Description
- Die Erfindung betrifft ein chirurgisches Instrument zur Behandlung von Harninkontinenz bei Frauen von der in WO-A-9003766 beschriebenen Art, das einen Schaft mit einem Griff an einem Ende davon und ein gebogenes nadelähnliches Element umfasst, das so konstruiert ist, dass es mit dem Schaft verbunden wird, um einen gebogenen Abschnitt auszubilden.
- Das Dokument WO-A-9606567, das Stand der Technik gemäß Artikel 54(3) EPÜ ist, umfasst eine chirurgische Inkontinenzvorrichtung, die erlaubt, Harninkontinenz bei Frauen zu lindern, während Kontinenz wiederhergestellt wird, indem zwei gebogene Nadeln an einem Band befestigt werden, das permanent in das Gewebe zwischen der Scheidenwand und der Abdomenwand eines Patienten implantiert werden soll, wodurch das Gewebe verstärkt wird, das erforderlich ist, um die Harninkontinenz wiederherzustellen. Das chirurgische Instrument gemäß der vorliegenden Erfindung ist eine Verbesserung gegenüber diesem Instrument, wo das Band ein Geflecht umfasst, das in einer dünnen Kunststoffhülle eingeschlossen ist, so dass das Einführen erleichtert wird, während Irritationen oder Beschädigung von Körpergewebe vermieden wird.
- Die Erfindung wird detaillierter unter Bezugnahme auf die beigefügten Zeichnungen erklärt werden, die das chirurgische Instrument gemäß der Erfindung zeigen und wobei

6 eine Seitenansicht von zwei Nadeln und eines Bandes ist, das die Nadeln miteinander verbindet. - In der folgenden Beschreibung sind die gleichen Bezugszeichen wie in WO-A-9606567 für entsprechende Details des Instrumentes verwendet worden.
- Das chirurgische Instrument umfasst einen zylindrischen röhrenförmigen Schaft
10 , der an einem Ende davon einen Griff11 umfasst. Am anderen Ende des Schaftes befindet sich eine Buchse14 . Ein zylindrischer Schaft15 ist drehbar in dem Schaft angebracht und kann manuell vermittels eines Knopfes16 gedreht werden, der an einem Ende des Schaftes angebracht ist. Das andere Ende des Schaftes bildet einen zylindrischen Abschnitt17 ,5 , mit einem kleineren Außendurchmesser als der Schaft aus, der einen Abschnitt18 mit Außengewinden verbindet, einen glatten Endabschnitt19 mit einem weiteren verringerten Durchmesser, der den mit einem Gewinde versehenen Abschnitt18 verbindet, wobei der Endabschnitt19 einen Führungsstift an dem anderen Ende des Schaftes ausbildet. Die Abschnitte18 und19 werden in dem Abschnitt der Buchse14 aufgenommen, der von dem Schaft vorsteht. Das bisher beschriebene chirurgische Instrument entspricht dem in WO-A-9606567 offenbarten Instrument mit der Ausnahme, dass der Endabschnitt14' der Buchse14 von gegenüberliegenden Seiten abgeflacht ist (siehe1 und2 ), so dass der Querschnitt des Endabschnittes nicht kreisförmig ist. - Das chirurgische Instrument umfasst auch eine austauschbare und Einwegnadel
21 , die an einem Ende davon mit dem Schaft an einem Ende der Nadel verbunden ist und sich über im wesentlichen ein Viertel eines Kreises zu dem anderen, freien Ende davon erstreckt, um im wesentlichen dem Profil des Schambeins zwischen der Scheiden- und der Abdomenwand zu folgen. Die Nadelform weist einen gleichmäßigen kreisförmigen Querschnitt auf und eine glatte, bevorzugterweise polierte äußere Oberfläche. An ihrem freien Ende bildet die Nadel eine Spitze22 aus, indem sie durch einen konischen Abschnitt beendet wird. - Zum Befestigen der Nadel
21 an dem Schaft10 bildet die Nadel an ihrem einen Ende einen geraden Abschnitt30 aus, der zylindrisch ist, aber gerändelte flache Seiten31 über dem Teil des Abschnittes30 aufweist, der sich von dem benachbarten Ende der Nadel erstreckt, der durch den Buchsenabschnitt14' aufgenommen werden soll. Die Nadel sollte in einer vorherbestimmten Drehposition bezüglich des Schaftes ausgerichtet sein und genauer sollte sie im rechten Winkel aus der Ebene des Griffes11 vorstehen. Diese Drehposition wird durch die nicht-kreisförmige Form des Buchsenabschnittes14' und den Endabschnitt der Nadel mit den flachen Seiten31 , der in den Buchsenabschnitt14' passt, gesichert. Der Endabschnitt der Nadel mit den flachen Seiten31 verbindet den Körper der Nadel über den konischen Abschnitt32 , der sich zu einer Schulter33 verjüngt. - Ein axiales Blindloch erstreckt sich von der Endoberfläche der Nadel, wobei das Loch einen mit einem Gewinde versehenen Abschnitt
23 und nach innen einen engeren zylindrischen Abschnitt24 aufweist. Der Führungsstift19 ist so dimensioniert, dass er führungsmäßig von dem letztgenannten Abschnitt aufgenommen wird, wenn der mit einem Gewinde versehen Abschnitt18 zum Befestigen der Nadel21 mit dem Rest des chirurgischen Instrumentes in dem mit einem Gewinde versehenen Abschnitt23 des Blindloches eingeschraubt wird, indem der Schaft15 durch manuelles Drehen des Knopfes16 gedreht wird, wobei die Endoberflächen des Schaftes und der Nadel gegeneinander gepresst werden. Diese Befestigung entspricht auch der in WO-A-9606567 beschriebenen. - Wenn das Verfahren, wie in WO-A-9606567 beschrieben, angewandt wird, werden zwei Nadeln
21A und21B ,6 der beschriebenen Ausführungsform, jeweils an einem Ende eines Bandes26 befestigt werden. Gemäß der vorliegenden Erfindung umfasst das Band der bevorzugten Ausführungsform ein Gitter oder Geflecht, das Öffnungen in der Größenordnung von 1 mm ausbildet. Ein geeignetes Material für das Band ist PROLENE®, ein gestricktes Polypropylengitter mit einer Stärke von 0,7 mm, das von Ethicon, Inc., Sommerville, New Jersey, USA, hergestellt wird. Dieses Material ist von der FDA in den USA zur Implantation in den menschlichen Körper zugelassen. Das Geflecht (Band) weist bevorzugterweise eine Weite von etwa 10 mm auf und ist in einer dünnen Polyethylenhülle34 eingeschlossen, die im abgeplatteten Zustand im wesentlichen die gleiche Weite wie das Band aufweist, obwohl ein Unterschied hinsichtlich der Weite in2 gezeigt ist, um die Bereitstellung der Hülle klarer zu machen. Die Länge des Geflechtes sollte etwa 400 mm betragen. Das Geflecht und die Hülle werden miteinander vermittels zweier Reihen35 aus nicht dem Zusammenziehen dienenden Nähten verbunden. Der Endabschnitt der Hülle ist mit dem konischen Abschnitt32 der Nadel vermittels eines geeigneten starken Klebers verbunden und die Verbindung der Nadel und der Hülle wird durch einen Schrumpfschlauch36 aus Gummi abgedeckt, der sich von der Schulter33 über den konischen Abschnitt32 und teilweise über den zylindrischen Endabschnitt33 der Nadel erstreckt. Der Schrumpfschlauch ist im wesentlichen bündig zu der Oberfläche der Nadel an der Schulter. Durch diese Anordnung ist das Geflecht fest mit der Nadel verbunden. - Der Zweck der Hülle
34 besteht vor allen Dingen darin, das Einführen des Geflechtes in der in WO-A-06567 beschriebenen Art und Weise zu erleichtern, d. h. wenn das Geflecht an seinen Enden von der Scheidenwand zu der Abdominalhaut gezogen wird und um zu vermeiden, dass raue Kanten des Geflechtes die Körpergewebe reizen oder beschädigen. - Wenn das Band als eine Schlinge um den Harnleiter herum in der richtigen Position positioniert worden ist, wird die Polyethylenhülle entfernt werden und, um das Entfernen zu erleichtern, sollte die Hülle entlang der Mitte ihrer Längsachse perforiert sein, wie in
6 mit der Linie37 aus Punkten und Strichen angegeben, so dass die zwei Hälften der Hülle aus dem Körper entfernt werden können, indem an ihren entsprechenden äußeren Enden gezogen wird, wobei die Hälften an der Perforation unter dem Einfluss der Zugkraft getrennt werden. - Der Zweck der Polyethylenhülle besteht auch darin, das Geflecht während der Befestigung an die Nadeln und während der Handhabung vor und während des Einführens in den Körper zu schützen.
- Die Mitte der Längsachse des Bandes und der Hülle sollten durch eine sichtbare Farbmarkierung
38 ,6 , angegeben sein, so dass der Chirurg leicht sehen kann, wenn das Geflecht hinsichtlich des Harnleiters während des chirurgischen Eingriffes symmetrisch angeordnet ist.
Claims (14)
- Band zur Behandlung von Harninkontinenz bei Frauen, dadurch gekennzeichnet, dass das Band ein Geflecht (
26 ) aufweist, das von einer dünnen Kunststoffhülle (34 ) umgeben ist. - Band nach Anspruch 1, dadurch gekennzeichnet, dass das Geflecht (
26 ) aus Polypropylen hergestellt ist. - Band nach Anspruch 1, dadurch gekennzeichnet, dass das Geflecht (
34 ) aus Polyethylen hergestellt ist. - Band nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass die Hülle (
34 ) in der Mitte der Längsachse perforiert ist. - Band nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, dass das Geflecht (
26 ) und die Hülle durch eine nicht dem Zusammenziehen dienende Naht (35 ) verbunden sind. - Band nach einem der Ansprüche 1 bis 5 mit einem ersten Ende, das mit einem ersten gebogenen nadelähnlichen Element (
21A ) an einem Ende des ersten Elementes befestigbar ist, und einem zweiten Ende des Bandes (26 ), das mit einem zweiten gebogenen nadelähnlichen Element (21B ) befestigbar ist an einem Ende des zweiten Elementes, wobei die Elemente dafür bestimmt sind, in den Körper durch die Scheidenwand eingebracht zu werden, und dimensioniert sind, damit sie sich von der inneren Oberfläche der Scheidenwand über die Rückseite des Schambeins zur Außenseite der Abdominalwand erstrecken. - Band nach einem der Ansprüche 1 bis 6, umfassend ein erstes gebogenes nadelähnliches Element (
21A ), das an einem Ende des ersten Elementes mit einem ersten Ende des Bandes (26 ) verbunden ist, und ein zweites gebogenes nadelähnliches Element (21B ), das an einem Ende des zweiten Elementes mit einem zweiten Ende des Bandes (26 ) verbunden ist, wobei die Elemente so konstruiert sind, dass sie unabhängig voneinander mit dem zweiten Ende eines Schaftes verbunden sein können, der einen Handgriff an einem ersten Ende des Schafts und ein zweites Ende aufweist, das angepasst ist, um das erste gebogene nadelähnliche Element (21A ) oder das zweite gebogene nadelähnliche Element (21B ) aufzunehmen, und die Elemente dazu bestimmt sind, in den Körper durch die Scheidenwand eingebracht zu werden, und so dimensioniert sind, dass sie sich von der inneren Oberfläche der Scheidenwand über die Rückseite des Schambeins zur Außenseite der Abdominalwand erstrecken. - Band nach Anspruch 7, dadurch gekennzeichnet, dass das nadelähnliche Element einen nichtkreisförmigen Endabschnitt aufweist, der in eine nichtkreisförmige Buchse (
14' ) am zweiten Ende des Schaftes (10 ) passt. - Band nach Anspruch 8, dadurch gekennzeichnet, dass der Endabschnitt des nadelähnlichen Elementes (
21 ) den Rest des Elementes durch einen konischen Abschnitt (32 ) verbindet, der sich zu einer Schulter (33 ) auf dem nadelähnlichen Element verjüngt. - Band nach Anspruch 9, dadurch gekennzeichnet, dass das Geflecht (
26 ) und die Hülle (34 ) mit dem nadelähnlichen Element (21 ) durch Anleimen an dem konischen Abschnitt (32 ) verbunden sind. - Band nach Anspruch 10, dadurch gekennzeichnet, dass das Geflecht (
26 ) und die Hülle (34 ) an ihrem Befestigungsort durch einen Schrumpfschlauch (36 ) beschichtet sind. - Band nach Anspruch 11, dadurch gekennzeichnet, dass ein Ende des Schrumpfschlauches (
36 ) an die Schulter (33 ) stößt und mit der Oberfläche des nadelähnlichen Elementes an der Schulter im Wesentlichen bündig ist. - Band nach Anspruch 11 oder 12, dadurch gekennzeichnet, dass das Geflecht (
26 ) und die Hülle (34 ) über den Schrumpfschlauch (36 ) an dessen anderem Ende vorstehen. - Band nach einem der Ansprüche 1 bis 13, dadurch gekennzeichnet, dass auf der Hülle (
34 ) eine sichtbare Markierung (38 ) in der Mitte ihrer Längsachse vorgesehen ist.
