Description
- Hintergrund der Erfindung
- Die minimalinvasive direkte Koronararterienbypass(MIDCAB)chirurgie, sowohl mittels Sternotomie als auch alternativen Inzisionen, ist eine wesentliche revolutionäre Entwicklung auf dem Gebiet der Chirurgie zum Ermöglichen einer Durchführung von Bypasschirurgie am schlagenden Herzen. Die Chirurgie am schlagenden Herzen weist jedoch eine unerwünscht höhere Rate des frühen Versagens von Transplantaten als herkömmliche Koronararterienbypassverfahren unter Verwendung von kardiopulmonalem Bypass und Kardioplegie auf. Die technische Schwierigkeit des Vernähens der Koronararterienanastomose am schlagenden Herzen ist wahrscheinlich ein wichtiger Faktor bei diesem Unterschied im Ergebnis zwischen den beiden Verfahren. Die gesteuerte intermittierende Asystolie (CIA) während kurzen Intervallen, die zur Platzierung von Anastomosenähten benötigt wird, ist zur Verbesserung der Genauigkeit von Koronaranastomosen, die an einem schlagenden Herzen durchgeführt werden, und zur Verminderung von Transplantatversagen, geeignet, während die leichte Durchführbarkeit der Operation erhöht wird.
- Kardiopulmonaler Bypass (CPB) und chemische Herzstillstände unter Verwendung von Kardioplegielösungen stellten traditionellerweise für Chirurgen optimale Operationsbedingungen bereit: hämodynamische Steuerung und Herzstilllegung. Dieses optimale Gebiet steuerte zum technischen Erfolg bei zunehmend komplexen kardiologischen chirurgischen Operationen bei. In letzter Zeit besteht jedoch ein Interesse an der Durchführung von Koronararterienbypasschirurgie ohne entweder vollständigen kardiopulmonalen Bypass oder Kardioplegie. Die Qualität der distalen Anastomose ist ein primä res Anliegen bei den Herzchirurgen, die Koronararterienbypasstransplantat(CABG)verfahren ohne Hilfe von kardioplegischen Stillständen und kardiopulmonalem Bypass observieren und durchführen. Koronararterienbypasstransplantatversagensraten mittels minimalinvasivem direktem Koronararterienbypass werden im Bereich von 3,8 bis 8,9 % berichtet, während traditionelle CABG an CPB mit einer Anastomoseversagensrate von 0,12 % berichtet wird. Dies kann einen Unterschied der Anastomosegenauigkeit zwischen MIDCAB und CPB-unterstützter CABG widerspiegeln. Obwohl die Vorteile der Vermeidung von extrakorporaler Zirkulierung und allgemeiner Kardioplegie bei Verfahren am schlagenden Herzen wichtig sind, überwiegen sie nicht die Leistung einer optimalen Koronaranastomose.
- Der wesentliche Unterschied in den Anastomoseergebnissen zwischen herkömmlicher CABG und CABG am schlagenden Herzen steht im Zusammenhang mit dem Erreichen einer elektiven Asystolie während des Erstellens der distalen Anastomose. Die Herzbewegung kann während den MIDCAB-Verfahren mittels pharmakologischer Bradykardie (Adenosin, β-Blockade) und mechanischer Stabilisierung unter Verwendung verschiedener Vorrichtungen minimiert werden. Obwohl diese Verfahren die Operationsbedingungen verbessern, erreichen sie nur näherungsweise die Vorteile der elektiven Asystolie, die mittels CPB und Kardioplegie erreicht werden.
- Valavanur, Eur. J. Cardio-Thorac. Surg. (1996) 10:1058-1063, offenbart ein Verfahren der Durchführung von Koronararterienbypasschirurgie, während dem die Herzrate des Patienten durch eine Kombination eines β-Adrenorezeptorenblockers (Esmolal) und eines Calciumkanalblockers (CardizemTM) verringert wird.
- Matheny et al, Ann. Thorac. Surg. (1997) 63:528-529, zeigt, dass die direkte intermittierende elektrische Stimulierung des Vagusnervs zusammen mit den Acetylcholinesteraseinhibitor Neostigmin in sicherer Weise kurze Perioden von Kardioplegie induzieren kann.
- Die Anmelder zeigen, dass ein Zustand einer gesteuerten intermittierenden Asystolie (CIA) abseits des CPB erzeugt wird, was einen Hauptvorteil, der sonst durch kardioplegischen Stillstand am CPB erhalten wird, ergibt. Insbesondere wird die CIA unter Verwendung unilateraler (oder bilateraler) Vagusnervenstimulierung in Zusammenhang mit pharmakologischer Unterdrückung der elektromechanischen Ersatzaktivität erreicht.
- Die Anmelder zeigen, dass elektive, gesteuerte, intermittierende Asystolie durch die Vagusnervenstimulierung nach der Behandlung mit einem Acetylcholinesteraseinhibitor, einem β-adrenergen Rezeptorblocker oder einem Calciumkanalblocker oder Kombinationen derselben erzeugt wird.

- Detaillierte Beschreibung der Erfindung
- Erhöhte Acetylcholinaktivität durch Acetylcholinesterasehemmung und Verhinderung der elektromechanischen Ersatzaktivität durch β-Adrenorezeptor- und Calciumkanalblockade während der Vagusstimulierung erzeugt eine deutliche Potenzierung der vagusinduzierten Asystolie und ein Mittel zum Erreichen einer CIA. Die durch pharmakologische Potenzierung einer vagusinduzierten Asystolie erreichte CIA ist ein geeignetes Verfahren zur Erleichterung von MIDCAB-Operationen. Insbesondere Anastomosen und andere komplexe Nahtverfahren werden während derartiger gesteuerter asystolischer Vorgänge erleichtert, was ein gern geschätzter Vorteil bei Chirurgie, die minimalinvasive direkte Koronararterienbypassoperationen am schlagenden Herzen umfasst, ist. Die CIA kann bei partiell oder vollständig endoskopischem CABG und möglicherweise bei perkutaner oder chirurgischer transmyokardialer Laserrevaskularisierung besondere Vorteile besitzen.
- Die vorliegende Erfindung stellt eine pharmazeutische Zusammensetzung, die einen Acetylcholinesteraseinhibitor, einen β-Adrenorezeptorblocker und einen Calcliumkanalblocker umfasst, bereit, wobei diese Zusammensetzung zur Durchführung von Chirurgie am schlagenden Herzen verwendbar ist. Die Erfindung stellt auch die Verwendbarkeit der Zusammensetzung für gesteuerte intermittierende Asystolie bei minimalinvasiver direkter Koronararterienbypasschirurgie bereit. Die Erfindung stellt weiter die mögliche Verabreichung der Zusammensetzungen in Kombination mit Vagusnervenstimulierung bereit. Vagusnervenstimulierung kann durch direkte oder indirekte elektrische Stimulierung erreicht werden.
- Bei bevorzugten unabhängigen Ausführungsformen kann der Acetylcholinesteraseinhibitor Pyridostigminbromid sein, der β-Adrenorezeptorblocker Propranololhydrochlorid sein und der Calciumkanalblocker Verapamilbromid sein.
- Die grundsätzliche Herausforderung der CABG-Chirurgie am schlagenden Herzen war die Nachahmung der vorteilhaften Operationsbedingungen eines ruhigen Operationsfeldes, die während herkömmlicher CABG mit CPB und kardioplegischem Stillstand bereitgestellt werden. Eine Vielzahl von pharmakologischen Manipulationen und mechanischen Stabilisierungsverfahren helfen bei der Durchführung der CABG ohne Pumpen. Diese Eingriffe minimieren zur Zeit die Bewegung des Herzens, beseitigen diese jedoch nicht. Das Konzept, dass ein Zustand einer gesteuerten intermittierenden Asystolie die Bedingungen zur Konstruktion einer distalen Koronararterienbypassanastomose bei nicht-CPB-assistierten Fällen verbessert, wurde durch die Anmelder dargestellt. Die CIA wird als vom Chirurgen initiierte und gesteuerte Intervalle eines mechanischen Herzstillstands definiert. Diese Intervalle können so abgestimmt werden, dass sie mit der Setzung von Nähten an der Anastomose zusammenfallen, wonach normaler Herzrhythmus und Hämodynamik wieder hergestellt werden, während die Vorbereitungen für den nächsten folgenden Stich gemacht werden. Experimente, die von dem Anwender berichtet wurden, zeigen, dass eine geringe Bradykardie, von der bekannt ist, dass sie durch Vagusnervenstimulierung erzeugt wird, durch pharmakologische Hemmung von Acetylcholinesterase und Blockade von β-Adrenorezeptoren und Calciumkanälen dramatisch zur Funktion als ein elektromechanischer "Ein-Aus-Schalter" erhöht wird. Gesteuerte intermittierende Asystolie kann sich als genauso verwendbar für CPB-assistierte Herzchirurgie ohne allgemeine Kardioplegie erweisen.
- Die chronotropen Wirkungen der Vagusnervenstimulierung wurden gut beschrieben und erzeugen typischerweise eine erste Pause, gefolgt durch einen "Vagus-Ersatz"-Schlag und auf rechterhaltene Bradykardie während der kontinuierlichen optimalen Stimulierung des Vagusnervs. Kardiologische Antworten auf eine 60-s-Vagusstimulierung ohne unterstützende Therapie erreichten eine durchschnittliche Pause von 1,6 s, die durch Vagus-Ersatz-Schläge beendet wurde, mit einer 19%igen Verringerung der Herzfrequenz. Die alleinige Stimulierung des Vagusnervs erzeugte keine gesteuerte Asystoliezeitspanne, die für die CIA erwünscht ist. Im Gegensatz dazu hemmte ein dreifaches pharmakologisches Verfahren mit z.B. Pyridostigmin, Propranolol und Verapamil den Vagus-Ersatzsystolie, und ermöglichte aufrechterhaltene Zeitspannen der Asystolie, die bis zu 60 s andauerten, und anschließender Asystolien von jeweils 15 s. Segmentierte Asystolien besaßen keine signifikanten hämodynamischen Konsequenzen.
- Es ist offensichtlich, dass die Unterdrückung der elektromechanischen Ersatzsystolie während der Vagusstimulierung zur Erzeugung eines ausreichenden Intervalls der Asystolie notwendig ist, um Zeit zu ermöglichen während der ein Einzelstich verlässlich während der Konstruktion einer distalen CABG-Anastomose platziert werden kann. Die negativen chronotropen Wirkungen der Vagusstimulierung werden durch Acetylcholinfreisetzung erzeugt. Die Acetylcholinaktivität kann durch Hemmung von Acetylcholinesteraseaktivität durch Mittel wie Pyridostigmin verstärkt werden. Zudem ist bekannt, dass die Calciumkanalblockade durch beispielsweise Verapamil die negative chronotrope Wirkung der Vagusnervenstimulierung potenziert. Eine weitere Komponente der elektromechanischen Ersatzsystolie kann mit einer erhöhten Catecholaminaktivität im sympathischen Nervensystem, die durch Hypotonie ausgelöst wird, zusammenhängen. Catecholamine erhöhen die Rate der diastolischen Depolarisierung und verringern das Schwellenpotential. Die β-Adrenorezeptorblockade mittels beispielsweise Propranolol vermindert die Wirkungen der Catecholaminaktivität und unterstützt die Unterdrückung elektromechanischer Ersatzsystolie.
- Die Verabreichung dieser Kombinationstherapie erzeugte eine signifikante Verringerung der Herzfrequenz und maximal entwickelten Ventrikulardruck zusammen mit einer Erhöhung des linksventrikulären enddiastolischen Drucks ohne jedoch den mittleren Arteriendruck zu verändern. Es zeigte sich keine offensichtliche Ermüdung dieser pharmakologischen Wirkung nach aufeinanderfolgenden Stimulierungen. Die Tiere, die für diese Pilotexperimente verwendet wurden, schienen dieses pharmakologische Verfahren ohne andere nachteilige hämodynamische Nebenwirkungen, wie Azidose, zu tolerieren.
- Die kurzzeitigen hämodynamischen Wirkungen einer einzelnen längeren Stimulierung wurden als im wesentlichen unbedeutend ermittelt. Auf ähnliche Weise waren die metabolischen Konsequenzen, die durch pH-Wert- und Veränderungen im Basendefizit ermittelt wurden, unbedeutend.
- Das pharmakologische Verfahren, das in dieser Untersuchung verwendet wurde, hielt die Zeitspanne der vagusinduzierten Asystolie etwa 60 min aufrecht. Dieses Intervall würde mehr als ausreichend Zeit für die Konstruktion einer distalen CABG-Anastomose ermöglichen. Tiere, die während 2 h nach der Verabreichung von Arzneimitteln überwacht wurden, wiesen ähnliche Reaktionen auf Vagusstimulierungen gegenüber denjenigen in einem Zustand ohne Arzneimittelbehandlung auf, wobei die Umkehrbarkeit der Arzneimittelwirkungen bestätigt wurde.
- Eine bedauerliche Wirkung des pharmakologischen Verfahrens, die vor der klinischen Anwendung in Betracht gezogen werden muss, sind vagusinduzierte Sekretionen. Alle Tiere zeigten bedeutende Speichelproduktion nach Initiierung der Vagus stimulierung. Es gab jedoch keine Probleme hinsichtlich Oxygenierung und Belüftung aufgrund von tracheobronchialen Sekretionen in diesen Experimenten. Vagusinduzierte oropharyngeale und tracheobronchiale Sekretionen sind in dem klinischen Rahmen sachdienlich. Zudem benötigen die Wirkungen auf die Funktionen des Nervus laryngeus recurrens Beachtung.
- Hinweise suggerieren, dass die Langzeitwirkungen dieses Vorgangs am Vagusnerv nicht schädlich sind. Chronische Vagusnervenstimulierung wurde als Therapie für renitente Krampfanfallstörungen ohne offensichtliche Nervenschädigung oder verletzter Funktion verwendet. Die Anmelder zeigten, dass eine vagusvermittelte chronotrope Steuerung 2 h nach Beendigung des experimentellen Protokolls ähnlich dem nicht-arzneimittelbehandelten Zustand war.
- Zusammengefasst kann die gesteuerte intermittierende Asystolie durch Potenzierung einer vagusinduzierten Asystolie über eine pharmakologische Kombinierung von z.B. Propranolol und Verapamil zur Unterdrückung des elektromechanischen Ersatzsystolie und beispielsweise Pyridostigmin zur Hemmung der Acetylcholinesterase erreicht werden. Die Asystolie kann über längere Intervalle und über kürzere mehrfache aufeinanderfolgende Intervalle unter Verwendung dieses Verfahrens erreicht werden.
- Nervenstimulierung
- Um eine beständige Asystolie zu erreichen, ermittelten die Anmelder, dass die Stimulierung des rechten Vagusnervs vor oder nach einer Behandlung mit den pharmakologischen Kombinationen der vorliegenden Erfindung bevorzugt ist.
- Die elektrische Stimulierung wird am rechten Vagusnerv, vorzugsweise an einer Stelle am Hals, durchgeführt. Andere geeignete Stellen zur Vagusnervenstimulierung umfassen, ohne hierauf beschränkt zu sein, unipolare oder bipolare elektrische Stimulierung des rechten oder linken Vagus oder beider, Stimulierung des Vagus in der Brust nach Sternotomie, Stimulierung mit einem perkutanen Katheter oder einer Elektrodensonde in der inneren Jugularisvene, Ösophagus oder Trachea oder einer Kombination von diesen. Der Nervenstimulator ist typischerweise ein "Grass Wire" mit einer einzigen Kontaktstelle, andere geeignete Stimulatoren umfassen jedoch ein Paar Schrittmacherdrähte oder Elektroden, die etwa 1 cm auseinander platziert sind, um eine bipolare Prodromalstimulierung zu ermöglichen. Ein einzelner kontinuierlicher Impuls wird zwischen etwa 5 s bis etwa 90 s, vorzugsweise zwischen etwa 5 s und etwa 15 s angewendet, um einen Einzelstich während der Chirurgie zu ermöglichen. Die Impulsparameter können ohne weiteres verändert werden, beispielsweise ein Frequenzbereich von zwischen etwa 1 Hz und etwa 500 Hz, zweckmäßigerweise zwischen etwa 20 Hz und etwa 80 Hz, vorzugsweise etwa 40 Hz mit einer Amplitude zwischen etwa 1 bis etwa 40 V.
- Pharmakologische Potenzierung
- Der Acetylcholinesteraseinhibitor ist auch als ein Cholinesteraseinhibitor bekannt. Geeignete Acetylcholinesteraseinhibitoren umfassen, ohne hierauf beschränkt zu sein, Tacrinhydrochlorid, Pyridostigminbromid, Neostigminmethylsulfat und Edrophoniumchlorid. Ein bevorzugter Acetylcholinesteraseinhibitor ist Pyridostigminbromid. Acetylcholinesteraseinhibitoren werden in einem Dosierungsbereich zwischen etwa 0,01 mg/kg und etwa 100 mg/kg, zweckmäßigerweise zwischen etwa 0,1 mg/kg und etwa 2,0 mg/kg und vorzugsweise etwa 0,5 mg/kg verabreicht.
- Der β-Adrenorezeptorblocker ist auch als ein β-adrenerges Blockierungsmittel bekannt. Geeignete β-Adrenorezeptorblocker umfassen, ohne hierauf beschränkt zu sein, Sotalol HCl, Timololmaleat, Esmololhydrochlorid, Carteololhydrochlorid, Propranololhydrochlorid, Betaxololhydrochlorid, Penbutololsulfat, Metoprololtartrat, Acetbutololhydrochlorid, die Kombination von Atenolol und Chlorthalidon, Metropsololsuccinat, Pindolol und Bisoprololfumarat. Ein bevorzugter β-Adrenorezeptorblocker ist Propranololhydrochlorid. β-Adrenorezeptorblocker werden in einem Dosisbereich zwischen etwa 0,01 mg/kg und etwa 100 mg/kg, zweckmäßigerweise zwischen etwa 0,1 mg/kg und etwa 2,0 mg/kg und vorzugsweise etwa 80 μg/kg verabreicht.
- Geeignete Calciumkanalblocker umfassen, ohne hierauf beschränkt zu sein, Nifedipin, Nicardipinhydrochlorid, Diltiazem HCl, Isradipin, Verapamilhydrochlorid, Nimodinpin, Amlodipinbesylat, Felodipin, Bepridilhydrochlorid und Nisoldipin. Ein bevorzugter Calciumkanalblocker ist Verapamilhydrochlorid. Calciumkanalblocker werden in einem Dosierungsbereich von zwischen etwa 1 mg/kg bis etwa 1000 mg/kg, zweckmäßigerweise zwischen etwa 10 mg/kg und etwa 200 mg/kg, vorzugsweise etwa 50 μg/kg verabreicht.
- Es ist klar, dass andere Dosierungskombinationen wirksam sein können. Die geeignete Dosierung wird durch das Alter, das Gewicht, Geschlecht, den Gesundheitszustand des Patienten bestimmt und kann mit einer Vielzahl anderer Faktoren gemäß herkömmlicher klinischer Praxis variieren.
- BEISPIEL 1
- Experimentelle Vorbereitung
- Die Schafe in den Beispielen der vorliegenden Erfindung erhielten menschliche Pflege in Übereinstimmung mit "Principles of Laboratory Animal Care", die durch die National Society for Medical Research formuliert wurden, und dem "Guide for Care and Use of Laboratory Animals", der von der National Academy of Sciences vorbereitet und von den National Institutes of Health veröffentlicht wurde (NIH Veröffentlichungsnummer 80-23, 1985 wieder bearbeitet). Das experimentelle Protokoll wurde durch das Institutional Animal Care and Use Committee of Emory University gebilligt.
- Sieben Schafe, die 44 bis 45 kg wogen, wurden mit Xylazin (0,1 mg/kg) und Atropin (0,2 mg/kg) 30 min vor der Einleitung der Anästhesie mit intravenösem Thiopental (2,2 mg/kg) und Lidocain (2,2 mg/kg) prämediziert. Die Tiere wurden endotracheal intubiert und an einem Volumenventilator mit Isofluran zur Aufrechterhaltung der Anästhesie platziert. Gliedmaßenableitungen und eine präkardiale Ableitung wurden zur elektrokardiographischen Überwachung platziert. Die rechte Femoralarterie wurde zur arteriellen Blutdruck- und arteriellen Blutgasüberwachung mit einer Kanüle versehen. Das Atemvolumen wurde auf 10 cm3/kg und eine Rate von 12 Atemzügen pro Minute eingestellt, wobei Anpassungen zur Aufrechterhaltung eines pH-Werts bei 7,35 bis 7,45, von pO2 größer als 100 mmHg und pCO2 zwischen 35-45 mmHg gemacht wurden.
- Eine rechte cervikale Inzision wurde durchgeführt, der Vagusnerv wurde vorsichtig freigelegt und eine Nervenstimulationssonde (Harvard Apparatus, South Natick, MA) wurde an dem Nerv platziert. Eine mediane Sternotomie wurde zur Freilegung des Herzens durchgeführt. Ein Festkörper-Mikromanometer hoher Genauigkeit (Millar Inc., Houston, TX) wurde in der aufsteigenden Aorta zur Überwachung des Aortenblutdrucks befestigt. Ein weiteres Mikromanometer wurde in den linken Ventrikel durch die Herzspitze zur links ventrikulären Drucküberwachung eingeführt.
- BEISPIEL 2
- Versuchsprotokoll
- Jedes Tier erfuhr vor und nach der Arzneimittelverabreichung eine Vagusstimulierung. Das pharmakologische Verfahren bestand aus Pyridostigmin (0,5 mg/kg) zur Acetylcholinesterasehemmung, Propranolol (80 μg/kg) zur β-Adrenorezeptorblockade und Verapamil (50 μg/kg) zur Calciumkanalblockade. Vagusstimulation wurde mit einem Nervenstiumulator (Grass Instrument Co., Quincy, MA) im monopolaren Modus mit einer Frequenz von 40 Hz, einer Impulsdauer von 0,4 ms und einer Amplitude von 2-6 V durchgeführt. Vagale Stimulierungen wurden in zwei Verfahren verabreicht:
- 1) kontinuierlicher 60-s-Impuls und
- 2) aufeinanderfolgende 15-s-Impulse.
- Die kontinuierliche 60-s-Stimulierung wurde zur Bestimmung der Langlebigkeit der vagusinduzierten Asystolie und der physiologischen Wirkungen der verlängerten vagusinduzierten Hypotonie entwickelt. Die aufeinanderfolgenden 15-s-Vagusstimulierungen wurden zur Simulierung der Nahtintervalle, die für Transplantatanastomosen benötigt werden, und zur Bestimmung, ob kardiologische Ermüdung, elektromechanische Ersatzsystolie und physiologische Wirkungen unter diesen praktischen Bedingungen auftreten, durchgeführt.
- BEISPIEL 3
- Sammeln von Daten und Analyse
- Elektrokardiographische und hämodynamische Daten wurden mittels einer analogdigital-Umwandlungstabelle gesammelt (Data Translation, Inc., Marlboro, MA) und mittels eines Mikroprozessors eines Personal 486 Computer (Compaq Computer Corp., Houston, TX) unter Verwendung interaktiver Markensoftware (SpectrumTM, Triton Technology, San Diego, CA) verarbeitet, gespeichert und analysiert. Das System wurde so konfiguriert, dass es 4 Kanäle physiologischer Daten bei einer Frequenz von 50 Hz (ausreichend für langsam-wellige Wellenformen und mittlere Druckdaten) über eine 200 s Zeitspanne, die die 60 s Stimulierung oder die aufeinanderfolgende 15-s-Abfolge von Stimulierungen umfasste, sammelte. Die Software ermöglichte anschließende videographische Wiedergabe und Analyse der hämodynamischen Daten.
- BEISPIEL 4
- Ergebnisse
- Vor der Arzneimittelverabreichung erzeugte die Vagusstimulierung über 60 s eine kurze Pause der elektromechanischen Aktivität (1,6 ± 0,9 s) und anschließend eine vagale Ersatzsystolie und Wiederaufnahme des Sinusrhythmus mit einer Verminderung der Herzfrequenz um 19,4 ± 11,9 % im Vergleich zur Herzfrequenz vor der Stimulierung. Auf ähnliche Weise erzeugte eine aufeinanderfolgende 15-s-Vagusstimulierung, die zur Stimulierung der Nahtintervalle, die für CABG-Anastomosen benötigt wird, durchgeführt wurde, einen kurze Pause (1,1 ± 0,4 s) und anschließend eine Vagus-Ersatzsystolie und einen Sinusrhythmus mit einer Reduktion der Herzfrequenz von 37 ± 6 %.
- Die Verabreichung des pharmakologischen Verfahrens (Propranolol, Verapamil, Pyridostigmin) verringerte die Herzfrequenz und erhöhte den linksventrikulären enddiastolischen Druck, beeinflusste jedoch nicht den mittleren arteriellen Druck oder das Maximum von dP/dt, wie in Tabelle 1 dargestellt.
- Nach der Arzneimittelverabreichung erzeugte eine 60-s-Vagusstimulierung eine Asystolie von im Schnitt 52 ± 5,6 s. Die individuellen Antworten der Tiere vor und nach Arzneimittelverabreichung sind in
1 dargestellt. Sechs Tiere erreichten eine gesteuerte Asystolie. Fünf dieser sechs erreichten eine gesteuerte Asystolie über mehr als 50 s. Die Wirkungen von 60-s-Vagusstimulierung vor und nach Arzneimittelbehandlung in den ansprechenden Tieren wurden mittels repräsentativer Aufzeichnungen des linksventrikulären und Aortendrucks gegeneinander dargestellt und für ein repräsentatives Experiment in2 dargestellt. Vor der Arzneimittelverfahrenbehandlung erzeugte die Vagusstimulierung keine deutliche Veränderung des Herzrhythums oder der Hämodynamik. Im Gegensatz dazu erleichterte das dreifache Arzneimittelverfahren eine beständige Asystolie und Kreislaufstillstand, bis der Stimulus entzogen wurde, wonach die Hämodynamik schnell auf Werte vor der Stimulierung zurückgeführt wurde. Die verzögerte Asystolie und der Kreislaufstillstand erzeugten keine signifikanten Unterschiede der hämodynamischen Parameter, die vor und nach der arzneimittelunterstützten 60-s-Vagusstimulierung gemessen wurden (Tabelle 2). - Auf ähnliche Weise bestand kein Unterschied der Parameter, die durch arterielle Blutgaswerte 1 und 5 min nach der 60-s-Stimulierung gemessen wurden, im Vergleich zu Werten vor der Stimulierung (Tabelle 3).
- Fünf bis sechs aufeinanderfolgende 15-s-Vagusstimulierungen während des arzneimittelbehandelten Zustands erzeugten beständige und stabile Asystolie (
3 ). Drei der sechs Tiere wiesen einen einzigen Ersatzschlag während einer der 15-s-Stimulierungen auf. Die anderen drei wiesen eine vollständige Asystolie während jeder der 15-s-Stimulierungen auf. Ein kardiologischer Rhythmus, der aufrechterhalten wurde, begann im Durchschnitt 5,3 ± 1,8 s nach Beendigung jedes 15-s-Impulses, wobei während diesem Intervall oft ein einzelner Schlag unmittelbar nach Entzug der Stimulierung beobachtet wurde. - Während die vorangegangenen Ausführungen die Prinzipien der vorliegenden Erfindung durch Beispiele, die zum Zwecke der Darstellung bereitgestellt werden, lehren, ist klar, dass die Durchführung der Erfindung alle gewöhnlichen Variationen, Anpassungen und Modifikationen, die innerhalb des Um fangs der folgenden Ansprüche und deren Äquivalenten liegen, umfasst.
Claims (12)
- Pharmazeutische Zusammensetzung, die einen Acetylcholinesteraseinhibitor, einen β-Adrenorezeptorenblocker und einen Calciumkanalblocker umfasst, wobei die Zusammensetzung zum Hervorrufen von gesteuerter intermittierender vagal induzierter Asystolie während einer Herzoperation verwendbar ist.
- Zusammensetzung nach Anspruch 1, wobei der Acetylcholinesteraseinhibitor Pyridostigminbromid ist.
- Zusammensetzung nach Anspruch 2, wobei der Acetylcholinesteraseinhibitor in einem Mengenbereich zwischen etwa 0,01 mg/kg und etwa 100,0 mg/kg, vorzugsweise zwischen etwa 0,1 mg/kg und etwa 2,0 mg/kg vorhanden ist.
- Zusammensetzung nach Anspruch 1, wobei der β-Adrenorezeptorenblocker Propranololhydrochlorid ist.
- Zusammensetzung nach Anspruch 4, wobei der β-Adrenorezeptorenblocker in einem Mengenbereich zwischen etwa 0,01 mg/kg und etwa 100,0 mg/kg, vorzugsweise zwischen etwa 0,1 mg/kg und etwa 2,0 mg/kg vorhanden ist.
- Zusammensetzung nach Anspruch 1, wobei der Calciumkanalblocker Verapamilbromid ist.
- Zusammensetzung nach Anspruch 6, wobei der Calciumkanalblocker in einem Mengenbereich zwischen etwa 1 mg/kg und etwa 1000 mg/kg, vorzugsweise zwischen etwa 10 mg/kg und etwa 200 mg/kg vorhanden ist.
- Pharmazeutische Zusammensetzung, die den Acetylcholinesteraseinhibitor Pyridostigminbromid, den β-Adrenorezeptorenblocker Propranololhydrochlorid und den Calciumkanalblocker Verapamilbromid umfasst, wobei die Zusammensetzung für gesteuerte intermittierende Asystolie bei einer minimal invasiven direkten Koronararterienbypass-Herzoperation, Koronararterienbypass-Transplantationsoperation mit Pumpenabschaltung oder einer an einem kardiopulmonalen Bypass durchgeführten Koronararterienbypassoperation verwendbar ist.
- Zusammensetzung nach Anspruch 8, wobei der Acetylcholinesteraseinhibitor in einem Mengenbereich zwischen etwa 0,01 mg/kg und etwa 100,0 mg/kg vorhanden ist, der β-Adrenorezeptorenblocker in einem Mengenbereich zwischen etwa 0,01 mg/kg und etwa 100,0 mg/kg vorhanden ist und der Calciumkanalblocker in einem Mengenbereich zwischen etwa 1 mg/kg und etwa 1000 mg/kg vorhanden ist.
- Zusammensetzung nach Anspruch 9, wobei der Acetylcholinesteraseinhibitor in einem Mengenbereich zwischen etwa 0,1 mg/kg und etwa 2,0 mg/kg vorhanden ist, der β-Adrenorezeptorenblocker in einem Mengenbereich zwischen etwa 0,1 mg/kg und etwa 2,0 mg/kg vorhanden ist und der Calciumkanalblocker in einem Mengenbereich zwischen etwa 10 mg/kg und etwa 200 mg/kg vorhanden ist.
- Verfahren zur Herstellung einer pharmazeutischen Zusammensetzung gemäß einem der Ansprüche 1 bis 10 durch Kombinieren pharmazeutisch aktiver Mengen eines Acetylcholinesteraseinhibitors, β-Adreno rezeptorenblockers, Calciumkanalblockers und eines pharmazeutisch akzeptablen Streckmittels, Verdünnungsmittels oder Trägers.
- Verwendung einer pharmazeutischen Zusammensetzung gemäß einem der Ansprüche 1 bis 10 bei der Herstellung eines Medikaments zur Verwendung beim Hervorrufen einer gesteuerten intermittierenden vagal induzierten Asystolie während einer Herzoperation.
