- Die Erfindung betrifft Materialien, die sich zur Knochenreparatur eignen.
- Es gibt eine Anzahl von Materialien, die bezüglich der Einleitung einer Knochenreparatur und/oder bezüglich der Wiederherstellung oder des Ersatzes von fehlendem Knochen untersucht worden sind, und zwar mit dem Ziel, die Schwierigkeiten bei der Stimulation der Knochenbildung an bestimmten Stellen zu bewältigen.
- Unter den Wegen, die zur Bewältigung dieser Schwierigkeiten eingeschlagen wurden, befindet sich ein Konformationsverfahren, bei dem ein Implantatmaterial, das üblicherweise aus einer Metallkeramik oder einem anderen anorganischen Material in einer Form, die die Form des fehlenden Knochens nachahmen soll, gefertigt worden ist, an die Stelle, an der der Knochenersatz benötigt wird, eingesetzt wird. Dabei besteht die Gefahr, dass der Wirt das Material abstößt oder dass die Integration des Implantats in das normale Skelettgewebe misslingt. Einige keramische Materialien, z. B. keramisches Tricalciumphosphat, sind zwar in bezug auf den Wirt und den Knochen bei Verwendung als Implantat in annehmbarer Weise biologisch verträglich, zeigen aber für einen allgemeinen Einsatz unzureichende mechanische Knocheneigenschaften, wobei der Knochen nicht in einwandfreier Weise in das Implantat einwächst und in dieses eingebaut wird.
- Ein weiterer Weg beinhaltet den Ersatz des fehlenden Knochengewebes durch eine Matrix, die als Träger dient, in die frischer Knochen einwachsen kann. Die Theorie besteht darin, dass die Matrix, die an einem osteogenen Stoffwechselweg beteiligten Zellen anzieht und der frische Knochen in die Matrix und durch diese hindurchwächst, und zwar aufgrund eines Vorgangs, der als Osteokonduktion bezeichnet wird. Allogene Knochentransplantate (nicht vom Wirt stammende Knochen) werden für dieses Verfahren herangezogen, jedoch ist dabei die Ausfallrate recht hoch. Selbst wenn allogene Knochentransplantate vom Wirt akzeptiert werden, ist die Heilungsdauer, die erforderlich ist, bis der Knochen konsolidiert und mechanische Belastungen aushält, im Vergleich zu autogenen Knochentransplantaten (vom Wirt stammende Knochen) recht lang. Bei der Verwendung von allogenen Knochen besteht auch das Problem von übertragbaren viralen Agentien.
- Ein drittes Verfahren beinhaltet den als Osteoinduktion bekannten Vorgang, der abläuft, wenn ein Material das Wachstum von frischem Knochen aus undifferenzierten Zellen oder Geweben des Wirts induziert, üblicherweise um eine temporäre Matrix herum. Für eine Anzahl von Verbindungen wurde eine derartige Fähigkeit dargelegt; vergl. z. B. die US-Patente 4 440 750 (Glowacki), 4 294 753 und 4 455 256 (Urist) sowie 4 434 094 und 4 627 982 (Seyedin et al.). Darunter scheint es sich bei den wirksamsten Verbindungen um Proteine zu handeln, die die Osteogenese stimulieren. Wenn diese Verbindungen jedoch aus natürlichen Quellen synthetisiert werden, liegen sie in äußerst geringen Konzentrationen vor, so dass große Mengen an Ausgangsmaterial erforderlich sind, um auch nur eine winzige Materialmenge für experimentelle Zwecke zu erhalten. Die Anwendung derartiger Proteine an sich kann aufgrund ihrer Verfügbarkeit durch rekombinante Verfahren für die Praxis noch interessanter werden. Jedoch besteht bei derartigen Proteinen vermutlich immer noch die Notwendigkeit, dass sie in einer geeigneten Matrix an den gewünschten Ort gebracht werden.
- Es wurden Zusammensetzungen mit einem Gehalt an Kollagen und verschiedenen Formen an Calciumphosphat beschrieben, die auf die Heilung und das Wachstum von Knochen abzielen.
- Das Patent 5 338 772 (Bauer et al.) beschreibt ein Verbundmaterial mit einem Gehalt an keramischen Calciumphosphatteilchen und einem biologisch resorbierbaren Polymeren, wobei das keramische Calciumphosphatmaterial mindestens 50 Gew.-% ausmacht und die Teilchen durch Polymerbrücken miteinander verbunden sind. Die beschriebenen keramischen Calciumphosphatteilchen weisen eine Größe von etwa 20 μm bis etwa 5 mm auf.
- Das Patent 4 795 467 (Piez et al.) beschreibt eine Zusammensetzung mit einem Gehalt an mineralischen Calciumphosphatteilchen im Gemisch mit rekonstituiertem, fibrillärem Atelopeptid-Kollagen. Die beschriebenen mineralischen Calciumphosphatteilchen weisen eine Größe im Bereich von 100–2000 μm auf.
- Das Patent 4 780 450 (Sauk et al.) beschreibt eine Zusammensetzung für die Knochenreparatur, die teilchenförmiges, polykristallines Calciumphosphat-Keramikmaterial, ein Phosphophoryn-calciumsalz und ein Typ I-Kollagen in einem Gewichtsverhältnis von 775–15 : 3–0,1 : 1 umfasst.
- Für die keramischen Teilchen wird angegeben, dass es sich um dichtes Hydroxylapatit mit einem Durchmesser von etwa 1–10 μm oder um größere, dichte keramische Hydroxylapatitteilchen mit einem Durchmesser von mehr als etwa 100 μm handelt.
- Die PCT-Anmeldung WO-94/15653 (Ammann et al.) beschreibt Zubereitungen mit einem Gehalt an Tricalciumphosphat (TCP), TGF-β und gegebenenfalls Kollagen. Für das TCP wird angegeben, dass es sich um einen Abgabeträger für das TGF-β handelt, wobei die Teilchengröße des TCP mehr als 5 μm und vorzugsweise mehr als etwa 75 μm beträgt. Der besonders bevorzugte Bereich für die Größe der TCP-Granalien wird mit 125–250 μm angegeben.
- Die PCT-Anmeldung WO-95/08304 beschreibt polymineralische Vorläuferteilchen von Hydroxylapatit im Gemisch mit unlöslichem Kollagen. Die Teilchengröße der polymineralischen Vorläuferteilchen liegt im Bereich von 0,5 bis 5 μm. Die Vorläufermineralien werden durch Hydrolyse in Hydroxylapatit umgewandelt. Man nimmt an, dass bei diesem Vorgang das Mineral unter Bildung von monolithischem Hydroxylapatit fusioniert.
- GB-PS 1 271 763 (FMC Corporation) beschreibt Komplexe aus Calciumphosphat und Kollagen.
- Zusammenfassende Darstellung der Erfindung
- Gemäß Anspruch 1 wird eine Knochentransplantationsmatrix bereitgestellt, die porös ist und nach der Implantation ihre strukturelle Unversehrtheit und Porosität für eine Zeitspanne behält, die ausreicht, den Vorgang des Knochenersatzes zu verstärken. Die Matrix umfasst mineralisiertes, fibrilläres, unlösliches Kollagen, ein Kollagenderivat oder modifizierte Gelatine, die mit einem Bindemittel gebunden sind. Die Mineralien umfassen teilchenförmiges Calciumphosphat, das innerhalb der Matrix immobilisiert ist und eine Teilchengröße von weniger als etwa 5 μm aufweist. Das erhaltene Produkt wird lyophilisiert, vernetzt, getrocknet und sterilisiert, wodurch eine poröse Matrix entsteht. Die Matrix kann als Transplantationsmaterial und/oder als Abgabeträger für einen osteogenen Wachstumsfaktor verwendet werden. Die Matrix kann mit autogenem Knochenmark vermischt und zur Knochenregeneration implantiert werden.
- Beschreibung spezieller Ausführungsformen
- Die Knochentransplantationsmatrix wird unter Verwendung eines in Wasser unlöslichen, biologisch abbaubaren Kollagen-, Kollagenderivat- oder modifizierten Gelatineprodukts hergestellt. Die Gelatine ist so modifiziert, dass sie in wässrigen Umgebungen unlöslich ist. Das Kollagen kann aus mineralisierten oder unmineralisierten Kollagenquellen und üblicherweise aus unmineralisierten Kollagenquellen stammen. Somit kann das Kollagen aus Knochen, Sehnen, Haut oder dergl. stammen, wobei es sich vorzugsweise um Typ I-Kollagen handelt, das eine Kombination aus zwei Strängen von α2- und einer α1-Kollagenkette umfasst. Das Kollagen kann aus einer jungen Quelle, z. B. vom Kalb, oder einer reifen Quelle, z. B. von einem Rind mit einem Alter von 2 oder mehr Jahren, stammen. Bei der Kollagenquelle kann es sich um eine beliebige zweckmäßige tierische Quelle von Säugetieren oder Vögeln handeln. Hierzu gehören Kollagenquellen von Rindern, Schweinen, Pferden, Hühnern, Truthahngeflügel oder anderen Haustieren. Das unlösliche kollagenöse Gewebe, das verwendet wird, wird normalerweise in einem Medium mit einem erhöhten pH-Wert, üblicherweise mit einem pH-Wert von mindestens etwa 8 und insbesondere von etwa 11 bis 12 dispergiert. Üblicherweise wird Natriumhydroxid verwendet, obgleich auch andere Hydroxide verwendet werden können, z. B. andere Alkalimetallhydroxide oder Ammoniumhydroxid.
- Erfindungsgemäß kann natives Kollagen verwendet werden. Natives Kollagen enthält an jedem Ende Regionen, die nicht die Triplett-Glycinsequenz aufweisen. Man nimmt an, dass diese Regionen (die Telopeptide) für die immunogene Beschaffenheit, die bei den meisten Kollagenpräparaten gegeben ist, verantwortlich ist. Die immunogene Beschaffenheit kann durch Entfernung dieser Regionen unter Bildung eines Atelopeptid-Kollagens abgemildert werden, indem man einen Verdau mit proteolytischen Enzymen, wie Trypsin und Pepsin, durchführt.
- Die Konzentration von Kollagen zur Mineralisierung liegt im allgemeinen im Bereich von etwa 0,1 bis 10 Gew.-% und insbesondere im Bereich von etwa 1 bis 5 Gew.-%. Das Kollagenmedium liegt im allgemeinen in einer Basenkonzentration im Bereich von etwa 0,0001 bis 0,1 N vor. Der pH-Wert wird im allgemeinen im Verlauf der Umsetzung im Bereich von etwa 10 bis 13 und vorzugsweise bei etwa 12 gehalten.
- Vorzugsweise wird unlösliches, fibrilläres Kollagen verwendet, das durch routinemäßige Verfahren hergestellt werden kann. Typischerweise erfolgt die Herstellung, indem man das Ausgangsprodukt zunächst mit Isopropanol (IPA), Diethylether, Hexan, Ethylacetat oder einem anderen geeigneten Lösungsmittel vermischt und anschließend das Kollagen abtrennt. Der pH-Wert wird typischerweise auf etwa 3 verringert, wonach man auf etwa 4°C abkühlt und eine Quellung vornimmt. Die erhaltene Aufschlämmung kann homogenisiert werden, bis die angestrebte Viskosität erreicht ist.
- Die homogenisierte Aufschlämmung wird mit einem Lösungsmittel vermischt und gerührt. Der pH-Wert wird auf etwa 7 angehoben. Das fibrilläre Kollagen wird abgetrennt, mit entionisiertem Wasser gespült und lyophilisiert. Zur Herstellung von mineralisiertem, fibrillärem Kollagen können die gereinigten, unlöslichen Kollagenfibrillen homogenisiert werden und in einen Reaktor gebracht werden, in den Calciumchlorid (typischerweise 0,05 M) und tribasisches Natriumphosphat (typischerweise 0,03 M) unter Rühren mit einer kontrollierten Geschwindigkeit eingeführt werden. Je nach Bedarf wird während dieses Vorgangs Natriumhydroxid zur Einstellung des pH-Werts auf 11,0 ± 0,5 verwendet. Nach der Mineralisierung wird das Kollagen mit entionisiertem Wasser oder Phosphatpuffer gespült, mit dem Bindemittel vereinigt und einer Einstellung des pH-Werts auf einen Bereich von 7,5 ± 1,5 unterzogen. Ein Verfahren zur Zugabe von Phosphat- und Calciumionen wird im US-Patent 5 231 169 beschrieben.
- Das Calciumphosphat kann andere Ionen, wie Carbonat, Chlorid, Fluorid, Natrium oder Ammonium, enthalten. Die Anwesenheit von Carbonat führt zu einem Produkt mit den Eigenschaften von Dahllit (cabonisiertes Hydroxylapatit), während Fluorid zu einem Produkt mit den Eigenschaften von fluoridiertem Apatit führt. Der gewichtsprozentuale Anteil an Carbonat liegt üblicherweise nicht über 10, während der gewichtsprozentuale Anteil an Fluorid üblicherweise nicht über 2 liegt und vorzugsweise im Bereich von 0 bis 1 liegt. Diese Ionen können zusammen mit der Calcium- und/oder Phosphatquelle vorhanden sein, sofern die Ionen verträglich sind und in den Reagenzlösungen nicht zu einer Ausfällung führen. Die Zugabezeit der Calcium- und Phosphationen beträgt im allgemeinen etwa 1 Stunde und nicht mehr als etwa 72 Stunden, um eine Teilchengröße von etwa 5 μm oder weniger zu erzielen. Im allgemeinen liegt die Zugabezeit im Bereich von etwa 2 bis 18 Stunden und insbesondere im Bereich von etwa 4 bis 16 Stunden. Es werden milde Temperaturen herangezogen, üblicherweise nicht mehr als etwa 40°C und vorzugsweise im Bereich von etwa 15 bis 30°C. Das Gewichtsverhältnis von Kollagen zu mineralischem Calciumphosphat liegt im allgemeinen im Bereich von etwa 8 : 2 bis 1 : 1 und beträgt typischerweise etwa 7 : 3.
- Weitere nicht-kollagenöse Proteine oder Faktoren, wie BMP's, TGF-β, Calcitonin und dergl., können in die Matrix aufgenommen werden, indem man sie vor oder nach der Zugabe von Calcium und Phosphat zu der Kollagenaufschlämmung gibt. Die Mengen derartiger Additive liegen im allgemeinen im Bereich von etwa 0,0001 bis 2 Gew.-%, bezogen auf das als Matrix verwendete Biopolymere, z. B. das Kollagen. Das zugesetzte Protein kann sich mit dem Mineral vereinigen, da es sich auf dem Kollagen bildet, wobei das zugesetzte Protein an das Kollagen gebunden wird.
- Die Menge des im mineralisierten Produkt vorhandenen Kollagens beträgt im allgemeinen etwa 80 bis 30%.
- Alternativ können die immobilisierten Calciumphosphatteilchen der Matrix zugesetzt werden, indem man die Teilchen mit dem zur Bindung der Kollagenfibrillen verwendeten Bindemittel vermischt.
- Zur Bildung einer porösen, dreidimensionalen Knochen-Transplantationsmatrix werden die mineralisierten Kollagenfasern mit einem Bindemittel vermischt.
- Vorzugsweise wird gereinigtes, lösliches Kollagen als Bindemittel verwendet, indem man zunächst das lösliche Kollagen mit einem Lösungsmittel, wie Isopropanol (IPA), vermischt und das Kollagen isoliert. Der pH-Wert wird auf etwa 3,0 verringert. Sodann wird nach Lösung des Kollagens der pH-Wert auf 5,0 erhöht. Nach zweimaligem Waschen mit dem Lösungsmittel und Spülen mit entionisiertem Wasser wird ein Sieb- und Lyophilisiervorgang durchgeführt.
- Zu weiteren Bindemitteln, die verwendet werden können, gehören (ohne Beschränkung hierauf) Gelatine, Polymilchsäure, Polyglycolsäure, Copolymere von Milchsäure und Glycolsäure, Polycaprolacton, Carboxymethylcellulose, Celluloseester (z. B. Methyl- und Ethylester), Celluloseacetat, Dextrose, Dextran, Chitosan, Hyaluronsäure, Ficol, Chondroitinsulfat, Polyvinylalkohol, Polyacrylsäure, Polypropylenglycol, Polyethylenglycol und wasserlösliche Methacrylat- oder Acrylatpolymere.
- Zur Herstellung der porösen Matrix wird das bevorzugte lösliche Kollagenbindemittel zu einer mineralisierten Kollagenaufschlämmung gegeben und vermischt. Vorzugsweise beträgt das Verhältnis von löslichem zu unlöslichem Kollagen etwa 10% (Gew./Gew.). Der pH-Wert wird je nach Bedarf auf 7,5 ± 0,5 eingestellt. Wenn der angestrebte Vermischungsgrad erreicht ist, wird die Dispersion auf –20 bis –80°C eingefroren.
- Die gefrorene Aufschlämmung wird lyophilisiert. Die poröse Matrix kann vernetzt werden, um die physikalische Stabilität zu erhöhen, die Resorptionszeit der Matrix zu verlängern und die Handhabung des Endprodukts zu erleichtern. Die lyophilisierte Matrix wird vorzugsweise unter Verwendung von Glutaraldehyd in Lösung (typischerweise 0,01%) oder unter Verwendung von Dampf vernetzt. Bei Verwendung einer Lösung wird die Matrix nach Entfernung von überschüssigem Reagenz durch Lyophilisation dehydratisiert. Die poröse Matrix kann auch durch Filtrieren der Aufschlämmung der mineralisierten Kollagenfasern und des Bindemittels unter Bildung einer Bahn erzeugt werden. Die getrocknete Bahn kann sodann vernetzt werden.
- Die poröse Struktur kann auch durch Vermischen der mineralisierten Kollagenfasern, des Bindemittels und der auslaugbaren Teilchen (lösliche Salze, wie Natriumchlorid) und/oder der Feststoffe mit hohem Dampfdruck, die später durch Sublimation entfernt werden können, erzielt werden. Die Aufschlämmung kann getrocknet werden. Anschließend lassen sich die auslaugbaren oder sublimierbaren Teilchen unter Bildung der porösen Struktur entfernen. Die poröse Matrix kann vernetzt werden.
- Zu weiteren Vorteilen einer vernetzten Matrix gehören eine größere Implantat-Verweilzeit und eine bessere Beibehaltung der Gestalt (keine Fragmentierung des Implantats).
- Weitere Vernetzungsverfahren und Vernetzungsmittel können herangezogen werden, z. B. Formaldehyd, Chromsalze, Diisocyanate, Carbodiimide, difunktionelle Säurechloride, difunktionelle Anhydride, difunktionelle Succinimide, Dibromisopropanol, Epichlorhydrin, Diepoxide, eine dehydrothermische Vernetzung, eine UV-Bestrahlung im trockenen Zustand oder die Anwendung von E-Strahlen oder eine γ-Bestrahlung in wässriger Lösung.
- Die endgültige Sterilisation des Produkts kann durch γ-Strahlung, E-Strahlung, trockenes Erwärmen oder Ethylenoxid erfolgen.
- Ein Vorteil der vorliegenden Erfindung besteht darin, dass die Kollagenfibrillen und das immobilisierte Calciumphosphatmineral eine Matrix bilden, die sich als besonders vorteilhaft für den Ersatz oder die Vermehrung von Knochen erweist. Die Matrix behält ihre physikalische Unversehrtheit für einen Zeitraum von mindestens etwa 3 Tagen nach der Implantation und behält ihre Porosität für einen Zeitraum von etwa 7 bis 14 Tagen nach Implantation in eine physiologische Umgebung, in der ein Knochenersatz erfolgt. Unter physikalischer Unversehrtheit ist zu verstehen, dass die Gestalt und die Größe der implantierten Matrix im wesentlichen beibehalten wird. Dies steht im Gegensatz zu Zusammensetzungen, die unmittelbar oder kurz nach der Implantation zu einer amorphen, nicht-porösen Masse zusammenfallen. Es erweist sich als Vorteil, dass die Matrix ferner ihre Porosität behält, was für den Knochenersatz oder für den Vorgang der Knochenvermehrung von Bedeutung ist.
- Die erfindungsgemäße Matrix erweist sich schließlich als biologisch abbaubar oder resorbierbar, so dass die Porosität und die physikalische Unversehrtheit über diesen begrenzten Zeitraum hinweg nicht aufrechterhalten werden können. Dieser Vorgang dauert durchschnittlich etwa 2 bis 12 Wochen und hängt selbstverständlich von der Größe der implantierten Matrix ab. Jedoch ist, solange die Zeitspanne, nach der eine vollständige Resorption oder ein vollständiger biologischer Abbau der Matrix vor dem Knochenersatz oder dem Vorgang der Knochenvermehrung nicht erfolgt ist, die Geschwindigkeit des biologischen Abbaus ausreichend.
- Ein Aspekt der vorliegenden Erfindung besteht darin, dass die Calciumphosphatmineralien, die typischerweise als Hydroxylapatit vorliegen, auf der Matrix immobilisiert werden, im Gegensatz zu einer freien Beweglichkeit über die gesamte Matrix hinweg. Es wurde festgestellt, dass das erfindungsgemäße Calciumphosphatmineral innerhalb der Matrix immobilisiert wird und aus Teilchen mit einer durchschnittlichen Größe von etwa 5 μm oder weniger besteht. Die Teilchengröße eines Materials kann die biologischen Wechselwirkungen im implantierten Zustand verändern, was die Gewebereaktion auf das Material beeinflussen kann. Die zelluläre Reaktion kann insofern verändert werden, als phagozytische Zellen, wie Riesenzellen und Makrophagen, um teilchenförmige Materialien herum stärker hervortreten und häufig Granulome bilden. Teilchen, die für eine Phagozytose ausreichend klein sind und eine Größe von etwa 3 bis 5 μm oder weniger aufweisen, werden von den phagozytischen Zellen aufgenommen, die ferner eine lokalisierte Gewebereaktion stimulieren. Beispielsweise lässt sich während der Knochenheilung beobachten, dass teilchenförmige Abriebbruchstücke in Verbindung mit künstlichen Gelenken in den Makrophagen von benachbartem Gewebe auftreten, womit eine erhöhte Knochenresorption in Tiermodellen in dosisabhängiger Weise einhergeht ("Macrophage/Particle Interactions: Effect of Size, Composition and Surface Area", A. S. Shanbhag et al., J. Biomed. Mater. Res., Bd. 28 (1), (1994), S. 81–90). Somit stellt es erfindungsgemäß einen Vorteil dar, dass das immobilisierte mineralische Calciumphosphat im Laufe der Zeit in Form von Teilchen mit einer Teilchengröße von 5 μm oder weniger freigesetzt wird, d. h. einer idealen Größe zur Aufnahme von phagozytischen Zellen. Ein weiterer Vorteil der Erfindung besteht darin, dass jegliche Freisetzung von mineralischen Calciumphosphatteilchen gesteuert wird, was ein Ergebnis der Tatsache ist, dass das Mineral innerhalb der Matrix immobilisiert ist. Die Vorteile in bezug auf die Teilchengröße und die Immobilisierung sind im nachstehenden Beispiel III dargelegt.
- Das Knochentransplantationsmaterial findet Anwendung als osteokonduktives Knochentransplantationsmaterial für spinale Fusionen, Füllung von Knochendefekten, Bruchreparatur und Transplantation bei periodontalen Defekten. Durch Kombination der vorliegenden Zusammensetzung mit einem osteogenen Material, z. B. autogenem Knochen oder autologem, abgesaugtem Knochenmark oder osteokonduktiven Knochenwachstumsfaktoren, BMP's, Calcitonin oder anderen Wachstumsfaktoren, lässt sich die Induktion und das Wachstum von Knochen noch weiter verstärken. Die Matrix kann auch ein Substrat ergeben, an das Wachstumsfaktoren binden können, so dass die vom Wirt erzeugten Faktoren oder von außen zugefügte Faktoren sich an der Matrix anreichern können. Die vorliegenden Zusammensetzungen lassen sich bei Bruchreparaturen, maxifazialen Rekonstruktionen, spinalen Fusionen, Gelenkrekonstruktionen und anderen orthopädischen Anwendungsmöglichkeiten einsetzen.
- Die folgenden Beispiele dienen der Erläuterung und sollen die Erfindung in keiner Weise beschränken.
- Beispiel I
- Die erfindungsgemäße mineralisierte Kollagenmatrix wird in Defekte, die in den parietalen Knochen von Ratten mit einem Alter von 8 Wochen erzeugt worden sind, implantiert. Histologische Untersuchungen werden nach 14 und 28 Tagen vorgenommen. Nach 14 Tagen wird ein Knochenwachstum von der Schnittkante des Defekts in die Kollagenmatrix beobachtet. Der neu gebildete, gewebte Knochen umgibt Stücke der restlichen Matrix und Bereiche von losem Bindegewebe, in dem eine Gefäßbildung offensichtlich ist. Nach 28 Tagen ist eine signifikante Neubildung aufgetreten, wobei Osteozyten im gesamten neuen Knochen vorliegen. Die nach 14 Tagen erkennbaren Bindegewebehohlräume verringern sich bei fortgesetztem Knochenwachstum.
- Beispiel II
- Die mit Calciumphosphat mineralisierte Kollagenmatrix von Beispiel I wurde unter Zusatz von Knochenmark in ausgewachsene männliche weiße Neuseeland-Kaninchen (3,7 bis 4,1 kg) implantiert. Ein Schnitt wurde am Mittelstück über die vordere mediale Oberfläche des rechten Vorderbeins zum Freilegen des Radius vorgenommen. Ein kritischer Defekt wurde unter Verwendung eines pneumatischen Bohrers durch Entfernen eines Segments von 1,5 cm des Radius erzeugt. Während der Osteotomie wurde für eine Spülung gesorgt, um eine Überhitzung und Schädigung des Knochens auf ein Minimum zu beschränken. Der Defekt wurde mit der mineralisierten Kollagenmatrix, die mit Knochenmark oder autogenem Transplantat vermischt war, gefüllt. Das Knochenmark wurde aus dem Schienbein des gleichen Tiers abgesaugt. Beim autogenen Transplantat handelte es sich um einen spongiösen Knochen, der vom Darmbeinkamm ähnlich wie bei den derzeitigen Knochenvermehrungs- oder Transplantationsverfahren geerntet worden war. Nach dem chirurgischen Eingriff wurden die Tiere täglich beobachtet. Röntgenaufnahmen wurden am operierten Radius während der ersten 8 Wochen in jeder zweiten Woche und bis zur Nekropsie nach 12 Wochen monatlich gemacht. Die Kaninchen wurden so eingeteilt, dass sie den chirurgischen Eingriff 12 und 24 Wochen überlebten.
- Bei der Nekropsie wurden der rechte und der linke Radius entnommen. Der operierte Radius wurde einer groben Bewertung auf Heilungsanzeichen (Callusbildung und Verbindung) unterzogen. Die Untersuchung umfasste das Auftreten von Knochen, was einen Hinweis auf eine Verbindung ergab oder das Vorliegen von Knorpel, Weichgewebe oder Rissen innerhalb des Defekts, was auf eine mögliche instabile Verbindung schließen ließ.
- Sodann wurden die Radien in 10% neutralem gepufferten Formalin fixiert und für die histologische und morphometrische Bewertung verarbeitet.
- Die nach 0, 2, 4, 6, 8 und 12 Wochen erstellten Röntgenaufnahmen zeigten eine robuste Heilungsreaktion bereits nach zwei Wochen, wobei die Defektstellen sich ständig besserten und eine Neubildung in Richtung zu einer Wiederherstellung der natürlichen Cortices des Radius erfolgte. Die beobachtete zunehmende Heilung stimmte zwischen den mit der Calciumphosphat-Kollagenmatrix behandelten Gruppen und der mit den Autotransplantaten behandelten Kontrollgruppe überein. Zwischen den beiden Verbindungsgruppen bestanden wenig Unterschiede in bezug auf die radiographische Vereinigung.
- Bei früheren Untersuchungen wurde gezeigt, dass Defekte, die leer oder unbehandelt blieben (negative Kontrolle), wenig oder gar keinen neuen Knochen enthielten. Bei diesem Test bildete das Autotransplantat (positive Kontrolle) eine stabile Knochenvebindung. Die mit Calciumphosphat-Kollagen behandelten Defekte, die Knochenmark enthielten, zeigten ebenfalls eine stabile Überbrückung mit neuem Knochen, vergleichbar mit der Situation beim Autotransplantat.
- Beispiel III (Vergleichsbeispiel)
- Ein Ansatz von Calciumphosphat-Mineral wurde ohne Zugabe von Kollagen hergestellt. Das Mineral wurde geerntet, gewaschen und zu einem trockenen Pulver lyophilisiert. Die IR-Spektroskopie ergab einen Hydroxylapatit-Charakter.
- Eine Mischmatrix wurde durch Mischen von unlöslichen fibrillären Kollagenfasern mit löslichem Kollagen in einem Gewichtsverhältnis von 9/1 bei einem Gesamtfeststoffgehalt von 4 Gew.-% hergestellt. Die Aufschlämmung wurde von Hand vermischt. Freies Mineral wurde zugesetzt, um den Gesamtfeststoffgehalt auf 25 Gew.-% zu bringen. Die Aufschlämmung wurde in quadratische Teflon-Formen von 2 Zoll Kantenlänge in einer Tiefe von etwa 5 mm gegossen, auf –80°C eingefroren und lyophilisiert. Die trockene Matrix wurde unter Verwendung von Glutaraldehyd 30 Minuten vernetzt, gewaschen und erneut lyophilisiert. Die erhaltene Matrix wies eine Dicke von etwa 4 mm auf. Stanzproben von 8 mm Durchmesser wurden aus der Matrix für Implantationszwecke gewonnen. Zum Vergleich wurde ein vor kurzem hergestellter Ansatz von mineralisiertem Kollagen (immobilisiertes Mineral) mit einem Aschegehalt von 28 Gew.-% für ausgestanzte Implantate von 8 mm Durchmesser verwendet.
- Die Implantate wurden subkutan in die Thoraxfaszie platziert, wobei zwei Implantatmaterialien des gleichen Typs bilateral bei vier Ratten zu jedem Implantationszeitpunkt nach 3, 7 und 14 Tagen verwendet wurden. Bei der Nekropsie wurden die Implantate in bezug auf Gewebereaktionen bewertet. Gewebeblöcke wurden für die Histologie entnommen. H & E-gefärbte Schnitte des Implantats und des umgebenden Gewebes wurden bei jedem Tier zu jedem Zeitpunkt untersucht, um die Gewebereaktion und -integratioin zu charakterisieren.
- Die klinischen Feststellungen bei der Nekropsie zeigen eine stärkere entzündliche Reaktion und eine Abbauwirkung bei der Mischzubereitung im Vergleich zum immobilisierten mineralisierten Kollagen beim subkutanen Ratten-Implantatmodell. Nach drei Tagen wird ein breiartiges Implantat festgestellt. Nach 7 und 14 Tagen wird ein verdicktes Implantat festgestellt, was möglicherweise auf die drastische fibröse Kapselreaktion der Gemischzubereitung entsprechend der histologischen Beobachtung zurückzuführen war. Die Zubereitung mit immobilisiertem Mineral zeigte im Vergleich ein klares umgebendes Gewebe und ein normales Implantataussehen zu sämtlichen drei Beobachtungszeitpunkten.
- Eine histologische Prüfung ergab, dass die Gemischzubereitung eine hochgradige akute und chronische Entzündung aufwies, wie durch die späte (14 Tage) PMN-Aktivität und die frühe (3 Tage) Riesenzellenaktivität gezeigt wurde. Die Riesenzellen stellen Anzeichen dafür dar, dass die phagozytische Aktivität vermutlich in Reaktion auf die große Menge an lose verbundenen Teilchen des Minerals organisiert wird. Eine Fibroblasteninvasion wurde noch beobachtet und es ergab sich keine offensichtlich Gewebenekrose.
- Im Gegensatz dazu zeigen die an den Kollagenfasern immobilisierten mineralischen Teilchen eine typischere Implantat-Gewebe-Reaktion. Nach drei Tagen wird eine akute Entzündung beobachtet, die rasch zu einer chronischeren Implantatreaktion nach 7 Tagen abklingt, wobei nur eine mäßige Entzündung vorliegt, während eine Fibroblasteninvasion und Gefäßneubildung am Implantatrand auftreten. Nach 14 Tagen sind Anzeichen einer verstärkten Entzündung sichtbar, was vermutlich ein Anzeichen für eine zusätzliche Mineralfreisetzung aus den Kollagenfasern aufgrund des Kollagenabbaus darstellt.
- Die Gemischzubereitung aus dem Kollagen und den Hydroxylapatit-Mineralkomponenten zeigt eine signifikante akute Entzündungsreaktion bei subkutanen Rattenimplantaten. Die Immobilisierung der mineralischen Komponente in der mineralisierten Kollagenzusammensetzung scheint die biologische Verfügbarkeit des Minerals zu verringern, wodurch die Entzündung vermindert wird, während die Gewebeintegration während der Wundheilung weiter unterstützt wird.
Claims (17)
- Poröse, bioabbaubare Matrix für den Knochenersatz, welche für einen Zeitraum von mindestens ungefähr drei Tagen nach der Implantation ihre physikalische Unversehrtheit und für einen Zeitraum von ungefähr sieben bis vierzehn Tagen nach Implantation in eine physiologische Umgebung, in der ein Knochenersatz erfolgt, ihre Porosität beibehält, umfassend ein Netzwerk unlöslicher Biopolymerfasern und immobilen Calciumphosphatminerals; dadurch gekennzeichnet, dass das Netzwerk mit einem Bindemittel gebunden ist, und dass das Mineral aus Teilchen mit einer durchschnittlichen Teilchengröße von 5 μm oder weniger besteht.
- Matrix nach Anspruch 1, wobei das Bindemittel aus der Gruppe ausgewählt ist, die aus löslichem Kollagen, Gelatine, Polymilchsäure, Polyglycolsäure, Copolymeren aus Milch- und Glycolsäure, Polycaprolacton, Carboxymethylcellulose, Celluloseestern, Dextrose, Dextran, Chitosan, Hyaluronsäure, Ficol, Chondroitinsulfat, Polyvinylalkohol, Polyacrylsäure, Polypropylenglycol, Polyethylenglycol, wasserlöslichen Polyacrylaten und wasserlöslichen Polymethacrylaten besteht.
- Matrix nach Anspruch 1, wobei das Biopolymer fibrilläres Kollagen umfasst.
- Matrix nach Anspruch 1, wobei das Mineral Hydroxylapatit umfasst.
- Matrix nach Anspruch 1, wobei das Mineral während des Austauschs mit Knochen mit einem zeitverzögerten Profil in Form von Teilchen in die physiologische Umgebung freigesetzt wird, wobei die physikalische Unversehrtheit und Poro sität jeweils für die jeweiligen Zeiträume beibehalten werden.
- Matrix nach Anspruch 3, wobei das Kollagen und immobilisiertes Calciumphosphat in Form von mineralisiertem Kollagen vorliegen, das etwa 30–80 Gew.-% Kollagen enthält.
- Matrix nach einem der Ansprüche 1 bis 6, die weiterhin Knochenmarkzellen umfasst.
- Matrix nach einem der Ansprüche 1 bis 6, die weiterhin autogene Knochen umfasst.
- Matrix nach einem der Ansprüche 1 bis 6, die weiterhin einen oder mehrere Knochenwachstumsfaktoren umfasst.
- Matrix nach Anspruch 1, wobei das Calciumphosphatmineral auf den Fasern immobilisiert ist.
- Verfahren zur Herstellung einer bioabbaubaren, porösen Knochentransplantationsmatrix, welches die Schritte umfasst: (a) Bilden einer Dispersion, welche ein bioabbaubares, wasserunlösliches Biopolymer, ein wasserlösliches Bindemittel und durch das Bindemittel oder Biopolymer immobilisiertes teilchenförmiges Calciumphosphatmineral umfasst; (b) Ausbilden der Dispersion zu einer porösen Matrix; (c) Vernetzen der porösen Matrix; wobei das Mineral aus Teilchen besteht, die eine durchschnittliche Größe von 5 μm oder weniger aufweisen.
- Verfahren nach Anspruch 11, wobei der Schritt (b) das Einfrieren und Lyophilisieren der Dispersion umfasst.
- Verfahren nach Anspruch 11, wobei die Dispersion weiterhin auslaugbare Teilchen umfasst, und Schritt (b) zur Bildung der Matrix das Trocknen der Dispersion und Auslaugen der Teilchen umfasst.
- Verfahren nach Anspruch 11, wobei die Dispersion weiterhin sublimierbare Teilchen umfasst, und Schritt (b) zur Bildung der Matrix das Trocknen der Dispersion und Sublimieren der Teilchen umfasst.
- Verfahren nach Anspruch 11, wobei das bioabbaubare wasserunlösliche Biopolymer Kollagenfasern umfasst.
- Verfahren nach Anspruch 11, wobei das Bindemittel lösliches Kollagen umfasst.
- Verfahren nach Anspruch 11, wobei das Mineral Hydroxylapatit umfasst.
