- the graduation corresponding to the milling of cup C will be spaced 10 mm from that corresponding to the milling of the femoral head T.
- the central hole of the femoral and acetabular cutters 11 and the hollow hexagonal head 16a of the screw 16 are similarly sized so that the same instrument 6 can be used.
- the support arm 4 always supports the cotyloid implant 15, via its distal end 4b which is screwed into the peripheral thread 15d of the cup.

- the latter comprises, analogously to the instrument 6, a handle 18b at its proximal portion.
- lugs 17a which are intended to fit in corresponding recesses or mortises 15c provided at the periphery of the internal concave housing of the acetabular implant 15. All of the lugs 17a and recesses 15c form a connection means of the tenon-mortise type for stably and rigidly assembling the insert 17 in the acetabular implant 15.
- the impactor element 19 is then separated from the insert 17, by exerting a simple pull on the instrument 18 until the frangible points break. Since the frangible points have a lower tensile strength than that of the tenon-mortise connection between the insert 17 and the cup 15, the impactor 19 will be detached without risking uncoupling the insert from the cup. Instead of exerting traction, one could also continue the rotation of the instrument 18 in the direction of its screwing on the threaded rod 19a to cause the breaking of the frangible points by shearing.
- This variant has the advantage of using the same instrument 6 for also fitting the insert 17.
- the shear strength of the frangible points between the impactor 19 and the insert 17 will be determined so as to be less than that of the anchoring points 15a of the cup 15 in the cup C.
- the frangible points can be three in number and consist of small polyethylene welds.
- the femoral implant 20 here has the general shape of a spherical cap whose convex external surface is smooth so as to be able to articulate in a slippery manner in the concave housing of the insert 17, and whose concave internal surface is provided with roughness or small points 20b which are intended to be anchored on the femoral head T previously milled.
- the femoral implant 20 also has at the center of its concave face a small threaded rod 20a similar to the aforementioned rod 19a and a peripheral thread 20c (see FIG. 16) for its connection with the support arm 4.
- tapping could be replaced by another equivalent means allowing the connection of the implants or cutters with the support arm 4.
- the instrument 18 is used in conjunction with a sleeve forming a stop 21 into which the rod of the instrument 18 is introduced. so as to be interposed between the trochanteric mass G and the sleeve 18b of the instrument 18.
- the abutment sleeve 21 is dimensioned so that the distal end 18a of the instrument 18 does not cross the surface of the femoral head, which makes it possible to impact the femoral implant 20 on the femoral head T by screwing fully the instrument 18 on the threaded rod 20a.
- an anchoring of the femoral implant 20 is thus obtained on the milled surface of the femoral head T with the threaded rod 20a inserted in the channel 22 crossing the femoral neck CF along its axis (FIG. 18 ).
- the threaded rod 20a ensures the centering of the femoral implant 20 on the femoral head T, but does not contribute to its primary anchoring because its outside diameter is less than that of the rod 18 and therefore of the channel 22 which has been practiced along of the axis of the femoral neck CF (see Figure 18).
- this threaded rod 20a can participate in the secondary anchoring during bone remodeling, it is preferable to use another separate element 23 to fix the rod 20a fixedly in place on the head of the femur T.
- This element 23 has the form of a nut whose internal thread corresponds to that of the rod 20a and which has on its external peripheral surface another thread whose bottom of the thread has a section corresponding to that of the aforementioned channel 22.
- the element 23 is introduced through the channel 22 from the trochanteric region G using another instrument 24 which supports at its distal end 24a the nut 23.
- the distal end 24a of the instrument 24 comprises for example three studs for securing in rotation the nut 23 to the instrument 24.
- the nut 23 is screwed into the channel 22, as indicated by the thread 25 in the figure 18, and is screwed onto the rod 20a at the end of the race.
- the nut 23 is screwed fully onto the threaded rod 20a until it abuts against the concave face of the implant 20.
- the instrument 24 is then easily removed because it is integral with the nut 23 only in rotation by simple contact.
- the introduction of this element 23 does not widen the main section of the channel 22. Furthermore, by providing an external thread of the nut 23 with a more spaced and wider pitch, the density of spongy bone tissue will be substantially preserved, which avoids weakening the femoral neck CF.
- the instrument 24 can be held by the support structure 1 at its proximal handle 24b, or it can be used alone, as shown in FIG. 16.
- the relative position between the femoral head T and the acetabulum C has been shown here in the drawings in a manner which does not completely reproduce the anatomical parameters for the sake of clarity and simplification.
