Description
- Hintergrund der Erfindung
- Technisches Gebiet
- Die vorliegende Erfindung bezieht sich auf ein knochenabgeleitetes Implantat, welches zumindest teilweise aus Festigkeits-verleihendem kortikalen Knochen hergestellt ist und zur Verwendung bei der Reparatur, dem Austausch und/oder der Augmentierung verschiedener Teile des tierischen oder menschlichen Skelettsystems vorgesehen ist. Genauer gesagt bezieht sich diese Erfindung auf ein knochenabgeleitetes Implantat, das aus zwei oder mehr Schichten besteht, von denen zumindest eine vollmineralisierter oder teilweise demineralisierter kortikaler Knochen ist, und optional eine oder mehrere Schichten, die aus einem anderen Material hergestellt sind. Die das Implantat bildenden einzelnen Schichten werden in eine einheitliche Struktur zusammengefügt, die zum Tragen von Lasten in der Lage ist.
- Beschreibung des verwandten Stands der Technik
- Die Verwendung von Autoimplantatknochen, Alloimplantatknochen oder Xenoimplantatknochen ist sowohl in menschlicher als auch in der Veterinärmedizin bekannt. Siehe Stevenson et al., Clinical Orthopedics and Related Research, 323, S. 66–74 (1996). Insbesondere ist bekannt, dass transplantierter Knochen Stütze bereitstellt, Heilung fördert, Knochenhöhlungen verfüllt, Knochenelemente wie etwa Wirbelkörper trennt, Verschmelzung fördert und Knochenbruchstellen stabilisiert. In jüngerer Zeit ist verarbeiteter Knochen in Formen zur Verwendung bei neuen chirurgischen Anwendungen oder als neue Materialien für Implantate, die historisch aus nicht biologisch abgeleiteten Materialien hergestellt waren, entwickelt worden.
- US-Patent Nr. 4,678,470 beschreibt ein nicht-geschichtetes Knochenimplantatmaterial, das aus Knochen durch ein Verfahren hergestellt wird, das ein Gerben mit Glutaraldehyd beinhaltet. Der Knochen kann pulverisiert werden, als ein großer Block verwendet oder in einer präzisen Form bearbeitet werden. Das Gerben stabilisiert das Material und macht es zudem nicht-antigen. Das Knochenmaterial kann auch demineralisiert werden.
- Die Verwendung einer kontinuierlichen Platte demineralisierten Knochens oder teildemineralisierten Knochens ist im US-Patent Nr. 5,556,430 beschrieben; jedoch muss die Platte hinreichend flexibel sein, somit Festigkeit opfernd, um sich an den Skelettort anzupassen, an dem sie angewendet wird.
- Die in US-Patent Nr. 5,507,813 beschriebene chirurgische implantierbare Platte wird aus länglichen Knochenpartikeln, die optional demineralisiert sind, gebildet, die biokompatible Zutaten, Adhäsive, Füllmaterialien, Weichmacher etc. enthalten.
- US Patent Nr. 4,932,973 offenbart eine künstliche organische Knochenmatrix mit Löchern oder Perforationen, die sich in die organische Knochenmatrix erstrecken. Diese Löcher oder Perforationen sind nach Implantation der Knochenmatrix als Zentren der Knorpel- und Knocheninduktion angezeigt.
- US Patent Nr. 4,394,370 offenbart ein einteiliges schwammartiges Knochenimplantatmaterial, das aus volldemineralisiertem Knochenpulver oder mikropartikulärem Knochen und rekonstituiertem Kollagen hergestellt wird. Das schwammartige Implantat wird optional mit Glutaraldehyd quervernetzt.
- Ein anderes einstückiges poröses Implantat ist im US-Patent Nr. 5,683,459 beschrieben. Das Implantat besteht aus einer biodegradierbaren Polymermakrostruktur, die als ein zwischenverbindendes offenzelliges Netzwerk strukturiert ist, und einer biodegradierbaren polymeren Mikrostruktur, die aus chemotaktischen gemahlenen Substanzen wie etwa Hyaluronsäure zusammengesetzt ist.
- GB-A-2 175 506 bezieht sich auf eine Prothese zum Ersatz von künstlichem Knorpel. Die Prothese besteht aus einem einzelnen Knochenstück, das einen nicht demineralisierten Teil und einen demineralisierten Teil umfasst. Der vollständig demineralisierte Teil hat eine schwammige Struktur ähnlich der von natürlichem Knorpel, während der nicht demineralisierte Teil eine Matrix bereitstellt, in welche angrenzender Wirtsknochen einwachsen kann.
- WO-A-88/03417 bezieht sich auf ein Biokompositmaterial, das bei knochenchirurgischen Anwendungen nützlich ist. Die Kompositmaterialien umfassen zumindest eine biokeramische Komponente und zumindest eine Polymer-enthaltende Komponente. Diese zwei Materialtypen können gestapelt werden, um eine Schichtstruktur zu bilden, in der die zwei unterschiedlichen Materialtypen alternierend auftreten.
- Zusammenfassung der Erfindung
- Die vorliegende Erfindung stillt ein lange bestehendes Bedürfnis auf dem Gebiet, indem ein knochenabgeleitetes Implantat bereitgestellt wird, das zum Aufnehmen von Lasten und, bei einer bevorzugten Ausführungsform, durch seine knochenheilende Aktivität und Fähigkeit, medizinisch/chirurgisch nützliche Substanzen am Operationsort zu inkorporieren, zum Fördern und/oder Beschleunigen neuen Knochenwachstums in der Lage ist.
- Es ist daher eine Aufgabe der vorliegenden Erfindung, ein knochenabgeleitetes Implantat bereitzustellen, das aus einer Mehrzahl von überlagerten Schichten, die alle zu einer einheitlichen Struktur aneinander befestigt sind und Kompressionsfestigkeitseigenschaften besitzen, die sich jenen natürlichen Knochens annähern.
- Es ist eine andere Aufgabe der Erfindung, ein Knochenimplantat bereitzustellen, das ein Netzwerk von Poren, Öffnungen, Perforationen, Kanälen oder Räumen enthält, das Penetration von endogenen und exogenen knochenheilenden Materialen und Blutzufuhr gestattet und fördert und gleichzeitig ein Mittel zum Inkorporieren einer oder mehrerer knochenheilender Substanzen bereitstellt.
- Es ist noch eine weitere Aufgabe der vorliegenden Erfindung, ein knochenabgeleitetes Implantat bereitzustellen, das in einer Vielzahl von Formen und Größen angefertigt werden kann, die nicht durch solche Beschränkungen limitiert sind, welche durch die normalen anatomischen Größen und/oder Arten von Spenderknochen auferlegt sind, welche für die Konstruktion des Implantats verfügbar sind.
- In Übereinstimmung mit diesen und anderen Aufgaben der Erfindung wird ein knochenabgeleitetes Implantat wie in Anspruch 1 definiert bereitgestellt.
- Das knochenabgeleitete Implantat der vorliegenden Erfindung besitzt einen signifikanten Vorteil gegenüber Knochen- und knochenabgeleiteten Implantaten des Stands der Technik hinsichtlich seiner Fähigkeit, sich mechanischen Festigkeitseigenschaften natürlichen Knochens anzunähern und über die Zeit einen graduellen Transfer der lastaufnehmenden Stützung davon zu neu gebildetem Knochengewebe zu gestatten.
- Ein anderer wichtiger Vorteil des knochenabgeleiteten Implantats hierin gegenüber Implantaten des Stands der Technik liegt in seiner Fähigkeit, als Träger für eine oder mehrere medizinisch/chirurgisch nützliche Substanzen zu dienen, die neues Knochenwachstum fördern und/oder die Heilung beschleunigen, zu dienen und effektiv zu inkorporieren.
- Der Ausdruck "osteogen", wie hierin verwendet, soll so verstanden werden, dass er auf die Fähigkeit einer Substanz Bezug nimmt, eine neue Knochenbildung über die Beteiligung lebender Zellen von innerhalb der Substanz aus zu induzieren.
- Der Ausdruck "osteokonduktiv", wie hier verwendet, soll so verstanden werden, dass er sich auf die Fähigkeit einer Substanz oder eines Materials bezieht, biologisch inerte Oberflächen bereitzustellen, welche für das Wachstum von neuem Knochengewebe rezeptiv sind.
- Der Ausdruck "osteoinduktiv", wie hier verwendet, soll so verstanden werden, dass er sich auf die Fähigkeit einer Substanz bezieht, Zellen des Wirts, die das Potential zum Reparieren von Knochengewebe aufweisen, zu rekrutieren.
- Kurze Beschreibung der Zeichnungen
- Verschiedene Ausführungsformen werden unten unter Bezugnahme auf die Zeichnungen beschrieben, in denen:

6 eine Teilansicht der menschlichen Wirbelsäule ist, welche die Installation des knochenabgeleiteten Implantats von5 an einem intervertebralen Ort zeigt. - Detaillierte Beschreibung der bevorzugten Ausführungsformen
- Das knochenabgeleitete Implantat der vorliegenden Erfindung umfasst zumindest zwei einander überlagernde Schichten, wobei zumindest eine der Schichten eine Kompressionsfestigkeitverleihende Schicht ist, die aus nicht demineralisiertem kortikalen Knochen oder kortikalem Knochen, der nur partiell demineralisiert worden ist, abgeleitet worden ist. Wenn als eine Kompressionsfestigkeit verleihende Schicht vorhanden, sollte eine partiell demineralisierte Kortikalknochenschicht einen ziemlich hohen Prozentsatz der Kompressionsfestigkeit einer vergleichbaren Schicht nicht demineralisierten kortikalen Knochens zeigen, z.B. vorzugsweise zumindest etwa 40, bevorzugtererweise zumindest etwa 50 und noch bevorzugtererweise zumindest etwa 60 % einer solchen Festigkeit. Schichten von partiell demineralisierten kortikalen Knochen, die signifikant weniger als etwa 40% der Kompressionsfestigkeit einer vergleichbaren Schicht von vollmineralisierten kortikalen Knochen zeigen, können im knochenabgeleiteten Implantat optional eingesetzt werden, aber in einer anderen Funktion als einer lastaufnehmenden. Abhängig von der Dicke der Schichten kann es im knochenabgeleiteten Implantat insgesamt zwischen zwei und etwa 200 Schichten geben. Das knochenabgeleitete Implantat kann Schichten verschiedener Dicken beinhalten, z.B. können die Kompressionsfestigkeit-verleihenden Schicht(en) beachtlich dicker oder dünner als jegliche optionalen Schicht(en) sein, die vorhanden sein können. Dicken, die von etwa 0,5 bis etwa 20, und vorzugsweise von etwa 1,5 bis etwa 15 mm reichen, können vorteilhafterweise verwendet werden. Im Allgemeinen ist die Anzahl und Dicke der Kompressionsfestigkeits-verleihenden Schichten in einem gegebenen knochenabgeleiteten Implantat so, dass sie eine Gesamtkompessionsfestigkeit für das Implantat von etwa 25 bis etwa 250 und vorzugsweise von etwa 100 bis etwa 200 MPa bereitstellen.
- Vorzugsweise sind die Quellen kortikalen Knochens für das knochenabgeleitete Implantat dieser Erfindung allogen, beinhalten jedoch auch xenogene Quellen wie etwa Rinder- oder Schweineknochen. Wenn partiell oder voll demineralisierter kortikaler Knochen verwendet wird, kann ein solcher Knochen durch Einsetzen bekannter Demineralisierungstechniken erhalten werden, z.B. solche, die starke Säuren wie etwa Salzsäure einsetzen, wie in Reddi et al., Proc. Nat. Acad. Sci. 69, S. 1601–5 (1972) beschrieben. Das Ausmaß der Demineralisierung ist eine Funktion der Stärke der Säurelösung, der Knochenform und der Dauer der Demineralisierungsbehandlung. In dieser Hinsicht wird Bezug genommen auf Lewandrowski et al., J. Biomed Materials Res., 31, S. 365–372 (1996). Die Verwendung von partiell oder volldemineralisierten Knochen kann hierbei günstig sein, da solche Substanzen eine größere anfängliche osteogene und/oder osteoinduktive Aktivität zeigen als volldemineralisierter Knochen.
- Die Kompressionsfestigkeit-verleihende(n) Schicht(en) des knochenabgeleiteten Implantats können als monolithische Abschnitte von Knochen oder als multisektionale Schichten, die aus zwei oder mehr Unterabschnitten gebildet sind, z.B. miteinander in einer Kante-an-Kante-Weise in einer Weise befestigt sein, die einer Beplanking analog ist, bereitgestellt werden. Auf diese Weise können relativ große Kompressionsfestigkeits-verleihende Schichten aus kleineren Knochenabschnitten aufgebaut werden, um ein Implantat bereitzustellen, dessen Gesamtgröße nicht durch die Größe und/oder Form des kortikalen Knochens beschränkt ist, der für seinen Aufbau verfügbar ist.
- Das Zusammenbauen der überlagerten Schichten in eine starke einheitliche Struktur kann durch eine Vielzahl von Mitteln/Verfahren erreicht werden, z.B. Aufbringen von bekannten und konventionellen biologisch kompatiblen Adhäsiven, wie etwa Cyanoacrylaten; epoxybasierten Verbindungen, Dentalharzdichtmitteln, Dentalharzzementen, Glasionomerzementen, Polymethylmethacrylat, Gelatineresorcinol-Formaldehyd-Leimen, kollagenbasierten Leimen, anorganischen Bindemitteln wie etwa Zinkphosphat, Magnesiumphosphat oder anderen phosphatbasierten Zementen, Zinkcarboxylat etc. und proteinbasierten Bindemitteln wie etwa Fibrinleimen und muschelabgeleiteten Adhäsivproteinen; die Verwendung mechanischer Befestiger wie etwa Stifte, Schrauben, Dübel etc., die aus natürlichen oder synthetischen Materialien und bioabsorbierbaren wie auch nichtbioabsorbierbaren Materialien hergestellt sein können; Lasergewebeverschweißen und Ultraschallbinden. Falls gewünscht, können die Schichten des knochenabgeleiteten Implantats mit mechanischen Zwischenverbindungsmerkmalen versehen sein, z.B. Zungen-und-Rillen- oder Zapfenloch-und-Zapfenelementen, um ihren Zusammenbau im Endprodukt zu erleichtern und/oder die Schichten in einer gesicherteren Weise aneinander zu fixieren. Zusätzlich zu seinen Kompressionsfestigkeits-verleihenden vollmineralisierten oder teilweise mineralisierten Kortikalknochenschichten kann das knochenabgeleitete Implantat dieser Erfindung optional eine oder mehrere Schichten besitzen, die aus einem oder mehreren anderen Materialien gebildet sind. Beispielsweise können diese optionalen Schichten auf hoch- oder teilweise demineralisiertem Knochen, Graphit oder pyrolytischem Kohlenstoff, einem Mineralmaterial wie etwa Hydroxyapatit, Tricalciumphosphat, Bioglas oder anderen biokeramischen oder natürlichen oder synthetischen Polymeren, z.B. bioabsorbierbare Materialien wie etwa Stärken, Polyglycolid, Polylactid, Glycolid-Lactid-Copolymer und dergleichen, und nicht bioabsorbierbaren Polymeren wie etwa Polymethylmethacrylat, Polytetrafluorethylen, Polyurethan, Polyethylen und Nylon basieren, oder diese enthalten.
- Falls gewünscht, kann die Kompressionsfestigkeitsachse einer oder mehrerer Kompressionsfestigkeit-verleihendender Schichten relativ zur Kompressionsfestigkeitsachse einer oder mehrerer der anderen Kompressionsfestigkeit-verleihenden Schichten in einer Anordnung versetzt werden, ganz ähnlich wie bei Spanplatten. Beispielsweise können die Kompressionsfestigkeitsachsen alternierender Kompressionsfestigkeits-verleihender Schichten um bis zu 90° gegenüber den Kompressionsfestigkeitsachsen der anderen Kompressionsfestigkeit verleihenden Schichten im Implantat versetzt werden, um die gesamte lastaufnehmende Kapazität des Implantats in zueinander quer laufenden Richtungen zu verteilen.
- Knochenabgeleitete Implantate jeder gewünschten Größe und/oder Konfiguration können vorgesehen sein, z.B. durch Bearbeiten oder andere mechanische Formvorgänge wie etwa Pressformen. Computerisiertes Modellieren eines spezifischen Implantats, gefolgt von computerisierter Steuerung der Formung des Implantats können verwendet werden, um ein kompliziert geformtes knochenabgeleitetes Implantat bereitzustellen, das für den beabsichtigten Einsatzort mit großer Präzision maßgefertigt ist.
- Das knochenabgeleitete Implantat kann eine oder mehrere Höhlen besitzen, die falls gewünscht mit der Oberfläche des Implantats durch Poren, Öffnungen, Perforationen oder Kanäle kommunizieren können, die für diesen Zweck vorgesehen sind und im Durchschnittsdurchmesser von einigen wenigen Mikron bis zu mehreren Millimetern reichen. Solche Hohlräume und ihre assoziierten Poren, Öffnungen, Perforationen und Kanäle können partiell oder vollständig mit einer oder mit mehreren medizinisch/chirurgisch nützlichen Substanzen gefüllt sein, die neues Knochenwachstum oder Knochenheilung fördern oder beschleunigen, z.B. aufgrund eines osteogenen, osteokonduktiven und/oder osteokonduktiven Effekts. Nützliche Substanzen dieser Art, die in das knochenabgeleitete Implantat dieser Erfindung inkorporiert werden können, beinhalten beispielsweise Kollagen, unlösliche Kollagenderivate etc. und lösliche Feststoffe und/oder darin gelöste Flüssigkeiten, z.B. antivirale Agenzien, insbesondere solche, die gegen HIV und Hepatitis wirken; Antimikrobiotika und/oder Antibiotika wie etwa Erythromycin, Bacitracin, Neomycin, Penicillin, Polymyxin B, Tetracycline, Viomycin, Chloromycetin und Streptomycine, Cefazolin, Ampicillin, Azactam, Tobramycin, Clindamycin und Gentamicin, etc.; biozide/biostatische Zucker wie etwa Dextroal, Glucose etc., Aminosäuren, Peptide, Vitamine, anorganische Elemente, Co-Faktoren der Proteinsynthese; Hormone; endokrines Gewebe oder Gewebefragmente; Synthetisierer; Enzyme wie etwa Kollagenase, Peptidasen, Oxidasen etc.; Polymerzellgerüste mit Parenchymalzellen; angiogene Medikamente und polymere Träger, die solche Medikamente enthalten; Kollagenmatrizen; Antigenreagenzien; Zytoskelettagenzien; Knorpelfragmente, lebende Zellen wie etwa Chondrozyten, Knochenmarkszellen, mesenchymale Stammzellen, natürliche Extrakte, Gewebetransplantate, Knochen, demineralisiertes Knochenpulver (oder "demineralisierte Knochenmatrix", wie es auch bezeichnet werden kann), autogene Gewebe wie etwa Blut, Serum, Weichgewebe, Knochenmark, etc.; Bioadhäsive, knochenmorphogene Proteine (BMPs), transformierender Wachstumsfaktor (TGF-beta), insulinartiger Wachstumsfaktor (IGF-1); Wachstumshormone wie etwa Somatrotropin; Knochenabdauer; Antitumoragenzien; Immunsuppressoren; Permeationsverstärker, z.B. Fettsäureester wie etwa Laureat, Myristat und Stearatmonoester von Polyethylenglykol, Enaminderivate, alpha-Ketoaldehyde, etc.; und Nukleinsäuren. Diese und ähnliche medizinisch/chirurgisch nützliche Substanzen können in das knochenabgeleitete Implantat dieser Erfindung oder irgendeine seiner Bildungsschichten während jeder Stufe des Zusammenbaus des Implantats inkorporiert werden. Geeignete Verfahren der Inkorporierung beinhalten Beschichten, Immersionssättigung, Verpacken etc. Die Mengen an medizinisch/chirurgisch nützlichen Substanzen, die verwendet werden, können breit variieren, wobei optimale Spiegel in einem spezifischen Fall durch Routineexperimente leicht bestimmt werden können.
- Das knochenabgeleitete Implantat hierin ist dafür vorgesehen, an einer Knochendefektstelle eingesetzt zu werden, die sich z.B. aus einer Verletzung, aus einem während des Verlaufs einer Operation aufgetretenen Defekt, einer Infektion, Bösartigkeit oder einer Entwicklungsmissbildung ergibt. Das knochenabgeleitete Implantat kann, nach Bedarf geeignet bemaßt und geformt, als ein Implantat oder Ersatz in einer großen Vielzahl von orthopädischen, neurochirurgischen und oralen und maxillofacialen chirurgischen Verfahren, wie etwa der Reparatur von einfachen oder komplexen Frakturen und Nicht-Verbindungen, externen und internen Fixierungen, Gelenksrekonstruktionen wie etwa Arthrodese, allgemeiner Arthroplatie, Pfannenarthroplastie der Hüfte, Femur- und Humuskopfersatz, Femurkopfoberflächenersatz und Gesamtgelenksersatz, Reparaturen der Wirbelsäule einschließlich Spinalfusion und interner Fixierung, Tumorchirurgie, z.B. Defizitverfüllung, Disektomie, Laminektomie, Exzision von Wirbelsäulentumoren, vordercervikalen und thoracalen Operationen, Reparatur spinaler Verletzungen, Skoliose-, Lordose- und Kyphosebehandlungen, Intermaxilliarfixierung von Brüchen, Mentoplastie, temporomandibularer Gelenksersatz, Zahnkammaugmentation und Rekonstruktion, Inlayknochenimplantate, Implantatersatz und -Revision, Sinusanhebungen etc. verwendet werden. Spezifische Knochen, die mit dem hiesigen knochenabgeleiteten Implantat repariert oder ersetzt werden können, beinhalten Siebbein-, Stirnbein-, Nasenbein-, Occipital-, Scheitelbein-, Schläfenbein-, Unterkiefer-, Oberkiefer-, Jochbein-, Cervicalwirbel-, Thoraxwirbel-, Lumbarwirbel-, Kreuzbein-, Rippen-, Brustbein, Schlüsselbein-, Schulterblatt-, Oberarm-, Speichen-, Ellen, Handwurzel-, Mittelhand-, Finger-, Iliums-, Sitzbein-, Schambein-, Oberschenkel-, Schienbein-, Wadenbein-, Patella-, Fersenbein-, Fußwurzel-, und Mittelfußknochen.
- Bezugnehmend auf die Zeichnungen, wie in
1 gezeigt, wird der kortikale Bereich des Knochens10 , der aus dem Diaphysen-Bereich entnommen ist, durch Längsschneiden des Knochens in kortikale Knochenschichten11 unterschiedlicher Breiten geschnitten. Falls gewünscht, können die kortikalen Knochenschichten11 weiter in gleichförmige größere Form geschnitten werden, wie bei den Knochenschichten22 des in2 gezeigten Implantats20 . -
2 illustriert ein knochenabgeleitetes Implantat20 , das alternierende Schichten von vollmineralisiertem kortikalen Knochen22 und teilweise demineralisiertem kortikalen Knochen23 umfasst. Alternativ können eine oder mehrere Schichten23 aus einem anderen Material als partiell demineralisiertem Knochen gemacht sein, wie etwa vollmineralisierter Knochen oder Mineralsubstanzen wie etwa Hydroxyapatit. Die Gesamtdicke des Knochenimplantats wird üblicherweise zumindest etwa 5 und vorzugsweise zumindest etwa 10 mm betragen. Das knochenabgeleitete Implantat20 kann geschnitten, bearbeitet, und/oder anders in jegliche andere gewünschte Form und Dimension gebracht werden, um in einen Körper implantiert zu werden. Beispielsweise kann ein im Wesentlichen zylindrisch geformtes Knochenimplantat zur Verwendung als ein Langknochensegmentersatz hergestellt werden, z.B. für einen Oberschenkelknochen. Um einen Zylinder zu bilden, kann ein im Wesentlichen quadratisches oder rechteckiges knochenabgeleitetes Implantat auf einer Drehbank bis zum benötigten Durchmesser geformt werden. Ein Hohlraum kann durch Entfernen von Materialien mit beispielsweise einem Bohrer gebildet werden oder alternativ kann ein Hohlraum durch Zusammenbauen von geeignet konfigurierten Schichten ausgebildet werden. - Wie in
3 gezeigt, ist das zylindrische oder dübelförmige knochenabgeleitete Implantat30 am intervertebralen Faserknorpelort auf der anterioren Seite der Wirbelsäule31 eingefügt gezeigt. - In
4 ist das knochenabgeleitete Jochbeinimplantat40 bemessen und geformt, um einen Teil des Jochbeinbogens und einen Teil des internen Orbits des Schädels41 zu bilden. - Wie in der Schnittansicht von
5 gezeigt, ist das knochenabgeleitete Implantat50 aus länglichen Abschnitten51 von Kompressionsfestigkeits-verleihendem vollmineralisierten oder teilweise demineralisierten kortikalen Knochen gleichförmigen quadratischen Querschnitts aufgebaut und in einer gitterartigen Weise unter Verwendung eines geeigneten Adhäsivs angeordnet, z.B. aus der Cyanacrylatreihe. Aufgrund der sich aus dem Muster von Längskanälen52 und Querkanälen53 ergebenden offenen Struktur des Implantats50 gestattet das Implantat die vaskuläre Penetration oder das Wirtsknocheneinwachsen hinein, und/oder die Diffusion einer oder mehrerer medizinisch/chirurgisch nützlicher Substanzen heraus. Vertikale Kanäle in dem gitterartigen Feld können, falls gewünscht, teilweise oder vollständig durch geeignet konfigurierte und dimensionierte Abschnitte54 besetzt sein, die aus vollmineralisiertem oder teilweise demineralisiertem kortikalen Knochen, wie in den kortikalen Knochenabschnitten51 , oder aus jeglicher anderer hierin offenbarter Substanz hergestellt werden können. Das voll zusammengebaute Knochenimplantat50 wird im Intervertebralraum61 der Wirbelsäule60 von6 installiert gezeigt. - Die folgenden Beispiele sind weitere Illustrationen des knochenabgeleiteten Implantats der vorliegenden Erfindung.
- Beispiel 1
- Ein kortikaler Abschnitt von Knochen aus der diaphysealen Region wurde in Längsrichtung geschnitten, während er kontinuierliche mit Wasser befeuchtet wurde, in ungefähr 1,5 mm dicken Schichten unter Verwendung einer Diamantblattsäge. Die Schichten wurden dann bei –70°C eingefroren und für 48 Stunden gefriergetrocknet. Die Schichten wurden dann mit Cyanoacrylatklebstoff zusammengebaut und über zwei Stunden in einer Klammer gehalten, während das Adhäsiv aushärtete. Die sich ergebende mehrschichtige einheitliche Struktur wurde auf einer Bandsäge geschnitten und durch Schleifen und Bearbeiten mit einem motorisierten Handformwerkzeug geformt, um ein geformtes Knochenimplantat bereitzustellen.
- Beispiel 2
- Ein Schnitt aus kortikalem Knochen aus der Diayhysenregion wurde, wie in Beispiel 1, in ungefähr 1,5 mm dicke Schichten gesägt. Die Hälfte der Schnitte wurden in bekannter Weise unter Einsatz von 0,6 normaler HCl-Säure vollständig demineralisiert. Alle Schichten wurden dann mit Cyanacrylatklebstoff zusammengebaut, wobei Schichten vollständig mineralisiertem Knochens mit Schichten von vollständig demineralisiertem Knochen abwechselten. Ein knochenabgeleitetes Implantat dieser Art ist in
2 illustriert. - Beispiel 3
- Ein Diaphysenknochensegment von etwa 50 mm Länge wurde gesägt, während es kontinuierlich mit Wasser befeuchtet wurde, um längliche Kortikalknochenabschnitte von gleichförmigem, quadratischem Querschnitt bereitzustellen. Die länglichen Knochenabschnitte wurden dann mit Cyanacrylatklebstoff zusammengebaut, um eine gitterartige Struktur bereitzustellen, in der Schichten von zueinander beabstandeten Knochensegmenten zueinander quer angeordnet wurden. Ein Abschnitt von knochenabgeleitetem Implantat dieser Art ist in
5 illustriert.
Claims (24)
- Knochenabgeleitetes Implantat (
20 ), welches eine Mehrzahl von überlagerten Schichten (22 ,23 ) umfasst, die mit einem Klebstoff zu einer einheitlichen Struktur verbunden sind, wobei zumindest eine Schicht in der Struktur eine Kompressionsfestigkeits-verleihende Schicht (23 ) ist, die aus nicht-demineralisiertem kortikalem Knochen oder teil-demineralisiertem kortikalen Knochen hergestellt ist. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, wobei die Kompressionsfestigkeits-verleihende Schicht (23 ) aus einer partiell demineralisierten Knochenschicht hergestellt ist, die nicht weniger als etwa 40 % der Kompressionsfestigkeit einer vergleichbaren Schicht von vollmineralisiertem Knochen zeigt. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, das zumindest zwei Kompressionsfestigkeits-verleihende Schichten (23 ) umfasst. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 3, wobei die Kompressionsfestigkeitsachsen von zumindest zwei aufeinanderfolgenden Kompressionsfestigkeitsverleihenden Schichten (23 ) relativ zueinander versetzt sind. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 3, wobei zumindest zwei aufeinanderfolgende Kompressionsfestigkeits-verleihende Schichten (23 ) jeweils aus voneinander beabstandeten Abschnitten gemacht sind, wobei die Kompressionsfestigkeitsachsen der Schichten relativ zueinander in einer gitterartigen Anordnung versetzt sind, die ein Feld von Kanälen in allen drei Richtungen definieren. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, das eine Gesamtdicke von zumindest 5 mm aufweist. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, das einen Querschnitt über zumindest einen Teil seiner Länge aufweist, der ein Kreis, Oval oder Polygon ist oder annähert, wobei das Implantat optional einen Hohlraum über zumindest einen Teil seiner Länge besitzt. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, weiterhin umfassend zumindest eine mechanische Festigkeit verleihende Schicht, die als ein monolithischer Knochenabschnitt vorgesehen ist. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, weiterhin umfassend zumindest eine mechanische Festigkeit verleihende Schicht, die als ein Mehrabschnittsknochenabschnitt vorgesehen ist, wobei Unterabschnitte von Knochen, die den Abschnitt bilden, miteinander in einer Kante-an-Kante-Weise verbunden sind. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, umfassend obere und untere Hauptexpositionsoberflächen, wobei die Oberflächen solche von Kompressionsfestigkeits-verleihenden Schichten (23 ) sind. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, weiterhin umfassend zumindest eine Schicht, die insgesamt oder teilweise aus zumindest einem anderen Material als nicht-demineralisiertem oder teildeminieralisiertem kortikalen Knochen hergestellt ist. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 11, wobei das Material vollständig demineralisierter Knochen, Graphit, pyrolytischer Kohlenstoff, Hydroxyapatit, Tricalciumphosphat, bioabsorbierbares Material oder nicht-bioabsorbierbares Polymer ist. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 11, wobei das Implantat Mittel zur Förderung der Diffusion von knochenheilendem Material umfasst. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 13, wobei das Mittel zur Förderung der Diffusion von knochenheilendem Material Kanäle bildet, die innerhalb einer oder mehreren Schicht(en) definiert sind. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, wobei jede Kompressionsfestigkeits-verleihende Schicht (23 ) dieselbe oder eine unterschiedliche Durchschnittsdicke im Bereich von etwa 0,5 mm bis etwa 20 mm besitzt. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, eine Kompressionsfestigkeit von etwa 25 bis etwa 250 MPa aufweisend. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, zwei bis etwa 200 überlagerte Schichten (22 ,23 ) besitzend. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, das insgesamt oder teilweise aus allogenem und/oder xenogenem kortikalen Knochen hergestellt ist. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, wobei die überlagerten Schichten (22 ,23 ) mit mechanischen Befestigern verbunden werden. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, das ein oder mehrere Hohlräume umfasst. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 19, wobei Poren, Öffnungen, Perforationen oder Kanäle eine Verbindung zwischen dem Hohlraum und der Oberfläche des Knochens bereitstellen. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 19, wobei der Hohlraum ein antimikrobielles Mittel, ein Antibiotikum, ein angiogenes Mittel, ein antigenes Mittel oder ein zytoskelettales Mittel enthält. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, wobei die überlagerten Schichten (22 ,23 ) mit einem Klebstoff zusammengesetzt werden, der aus Cyanacrylat, einer Epoxy-basierten Verbindung, Dentalharzdichtmittel, Dentalharzzement, Polymethylmethacrylat, einem Gelatine-Resorcinol-Formaldehyd-Klebstoff, einem Kollagenbasierten Klebstoff, Zinkphosphat, Magnesiumphosphat, Zinkcarboxylat und einem Fibrinkleber ausgewählt ist. - Knochenabgeleitetes Implantat (
20 ) gemäß Anspruch 1, wobei das knochenabgeleitete Implantat als ein Siebbein, ein Stirnknochen, ein Nasalknochen, ein Occipitalknochen, ein Scheitelbein, ein Schläfenbein, ein Mandibularknochen, ein Maxillarknochen, ein Jochbein, ein zervikaler Wirbelsäulenknochen, ein thorakaler Wirbelsäulenknochen, ein Lumbarknochen, ein Mittelhand-Knochen, ein Phalanxknochen, ein Iliumknochen, ein Ischiumknochen, ein Schambein, ein Femurknochen, ein Tibiaknochen, ein Patellaknochen, ein Fersenbein, ein Tarsalknochen oder ein Metatarsalknochen konfiguriert ist.
