The ornamental design for a commemorative memorial flag holder, as shown and described.

The commemorative memorial flag holder may have double stick adhesive on the back to make it adhere to cemetery monuments. The flag, flagpole, and design shown in broken lines in the drawings are provided for illustrative purposes only and form no part of the claimed design.
