- GEBIET DER ERFINDUNG
- Die vorliegende Erfindung betrifft eine Vorrichtung, die in der Arthroplastik verwendet wird, wie eine Probe zum Bestimmen der erforderlichen Abmessungen einer Oberschenkelprothesenkomponente.
- ALLGEMEINER STAND DER TECHNIK
- Ein erfolgreicher Hüftgelenkersatz oder ein erfolgreiches Arthroplastikverfahren resultieren, zumindest teilweise, aus der Auswahl der Gelenkprothesenkomponenten, die so bemessen und positioniert sind, daß sie an die Geometrie und Funktionseigenschaften eines natürlichen, gesunden Hüftgelenks nahe heranreichen oder diese nachbilden. Üblicherweise umfaßt der Komponentenauswahlprozeß eine präoperative Analyse von Gelenkbildern. Jedoch hat sich herausgestellt, daß die temporäre Fixierung einer oder mehrerer provisorischer Komponenten an einem oder mehreren Knochen von Interesse in einem Stadium des Arthroplastikverfahrens vor der permanenten Fixierung der Gelenkprothese eine nützliche Ergänzung zur Bildanalyse ist. Die provisorischen Komponenten sollen bestimmte Aspekte der permanenten Gelenkprothese nachahmen, damit ein Chirurg Messungen validieren und mehrere unterschiedliche mögliche Komponentengrößen und -konfigurationen testen oder „ausprobieren" kann. Folglich sind provisorische Komponenten geeigneterweise als „Proben" bekannt.
- In einem bekannten Verfahren wird eine Probe für eine Oberschenkelkomponente auf folgende Art und Weise verwendet. Das proximale Ende eines Oberschenkelknochens wird reseziert und der Knochenmarkkanal des Oberschenkelknochens wird erweitert. Eine Räumnadel wird in das resezierte proximale Ende des Oberschenkelknochens eingeführt, um einen Hohlraum innerhalb des Knochens bereitzustellen, welcher so bemessen und konturiert ist, um einen Oberschenkelschaft aufzunehmen. Jedoch können vor dem Entfernen der Räumnadel ein Probenhals oder -zapfen und Probenkopf an der Räumnadel befestigt werden, um einen kompletten Oberschenkelschaft zu simulieren. Normalerweise werden bei einem Versuch, eine geeignete Halslänge und Gesamtlänge des Oberschenkelschaftes zu bestimmen, nacheinander mehrere Probenhälse und Probenköpfe mit variierenden Längen und Geometrien mit der Räumnadel verbunden. Nachdem diese Längen bestimmt wurden, werden der Probenhals und Probenkopf von der Räumnadel entfernt und die Räumnadel wird aus dem Oberschenkelknochen entfernt. Anschlie ßend wird ein Oberschenkelschaft geeigneter Länge für die Einführung in den Hohlraum ausgewählt, welche durch die Räumnadel mit Hilfe von Techniken, die dem Fachmann auf dem Gebiet bekannt sind, definiert wurde.
- Andere Techniken erfordern, daß die Räumnadel aus dem Knochenmarkkanal entfernt wird, um das Einführen einer Probe zu ermöglichen, bei welcher zusätzlich zu einem Probenkopf und Probenhals ein Schaftabschnitt verwendet werden muß. Zum Beispiel offenbart
US-Patentschrift Nr. 5,100,407 ein System, welches eine Gruppe verschieden großer Probenhals/Körperabschnitte und eine Gruppe unterschiedlich langer Probenschaftabschnitte umfaßt, welche so zusammengestellt werden, daß eine geeignete Probe entsteht. Jedoch kann das wiederholte Entfernen und Einführen von Abfolgen von Probenschäften begleitet von fortlaufender Montage und Demontage hinsichtlich des Körpers einen recht langen und kostspieligen Zeitraum beanspruchen. - Eine weitere bekannte Probe umfaßt einen Schaft, an dem an aufeinanderfolgenden Punkten entlang der Länge der Probe ein Bund befestigt wird, bis eine geeignete Halslänge und Schaftlänge sichergestellt wurde. Unerwünschterweise verursacht diese Art der Probe Messungsungenauigkeiten, die aus der Schaftbewegung entstehen, wenn der Bund wiederholt mit dem Schaft in Eingriff gebracht und wieder daraus gelöst wird. Außerdem befindet sich, wenn der Bund in Richtung des distalen Endes des Schaftes bewegt wird, ein immer kleinerer Abschnitt des Schaftes innerhalb des Knochenmarkkanals, wodurch die Probe zunehmend instabil wird und das Erreichen genauer Messungen sehr schwierig ist.
- Weitere anatomische Überlegungen können ferner, und unerwünschterweise, die Zahl der Probenkomponenten in einem Satz erhöhen. Zum Beispiel müssen Proben für lange Hüftschäfte aufgrund der Krümmung oder des Bogens der entsprechenden Oberschenkelknochen für den rechten und den linken Oberschenkelknochen unterschiedlich sein. Mit anderen Worten, ein langer linker Probenschaft kann nicht bei einem rechten Oberschenkelknochen verwendet werden und umgekehrt. Man glaubt, daß ein Probensystem, welches aus zahlreichen Teilen besteht, die auszuwählen und in verschiedenen Kombinationen zusammenzustellen sind, und das möglicherweise viele Male, umständlich und unnötig komplex ist, wodurch, unter anderen Defiziten, Eingriffszeit verschwendet wird.

US-A-5 002 578 beschreibt eine modulare Hüftschaftprothese mit einer austauschbaren Schulter, welche als eine separate Komponente hergestellt wird. -
EP-A-0 428 303 beschreibt eine modulare Hüftprothese mit einstellbaren Verbindungsmitteln zwischen dem Schaftabschnitt und einem Modulabschnitt. - KURZDARSTELLUNG DER ERFINDUNG
- Die vorliegende Erfindung überwindet die Nachteile der bekannten Proben durch die Bereitstellung einer vereinheitlichten Anordnung, welche sehr genaue Messungen in einer bequemen, leicht zu verwendenden Art und Weise vereinfacht. Die Probe erfordert keine wiederholte Montage und Demontage, und sie ist einmalig in der Lage, eine Geometrie bereitzustellen, welche ungeachtet der Höhe der Probe einem geräumten Hohlraum nahezu entspricht.
- Die vorliegende Erfindung ist in den beigefügten Ansprüchen definiert.
- Eine Probenprothese umfaßt einen Schaft mit einem proximalen und einem distalen Ende und einem Körper. Der Körper steht in Eingriff mit dem Schaft nahe dem proximalen Ende des Schaftes und ist gegenüber dem Schaft verschiebbar. Ein Verriegelungsmechanismus kann zum Verhindern einer Bewegung des Körpers gegenüber dem Schaft bereitgestellt sein. Ein Bund kann sich radial von dem Schaft nach Außen erstrecken, um einen Abschnitt des Körpers zu umgeben. Der Körper und der Schaft können derart konfiguriert sein, daß sich der Durchmesser der Probenprothese an einer Stelle zwischen dem Bund und dem distalen Ende des Schaftes erhöht, wenn der Körper in Richtung des distalen Endes des Schaftes bewegt wird, und er verringert sich, wenn der Körper vom distalen Ende des Schaftes wegbewegt wird. Außerdem kann das proximale Ende des Schaftes Eingriffstrukturen wie gegenüberliegende Kerben aufweisen, in die durch ein gegabeltes Werkzeug eingegriffen werden kann.
- Bei einer weiteren Ausführungsform der Erfindung umfaßt eine Probenprothese einen proximalen Schaftabschnitt, der mit einem distalen Schaftabschnitt in Eingriff bringbar ist. Ein einzelner distaler Schaftabschnitt kann gebogen und reversibel an dem proximalen Schaftabschnitt sicherbar sein, um einen langen Probenschaft bereitzustellen, welcher für Eingriffe entweder für den rechten oder den linken Oberschenkelknochen geeignet ist. Die Probe kann einen verschiebbaren Körperabschnitt umfassen, damit die Höhe der Probe angepaßt werden kann.
- Bei noch einer weiteren Ausführungsform der Erfindung umfaßt eine Probenprothese einen Schaft mit einem proximalen Ende und einem distalen Ende und einen Körper, welcher einen Zapfen definiert. Der Körper steht drehbar in Eingriff mit dem Schaft, um eine einzige Probe bereitzustellen, die für die Verwendung sowohl in einem linken als auch einem rechten langen Knochen wie dem Oberschenkelknochen geeignet ist.

- Bezugnehmend auf
1 , ist eine Probe in einer auseinandergezogenen Ansicht gezeigt, um einen Schaft10 , einen Körper12 , einen Stift14 , eine Feder16 und einen Druckknopf18 zu zeigen. Der Schaft10 weist ein proximales Ende20 und ein distales Ende22 auf, und der Körper12 weist ein proximales Ende, welches einen Zapfen24 definiert, und ein distales Ende26 auf. Der Körper12 ist mit dem Schaft10 nahe dem proximalen Ende des Schaftes in Eingriff bringbar, damit er gegenüber dem Schaft um eine vorbestimmte Distanz zwischen dem proximalen Ende und dem distalen Ende des Schaftes verschiebbar ist. - Eine Führung oder Schiene kann mit einem oder beiden aus dem Körper
12 und dem Schaft10 in Zusammenhang stehen, um die Bewegung des Körpers gegenüber dem Schaft zu führen. Zum Beispiel umfaßt der Schaft10 , wie in2 gezeigt, eine Schiene28 , welche von einer ersten Fläche30 des Schaftes hervorsteht und sich um eine vorbestimmte Distanz zwischen dem proximalen Ende20 und dem distalen Ende22 des Schaftes erstreckt. Wie in1 gezeigt, umfaßt der Körper10 eine Kerbe32 zum Aufnehmen der Schiene28 . Jedoch umfaßt der Körper12 in anderen Ausführungsformen einen erhöhten Abschnitt, welcher mit einer Schiene in Eingriff bringbar ist, die in den Schaft eingelassen ist. Ungeachtet ihrer Konfiguration dienen die komplementäre Führung/Schiene/Kerbe des Körpers und des Schaftes der Einschränkung der Bewegung des Körpers12 entlang eines vorbestimmten Pfades, wie einer Längsbewegung, sowie der Verhinderung unerwünschter Bewegungen wie Verdrehen oder seitliche Verschiebung. - Außerdem kann ein Verriegelungsmechanismus bereitgestellt sein, um eine Bewegung des Körpers
12 gegenüber dem Schaft10 zu verhindern. Wie in1 und3 gezeigt, kann der Körper12 einen Kanal34 aufweisen, welcher quer zu der Kerbe32 verläuft und welcher dafür eingerichtet ist, einen länglichen Abschnitt des Knopfes18 darin aufzunehmen. Der Knopf18 ist innerhalb des Kanals von einer ersten Lage, in der ein Abschnitt des Knopfes in Kontakt und in Eingriff mit einem Abschnitt der Schiene28 des Schaftes steht, in eine zweite Lage, in welcher der Knopf aus der Schiene gelöst ist, beweglich. Wie in4 gezeigt, ist der Knopf18 durch die Feder16 in die erste Lage vorgespannt.5 veranschaulicht den Knopf18 in der zweiten Lage. Der Knopf18 weist einen erweiterten Knopfabschnitt36 auf, welcher mit dem Körper12 in Eingriff steht, um die Einführungstiefe des Knopfes18 in den Körper zu beschränken. Der Knopf18 weist auch einen Ausschnittabschnitt38 auf, in welchen der Stift14 und ein Abschnitt der Schiene28 aufgenommen werden. In2 dürfte am besten zu sehen sein, daß die Schiene erste und zweite Querrillen40 und42 umfaßt. Wenn der Körper12 und der Schaft10 gegeneinander verschoben werden, bewegt sich der Ausschnittabschnitt38 über die/entlang der Schiene28 , bis eine Rille40 ,42 erreicht ist, woraufhin ein Endabschnitt44 des Knopfes in die Rille40 ,42 vorgespannt wird. Der Endabschnitt44 wird durch Drücken des Knopfes18 in den Körper12 mit ausreichender Kraft, um die Vorspannkraft der Feder16 zu überwinden, aus der Rille40 ,42 freigegeben. - Wieder bezugnehmend auf
1 und2 , erstreckt sich ein Bund46 radial von dem Schaft nach außen. Der Bund16 umgibt einen Abschnitt des Körpers12 , wie in6 gezeigt, und er ist so bemessen, daß er auf einer resezierten Knochenfläche aufliegt, wie in7 und8 gezeigt. Der Bund46 und das distale Ende des Schaftes22 sind um eine feste Distanz voneinander entfernt. Der Bund definiert eine Öffnung48 mit Bögen zum Ergänzen der Form des Körpers12 und in welcher der Körper aufgenommen wird. - Nun bezugnehmend auf
7 und8 , ist die Verwendung der Probenprothese in bezug auf die resezierten Oberschenkelknochen50 bzw.52 veranschaulicht. Nachdem der Oberschenkelknochen vorbereitet wurde, um die Probe aufzunehmen, wird das distale Ende22 der Probe in den Knochenmarkkanal54 ,56 eingeführt. Es sei darauf hingewiesen, daß in beiden7 und8 die komplette Länge des Schaftes10 vom Bund46 bis zum distalen Ende des Schaftes22 in den Knochenmarkkanal eingeführt ist. Dies ermöglicht es einem Chirurgen zu verifizieren, daß der Knochenmarkkanal auf eine ausreichende Tiefe und Weite geräumt wurde, um einen Ersatzhüftschaft unterzubringen. Der Chirurg verschiebt dann, wie erforderlich, den Körper12 gegenüber dem Schaft, um die Kopfhöhe oder Distanz zwischen einem Referenzpunkt58 auf dem Kopf60 , der am Körper12 befestigt ist, und dem Bund46 anzupassen, um eine Hüftprothesen-Schaftlänge zu bestimmen. In7 ist der Körper12 gegenüber dem Schaft10 auf einer Kopfhöhe von 45 mm positioniert, wohingegen in8 die Kopfhöhe 55 mm beträgt. Die Probe wird dann aus dem Knochenmarkkanal entfernt und ein Hüftprothesenschaft mit der bestimmten Länge wird aus einer Gruppe von Hüftschäften ausgewählt. Der ausgewählte Hüftschaft wird in den Knochenmarkkanal zementiert. - In diesen Veranschaulichungen sollte zu sehen sein, daß der proximale Abschnitt der Probe unterhalb des Bundes breiter ist als ein Zwischenabschnitt des Schaftes oder sein distales Ende
22 , um ein Festsitzen der Probe innerhalb des Oberschenkelknochens sicherzustellen. Die Probe ist im proximalen Abschnitt unterhalb des Bundes geringfügig größer als der tatsächliche Ersatzschaft, damit die Probe den Knochenmarkkanal füllt, welcher so geräumt wurde, daß er geringfügig größer als ein tatsächlicher Ersatzschaft ist (um Raum für Knochenzement zu lassen, welcher den Ersatzschaft umgibt). Der Schaft10 und der Körper12 sind auch derart konfiguriert, daß sich ein Durchmesser der Probe ein einem Punkt zwischen dem Bund und dem distalen Ende des Schaftes erhöht, wenn der Körper in Richtung des distalen Endes des Schaftes bewegt wird, und er verringert sich, wenn der Körper vom distalen Ende des Schaftes wegbewegt wird. Somit entsprechen, ungeachtet dessen, wo ein Resektionsschnitt gemacht wird, die Abmessungen und die Form der Probe den Abmessungen und der Form des geräumten Knochenmarkkanals. - Da die Probe üblicherweise sehr fest innerhalb des Knochenmarkkanals sitzt, umfaßt die Probe gemäß der Erfindung ferner Merkmale, die von Nutzen sind, wenn die Probe in den Knochenmarkkanal eingeführt oder die Probe daraus heraus extrahiert wird. Zum Beispiel umfaßt, bezugnehmend auf
2 , das proximale Ende des Schaftes eine Werkzeugeingriffstruktur, wie ein Paar gegenüberliegender Kerben62 und64 an dem Schaft. Ein Werkzeug66 , in10 gezeigt, umfaßt einen ersten Gabelabschnitt oder eine Gabelung68 und eine zweite Gabelung70 für den Eingriff in die gegenüberliegenden Kerben62 und64 . Der Raum zwischen den Gabelungen entspricht der Form des Körpers12 , damit das Werkzeug fest in den Körper paßt. Dies stellt sicher, daß das Werkzeug axial am Körper12 und Schaft10 ausgerichtet bleibt. Jede Gabelung68 und70 kann gewinkelte Endabschnitte oder Zinken72 bzw.74 umfassen. Ein Griff76 stellt eine leicht greifbare Struktur zum Ziehen oder Drücken des Werkzeuges66 sowie eine geeignete Oberfläche für Hammerschläge bereit. Weil die Kerben62 ,64 ähnlich der Kerben eines Ersatzschaftes sind, die verwendet werden, um Cerclagekabel zu orientieren, kann das gleiche Werkzeug66 verwendet werden, um sowohl die Probe als auch den Ersatzschaft einzuführen und zu extrahieren. -
10 zeigt das Werkzeug66 positioniert hinsichtlich der Probe zum Einführen der Probe in einen Knochenmarkkanal, wobei die Zinken72 ,74 nicht mit den Kerben62 ,64 in Eingriff stehen, sondern direkt auf dem Bund46 auf gegenüberliegenden Seiten des Körpers12 ruhen. -
11 zeigt das Werkzeug66 positioniert hinsichtlich der Probe zum Extrahieren der Probe aus einem Knochenmarkkanal, wobei die Zinken72 ,74 mit den Kerben62 ,64 in Eingriff stehen. - Das gegabelte Werkzeug erlaubt so das Aufbringen einer gleichmäßigen und verteilten Kraft auf den Schaft
10 , sowohl während des Einführens als auch während des Extrahierens. - Noch ein weiteres Problem mit Proben, insbesondere Proben für lange Hüftschäfte, ist, daß eine lange Probe die Krümmung oder den Bogen des Oberschenkelknochens berücksichtigen muß. Ein modularer Schaft kann, abhängig von der Konfiguration eines distalen Schaftabschnittes, den Probenschaft verlängern und die erforderliche Krümmung für eine angemessene Oberschenkelimplantation bereitstellen.
- Zum Beispiel ist
12 eine auseinandergezogene Ansicht einer Ausführungsform einer Probe mit einem proximalen Schaftabschnitt 78, einem distalen Schaftabschnitt80 und einem Verbindungselement82 . Der proximale Schaftabschnitt78 kann eines oder mehrere der Merkmale umfassen, die oben hinsichtlich des Eingriffs eines Körpers12 oder hinsichtlich eines Werkzeugs66 beschrieben wurden. Alternativ dazu kann der proximale Schaftabschnitt78 keines der obigen erfinderischen Merkmale umfassen, sondern eher Merkmale, die bei Proben, die in der Technik bekannt sind, zu finden sind. - Bei der veranschaulichten Ausführungsform weist der proximale Schaftabschnitt
78 etwa die gleiche Länge wie der in1 –11 veranschaulichte Schaftabschnitt10 auf und ist daher wie oben beschrieben ohne die Hinzufügung des Verbindungselementes82 oder des distalen Schaftabschnittes80 einsetzbar. Jedoch ist, wenn der distale Schaftabschnitt80 mit dem proximalen Schaftabschnitt78 verbunden wird, die Gesamtlänge der Probe äquivalent der einer traditionellen einteiligen Langschaftprobe. Bei weiteren Ausführungsformen ist der proximale Schaftabschnitt78 länger oder kürzer als der Schaftabschnitt10 , und der distale Schaftabschnitt80 ist entsprechend kürzer oder länger als der in12 veranschaulichte. - Obwohl der distale Schaftabschnitt
80 auch gerade oder co-axial mit dem proximalen Schaftabschnitt78 sein kann, ist er in der veranschaulichten Ausführungsform gebogen oder weist eine Winkeligkeit auf, so daß ein Teil des Schaftabschnittes oder der gesamte distale Schaftabschnitt nicht co-axial mit dem proximalen Schaftabschnitt ist. Die Krümmung oder die Winkeligkeit kann durch die Form des distalen Schaftabschnittes und/oder durch das Verbinden der Schaftabschnitte in einem Winkel bereitgestellt sein. Zusammen werden die Krümmung oder Winkeligkeit des Schaftes oder der Schäfte als „gebogen" bezeichnet. - Weiter bezugnehmend auf
12 , ist der distale Schaftabschnitt80 selektiv, reversibel am proximalen Schaftabschnitt78 anbringbar, damit die Probe einen Bogen in einer ersten oder einer zweiten entgegengesetzten Richtung aufweist. So kann die Probe entweder für den rechten oder den linken Oberschenkelknochen verwendet werden, ohne daß ein zusätzliches Teil erforderlich ist. Bei der veranschaulichten Ausführungsform beinhalten die Strukturen, welche die Reversibilität vereinfachen, einen Dorn84 am distalen Schaftabschnitt80 , welcher in einen Schlitz86 aufgenommen wird, der durch den proximalen Schaftabschnitt78 definiert ist. Der Dorn84 und der Schlitz86 stellen sicher, daß der distale und proximale Schaftabschnitt in einer präzisen Orientierung zusammengefügt werden, und daß die Orientierung nicht unbeabsichtigt verändert werden kann. Wenn zum Beispiel die Schaftabschnitte einfach durch Reibpassung wie ein zylindrisches Element in einer zylindrischen Bohrung oder durch eine Schraubverbindung verbunden werden sollten, könnte die präzise Orientierung nicht sichergestellt werden. - Der Schlitz
86 und der Dom84 können sich auf Abschnitten mit reduziertem Durchmesser88 und90 des entsprechenden proximalen Schaftabschnittes78 und des distalen Schaftabschnittes80 befinden. Die Abschnitte mit reduziertem Durchmesser88 und90 können mit einem Gewinde versehen sein und innerhalb einer Gewindebohrung92 des Verbindungselementes82 aufgenommen werden. Das Verbindungselement82 kann eine Textur aufweisen, wie abwechselnde längliche Erhöhungen94 und Vertiefungen96 , um zu ermöglichen, daß das Verbindungselement von Hand oder mit einem Werkzeug gegriffen und gegenüber dem Schaftabschnitt oder der Schaftabschnitte gedreht wird, um einen geschraubten Eingriff zwischen den Schaftabschnitten zu erzeugen. So verriegelt das Verbindungselement die Schaftabschnitte miteinander. Jedoch werden auch weitere Ausführungsformen in Betracht gezogen, die erlauben, daß die Schaftabschnitte in einer von zwei oder mehr präzisen Orientierungen verbunden und ohne ein separates Verbindungselement zusammengehalten werden. -
13 zeigt die Probe von12 in einem zusammengebauten Zustand. Es sei darauf hingewiesen, daß das Verbindungselement82 einen maximalen Durchmesser aufweist, welcher dem der Schaftabschnitte nahekommt, wo sie auf das Verbindungselement treffen. -
14 ist eine teilweise Ausschnittansicht einer Verbindung zwischen dem proximalen Schaftabschnitt78 und dem distalen Schaftabschnitt80 , welche klar den Eingriff des Domes84 in den Schlitz86 zeigt. Obwohl der veranschaulichte Dorn und Schlitz eine ausgezeichnete reversible Verbindung bereitstellen, werden weitere Strukturen zum Verbinden der Schaf tabschnitte in Betracht gezogen und die bestimmte Verbindung ist keine Einschränkung der Erfindung. -
15 ist eine auseinandergezogene Ansicht noch einer weiteren Ausführungsform der Probe, wobei ein Schaft98 gegenüber einem Körper100 , welcher einen Zapfen102 definiert oder aufweist, drehbar ist. Der Schaft98 umfaßt ein proximales Ende104 und ein distales Ende106 . Der Schaft98 ist für anatomische Korrektheit wie durch seine Länge erforderlich gebogen. - Die Abschnitte mit reduziertem Durchmesser
108 ,110 ,112 des Schaftes98 können in eine Bohrung114 innerhalb des Körpers100 eingeführt werden. Ein erster Verriegelungsmechanismus116 ist mit dem Körper in Eingriff bringbar, um ein sich Zurückziehen des proximalen Endes104 des Schaftes98 aus der Bohrung114 zu verhindern und das proximale Ende mit einem eingeschränkten Bewegungsbereich innerhalb der Bohrung zu versehen. Ein Stift118 , der durch eine Öffnung120 in den Körper100 eingeführt werden kann, wird verwendet, um die Bewegung des ersten Verriegelungsmechanismus116 gegenüber dem Körper zu begrenzen. -
16 zeigt den Stift118 detaillierter. Der Stift118 ist ein im wesentlichen zylindrischer Körper122 mit einem Vorsprung124 , der sich aus einem Ende heraus erstreckt. Ein Vorspannungselement126 , wie eine Feder oder ein elastischer Körper, ist derart innerhalb des Körpers122 positioniert, daß es eine nach außen gerichtete Vorspannung auf den Vorsprung124 ausübt. Das Äußere des Körpers kann Gewinde128 aufweisen, die mit Gewinden (nicht gezeigt) in der aufnehmenden Öffnung120 des Körpers übereinstimmen, für einen sicheren Eingriff des Stiftes118 innerhalb des Körpers110 . Ein Schlitz130 , eine Buchse oder ein anderes Eingriffmerkmal für eine Werkzeug, wie einen Schraubendreher oder einen Inbusschlüssel, kann an einem Ende des Stiftes bereitgestellt sein, um das Zusammenfügen von Stift und Körper zu vereinfachen. -
17 ist eine Draufsicht des ersten Verriegelungselementes116 . Der Verriegelungsmechanismus116 umfaßt eine Region132a mit einem ersten Durchmesser und eine Region132b mit einem zweiten Durchmesser, wobei der erste Durchmesser größer ist als der zweite Durchmesser. Wenn das erste Verriegelungselement im Eingriff mit dem Körper100 steht, definiert die Region132 einen Abschnitt der Bohrung114 (oder eine Erweiterung davon) durch den Körper. Das Bewegen des ersten Verriegelungsmechanismus116 gegenüber dem Körper100 , wie gezeigt und in bezug auf18 –21 unten beschrieben, erhöht oder verringert den lokalen Durchmesser der Bohrung114 . - Die Fingernasen
134 und136 sind bereitgestellt, um das Drücken und Ziehen der ersten Verriegelungsvorrichtung116 gegenüber dem Körper100 zu vereinfachen. Ein Arm138 erstreckt sich senkrecht von der Ebene, die durch die Fingernasen134 ,136 definiert ist, in oder um einen Abschnitt des Körpers100 . Ein kleiner Vorsprung140 erstreckt sich von dem Arm138 aus, um eine erste Anschlagfläche142 auf der gegenüberliegenden Seite der Bohrung114 von den Fingernasen bereitzustellen. Eine zweite Anschlagfläche144 liegt der ersten Anschlagfläche142 gegenüber. - Wenn die erste Verriegelungsvorrichtung
116 und der Stift118 mit dem Körper100 in Eingriff stehen, befinden sich die erste und zweite Anschlagfläche142 bzw.144 auf gegenüberliegenden Seiten des Vorsprungs124 . Der Vorsprung124 ist gegen eine Fläche146 des Armes138 vorgespannt, und der Abstand zwischen den Anschlagflächen140 ,144 definiert das Bewegungslimit der ersten Verriegelungsvorrichtung. Eine Aussparung, Rille oder Vertiefung148 in der Fläche146 ist so bemessen, daß sie mindestens einen Abschnitt des Vorsprungs124 aufnimmt. Wenn die erste Verriegelungsvorrichtung116 derart positioniert ist, daß der Vorsprung124 innerhalb der Vertiefung148 aufgenommen ist, wird die Bewegung der ersten Verriegelungsvorrichtung verhindert, bis genügend Kraft auf die erste Verriegelungsvorrichtung aufgebracht wird (durch Ziehen), um die Vorspannkraft zu überwinden, die durch das Vorspannelement126 auf den Vorsprung aufgebracht wird. Ein Arretierstift150 , der innerhalb einer Bohrung (nicht gezeigt) im Körper100 aufnehmbar ist, ist auf der ersten Verriegelungsvorrichtung116 bereitgestellt, um zu verhindern, daß die erste Verriegelungsvorrichtung vom Körper weg in Richtung der Vorspannkraft verschoben wird. - Nun bezugnehmend auf
18 , ist eine komplett zusammengebaute Probe im verriegelten Zustand und für die Implantation konfiguriert veranschaulicht. In dieser Ansicht ist eine zweite Verriegelungsvorrichtung gezeigt, welche einen Dorn152 in Zusammenhang mit dem Schaft98 aufweist, welcher innerhalb einer Kerbe154 in Zusammenhang mit dem Körper100 sitzt. Eine zweite Kerbe (nicht gezeigt) ist im Körper100 direkt gegenüber (180 Grad von) der ersten Kerbe154 bereitgestellt. Der zweite Verriegelungsmechanismus verhindert die Rotation des Körpers100 gegenüber dem Schaft, wenn die Kerbe und der Dorn zusammengefügt sind. Bei weiteren Ausführungsformen steht der Dorn in Zusammenhang mit dem Körper und die Kerben stehen in Zusammenhang mit dem Schaft. - Ein Schlitz
156 , welcher Zugang zu der Bohrung114 in dem Körper100 gewährt, kann bereitgestellt sein, um das Entfernen von Gewebe oder Fremdkörpern aus der Bohrung zu ermöglichen, oder diese einfach reinigen zu können, ohne die Probe komplett demontieren zu müssen. - Es sei darauf hingewiesen, daß die erste Verriegelungsvorrichtung
116 im verriegelten Zustand im wesentlichen bündig mit einem Abschnitt des Körpers100 oder innerhalb des geometrischen Profils des Körpers ist. Bei der veranschaulichten Ausführungsform ist die erste Verriegelungsvorrichtung bündig mit einem Flanschelement158 . Jedoch ist weder das Vorhandensein eines Flansches wichtig für die Erfindung, noch die bestimmte Form des Körpers. - Wie in
19 veranschaulicht, ist im verriegelten Zustand der Abschnitt mit reduziertem Durchmesser110 des Schaftes von der Region132b der Verriegelungsvorrichtung116 umgeben, welche einen kleineren Durchmesser als der Abschnitt112 des Schaftes aufweist. Somit wird der Abschnitt112 durch den ersten Verriegelungsmechanismus116 an seiner am weitesten eingeführten Distanz in der Bohrung114 eingeklemmt oder gehalten und der Schaft98 kann nicht weit genug vom Körper100 weggezogen werden, um den Dom152 aus der Kerbe154 zu befördern. Der Vorsprung124 des Stiftes118 sitzt in der Vertiefung148 (siehe17 ). Somit agiert der Stift118 als ein Verriegelungselement für den ersten Verriegelungsmechanismus116 . -
19 zeigt auch eine Querbohrung160 durch den Zapfen. Die Bohrung160 identifiziert die Vorrichtung nicht nur leicht als eine Probe, sondern sie ermöglicht es auch, daß ein Stab oder ein anderes Werkzeug (nicht gezeigt) in oder durch die Bohrung160 eingeführt wird, um die Extraktion der Probe aus einem geräumten Knochenkanal zu vereinfachen, wo sie temporär implantiert wurde. Ein Schlitz162 ermöglicht das Reinigen der Probe ohne eine vollständige Demontage der Probe oder die Entfernung von Körpergewebe oder Fremdstoffen aus der Bohrung114 . - Nun bezugnehmend auf
20 , ist eine vollständig montierte Probe im unverriegelten Zustand veranschaulicht, wobei die erste Verriegelungsvorrichtung116 aus dem Körper100 herausgezogen ist. Der Schaft98 wird in Richtung des Pfeils gedreht, um die zweite Kerbe (die sich gegenüber der Kerbe154 befindet) an dem Dorn152 auszurichten (siehe18 ), um die Langschaftprobe für einen linken Oberschenkelknochen zu konfigurieren. Im urverriegelten Zustand sitzt der Dorn152 nicht innerhalb der Kerbe und der Schaft98 ist gegenüber dem Körper100 frei drehbar. - Wie in
21 veranschaulicht, ist die erste Verriegelungsvorrichtung116 im unverriegelten Zustand weit genug vom Körper100 weggezogen, damit sich der Abschnitt mit reduziertem Durchmesser112 in den Bereich der Bohrung114 bewegen kann, der teilweise durch die Region132a der Verriegelungsvorrichtung116 definiert ist. Wenn sich der Schaft absenkt (sich also vom Körper weg bewegt), löst sich die zweite Verriegelungsvorrichtung (Dorn und Kerbe) aus dem Eingriff. Weil jedoch der Schaft98 mit einem Kopf164 endet, der breiter ist als der Durchmesser der Bohrung, die teilweise durch die Region132a definiert ist, trennt sich der Schaft nicht komplett vom Körper. Wie in21 gezeigt, ist der Vorsprung124 des Stiftes118 aus dem Eingriff mit der Vertiefung148 gelöst und der Vorsprung und der Stift treffen auf eine Anschlagfläche142 . - Viele funktionale Vorteile sind aus den oben beschriebenen Merkmalen abgeleitet. Zum Beispiel läßt die Antirotationsverriegelung aus Kerbe und Dorn nur zwei zusammengebaute Konfigurationen zu, wobei jede Position eine entsprechende Geometrie bereitstellt. Es gibt keine mögliche unbeabsichtigte oder „leicht verschobene" Lage. Obwohl eine Kerbe und ein Dorn offenbart sind, werden auch andere Geometrien und Strukturen in Betracht gezogen, welche eine ähnliche Funktion erreichen. Auch sei darauf hingewiesen, da die erste und zweite Verriegelungsvorrichtung, wenn sie verriegelt sind, komplett innerhalb der Implantatgeometrie liegen, die Probe das Profil eines tatsächlichen Implantates genau repliziert. Ferner kann, weil die Probe derart konfiguriert sein kann, daß ein Abschnitt mit reduziertem Durchmesser des Schaftabschnittes nahe dem proximalen Ende der Probe in den Körperabschnitt ragt, der Schaftabschnitt leicht entnommen werden, wenn der Körper und der Schaft getrennt werden, weil sich das proximale Ende des Schaftes nahe der Öffnung des geräumten Knochenkanals befindet oder daraus hervorsteht.
- Obwohl die Erfindung hinsichtlich exemplarischer Ausführungsformen von dieser gezeigt und beschrieben wurde, können verschiedene andere Änderungen, Weglassungen und Hinzufügungen in Form und Detail davon erfolgen, ohne den Umfang der Erfindung zu verlassen.
Claims (3)
- Probenprothese, die folgendes aufweist: einen Schaft (
10 ,98 ) mit einem proximalen Ende (104 ) und einem distalen Ende (106 ); einen Körper (12 ,100 ), der einen Zapfen (24 ,102 ) aufweist, wobei der Körper (12 ,100 ) drehbar mit dem Schaft (10 ,98 ) in Eingriff steht; und einen ersten Verriegelungsmechanismus (116 ), der dafür eingerichtet ist, eine Längsbewegung des Schafts (10 ,98 ) gegenüber dem Körper (12 ,100 ) zu verhindern, wobei das proximale Ende (104 ) des Schafts (10 ,98 ) einen Abschnitt mit reduziertem Durchmesser (108 ,110 ,112 ) aufweist; und wobei der Körper (12 ,100 ) eine Bohrung (114 ) aufweist, die so bemessen ist, um den Abschnitt mit reduziertem Durchmesser des Schafts (10 ,98 ) aufzunehmen und der erste Verriegelungsmechanismus (116 ) dafür eingerichtet ist, eine Bewegung des Abschnitts mit reduziertem Durchmesser (108 ,110 ,112 ) des Schafts (10 ,98 ) innerhalb der Bohrung (112 ) des Körpers (12 ,100 ) zu verhindern, dadurch gekennzeichnet, daß der erste Verriegelungsmechanismus (116 ) einen Abschnitt (132 ) der Bohrung (114 ) definiert, und der erste Verriegelungsmechanismus (116 ) dafür eingerichtet ist, aus einer ersten Lage in eine zweite Lage bewegt zu werden, um den Durchmesser der Bohrung (114 ) zu erhöhen. - Probenprothesen nach Anspruch 1, die weiterhin einen zweiten Verriegelungsmechanismus (
118 ) aufweist, der dafür eingerichtet ist, den ersten Verriegelungsmechanismus (116 ) in der ersten Lage zu halten. - Probenprothesen nach Anspruch 2, wobei der zweite Verriegelungsmechanismus (
118 ) ein Element (124 ) aufweist, das gegenüber dem ersten Verriegelungsmechanismus (116 ) vorgespannt ist und mit diesem in Eingriff bringbar ist, wenn der erste Verriegelungsmechanismus (116 ) sich in der ersten Lage befindet.
