Fläche der oberen Griffplatte276 montiert. Der elektrische Rezeptor294 ist in den jeweiligen Hohlräumen296 und298 in der jeweils daran befestigten oberen Griffplatte276 und unteren Griffplatte300 aufgenommen. Die untere Griffplatte300 weist einen Mittelhohlraum302 auf, der die Elektroden- und Hüllenschieberblöcke und damit assoziierten Elemente aufnimmt. - Die Mikroschalter-Aktivatorblöcke
304 (nur der linke Hüllenblock ist dargestellt) sind mit den Hüllenschieberblöcken290 und292 verbunden. Sie sind so positioniert, dass sie die Mikroschalter306 betätigen, wenn der jeweilige Hüllenblock (und die daran befestigte Hülle) vorgeschoben wurde. Die Mikroschalter306 halten die elektromagnetischen Energiekreise geöffnet, bis die jeweiligen Hüllen zu einer Position über die Harnröhrenwand hinaus und in die Prostata vorgeschoben werden, um zu verhindern, dass die Harnröhre direkt den mit Energie angereicherten Elektroden ausgesetzt wird. Das Erstrecken der Hülle 5 mm über die Führung hinaus reicht gewöhnlich aus, um die Harnröhre zu schützen. - Die Spannungs-Drehkraft-Röhrenanordnung
308 ist im distalen Ende des Gehäuses im Rezeptor310 montiert. -
7 ist eine isometrische Ansicht der in6 dargestellten Block- und Spannungsröhrenanordnung308 des Reglers des in6 dargestellten Ablationskatheters. Die Drehmomentröhre312 erstreckt sich vom Drehmomentkuppler314 durch den Drehungssteuerknopf316 zur Stilettführung6 . Das Biegen der Drehkraftröhre312 während der Verwendung verlängert den Weg vom Griff zur Führungsspitze6 . Um ein dadurch hervorgerufenes Rückziehen der Stiletthülle und der Elektrodenkomponenten zu verhindern, wenn die Drehmomentröhre312 gebogen wird, ist eine Spannungsröhre318 mit einer fixen Länge und einem kleineren Durchmesser als der Innendurchmesser der Drehmomentröhre312 vorgesehen. Das distale Ende der Spannungsröhre318 ist fest an der Stilettführung6 befestigt, und das proximate Ende320 ist am Einstellblock322 z. B. mittels Klebstoff angebracht. Die Axial- oder Längsposition des Reglerblocks322 kann so eingestellt werden, dass sichergestellt ist, dass die Stilette anfänglich unmittelbar innerhalb der Auslassöffnungen in der Stilettführung6 positioniert sind. Der Drehmomentkuppler314 ist auf dem Kupplerblock324 montiert. Der Drallsteuerknopf-Anschlagstift326 erstreckt sich in eine (nicht dargestellte) Rille und begrenzt die Drehung des Steuerknopfs316 . -
8 ist eine detaillierte Ansicht "A" der distalen Ende der Spannungsröhrenverbindungen der in7 dargestellten Spannungsröhre. Die Spannungsröhre318 ist fest mit dem proximalen Ende328 der Stilettführung6 z. B. über eine Länge eines Schrumpfschlauchs330 verbunden. -
9 ist eine Explosionsansicht der Hüllen- und Elektrodenschieberblockanordnung der in6 dargestellten Ausführungsform. Der rechte Hüllenschieberblock292 weist eine Vorkragung332 auf, die sich nach innen unter den rechten Elektrodenschieberblock284 erstreckt. Der rechte Hüllenanschluss334 ist auf dem inneren Ende der Vorkragung332 montiert und mit dem Ende des proximalen Endes der Hülle336 befestigt. Der rechte Elektrodenanschluss338 ist mit einer Innenfläche des Elektrodenschieberanschlags284 verbunden und am proximalen Ende der Elektrode340 befestigt. Die rechten Hüllen- und Elektrodenschieberanschläge292 und284 sind verschiebbar an der rechten Reibungseinstellschiene432 mittels Schrauben (nicht dargestellt) durch die Schlitze344 und346 befestigt, wobei die Schrauben so einstellbar sind, dass sie zwischen den Anschlägen und der Schiene342 für ausreichend Reibung sorgen, um auf diese Weise eine sichere Kontrolle der Stilettbewegung bereitzustellen. Der linke Hüllenschieberblock290 und der linke Elektrodenschieberblock282 sind gespiegelte Kopien der rechten Blöcke und sind ähnlich auf der linken Reibungsschiene345 montiert. Die linke Hülle und Elektroden sind nicht dargestellt. -
10 ist eine schematische Ansicht des Anwendens zweier Stilette in einer Prostata, wobei die Ausrichtung des Stiletts für das Verfahren der überlappenden Ablationszone dieser Erfindung, das z. B. mit HF-Strom oder einem Mikrowellenfeld versorgt wird, dargestellt ist. Zur Veranschaulichung aber nicht zur Beschränkung wurde die Prostata für diese Beschreibung ausgewählt, wobei die Anwendung dieses Verfahrens und dieser Anordnung auch auf andere Körpergebiete ebenfalls umfasst sein soll. - Das zu behandelnde Gewebe für die BPH-Behandlung befinden sich in der Übergangszone
428 der Prostata. Ein Katheter dieser Erfindung430 wurde bis zur Harnröhre432 in eine an die Prostata angrenzende Position vorgeschoben. Zwei Stilette434 und436 wurden durch die Harnröhrenwand432 durch eine Vorwärtsbewegung der Nasen10 und12 (1 ) und durch die umgebenden Gewebe in die Zielgewebe vorgeschoben. Die nicht-leitenden Hüllen438 und440 wurden durch eine Rückbewegung der Hüllenstreifen10 zurückgezogen, um einen Abschnitt der jeweiligen elektrischen Leiter442 und444 am Ende jedes Stiletts auszusetzen. Der Winkel zwischen den Stilettachsen in dieser Ausführungsform beträgt weniger als 180°, vorzugsweise weniger als 110°. Für die meisten überlappenden Ablationen sind Winkel von 15° bis 90° und insbesondere von 20° bis 70° am praktischsten. Eine Erdungsplatte (nicht dargestellt) ist außerhalb des Körpers positioniert. - Werden die Elektroden
442 und444 mit HF-Strom versorgt, so wird der Schaltkreis von den Elektroden zur Erdungsplatte geschlossen. Die Stromdichte, die durch das Gewebe strömt, fließt durch das zu behandelnde Zielgewebe und erzeugt dabei Läsionen mit der Querschnittsgestalt von etwa überlappenden Zonen446 und448 . Die Stromdichte nimmt als Funktion der Entfernung rapide ab, wodurch die Größe der Läsionen begrenzt ist. Auf diese Weise können Läsionen einander überlappen und eine größere Läsion ausbilden, wodurch wiederum die Wirksamkeit der Behandlung erhöht wird. Es ist leicht zu verstehen, dass diese Vorgänge gleichzeitig, wie beschrieben, oder nacheinander durchgeführt werden können, und dass diese Variationen in dieser Erfindung umfasst sein sollen. - Während die zuvor beschriebene Erfindung in der Behandlung von gutartiger Prostata-Hyperplasie verwendet wird, kann ein Arzt in einigen Fällen den Wunsch haben, weiterzugehen und tatsächlich zu bestimmen, ob die Prostata Krebsgewebe enthält. Während die zuvor beschriebene Ablationstechnik gegen BPH eingesetzt wird, können andere und möglicherweise drastischere Verfahren notwendig sein, sollte die Prostata krebsbefallen sein. Für die Möglichkeit, dass der Arzt die Prostata für krebsbefallen hält, oder dass diese krebsartiges Gewebe oder Knötchen enthält, kann er/sie das verdächtige Gewebe weiter untersuchen, indem er/sie eine Gewebeprobe für eine histologische Untersuchung entnimmt.
- Die diesbezügliche Offenbarung betreffend Biopsie und Gewebesammlung der Prostata soll den Umfang der Erfindung nicht begrenzen. Die Prinzipien der offenbarten Erfindung sind auch auf andere Körpergewebe verwend- und anwendbar.

11 ist eine Seitenansicht eines Kernprobenbiopsiestiletts1100 . Wie in den5 ,6 und7 erkennbar ist, umfasst das Stilett eine Hülle und eine Steuerelektrode. In dieser Ausführungsform wurde die Elektrode durch die hohle Kernprobenröhre1102 ersetzt, die in der Hülle1104 eingeschlossen ist. Die zugespitzte Spitze1106 der Hülle1104 passt zur Form der Spitze1108 des Kernprobenstiletts1102 . Diese zugespitzte Kante ermöglicht eine leichtere Penetration des Gewebes, das zum Zielgewebe hin führt und dieses umfasst. Nachdem der in den vorangegangenen Figuren dargestellte Katheter in die Harnröhre oder in eine andere Körperöffnung eingeschoben wurde, wird die Hülle1104 vom Katheter in die Harnröhrenwand und in die Prostatadrüse unter Verwendung des geeigneten Stiletthüllenstreifens10 oder11 , wie sie in1 dargestellt sind, hinein geschoben. Zum richtigen Zeitpunkt wird das Stilett1102 aus der Hülle1104 heraus gestreckt, indem irgendeine der Elektrodennasen12 ,13 der1 ihre Wirkung ausüben. Bei Anwendung des Stiletts1102 wird eine Gewebeprobe mit einer scharfen Spitze1108 herausgeschnitten und im hohlen distalen Endabschnitt1114 gesammelt. Bei Bedarf kann das Stilett1102 um seine Längsachse herum gedreht oder oszilliert werden, um dadurch den Schneidvorgang zu erleichtern. Das Kernprobenstilett1102 kann daraufhin in die Hülle1104 zurückgezogen werden, die Hülle1104 wird in den Katheter zurückgezogen und danach wird der Katheter vollständig wieder aus dem Körper zurückgezogen. Das Biopsiegewebe kann daraufhin aus dem Kern1114 der Röhre1102 herausgenommen und von einem Fachmann auf Anzeichen von BPH oder Krebs untersucht werden. -
12 ist eine Seitenansicht einer Biopsiesampler-Stilettanordnung1200 mit einer Harpunenblatt-Stilettstruktur1204 . Die Hülle1208 endet in ein vorderes Ende1210 , das sich so verjüngt, dass es eine blattartige Fläche bildet. Der Stilettschaft1204 erstreckt sich durch die Hülle1208 und endet in der allgemein bei1206 dargestellten harpunenförmigen Struktur. Eine Harpune ist eine speerartige Vorrichtung mit Widerhaken, die aufgrund ihrer mit Widerhaken versehenen Spitze schwierig von einem Zieltier oder -gewebe zu entfernen ist. In diesem Fall ist die Harpunenblatt-Struktur eine zylindrische Röhre1204 mit einer Seitenöffnung oder -tasche1202 als Rezeptor für das gesammelte Gewebe. Die Rückseite der Harpune besitzt eine scharfe Schneidkante1214 . Die Harpunenblatt-Struktur kann sich in jeder beliebigen Weise in Bezug auf die Verjüngung auf der Hülle1208 beim vorderen Ende1210 ausgerichtet werden, so lange die scharfe Harpunenkante1214 eine zerstörerische Wirkung ausübt, wenn sie in die Hülle1208 über die Kante1210 zurückgezogen wird. Das Harpunenende1206 umfasst auch eine Spitze1216 , die zum Punktieren des Gewebes, das gesammelt werden soll, verwendet wird, und auch eine Tasche1202 , die hinter dem harpunenartigen Widerhaken1206 ausgebildet ist. - Ist der Katheter in die Harnröhre eingesetzt, so wird die Hülle
1208 vom Katheter (siehe5 ) in die Harnröhrenwand und in die Prostatadrüse erstreckt, indem die geeignete Stiletthüllennase (siehe6 ) verwendet wird. Danach wird das Stilett1204 in der Prostata am erwünschten Zielgewebe anwendet. Das Positionieren der Harpunenspitze1206 in Bezug auf das vordere Ende1210 der Hülle1208 erfolgt so, dass bei Bewegung des geeigneten Hüllenanschlags das Harpunenstilett1204 sich über das Ende1210 hinaus bewegt und in die von der Vorderkante1210 der Hülle1208 gebildete Öffnung zurückgezogen werden kann. Nach dem Zurückziehen der Harpunenstilettanordnung1204 in die Hülle1208 zerstört die scharfe Kante1214 jegliches Gewebe, das sich in die Tasche1202 erstreckte. Wird das Stilett1204 weiter nach hinten in die Hülle1208 zurückgezogen, so wird die Gewebeprobe in der Tasche1202 der Harpune1206 innerhalb der Hülle1208 gehalten. Daraufhin wird, ähnlich wie in der obigen Ausführungsform in Zusammenhang mit11 , die Hülle1208 vom Körper des Patienten zurückgezogen und das Gewebe aus der Tasche1202 für eine weitere externe histologische Untersuchung entfernt. -
12 ist eine Seitenansicht einer Ausführungsform dieses Aspekts der Erfindung, wobei die spitze Kante1216 in einem Winkel zur Schneidkante1214 zurückführt, während13 eine Seitenansicht einer zweiten Ausführungsform dieses Aspekts der Erfindung ist, worin die scharfe Kante1216 im Wesentlichen parallel zur Führungskante1210 der Hülle1208 verläuft. Die Schneidkante1214 arbeitet ähnlich wie in der in12 gezeigten Ausführungsform, um eine Gewebeprobe, die in der Tasche1202 aufbewahrt werden soll, zu schneiden. - Die
14 und15 sind Seitenansichten einer weiteren Biopsiesampler-Stilettanordnung1300 , worin die Zielgewebeproben durch die Hilfe von Zweischalengreifern oder Alligator-Spannbacken gewonnen werden. Die in13 veranschaulichten Spannbacken sind doppeltwirkend, wobei die um den Punkt1304 herum gedrehten Spannbacken1302 umfasst sind. Die Spannbacken1302 umfassen die Spannbacke1306 und die Spannbacke1308 . Die Spannbacke1306 und die Spannbacke1308 sind über einen biegsamen Drehpunkt1302 miteinander verbunden. Am vorderen Ende jeder Spannbacke1306 ,1308 befinden sich Zähne vom Alligator-Typ (nicht dargestellt) zum Ergreifen und/oder Schneiden des erwünschten Gewebes. Alternativ dazu können die vorderen Enden der Spannbacken1306 ,1308 sehr scharf ausgeführt sein, um den Schneidvorgang nach Aussetzen und Bedienung der Biopsie-Spannbacken1302 zu erleichtern. - An den anderen Enden der beiden Spannbacken
1306 und1308 sind Arme1316 und1314 über Drehzapfen1312 und1310 befestigt. Innerhalb der Stiletthülle1320 ist das Stilettkabel1322 positioniert. Der Drehpunkt1304 befindet sich am distalen Ende des Stiletts1322 , um die Spannbacken1302 beim Aussetzen der Vorrichtung in das Zielgewebe zu halten und zu betätigen. Wird das Kabel1324 durch den die Operation durchführenden Chirurgen zurückgezogen, werden die Arme1314 und1316 zum operierenden Chirurgen hin gezogen, und die Spannbacken1302 werden durch die Doppeltwirkung der Spannbacken um den Drehpunkt1304 geschlossen. Kommen die Spannbacken1306 und1308 zusammen, so schneiden die scharfen Vorderkanten davon eine kleine Probe aus dem Zielgewebe oder zerstören dieses. Die Stilett-Spannbackenanordnung1302 wird daraufhin in die Hülle1320 zurückgezogen, die Hülle zieht sich aus dem Gewebe zurück, und die Gewebeprobe wird aus dem Körper des Patienten für eine externe histologische Untersuchung zurückgezogen und entfernt. - Die
16 bis18 zeigen ein Biopsiemesser-Stilett1400 in Kombination mit einer HF-Antennenanordnung.16 zeigt das von der Hülle1402 umgebene Stilett1404 . Die zugespitzte Kante1406 des Stiletts1404 unterstützt den Schnitt in das ausgewählte Körpergewebe. Um das Mittelstilett1404 herumgewunden befindet sich ein Antennendraht1408 , der über ein Antennenanschlussstück1410 mit einer Quelle für Hochfrequenzsignale verbunden ist. Wird die Anordnung1400 als Mikrowellenanordnung verwendet, wie dies in den17 und18 dargestellt ist, so liegen die angelegten Signale im Mikrowellenbereich (z. B. von 915 MHz bis 2,45 GHz). Wird die Stilettanordnung1400 als HF(Hochfrequenz)-Stromgenerator für eine separate Körperkontaktelektrode (nicht dargestellt) verwendet, so liegen die HF-Signale im Bereich von 10 bis 20 MHz. -
17 zeigt eine vergrößerte Querschnittsdarstellung B der in14a veranschaulichten Stilettanordnung. Das hohle Lumen1404 wird so dargestellt, dass es von einer dielektrischen Isolierungshülle1402 umgeben ist. Der Antennendraht1408 , der in das hohle Lumen1404 gewunden ist, ist im Querschnitt in verschiedenen Positionen dargestellt. -
18 ist eine vergrößerte Querschnittsdarstellung der Ausführungsform eines Biopsiestiletts gemäß dieser Erfindung, wie es in Verbindung mit16 dargestellt ist. Dieses Stilett umfasst eine biegsame Röhre1404 , in deren Oberfläche ein Elektrodendraht1408 eingebettet ist. Der Elektrodendraht ist von einer zurückziehbaren Isolierungshülle1402 umgeben. Am Ende der Röhre1404 erstreckt sich ein Biopsiemesser1412 mit einer Schneidspitze1412 durch einen biegsamen Zweischalengreifer oder ein Pufferschutzschild1414 . Während des Vorschiebens des Biopsiestiletts wird das Schild1414 geschlossen, und das Biopsiemesser1412 befindet sich hinter dem Schild in einer zurückgezogenen Position. Wird das Biopsiemesser vorgeschoben, so gleitet die scharfe Schneidspitze1418 durch das Schild und durchdringt das Gewebe mit nur minimalem Trauma. Die Schneidspitze1418 besitzt eine Seitenöffnung1420 , durch die eine Gewebeprobe oder eine Flüssigkeit durch ein konzentrisches Ansaugrohr1422 angesaugt werden kann. Das Zurückziehen des Biopsiemessers1412 ermöglicht das Schließen des Schilds1414 in seine Ausgangsposition, wodurch das Gewebe vor der scharfen Kante1418 abgeschirmt ist. - In Verwendung wird der Katheter in die Harnröhre in einer wie oben erklärten Weise eingesetzt. Die Hülle
1402 wird daraufhin selektiv vom Arzt im Zielgewebe ausgesetzt. Danach wird das Lumen1404 mit dem Antennendraht1408 aus dem Lumen hinaus in das Zielgewebe erstreckt. Zu diesem Zeitpunkt bieten sich dem Chirurgen verschiedene Möglichkeiten. Er/sie kann daraufhin selektiv das Biopsiemesser1412 in das Zielgewebe aussetzen, um Gewebeproben für eine spätere Untersuchung zu sammeln. Er/sie kann aber auch das Biopsiemesser1412 außer Acht lassen und, wie dies zuvor ausgeführt wurde, die ausgestreckte Antennensonde1414 mit Energie versorgen, um das umgebende Gewebe zu erhitzen und somit abzutragen. Der Arzt kann aber auch beide Aspekte dieser Erfindung in seiner beliebigen Reihenfolge zur Anwendung bringen. D. h. das Biopsiemesser1412 kann angewandt werden, um aus dem Zielgewebe eine Probe zu entnehmen. Das Biopsiemesser kann durch das hohle Lumen1404 zurück- und aus dem Körper des Patienten für eine externe histologische Untersuchung herausgezogen werden. Enthält das Gewebe krebsartiges Gewebe oder BPH-Gewebe, so kann der Arzt sich danach dazu entscheiden, den Mikrowellen-Aspekt dieser Ausführungsform anzuwenden und das ausgewählte Zielgewebe zu ablatieren. Wählt er diese Ausführungsform, so bleiben das hohle Lumen und der Katheter im Körper, während die Biopsiemesser-Anordnung1412 vollständig für eine externe Untersuchung herausgezogen wird. Schließlich besteht die dritte Option für den Chirurgen darin, die Mikrowellenantenne1414 zu betätigen, um das Zielgewebe abzutragen, und danach das Biopsiemesser1412 auszusetzen, um eine Gewebeprobe für eine externe Untersuchung und Überprüfung zu entnehmen. Diese Ausführungsform bietet dem Arzt und Patienten insbesondere Vorteile, da das Detektieren, Untersuchen und Ablatieren des ausgewählten Gewebes mit nur einem Einsetzen des Katheter und der Stilettanordnung erfolgen kann. Während die Erfindung mit Bezug auf die spezifischen bevorzugten Ausführungsformen beschrieben wurde, ist es für Fachleute auf dem Gebiet der Technik klar verständlich, dass verschiedene Änderungen gemacht werden können und äquivalente Elemente die Elemente dieser Ausführungsform ersetzen können, ohne dabei den Schutzumfang der Erfindung zu verlassen. Zusätzlich dazu können viele Modifikationen durchgeführt werden, ohne dabei von den wesentlichen Punkten der Erfindung abzuweichen.
Claims (11)
- Medizinische Sonde umfassend einen Katheter (
2 ) mit einer Stilettöffnung (120 ) in Richtung des distalen Endes desselben, ein Stilett (8 ), das innerhalb des Katheters angeordnet ist und eine scharfe distale Spitze zum Durchstechen eines Gewebes aufweist, sowie Mittel (12 ,13 ) zum Vorbewegen des Stiletts durch das Zielgewebe, Mittel (1108 /1114 ,1202 /1214 ,1302 ,1418 /1420 ), die vom Stilett getragen werden, um eine Probe des Zielgewebes abzutrennen und zu sammeln, und ein Energieversorgungsmittel, um das Stilett elektromagnetisch zu versorgen, worin das Stilett biegsam ist und das Vorbewegungsmittel dafür gedacht ist, das Stilett durch die Stilettöffnung und durch das dazwischenliegende Gewebe zum Zielgewebe vorzubewegen, wobei eine Isolierungshülle (438 ,440 ,1104 ,1208 ,1320 ,1404 ) koaxial um das Stilett angeordnet ist und in Längsrichtung entlang des Katheters und des Stiletts bewegt werden kann, um einen ausgewählten Abschnitt des Stiletts dem Zielgewebe auszusetzen, wobei das Energieversorgungsmittel (16 ,306 ) dafür gedacht ist, das Stilett mit Energie zu versorgen, um ablative Leistung in das Zielgewebe einzubringen. - Medizinische Sonde nach Anspruch 1, worin das Mittel zum Abtrennen und Sammeln einer Probe des Zielgewebes eine Biopsienadel (
1102 ,1204 ,1420 ) umfasst. - Medizinische Sonde nach Anspruch 1, worin das Mittel zum Abtrennen und Sammeln einer Probe des Zielgewebes eine Kernprobenvorrichtung (
1108 /1114 ,1202 /1214 ,1418 /1420 ) umfasst. - Medizinische Sonde nach Anspruch 1, worin das Mittel zum Abtrennen und Sammeln einer Probe des Zielgewebes eine hohle Röhre mit einem scharfen distalen Ende mit Widerhaken (
1206 ,1412 ) zum Schneiden und Einsammeln der Probe umfasst. - Medizinische Sonde nach Anspruch 1, worin das Mittel zum Abtrennen und Sammeln einer Probe des Zielgewebes einen Zweischalengreifer oder eine Alligatorklemme (
1302 ) umfasst. - Medizinische Sonde nach Anspruch 1, worin das Mittel zum Abtrennen und Sammeln einer Probe des Zielgewebes eine hohle Röhre mit einem zugespitzten Ende (
1108 ,1216 ,1418 ) umfasst, um eine Penetration des Gewebes und die Probenentnahme zu erleichtern. - Medizinische Sonde nach Anspruch 1, worin das Mittel zum Abtrennen und Sammeln einer Probe des Zielgewebes eine Schneidkante (
1214 ) einschließt, die vom distalen Ende des Stiletts weg ausgerichtet ist, und die Hülle ein zugespitztes distales Ende (1210 ) aufweist, das mit der Schneidkante beim Schneiden des Gewebes, wenn das Stilett relativ zur Hülle zurückgezogen wird, zusammenwirkt. - Medizinische Sonde nach Anspruch 1, worin das Stilett eine hohle Röhre mit einer Öffnung (
1420 ) entlang der lateralen Seite davon in Richtung des distalen Endes der Röhre sowie Mittel (1422 ) zum Ausüben eines negativen Drucks auf die hohle Röhre umfasst, um eine Gewebeprobe durch die Öffnung in die Röhre zu ziehen. - Medizinische Sonde nach einem der Ansprüche 1–4, 6 und 8 weiters umfassend einen Antennendraht (
1408 ), der vom proximalen zum distalen Ende der Hülle hin um die Isolierungshülle (1404 ) gewickelt ist, und ein elektromagnetisches Antennenmittel (1414 ), das an den Antennendraht gekoppelt ist und von der Isolierungshülle ausgesetzt werden kann. - Medizinische Sonde nach einem der vorhergehenden Ansprüche, worin die ablative Leistung in Form von Mikrowellenenergie an das Zielgewebe angelegt wird.
- Medizinische Sonde nach einem der vorhergehenden Ansprüche weiters umfassend eine zweite Stilettöffnung (
120 ) in Richtung des distalen Endes des Katheters, ein zweites biegsames Stilett (8 ), das innerhalb des Katheters angeordnet ist und eine scharfe distale Spitze zum Durchstechen von Gewebe aufweist, Mittel (12 ,13 ) zum Vorbewegen des zweiten Stiletts durch die zweiten Stilettöffnung und durch das dazwischenliegende Gewebe zum Zielgewebe, Mittel (1108 /1114 ,1202 /1214 ,1302 ,1418 /1420 ) zum Abtrennen und Sammeln einer Probe des Zielgewebes, die vom zweiten Stilett getragen werden, eine Isolierungshülle (438 ,440 ,1104 ,1208 ,1320 ,1404 ), die koaxial um das zweite Stilett angeordnet ist und in Längsrichtung des Katheters und des zweiten Stiletts bewegt werden kann, um einen ausgewählten Abschnitt des zweiten Stiletts dem Zielgewebe auszusetzen, sowie Mittel (16 ,306 ) zur Energieversorgung des zweiten Stiletts, um ablative Leistung in der Form von elektromagnetischer Energie in das Zielgewebe einzubringen.
