Description
- Die vorliegende Erfindung betrifft allgemein Vorrichtungen zum Stabilisieren der Wirbelsäule sowie Vorrichtungen zur Implantation zwischen Wirbeln und insbesondere im Zwischenwirbelraum. Die Erfindung betrifft spezifisch ein verstärktes, poröses Wirbelsäulenimplantat. Zwischenwirbelscheiben, die sich zwischen den Endplatten benachbarter Wirbel befinden, stabilisieren die Wirbelsäule, verteilen Kräfte zwischen Wirbeln und polstern Wirbelkörper. Eine normale Zwischenwirbelscheibe hat eine halbgelatinöse Komponente, den Nucleus pulposus, der von einem äußeren fasrigen Ring mit der Bezeichnung Annulus fibrosus umgeben und eingefasst ist. Bei einer gesunden, unbeschädigten Wirbelsäule verhindert der Annulus fibrosus, dass der Nucleus pulposus aus dem Zwischenwirbelraum vorsteht.
- Bandscheiben können durch Trauma, Erkrankung oder Alterung verschoben oder beschädigt werden. Der Nucleus pulposus kann aufgrund einer Disruption des Annulus fibrosus in den Wirbelkanal vorstehen, ein Zustand, der gewöhnlich als Scheibenprolaps oder -bruch bezeichnet wird. Der ausgepresste Nucleus pulposus kann auf den Wirbelsäulennerv drücken, was zu Nervenschäden, Schmerzen, Taubheit, Muskelschwäche und Paralyse führen kann. Zwischenwirbelscheiben können sich auch im normalen Alterungsprozesses oder aufgrund einer Erkrankung verschlechtern. Wenn eine Scheibe austrocknet und sich verhärtet, nimmt die Höhe des Zwischenwirbelraums ab, was zu Instabilität der Wirbelsäule, Beträchtigung der Beweglichkeit und Schmerzen führt.
- Die einzige Erleichterung von den Symptomen dieser Zustände ist häufig eine Diskektomie oder ein vollständiges oder teilweises chirurgisches Entfernen einer Zwischenwirbelscheibe, gefolgt von Fusionen der benachbarten Wirbel. Aufgrund des Entfernens der beschädigten oder ungesunden Scheibe kann der Zwischenwirbelraum kollabieren. Ein Kollaps des Zwischenwirbelraums kann Instabilität der Wirbelsäule, eine anormale Gelenkmechanik, vorzeitige Entwicklung von Arthritis oder Nervenschäden zusätzlich zu ernsthaften Schmerzen verursachen. Um einen Kollaps des Raums zu verhindern, werden häufig prothetische Implantate verwendet. Das Implantat muss vorübergehende Abstützung bieten und ein Einwachsen des Knochens zulassen. Ein Erfolg der Diskektomie und des Fusionsverfahrens erfordert die Entwicklung eines zusammenhängenden Knochenwachstums, damit eine feste Masse entstehen kann, weil das Implantat die Druckbelastungen auf die Wirbelsäule möglicherweise nicht für die Lebenszeit des Patienten aushalten kann.
- Viele Versuche, den Zwischenwirbelraum nach der Beseitigung der Bandscheibe wiederherzustellen, beruhten auf Metallvorrichtungen. Das US-Patent Nr. 4878915 von Brantigan lehrt einen massiven Metallstopfen; die US-Patente Nr. 5044104; 5026373 und 4961740 von Ray; 5015247 von Michelson und US-Patent Nr. 4820305 von Harms et al. lehren hohle Metallkäfigkonstruktionen. Mit dem Gebrauch dieser Metallimplantate sind mehrere Nachteile verbunden. Vollkörpermetallimplantate erlauben kein Einwachsen des Knochens, was schließlich zu einem Ausfall des Implantats führen kann. Die Oberflächenporösität solcher massiver Implantate korrigiert dieses Problem nicht, weil sie kein ausreichendes Einwachsen zulässt, um eine massive Knochenmasse entstehen zu lassen, die stark genug ist, um die Belastung der Wirbelsäule auszuhalten. Andererseits erlauben die Hohlkäfigkonstruktionen von Harms, Ray und Michelson ein Einwachsen. Diese Vorrichtungen können auch mit Knochengraftmaterial zum Fördern von Knochenwachstum gefüllt werden. Leider lassen sich viele dieser Vorrichtungen nur schwer bearbeiten und sind daher kostspielig. Außerdem können Metallimplantate den Knochengraft vor Belastung schützen, was die für eine Fusion benötigte Zeit erhöht.
- Das Michelson-Implantat erfordert ferner ein Spezialwerkzeug und zusätzliche Vorbereitung der benachbarten Wirbelkörper, um eine Fusion sicherzustellen. Es wird eine Spezialpresse benötigt, um einen komprimierten Kern aus osteogenem Material mit Kraft in die Vorrichtung einzuspritzen. Das osteogene Material, das vom Beckenkamm des Patienten genommen wird, muss so komprimiert werden, dass das Graftmaterial durch Öffnungen im Implantat verläuft, so dass das Graftmaterial den Knochen der benachbarten Wirbelkörper direkt berührt. Michelson verlangt auch das Auskernen eines Bereiches jedes benachbarten Wirbelkörpers, um einen ausreichenden Oberflächenkontaktbereich zwischen dem Implantat und dem Rindenknochen des Wirbels herzustellen.
- Die EP-A-0179695 offenbart eine Wirbelprothese, in der ein Metallgehäuse einen künstlichen Graft aus gesintertem Aluminiumoxid trägt. Der Graft entspricht der Innenform des Gehäuses und hat Bohrungen, die zwischen einer oberen und unteren Querfläche verlaufen.
- Die Verwendung von Knochengraftmaterialien in diesen früheren Metallkäfig-Fusionsvorrichtungen ist mit mehreren Nachteilen behaftet. Autografts, Knochenmaterial, das chirurgisch vom Patienten entfernt wurde, sind unerwünscht, weil sie keine ausreichende Graftmaterialmenge ergeben. Der zusätzliche chirurgische Eingriff zum Extrahieren des Autografts erhöht auch das Infektionsrisiko und kann die strukturelle Intaktheit am Spenderort herabsetzen. Die Versorgung mit Allograftmaterial, das von Spendern derselben Spezies erhalten wurde, ist unbegrenzt. Allografts sind jedoch auch aufgrund des Risikos von Krankheitsübertragung und Immunreaktionen nachteilig. Ferner hat allogener Knochen nicht das osteogene Potential von autogenem Knochen und bietet daher nur temporäre Abstützung.
- Aufgrund des Bedarfs an sichereren Knochengraftmaterialien haben Knochengraftsubstitute wie biokeramische Produkte in neuerer Zeit erhebliche Aufmerksamkeit erhalten. Calciumphosphatkeramiken sind biokompatibel und haben nicht die infektiösen oder immunologischen Risiken von Allograftmaterial. Keramiken können in jeder beliebigen Menge hergestellt werden, was gegenüber Autograft-Knochengraftmaterial von großem Vorteil ist. Ferner sind Biokeramiken osteokonduktiv, stimulieren Osteogenese an knöchrigen Orten und werden auch für osteogen d.h. fähig gehalten, an nichtknöchrigen Stellen eine Osteogenese einzuleiten. Biokeramiken bieten eine poröse Matrix, die neues Knochenwachstum noch weiter fördert. Leider fehlt Keramikimplantaten die Festigkeit, um hohe Wirbelsäulenbelastungen auszuhalten, und sie benötigen daher eine separate Befestigung vor der Fusion.
- Von den Calciumphosphatkeramiken wurden Hydroxyapatit- und Tricalciumphosphatkeramiken für Knochengrafts am häufigsten eingesetzt. Hydroxyapatit ist anorganischer Knochensubstanz chemisch ähnlich und mit Knochen biokompatibel. Es wird jedoch nur langsam abgebaut. β-Tricalciumphosphat wird in vivo schnell abgebaut und ist zu schwach, um als Abstützung zu wirken. Es hat sich gezeigt, dass diese Keramiken nach einer Diskektomie beim Warten auf Fusion als temporäre Abstützung nicht zufriedenstellend sind.
- Es besteht weiterhin Bedarf an Fusionsvorrichtungen, die ein Einwachsen von Knochen fördern ohne Belastungsschutz und dabei doch eine ausreichende Festigkeit ergeben, um die Wirbelsäule zu stützen, bis die benachbarten Wirbel fusioniert sind.
- Gemäß der vorliegenden Erfindung wird ein Wirbelsäulenimplantat für den Eingriff zwischen Wirbeln bereitgestellt, das Folgendes umfasst:
einen Körper mit zwei entgegengesetzten Flächen und einer Außenfläche, die zwischen den zwei Flächen angeordnet ist, wobei der Körper ein poröses biokompatibles Material umfaßt, das ein Einwachsen des Gewebes dazwischen gestattet, wobei der Körper für einen Eingriff zwischen zwei Wirbeln bemessen und gestaltet ist; und
eine Hülse, die um die Außenfläche des Körpers angeordnet ist, wobei die Hülse ein zweites Material umfaßt, das unter Druckbelastungen relativ fester ist als das poröse biokompatible Material, wobei die Hülse eine Höhe aufweist, die geringer ist als eine Höhe der Außenfläche, um einen Kontakt der entgegengesetzten Flächen mit Endplatten der entsprechenden Wirbel zu gestatten, wenn das Implantat zwischen den Wirbeln implantiert wird. - Die Erfindung sieht auch eine Zwischenkörperfusionsvorrichtung mit einer Höhe vor, die der Höhe eines menschlichen Zwischenwirbelraums ähnlich ist. In einer weiteren spezifischen Ausgestaltung wird eine Wirbelkörperaustauschvorrichtung bereitgestellt, um den nach dem Entfernen eines defekten Wirbelsäulenelementes gebliebenen Raum zwischen benachbarten gesunden Wirbelkörpern wiederherzustellen.
- In einer spezifischen Ausgestaltung der Erfindung wird eine Mehrzahl von Öffnungen durch die Hülse in Verbindung mit der Außenfläche des Körpers für ein Einwachsen von Knochen bereitgestellt. In einer anderen Ausgestaltung wird die Hülse aus einem temperaturempfindlichen Material gebildet, so dass die Kammer eine erste Innenabmessung hat, die geringfügig größer ist als eine Außenabmessung des Körpers, wenn die Hülse sich in einem erwärmten Zustand befindet, um den Körper gleitend in der Hülse aufzunehmen. Die Kammer hat eine zweite Innenabmessung, die geringfügig kleiner ist als der Körper, wenn sich die Hülse in einem gekühlten Zustand befindet, um dadurch den Körper darin festzuklemmen.
- In einer weiteren spezifischen Ausgestaltung der Erfindung wird ein Befestigungsmittel zum Befestigen der Hülse an den Endplatten von benachbarten Wirbelkörpern bereitgestellt.
- Es ist eine Aufgabe der Erfindung, ein Implantat für den Eingriff zwischen Wirbeln bereitzustellen, das das Einwachsen von Knochen fördert ohne Belastungsschutz. Es ist eine weitere Aufgabe der Erfindung, ein Implantat bereitzustellen, das den Zwischenwirbelraum wiederherstellt und die Wirbelsäule abstützt und gleichzeitig das Einwachsen von Knochen fördert. Es ist eine weitere Aufgabe, eine Wirbelkörperaustauschvorrichtung für die Verwendung beim Wiederherstellen des Raums bereitzustellen, der nach dem Entfernen eines fehlerhaften Wirbelsäulenelementes bleibt, die gleichzeitig Fusion zwischen den benachbarten gesunden Wirbelkörpern fördert.
- Ein Vorteil der Implantate der vorliegenden Erfindung ist, dass sie die Vorteile von Keramiken und anderen porösen, biokompatiblen Materialien mit den Vorteilen von Metallen verbinden, ohne die entsprechenden Nachteile. Ein zusätzlicher Vorteil ist, dass die Erfindung ein stabiles Gerüst für das Einwachsen von Knochen erzeugt, bevor eine Fusion stattfindet. Weitere Aufgaben und Vorteile der vorliegenden Erfindung werden für die durchschnittliche Fachperson anhand der nachfolgenden schriftlichen Beschreibung und der Begleitzeichnungen offensichtlich.
- KURZBESCHREIBUNG DER ZEICHNUNGEN

- Für die Zwecke des Vermittelns eines Verständnisses der Grundsätze der Erfindung wird nunmehr auf die in den Zeichnungen illustrierten Ausgestaltungen Bezug genommen, für deren Beschreibung eine bestimmte Sprache verwendet wird. Es ist jedoch zu verstehen, dass dadurch der Umfang der Erfindung keineswegs begrenzt werden soll und dass der Umfang der vorliegenden Erfindung durch die Ansprüche definiert wird.
- Die vorliegende Erfindung stellt ein Wirbelsäulenimplantat für den Eingriff zwischen Wirbeln bereit, das Folgendes umfasst:
einen Körper mit zwei entgegengesetzten Flächen und einer Außenfläche, die zwischen den zwei Flächen angeordnet ist, wobei der Körper ein poröses biokompatibles Material umfaßt, das ein Einwachsen des Gewebes dazwischen gestattet, wobei der Körper für einen Eingriff zwischen zwei Wirbeln bemessen und gestaltet ist; und
eine Hülse, die um die Außenfläche des Körpers angeordnet ist, wobei die Hülse ein zweites Material umfasst, das unter Druckbelastungen relativ fester ist als das poröse biokompatible Material, dadurch gekennzeichnet, dass die Hülse eine Höhe aufweist, die geringer ist als eine Höhe der Außenfläche, um einen Kontakt der entgegengesetzten Flächen mit Endplatten der entsprechenden Wirbel zu gestatten, wenn das Implantat zwischen den Wirbeln implantiert wird. - Die vorliegende Erfindung stellt Rückenwirbelimplantate bereit, die einen Körper aus einem porösen biokompatiblen Material beinhalten, der von einer Außenhülse aus einem zweiten Material verstärkt wird, das unter der Druckbelastung der Wirbelsäule relativ fester ist als das biokompatible Material. Die erfindungsgemäßen Implantate stellen den Zwischenwirbelraum wieder her, erzeugen einen großen Oberflächenbereich für das Einwachsen von Knochen und eliminieren die Notwendigkeit für Autografts. Implantate gemäß der vorliegenden Erfindung bieten eine sofortige Belastungsfähigkeit und Abstützung für die Wirbelsäule ohne Belastungsschutz des Knochenimplantatmaterials.
- Ein Wirbelsäulenimplantat
10 für den Eingriff zwischen Wirbeln ist in den1 und2 dargestellt. Das Implantat10 beinhaltet einen Körper11 aus einem porösen biokompatiblen Material, in das Knochen einwachsen kann. Der Körper11 beinhaltet zwei gegenüberliegende Flächen12 ,14 sowie eine Außenfläche13 zwischen den beiden Flächen12 ,14 . Der Körper11 ist für einen Eingriff zwischen zwei Wirbeln V bemessen und konfiguriert, wie in den3 und4 dargestellt ist. -
3 zeigt eine Zwischenkörperfusionsvorrichtung20 gemäß der vorliegenden Erfindung, die in den Zwischenwirbelraum IS zwischen zwei Wirbeln V eingreift. Der Körper25 hat vorzugsweise eine Höhe, die etwa der Höhe eines menschlichen Zwischenwirbelraums IS entspricht, und die gegenüberliegenden Flächen26 ,27 des Körpers haben jeweils eine solche Größe und Form, dass sie den Endplatten jedes der Wirbel V entsprechen. Wie in5 gezeigt, kann der Körper41 einen nierenförmigen Querschnitt haben, der sich in Größe und Form einem Nucleus pulposus nähert, der aus einem Zwischenwirbelraum entfernt wurde, oder der kompletten Scheibe oder Wirbelendplatte. In der in3 gezeigten Ausgestaltung ist der Körper vorzugsweise so bemessen und geformt, dass er satt in den Raum IS passt, der von den Endplatten und den Annuli der benachbarten Wirbelkörper V definiert wird. Eine alternative Form für den Körper ist in den6 und7 dargestellt. In einigen Anwendungen wird man es möglicherweise bevorzugen, dass die Höhe des Körpers25 geringfügig größer ist als die Höhe eines menschlichen Bandscheibenraums IS, um die Bandscheibenraumhöhe unter den Druckkräften der Wirbelsäule zu erhalten und damit die Effekte von Knochenerosion zu vermeiden. - Die Erfindung sieht auch Wirbelkörperersatzvorrichtungen
30 für die Verwendung beim Wiederherstellen des Raums vor, der nach dem Entfernen eines defekten Wirbelsäulenelementes zwischen benachbarten gesunden Wirbelkörpern V wie in4 gezeigt geblieben ist. In solchen Fällen ist der Körper35 so konfiguriert, dass er in dem Raum zwischen den benachbarten Wirbelkörpern V positioniert werden kann. Die Erfindung sieht vor, dass der Körper35 eine beliebige Form oder Größe haben kann, die für ein Wirbelsäulenimplantat für den Eingriff zwischen Wirbeln V wünschenswert ist. - Der Körper besteht aus einem porösen biokompatiblen Material, um das Einwachsen von Knochen zu fördern. Es kann jedes beliebige poröse biokompatible Material vorgesehen werden. Solche Materialien und Verfahren zu ihrer Herstellung sind in der Technik hinlänglich bekannt. Biokeramiken werden deshalb bevorzugt, weil sie eine bioaktive Osteokonduktivität aufweisen. Calciumphosphatkeramiken wie Hydroxyapatit, tricalciumphosphat, Tetracalciumphosphat, α-Calciumpyrophosphat, β-Calciumpyrophosphat und Gemische davon werden bevorzugt. Es ist vorgesehen, dass das biokompatible Material kollagenkeramische Composite wie in Toth, 1993 beschrieben oder Keramikwachstumsfaktor-Composite enthält. Das Keramikwachstumsfaktor-Composite wäre vorzugsweise ein Keramik-Knochen-Morphogenprotein- (BMP) Composite. BMP kann über Poren der Keramik zum Fusionsort geführt werden.
- Biphasische Calciumphosphat-Composite werden für ein optimales Einwachsen des Knochens und optimale Festigkeit bevorzugt und können mit in der Technik bekannten Methoden hergestellt werden (Dilecrin, 1992; Yubaoi 1993; Xingdong, 1993). Die biphasischen Composite können etwa 10 bis etwa 90 Gew.-% Hydroxyapatit und etwa 90 bis etwa 10 Gew.-% eines resorbierbaren Calciumphosphats enthalten. Biphasische osteokonduktive Hydroxyapatit/β-Tricalciumphosphat- (HA/TCP) Keramiken werden besonders bevorzugt, wie sie von Lynch im US-Patent Nr. 5306303 beschrieben wurden. Diese biphasische Keramik ist dem menschlichen trikortikalen Beckenkamm unter Druckbelastung biomechanisch überlegen. Es wurde gefunden, dass die Fusionsgeschwindigkeiten mit dieser Keramik mit Porösitäten von 30%, 50% und 70% gegenüber dem Beckenkamm-Autograft in einem Ziegenmodell überlegen sind (An et al., 1995).
- Das HA/TCP-Verhältnis beeinflusst die biologische Abbaugeschwindigkeit. Hydroxyapatit fördert die Festigkeit, wird jedoch nur langsam abgebaut. β-Tricalciumphosphat ist relativ schwach und wird schnell abgebaut. Biphasische HA/TCP-Keramiken können etwa 10 bis etwa 90 Gew.-% Hydroxyapatit und etwa 90 bis etwa 10 Gew.-% Tricalciumphosphat enthalten. Die Keramik kann 60 Gew.-% Hydroxyapatit und 40 Gew.-% Tricalciumphosphat enthalten, aber am meisten bevorzugt wird, wenn die Keramik 50 Gew.-% Hydroxyapatit und 50 Gew.-% Tricalciumphosphat enthält.
- Porösität des biokompatiblen Materials ist für ein Einwachsen erforderlich, aber es wird allgemein verstanden, dass die Festigkeit mit zunehmender Porösität abnimmt. Implantate gemäß der vorliegenden Erfindung optimieren Porösität mit Festigkeitsanforderungen, um Frakturen zu verhüten. Die biokompatiblen Materialien der vorliegenden Erfindung beinhalten vorzugsweise Porösitäten zwischen etwa 40% und etwa 60%, wobei eine Porösität von 50% am meisten bevorzugt wird. Ein Porendurchmesser von wenigstens 200 bis 600 Mikron wird für das Einwachsen von Knochen benötigt. Daher werden die Keramiken der vorliegenden Erfindung mit einer mittleren Porengröße von etwa 200 bis etwa 700 Mikron vorgesehen. Die mittlere Porengröße beträgt vorzugsweise etwa 400 Mikron.
- Wieder mit Bezug auf die
1 und2 , ein Wirbelsäulenimplantat10 beinhaltet eine Hülse15 , die um die Außenfläche13 des Körpers11 herum angeordnet ist. Die Hülse15 besteht aus einem zweiten Material, das unter Druckbelastungen relativ stärker ist als das poröse biokompatible Material. Das zweite Material ist vorzugsweise ein Metallmaterial. Das Metallmaterial beinhaltet ein beliebiges chirurgisch geeignetes Metall wie z.B. Titan, Titan-Vanadium-Aluminium-Legierung, Cobalt-Chrom-Legierung, Cobalt-Chrom-Molybdän-Legierung, Cobalt-Nickel-Chrom-Molybdän-Legierung, biokompatibler Edelstahl, Tantal, Niob, Hafnium, Wolfram und Legierungen davon. Das Metall ist vorzugsweise ein Edelstahl 316 LVM, ein Titan- oder Tantalschaumstoff. Am meisten bevorzugt wird, wenn das Metallmaterial ein offenzelliger Tantalschaumstoff ist, wie er von Kaplan im US-Patent Nr. 5282861 beschrieben wurde. Dieses Material ist ein Kohlenstoff-Metall-Verbundstoff mit einem Gerüst aus glasartigem Kohlenstoff, das ein Netzwerk aus untereinander verbundenen Poren definiert, die mit Tantal oder einem anderen geeigneten Material durch chemische Aufdampfung infiltriert werden. Dieses Material ist deshalb von Vorteil, weil es die Festigkeit anderer Metalle bietet, um als Gerüst und Prothese zu dienen, aber auch die Struktur von Knochen nachahmt. Die miteinander verbundenen Poren des Schaumstoffs dienen als weitere Stelle für das Einwachsen von Knochen. Gemäß der vorliegenden Erfindung kann eine Tantalschaumstoffscheibe gemäß dem Kaplan-Patent hergestellt werden. Der Mittelpunkt der Tantalschaumstoffscheibe kann dann ausgebohrt werden, so dass eine Metallschaumstoffhülse entsteht. Das in12 veranschaulichte Implantat50 kann aus einer Metallschaumstoffhülse51 und einem geeigneten Körper52 hergestellt sein. - Mit Bezug auf
2 , die Hülse15 hat eine Innenfläche17 für den Kontakt mit der Außenfläche13 des Körpers11 . Die Innenfläche17 der Hülse15 definiert eine Kammer18 . Die Hülse15 wird vorzugsweise aus einem temperaturempfindlichen Material wie Metall gebildet, so dass die Kammer18 eine erste Innenabmessung hat, die geringfügig größer ist als eine Außenabmessung des Körpers11 , wenn sich die Hülse15 im erhitzten Zustand befindet. In diesem Zustand kann die Hülse15 den Körper11 gleitend aufnehmen. Die Kammer18 hat eine zweite Innenabmessung, die geringfügig kleiner ist als der Körper11 , wenn sich die Hülse15 in einem gekühlten Zustand befindet, um den Körper11 darin einzuklemmen. Mit anderen Worten, die Hülse15 kann durch Anwendung von Wärme ausgedehnt und dann durch Kühlung um den Körper11 herum geschrumpft werden. Alternativ kann der Körper11 , wenn er aus Keramik besteht, in der Kammer18 der Hülse15 ausgebildet werden. - Größe und Form der Hülse
15 entsprechen der Konfiguration der Außenfläche13 des Körpers11 , wie in den2 sowie5 -11 gezeigt ist. Wenn beispielsweise der Körper zylindrisch ist, dann ist die Hülse ein Hohlzylinder wie in den6 -11 gezeigt. Gemäß der Erfindung hat die Hülse15 eine Höhe, die geringer ist als eine Höhe der Außenfläche13 des Körpers11 , um einen Kontakt der gegenüberliegenden Flächen12 ,14 des Körpers11 mit Endplatten der entsprechenden Wirbel V zuzulassen, wenn das Implantat zwischen den Wirbeln V implantiert wird. - Gemäß
1 beinhaltet ein Implantat vorzugsweise eine Hülse15 , durch die eine Mehrzahl von Öffnungen16 in Verbindung mit der Außenfläche13 des Körpers11 für das Einwachsen von Knochen definiert sind. Es ist jede geeignete Form für die Öffnungen vorgesehen. Wie in den6 -11 gezeigt, können die Öffnungen allgemein kreisförmig, oval, diamantförmig oder rechteckig sein. Größe und Anzahl der Öffnungen müssen jedoch so gewählt werden, dass die Gesamtfestigkeit der Hülse15 erhalten bleibt. - Die Implantate der vorliegenden Erfindung beinhalten vorzugsweise Befestigungsmittel zum Befestigen der Hülse
21 ,31 der Implantate an den benachbarten Wirbelkörpern V, wie in den3 und4 gezeigt ist. In einer in3 gezeigten spezifischen Ausgestaltung beinhaltet das Befestigungsmittel Zähne23 , die am oberen26 und unteren27 Ende der Hülse21 angeordnet sind. Wie in4 gezeigt, kann das Befestigungsmittel alternativ geraute Flächen33 ,34 aufweisen, die am oberen36 und unteren37 Ende der Hülse31 definiert sind. Die gerauten Flächen können mit konventionellen Bearbeitungstechniken wie Rändeln ausgebildet werden. - In einer in
13 gezeigten Ausgestaltung beinhaltet die Hülse56 Anschläge57 ,58 , die die benachbarten Wirbelkörper berühren, um zu verhindern, dass die Vorrichtung55 im Bandscheibenraum nach vorne rutscht. - Die Hülse
15 hat in einer Ausgestaltung einen Innendurchmesser, der geringfügig kleiner als 11 mm ist, und eine Wanddicke von etwa 1 mm. Die Hülse15 ist mit vierundzwanzig gleichmäßig bemessenen und beabstandeten Öffnungen16 mit einem Durchmesser von etwa 2 mm versehen. - Die Implantate der vorliegenden Erfindung können mit chirurgischen Verfahren implantiert werden, die in der Technik gut bekannt sind. Darüber hinaus können die Implantate nach einer vollen oder teilweisen Diskektomie des instrumentierten Bandscheibenraums implantiert werden.
- Implantate gemäß der vorliegenden Erfindung haben Druckfestigkeiten, die ausreichen, um die normalen Belastungen der Wirbelsäule auszuhalten. Diese erfindungsgemäßen Implantate sind wenigstens so fest wie trikortikale Beckenkammgrafts. Die Implantate haben eine Druckfestigkeit gemäß ASTM C-773 von wenigstens 7 MPa, vorzugsweise von wenigstens 20 MPa und am meisten bevorzugt von wenigstens 40 MPa. Unter diesen Belastungen trägt die Außenhülse den größten Teil der Last ohne Biegen oder Fraktur, um das brüchigere und schwächere Keramikmaterial darin zu schützen.
- Implantate gemäß der vorliegenden Erfindung kombinieren die Vorteile von porösen biokompatiblen Materialien mit festeren Materialien wie Metallen. Die Implantate bieten eine sofortige Lasttragfähigkeit ohne Belastungsschutz. Die Hülse aus den festeren Materialien bildet ein Spannungsband um das poröse biokompatible Material, um eine Fraktur des Körpers zu verhüten. Der Körper trägt die Anfangslast und überträgt sie langsam auf den neu gebildeten Knochen. Das poröse biokompatible Material bietet einen großen Oberflächenbereich für das Einwachsen von Knochen und eliminiert die Notwendigkeit für Autografts. Wo das biokompatible Material eine Biokeramik ist, da wird sie schließlich vom Host-Knochen resorbiert und ersetzt.

Claims (26)
- Wirbelsäulenimplantat für einen Eingriff zwischen Rückenwirbeln, das aufweist: einen Körper (
11 ) mit zwei entgegengesetzten Flächen (12 ,14 ) und einer Außenfläche (13 ), die zwischen den zwei Flächen angeordnet ist, wobei der Körper ein poröses biokompatibles Material umfaßt, das ein Einwachsen des Gewebes dazwischen gestattet, wobei der Körper für einen Eingriff zwischen zwei Rückenwirbeln (V) bemessen und gestaltet ist; und eine Buchse (15 ), die um die Außenfläche (13 ) des Körpers angeordnet ist, wobei die Buchse ein zweites Material umfaßt, das unter Druckbelastungen relativ fester ist als das poröse biokompatible Material, dadurch gekennzeichnet, daß die Buchse eine Höhe aufweisst, die geringer ist als die Höhe der Außenfläche (13 ), um einen Kontakt der entgegengesetzten Flächen (12 ,14 ) mit den Endplatten der entsprechenden Rückenwirbel (V) zu gestatten, wenn das Implantat zwischen die Rückenwirbel implantiert wird. - Implantat nach Anspruch 1, bei dem die Buchse (
15 ) eine Vielzahl von Öffnungen (16 ) dort hindurch in Verbindung mit der Außenfläche (13 ) des Körpers für das Einwachsen des Knochens definiert. - Implantat nach Anspruch 1 oder 2, bei dem die Buchse (
15 ) eine Innenfläche (17 ) für das Berühren der Außenfläche (13 ) des Körpers (11 ) umfaßt, wobei die Innenfläche der Buchse eine Kammer (18 ) definiert; wobei die Buchse (15 ) aus einem temperaturempfindlichen Material gebildet wird, so daß die Kammer (18 ) eine erste Innenabmessung aufweist, die etwas größer ist als eine Außenabmessung des Körpers, wenn sich die Buchse in einem erwärmten Zustand befindet, um den Körper darin verschiebbar aufzunehmen; und die Kammer (18 ) eine zweite Innenabmessung aufweist, die etwas kleiner ist als der Körper (11 ), wenn sich die Buchse (15 ) in einem abgekühlten Zustand befindet, um dadurch den Körper darin festzuklemmen. - Implantat nach Anspruch 1 oder 2, bei dem die Buchse (
15 ) umfaßt: ein tieferes Ende (37 ) und ein entgegengesetztes höheres Ende (36 ); und starre Zähne (33 ,34 ), die an jedem des tieferen und höheren Endes der Buchse für einen Eingriff des Implantates mit den Rückenwirbelkörpern angeordnet sind. - Wirbelsäulenimplantat nach Anspruch 1 oder 2, wobei der Körper eine Höhe aufweist, die sich der Höhe eines menschlichen Scheibenzwischenraumes nähert, und wobei das poröse biokompatible Material biologisch abbaubar ist.
- Implantat nach Anspruch 5, das außerdem aufweist: eine Befestigungseinrichtung für das Befestigen der Buchse (
15 ) an den Endplatten der benachbarten Rückenwirbelkörper (V). - Implantat nach Anspruch 6, bei dem: die Buchse ein tieferes Ende und ein höheres Ende umfaßt; und die Befestigungseinrichtung Zähne umfaßt, die am höheren Ende und am tieferen Ende der Buchse angeordnet sind.
- Implantat nach Anspruch 5, bei dem das Implantat eine Druckfestigkeit nach ASTM C-773 von mindestens 7,1 MPa aufweist.
- Implantat nach Anspruch 5, bei dem das Implantat eine Druckfestigkeit von mindestens 20 MPa aufweist.
- Implantat nach Anspruch 5, bei dem das Implantat eine Druckfestigkeit von mindestens 40 MPa aufweist.
- Vorrichtung nach Anspruch 6, bei der die Buchse ein tieferes Ende (
37 ) und ein höheres Ende (36 ) umfaßt; und die Befestigungseinrichtung aufgerauhte Flächen (33 ,34 ) umfaßt, die am höheren Ende und am tieferen Ende der Buchse definiert werden. - Implantat nach einem der vorhergehenden Ansprüche, bei dem die Buchse (
15 ) aus einem Metallmaterial besteht. - Implantat nach Anspruch 12, bei dem das Metallmaterial ein Metall umfaßt, das aus der Gruppe ausgewählt wird, die besteht aus: Tantal; Niob; Hafnium; Wolfram; und deren Legierungen.
- Implantat nach Anspruch 13, bei dem das Metallmaterial Tantalschaum ist.
- Implantat nach Anspruch 12, bei dem das Metallmaterial Titan umfaßt.
- Implantat nach einem der vorhergehenden Ansprüche, bei dem der Körper (
11 ) einen nierenförmigen Querschnitt aufweist, um sich an die Form der Rückenwirbelendplatten anzupassen. - Implantat nach Anspruch 5, bei dem die Höhe des Köpers (
11 ) etwas größer ist als die Höhe eines menschlichen Scheibenzwischenraumes. - Implantat nach Anspruch 5, bei dem der Körper (
11 ) so gestaltet ist, daß er sich einer Größe und Form eines Nukleus pulposus einer natürlichen Zwischenwirbelscheibe nähert. - Implantat nach einem der vorhergehenden Ansprüche, bei dem das biokompatible Material ein Keramikmaterial ist.
- Implantat nach Anspruch 19, bei dem das biokompatible Material ein zweiphasiges Calciumphosphat-Keramikmaterial ist.
- Implantat nach Anspruch 19, bei dem das biokompatible Material ein Hydroxyapatit/Tricalciumphosphat-Keramikmaterial ist.
- Implantat nach Anspruch 19, bei dem das Keramikmaterial eine Porosität von mindestens etwa 50 % aufweist.
- Implantat nach Anspruch 19, bei dem das Keramikmaterial eine Porosität von mindestens etwa 70 % aufweist.
- Implantat nach Anspruch 19, bei dem das Keramikmaterial eine mittlere Porengröße von etwa 200 bis 700 μm aufweist.
