Description
- Diese Erfindung betrifft die transdermale Arzneiabgabe. Insbesondere betrifft sie Pflaster zur transdermalen Verabreichung von 17-Deacetyl-Norgestimat in Kombination mit einem Östrogen, insbesondere Ethinylöstradiol.
- Kombinationen aus Norgestimat (Ngm) und Ethinylöstradiol (EE) werden oral an Frauen als ein Kontrazeptivum verabreicht. Bringer J., Am. J. Obstet. Gynecol. (1992) 166: 1969–77. McGuire, J. C. et al., Am. J. Obstet. Gynecol. (1990) 163: 2127–2131, schlägt vor, daß oral verabreichtes Ngm zu 17-Deacetyl-Norgestimat (17-d-Ngm), 3-Ketonorgestimat und Levonorgestrel (Lng) metabolisiert, und daß diese Metaboliten zu der pharmakologischen Antwort auf die oral verabreichte Arznei beitragen können.
- Chien et al.,
US Pat. Nr. 4,906,169 beschreibt transdermale Pflaster zum Zusammen-Verabreichen von Östrogenen und Progestinen an Frauen zur Kontrazeption. EE wird als eines der Östrogene erwähnt, die aus dem Pflaster verabreicht werden können, und Ngm und Lng werden als mögliche Progestine erwähnt, die verabreicht werden können. - Die Anmelder sind sich keines Stands der Technik bewußt, der die Verabreichung von 17-d-Ngm alleine oder in Kombination mit einem Östrogen über die transdermale oder eine andere Verabreichungsroute beschreibt.
- Diese Erfindung stellt ein transdermales Pflaster zur Verhinderung der Ovulation bei einer Frau bereit, wie definiert in Anspruch 1, das dazu angepaßt ist, eine ovulationsinhibierende Menge von 17-Deacetyl-Norgestimat zu verabreichen. Eine effektive Menge kann von 150 bis 350 μg/Tag und bevorzugt von 175 bis 300 μg/Tag an 17-Deacetyl-Norgestimat betragen. Unter einem bevorzugten Gesichtspunkt ist das Pflaster dazu angepaßt, eine ovulationinhibierende Menge eines Östrogens, wie etwa Ethinylöstradiol, zusammen zu verabreichen. Eine wirksame Menge beträgt von 150 bis 350 μg/Tag und bevorzugt von 175 bis 300 μg/Tag an 17-Deacetyl-Norgestimat und von 10 bis 35 μg/Tag an Ethinylöstradiol.
- Die transdermalen Pflaster der Erfindung ermöglichen Empfängnisverhütung für Frauen. Sie sind ebenso für eine Hormonersatztherapie angepaßt. Diese Erfindung stellt deshalb ebenso ein transdermales Pflaster zum Ermöglichen einer Hormonersatztherapie bei einer Frau bereit, wie in Anspruch 5 definiert.
- Die Pflaster sollen 17-d-Ngm und ein Östrogen an die Haut kontinuierlich für eine ausgedehnte Zeitperiode abgeben, typischerweise 1–7 Tage und bevorzugt für 7 Tage.
- Wenn die Pflaster zur Empfängnisverhütung getragen werden, wird typischerweise ein Pflaster auf die Haut am 5. Tage des Menstruationszyklus aufgebracht und nach Bedarf ersetzt, bis 21 Tage des Tragens verstrichen sind. Zum Beispiel im Falle eines 7-Tage-Pflasters werden 3 Pflaster benötigt, um die Arznei(en) für die 21-Tages-Periode abzugeben. Wenn erwünscht, kann ein Placebo-Pflaster danach bis zum 5. Tage des darauffolgenden Menstruationszyklus getragen werden. Dieses Schema wird für jeden Menstruationszyklus wiederholt.
- 17-d-Ngm und Östrogene inhibieren beide die Ovulation, obgleich auf getrennten Wegen. 17-d-Ngm inhibiert die Freisetzung von Luteinisierungshormon (LH), während der überwiegende Effekt von Östrogen es ist, die Sekretion von Follikel-stimulierendem Hormon (FSH) zu inhibieren. Daher wird, wenn eine Kombination aus 17-d-Ngm und Östrogen unter Verwendung eines Pflasters gemäß der Erfindung verabreicht wird, die Ovulation verhindert durch Inhibieren des ovulatorischen Stimulus und/oder durch Inhibieren des Wachstums von Follikeln. Es wird geglaubt, daß die Verabreichung von 17-d-Ngm vorteilhaft ist gegenüber der Elternverbindung (Ngm) oder ihrer anderen Metaboliten, dahingehend, daß 17-d-Ngm wenig oder keine androgene Aktivität inhibiert.
- Die effektive Dosis von 17-d-Ngm zum Inhibieren der Ovulation ist normalerweise im Bereich von 150 bis 350 μg/Tag, bevorzugt von 175 bis 300 μg/Tag und bevorzugter von 175 bis 250 μg/Tag. Die Pflaster der Erfindung werden typischerweise eine basale Oberfläche (d. h. die Fläche in Diffusionskontakt mit der Haut) zwischen 10 und 50 cm2 haben. Die effektive Dosis an Östrogen zum Inhibieren der Ovulation wird von dem jeweiligen Östrogen, das zusammen verabreicht wird, abhängen. Zum Beispiel wird, wenn das Östrogen Ethinylöstradiol ist, die Dosis normalerweise wenigstens 10 μg/Tag, bevorzugt von 10 bis 35 μg/Tag und am bevorzugtesten näherungsweise 20 μg/Tag betragen. Die Pflaster werden ausreichende Mengen an 17-d-Ngm und Östrogen enthalten, um solche täglichen Dosierungen für die beabsichtigte Pflaster-Tragezeit zu ermöglichen. Typischerweise betragen solche Dosierungen von 20 μg/Tag bis 200 μg/Tag, und bevorzugt von 30 μg/Tag bis 150 μg/Tag an Ethinylöstradiol.
- Die Pflaster dieser Erfindung sind matrix- oder monolith-artige laminierte Strukturen. Solche transdermalen Pflaster sind auf dem Gebiet gut bekannt. Sie umfassen eine Matrixschicht der Arznei(en), vermischt mit einem druckempfindlichen Klebstoff und einer Rückschicht. Die Matrix dient sowohl als das Arznei-Reservoir als auch als das Mittel, mit dem das Pflaster an der Haut befestigt wird. Vor der Verwendung wird das Pflaster ebenso eine undurchlässige Abziehfolienschicht umfassen.
- Die Rückschicht ist gegenüber der Arznei und anderen Bestandteilen der Matrix undurchlässig und definiert die obere Oberfläche des Pflasters. Sie kann aus einer einzelnen Schicht oder Film aus Polymer gemacht sein oder ein Laminat aus einem oder mehreren Polymerschichten und Metallfolie sein. Beispiele für Polymere, die zur Verwendung beim Herstellen der Rückfilme nützlich sind, sind Polyvinylchlorid, Polyvinylidenchlorid, Polyolefine, wie etwa Ethylen-Vinylacetat-Copolymere, Polyethylen und Polypropylen, Polyurethan und Polyester, wie etwa Polyethylenterephthalat.
- Der druckempfindliche Klebstoff der Matrix wird ein Silikon oder Polyisobutylen (PIB) sein. Solche Klebstoffe sind auf dem transdermalen Gebiet wohl bekannt. Siehe zum Beispiel das Handbook of Pressure Sensitive Adhesive Technology, 2. Ausgabe (1989) Van Nostrand, Reinhold.
- Polyisobutylen-Klebstoffe sind Mischungen aus hochmolekulargewichtigem (HMW) PIB und niedermolekulargewichtigem (LMW) PIB. Solche Mischungen sind auf dem Gebiet beschrieben, z. B.
PCT/US91/02516 - Die Silikon-Klebstoffe, die beim Bilden der Matrix verwendet werden können, sind typischerweise hochmolekulargewichtige Polydimethylsiloxane oder Polydimethyldiphenylsiloxane. Formulierungen von Silikon-Klebstoffen, die bei transdermalen Pflastern nützlich sind, werden in
US Patenten Nr. 5,232,702 ,4,906,169 und4,951,622 beschrieben. - Östrogene, die mit 17-d-Ngm in der Matrix kombiniert werden können, schließen 17-β-Östradiol und Ester davon, wie Östradiolvalerat, Östradiolcypionat, Östradiolacetat, Östradiolbenzoat und EE ein. EE ist ein bevorzugtes Östrogen zur Verwendung in Kombination mit 17-d-Ngm. Die EE/17-d-Ngm-Kombinationen können Stoffwechselparameter vorteilhafterweise beeinflussen, wie etwa eine Erhöhung von Lipoproteinen mit hoher Dichte im Serum sowie eine Erniedrigung des Verhältnisses von Lipoprotein mit niedriger Dichte/Liproprotein mit hoher Dichte im Serum.
- Zusätzlich zu dem druckempfindlichen Klebstoff, 17-d-Ngm und Östrogen wird die Matrix typischerweise hinreichende Mengen an Permeationsverstärkern enthalten, um die Permeabilität des 17-d-Ngm und Östrogen durch die Haut zu erhöhen und Flüsse in den oben beschriebenen Bereichen zu ermöglichen. Beispiele für Haut-Permeationsverstärker, die in der Matrix eingeschlossen sein können, werden in
US-Patenten Nr. 5,059,426 ,4,973,468 ,4,906,463 und4,906,169 beschrieben und schließen ein, sind aber nicht beschränkt auf Lactatester von C12- bis C18-aliphatischem Alkohol, Lauryllactat, Ölsäure oder PGML. Die Menge an Permeationsverstärker, die in der Matrix eingeschlossen ist, wird von dem (den) jeweiligem (jeweiligen) verwendetem (verwendeten) Verstärker(n) abhängen. In den meisten Fällen wird der Verstärker einen Bereich von 1 bis 20 Gew.% der Matrix ausmachen. - Die Matrix kann andere Zusatzstoffe, abhängig von dem jeweiligen verwendeten Klebstoff, enthalten. Zum Beispiel können Materialien, wie etwa Polyvinylpyrrolidon (PVP), die die Arzneikristallisierung inhibieren, hygroskopische Mittel, die die Dauer der Abnutzung verbessern, oder Zusatzstoffe, die die physikalischen Eigenschaften (z. B. kalter Fluß) oder Klebstoff Eigenschaften (z. B. Klebrigkeit, kohäsive Festigkeit) der Matrix verbessern, eingeschlossen werden.
- Die oben beschriebenen Pflaster sind ebenso zum Ermöglichen einer Hormonersatztherapie nützlich. Bei Verwendung zum Ermöglichen einer Hormonersatztherapie wird die Matrix so konstruiert, daß sie eine wirksame Menge an 17-d-Ngm und Östrogen für den beabsichtigten Zweck bereitstellt. Typischerweise ist die Matrix und deshalb das Pflaster so konstruiert, daß es von 150 bis 350 μg/Tag und bevorzugt von 175 bis 300 μg/Tag an 17-d-Ngm bereitstellt, zusammen verabreicht mit von 5 bis 45 μg/Tag und bevorzugt von 10 bis 35 μg/Tag an Ethinylöstradiol. In einer alternativen Ausführungsform wird das Pflaster von 200 bis 350 μg/Tag und bevorzugt von 175 bis 300 μg/Tag 17-d-Ngm verabreichen, zusammen verabreicht mit von ungefähr 20 bis ungefähr 175 μg/Tag und bevorzugt von 30 bis 150 μg/Tag an 17-β-Östradiol. Das Pflaster wird für 7 Tage angebracht und durch ein neues Pflaster (für 7 Tage) für die Dauer der Therapie ersetzt.
- Die Pflaster der Erfindung können unter Verwendung von auf dem Gebiet der transdermalen Pflaster bekannten Prozeduren hergestellt werden. Die Prozedur wird im allgemeinen das Formulieren der Matrix (d. h. Mischen des Klebstoffs, der Arzneien, des Permeationsverstärkers und der Zusatzstoffe, sofern vorhanden), das Gießen der Matrix auf die Rückschicht oder Abziehfolienschicht, Entfernen des Lösungsmittels von der Matrix und Aufbringen der Rück/Abziehfolienschicht umfassen, je nachdem wie der Fall liegt. Wie für Fachleute klar ist, kann die Matrixzusammensetzung, die eine wirksame Menge der Arznei in sich dispergiert hat, in verschiedene transdermale Konstruktionen eingebaut werden, und deshalb sind die Anmelder nicht auf die unten beispielhaft dargestellten Ausführungsformen beschränkt.
- Die folgenden Beispiele veranschaulichen die Erfindung näher. Diese Beispiele sollen nicht die Erfindung auf irgendeine Weise beschränken. Sofern nicht anderweitig angezeigt, sind die angegebenen prozentualen Angaben Gewichtsprozentangaben.
- BEISPIELE
- Beispiel 1
- Silikon 4202 ist ein Polydimethylsiloxan-Klebstoff von Dow Corning. Es wurde mit 17-d-Ngm, EE, 7% PVP (K30 von BASF; aufgelöst in n-Propanol) und verschiedenen Verstärkern gemischt. Diese Mischungen wurden als eine 100 Micron dicke (nasse) Schicht auf eine 3M 1022-Polyester-Rückschicht gegossen und getrocknet. Hautflußtests wurden an den resultierenden Konstruktionen gemäß der in Spalte 7 von
US 5,252,334 beschriebenen Prozedur durchgeführt. HPLC wurde verwendet, um auf 17-d-Ngm und EE zu testen. Ein Perkin-Elmer-HPLC-System mit einem Diodenarray-Detektor, eingestellt auf 245 nm bzw. 215 nm für 17-d-Ngm bzw. EE. Die mobile Phase war 55% Wasser, 45% Acetonitril bei einer Flußrate von 1,0 ml/min. Die Retentionszeit war 4,5 und 3,0 min für 17-d-Ngm bzw. EE. Die Details der Formulierungen und die Resultate der Fluß-Studien werden in Tabelle 1 unten dargestellt. - Es wurden Vergleichsstudien durchgeführt an Silikon-Klebstoff-17-d-Ngm/EE-Pflastern unter Verwendung von zwei Arten von PVP: Einem löslichen niedermolekulargewichtigen PVP, bezeichnet als PVP-K30 von BASF und einem unlöslichen, quervernetzten mikronisierten PVP, bezeichnet als PVP-CLM von BASF.
- PVP-K30 wird in absolutem Ethanol aufgelöst. Es wurden Mischungen von 17-d-Ngm, EE und PVP-K30 hergestellt und Silikon 4202 und Methyllaurat wurden danach zugegeben. Die Mischung wurde über Nacht gemischt. Die Mischung wurde auf eine 3M-1022-Folie in einer Dicke von 0,38 mm (15 mil) (naß) gegossen und bei 70°C für 40 min getrocknet.
- PVP-CLM ist als mikronisierte Feststoffe erhältlich. Silikon-4202 und PVP-CLM wurden zusammengemischt, und dann wurden Methyllaurat, EE, 17-d-Ngm und Ethanol zugegeben. Die Mischung wurde über Nacht gemischt und auf eine 3M-1022-Folie gegossen und wie oben getrocknet.
- Hautflußstudien wurden an den oben beschriebenen Konstruktionen wie in Beispiel 1 durchgeführt. Die Details dieser Formulierungen und die Resultate der Hautflußtests werden in Tabelle 2 unten dargestellt. Tabelle 2

- PIB-Lösungen wurden durch Auflösen von VISTANEX L100, Vistanex LM-MS-LC und Polybuten (Indopol H1900) in Hexan hergestellt. Suspensionen von PVP-CLM, 17-d-Ngm, EE und verschiedenen Verstärkern in Ethanol/Ethylacetat wurden hergestellt. Die PIB-Lösung wurde zu den Arzneisuspensionen zugegeben, und die resultierenden Mischungen wurden vollständig vermischt. Die Mischungen wurden als eine 0,25 mm (10 mil) dicke (nasse) Schicht auf Abziehfolien gegossen und bei 70°C für 40 min getrocknet. Eine Sarimex 2015-Rückschicht wurde an die Konstruktion laminiert. Hautflußstudien wurden an diesen Konstruktionen wie in Beispiel 1 durchgeführt. Die Details dieser Konstruktionen und die Resultate der Hautflußstudien werden in Tabelle 3 unten dargestellt.
- Beispiel 4
- Eine für die Hormonersatztherapie geeignete Zusammensetzung und Matrix wird wie folgt hergestellt. 2% 17β-Östradiol, 2% 17-Deacetylnorgestimat, 20% PVP-CLM und 76% PIB-Klebstoff (1:5:2,5:1,5 Vistanex L100:Vistanex LM-MS-LC: Polybuten:Zonester 85FB) wird in einer Kombination aus Hexan, Ethylacetat und Ethanol aufgelöst. Sie wurde auf eine Polyester-Abziehfolie gegossen und bei 70°C für 45 Minuten getrocknet. Eine Polyester-Rückschicht wurde vor der Flußstudie angebracht. Die Flußstudie wurde, wie in Beispiel 1 beschrieben, durchgeführt. Tabelle 4

- Transdermale Pflaster mit einer Matrix, zusammengesetzt aus PIB-Klebstoff, PVP = CLM, Lauryl- oder Myristyllactat, 17-d-Ngm und EE, wurden wie folgt herstellt:
Das 17-d-Ngm und EE wurden in Ethylacetat aufgelöst, und das PVP-CL und Lauryllactat oder Myristyllactat (erhalten von ISP VanDYK aus Bellville, New Jersey) wurden zu dieser Lösung zugegeben. Eine Lösung des PIB-Klebstoffs (1:5:4 Vistanex L100:Vistanex LM-MS-LC:Indophol H1900) in Hexan wurde zu den Steroidlösungen unter kräftigem Mischen zugegeben. N-Propanol, in 10% des PVP-CLM-Gewichts, wurde langsam zu den endgültigen Mischungen zugegeben. Die Mischungen wurden auf eine Abziehfolie gegossen und in einem Ofen bei 70°C für 40 Minuten getrocknet. Die getrocknete Matrix wog 7,5 mg/cm2. Die Matrix-Abziehfolien-Subkonstruktion wurde an eine Polyester(Scotchpak 1012)-Rückschicht laminiert. - Eine andere Subkonstruktion wurde an eine Polyester-Vliesschicht (Remay 2250) laminiert. Die Abziehfolie wurde von der Rückschichtkonstruktion entfernt, und sie wurde auf die Vliesschicht-Konstruktion aufgetragen, um einen 5-schichtigen Verbundstoff zu ergeben aus: Rückschicht/Klebstoff-Matrix/Vlies/Klebstoff-Matrix/Abziehfolie.
- Hautflußstudien wurden an diesen Pflastern, wie in Beispiel 1 beschrieben, durchgeführt. Einzelheiten der Pflaster und die Resultate der Flußstudien werden in Tabelle 5 unten dargestellt. Tabelle 5

Claims (13)
- Transdermales Pflaster zur Verhinderung der Ovulation bei einer Frau, umfassend: (a) eine Rückschicht; und (b) eine Matrixschicht, die unter der Rückschicht liegt, wobei die Matrixschicht eine Mischung aus 17-Deacetyl-Norgestimat, einem Östrogen, einem Permeationsverstärker und einem druckempfindlichen Klebstoff umfasst, und die dazu angepaßt ist, daß sie in Diffusionsverbindung mit der Haut der Frau steht und daß sie der Frau eine ovulationsinhibierende Menge an 17-Deacetyl-Norgestimat und Östrogen verabreicht, wobei der druckempfindliche Klebstoff ein Silikon-Klebstoff oder ein Polyisobutylen-Klebstoff ist.
- Pflaster nach Anspruch 1, wobei die ovulationsinhibierende Menge an 17-Deacetyl-Norgestimat 150–350 μg/Tag ist.
- Pflaster nach Anspruch 2, wobei die ovulationsinhibierende Menge an 17-Deacetyl-Norgestimat 175–300 μg/Tag ist.
- Pflaster nach einem der Ansprüche 1–3, wobei die Mischung zusätzlich Polyvinylpyrrolidon umfaßt.
- Transdermales Pflaster zum Ermöglichen einer Hormonersatztherapie bei einer Frau, umfassend: (a) eine Rückschicht; und (b) eine Matrixschicht, die unter der Rückschicht liegt, wobei die Matrixschicht eine Mischung aus 17-Deacetyl-Norgestimat, einem Östrogen, einem Permeationsverstärker und einem druckempfindlichen Klebstoff umfaßt, und die dazu angepaßt ist, daß sie in Diffusionsverbindung mit der Haut der Frau steht und daß sie der Frau eine therapeutische Menge an 17-Deacetyl-Norgestimat und Östrogen zusammen verabreicht, wobei der druckempfindliche Klebstoff ein Silikon-Klebstoff oder ein Polyisobutylen-Klebstoff ist.
- Pflaster nach Anspruch 1 oder Anspruch 5, wobei das Östrogen ein Östradiol ist.
- Pflaster nach Anspruch 6, wobei das Östradiol Ethinylöstradiol ist.
- Pflaster nach Anspruch 7, wobei die Menge 175 bis 300 μg/Tag 17-Deacetyl-Norgestimat und 10 bis 35 μg/Tag Ethinylöstradiol ist.
- Pflaster nach Anspruch 6, wobei das Östradiol 17-β-Östradiol ist.
- Pflaster nach Anspruch 9, wobei die Menge 175 bis 300 μg/Tag 17-Deacetyl-Norgestimat und 30 bis 150 μg/Tag 17-β-Östradiol ist.
- Pflaster nach einem der vorangehenden Ansprüche, wobei der druckempfindliche Klebstoff ein Polyisobutylen ist.
- Pflaster nach einem der Ansprüche 1–11, wobei der Haut-Permeationsverstärker ein Milchesäureester eines C12 bis C18-aliphatischen Alkohols, Ölsäure oder PGML ist.
- Pflaster nach Anspruch 12, wobei der Haut-Permeationsverstärker Lauryllactat ist.
